In order to improve static stability and healing of reattached labrum, we combined the advantages of suture bridge and transosseous technique. Using the conventional 3 portal for anterior instability, check stability of bony Bankart and preparation of glenoid bed in 3 way including removal, reshaping or mobilization of bony fragment. Two anchors were inserted to the superior and inferior portion and medial edge of bony Bankart lesion. It usually corresponded to the area of IGHL. Medial mattress sutures were applied around IGHL complex to get enough depth of glenoid coverage using suture hook. Make 3.5mm pushlock anchor hole to the articular edge of glenoid cartilage. Proximal suture bridge was applied at first and then distal suture bridge was inserted to mobilize the labrum in proximal direction. These construction can provide more stable labral repair with wide contact and compression in case of deficient bony stability. It not only avoids technical disadvantage of point contact with anchor fixation, but also decreasing gap formation through cross compression of labrum that couldn't gain even with the transosseous fixation which affords linear compression effect. Additional bony stability could be gained if the the bony fragment was mobilized to the glenoid margin with potential healing bed or reshaped for the good contact with reattached labrum.
Background: The blunted tip of a reusable with multiple uses can cause problems with the passing procedure in arthroscopic Bankart repair. This study assessed the advantage of Scorpion with a $70^{\circ}$ arthroscope in arthroscopic Bankart repair compared to hook typed suture passer. Methods: Scorpion in 19 patients, the hook type suture passer (conventional group) in 18 patients were used. All patients underwent the same procedure except for the type of suture passer used. Another different point of the procedure were telescopes and the number of portals used; three arthroscopic portals (posterior, anterorsuperiorlateral, and mid-anterior) and a $30^{\circ}$ arthroscope in the conventional group, but two portals and a $70^{\circ}$ arthroscope as well as the $30^{\circ}$ one in the Scorpion group. The surgery time and the surgical complications including an iatrogenic axillary nerve injury were recorded. Results: The Scorpion group showed a significant decrease in surgery time compared to the conventional group. In contrast to the conventional group, Scorpion provided an easy estimation of the exit of suture passing, no iatrogenic labral injury during the passing procedure with straight movement and the sharp tip of the knife installed. Iatrogenic supraspinatus injuries could be avoided when making an accessory anteosuperiorlateral portal due to the $70^{\circ}$ arthroscope. Conclusions: In arthroscopic Bankart repair, the use of the Scorpion suture passer and a $70^{\circ}$ arthroscope can reduce the surgery time, avoid unnecessary supraspinatus injury, and avoid iatrogenic axillary nerve damage through the relatively easy and precise suture passing and saving of the anterosuperior portal.
Purpose: To Analyze the exit site of pin inserted at the anterior glenoid rim in the reconstruction of the Bankart lesion and SLAP lesion using transglenoid suture technique. Materials and Methods: In the twenty adult right cadeveric scapula, insertions of pin were performed using guide at the position of 1, 2, 3 O'clock of glenoid rim. We measured the exit site of dorsal surface of the scapula by medial distance from sagittal plane of lateral border of scapular spine and the vertical distance from posterior border of the scapular spine. Results: When the pin was inserted caudally within 10 degree, at the position of 1, 2, 3 O'clock, the medial distance from lateral border of the scapula is averaged 29.4, 19.2, 34.0 mm respectively and the vertical distance from posterior border of the scapular spine is averaged 15.0, 18.6, 17.2 mm respectively. When the pin was inserted caudally within 20-30 degree, the medial distance is averaged 14.6, 14.2, 15.8 mm respectively and the vertical distance is averaged 31.6, 31.9, 32.1 mm respectively. Conclusion: When the pin was inserted caudally within ten degrees using the guide, the pin exit appeared at the more medial side of the base of scapular spine and the more inferior of scapular spine. This can make the firm suture tied over scapular spine during repair SLAP and the Bankart lesion, and also prevent the injury of suprascapular nerve.
Background: The purpose of this study is to investigate clinical outcomes and radiological findings of cyst formation in the glenoid around suture anchors after arthroscopic Bankart repair with either biocomposite suture anchor or all-suture anchor in traumatic anterior shoulder instability. We hypothesized that there would be no significant difference in clinical and radiological outcomes between the two suture materials. Methods: This retrospective study reviewed 162 patients (69 in group A, biocomposite anchor; 93 in group B, all-suture anchor) who underwent arthroscopic Bankart repair of traumatic recurrent anterior shoulder instability with less than 20% glenoid defect on preoperative en-face view three-dimensional computed tomography. Patient assignment was not randomized. Results: At final follow-up, the mean subjective shoulder value, Rowe score, and University of California, Los Angeles shoulder score improved significantly in both groups. However, there were no significant differences in functional shoulder scores and recurrence rate (6%, 4/69 in group A; 5%, 5/93 in group B) between the two groups. On follow-up magnetic resonance arthrography/computed tomography arthrography, the incidence of peri-anchor cyst formation was 5.7% (4/69) in group A and 3.2% (3/93) in group B, which was not a significant difference. Conclusions: Considering the low incidence of peri-anchor cyst formation in the glenoid after Bankart repair with one of two anchor systems and the lack of association with recurrence instability, biocomposite and all-suture anchors in Bankart repair yield satisfactory outcomes with no significant difference.
관절와 및 상완골의 의미 있는 골결손을 동반한 불안정한 견관절에 대해 어떻게 최선의 치료를 할 것인가에 대한 의문은 남아있다. 어떤 저자들은 외회전과 외전을 제한하는 것으로 Hill-Sachs 병변의 진입을 예방하는 일종의 관혈적 관절낭 변위를 통한 제한된 상완와관절 운동을 주장해왔으며, 다른 관혈적 시술들은 극하근건과 관절낭을 Hill-Sachs 병변 내로 관혈적 전이시키는 것으로 기술되어왔다. 최근에는 여러 저자들에 의해 Hill-Sachs 병변의 진입을 치료하는 더 새로운 접근법들이 기술되었으며, Bankart 병변 복원술에 더하여 Hill-Sachs 병변을 채우기 위해 관절경 하 후방 관절낭 고정술과 극하근건 고정술로 이루어진 이른바 'Remplissage'술기가 소개된 바 있어, 이에 저자는 외상성 견관절 불안정성으로 관절경 하 Bankart 병변 복원술 및 관절낭 축화술 시행 후 외상으로 재발된 견관절 불안정성이 발생하여 관절경 하 Bankart 병변 복원술과 함께'Remplissage'술기를 이용한 Hill-Sachs 병변 복원술을 시행한 예를 보고하는 바이다.
목적: 견관절의 Bankart 병변에 대하여서 관절경적 수술하에서도 골경유 봉합이 가능하도록 유연성 골 천공기를 개발하였으며 이에 대한 동물 실험 및 임상 증례를 보고하는 바이다. 대상 및 방법: 유연성 천공기 세트는 유연성 천공기와 유도관으로 구성되어 있다. 유연성 천공기는 49개의 미세강선들이 단일 강선 (직경 1.2 mm)으로 꼬인 구조로 이루어져 유연성이 있으며 이러한 유연한 강선의 한 쪽 끝에는 천공기 (직경 1.2 mm)가 용접되어 있는 구조로 이루어져 있다. 유도관은 외경 3.0 mm, 내경 2.0 mm의 원통형 금속 도관으로 이루어져 있으며 한 쪽 끝은 30도 정도 굴곡되어 있다. 기기의 내구성 및 임상적 유용성을 시험하기 위하여 돼지의 견갑골 관절와에 골 천공을 실시하는 실험을 30회 이상 실시하였다. 유연성 강선 부위의 기계적 파단이나 골 천공의 실패 등은 발생하지 않았다. 일반적 견관절 관절경 술식대로 견관절의 후방 입구 및 전 상방, 전 후방 입구들을 확보한 후 관절경하에서 Bankart 병변이 확인이 되면 면도기를 이용하여 견갑와 내측면의 변연 절제를 실시하여 골 출혈을 유도한다. 이후 관절경을 전 상방 입구로 이전 시키고, 후방 입구를 통하여서 유연성 골천공기 세트의 유도 강관을 삽입하는데 유도 강관의 끝이 견갑와연에서 약 5 mm 정도 안쪽에 위치되도록 한다. 유도강관 내로 유연성 천공기를 삽입한 후 구동기를 이용하여서 견갑와의 골 천공을 실시한다. 골 천공 후에는 유연성 천공기만 유도강관에서 제거한 후에 유도 강관내로 봉합사를 삽입하여 천공된 골구로 통과시키도록 한다. 봉합 갈고리를 이용하여 관절와순에 유도봉합사를 통과시킨 후에 이 유도봉합사에 골 천공구를 통과한 봉합사를 끼워서 관절완순을 통과하도록 한 후 활주 결찰을 실시하여 골 관통 봉합을 이루도록 한다. 동일한 방식으로 견갑와의 2시, 4시 방향에 골 천공을 시행하여 견갑와순의 봉합을 실시한다. 결과: 외상성 견관절 탈구 환자 5예에서 유연성 골 천공기를 이용한 Bankart 병변의 봉합을 실시하였다. 수술 도중 또는 수술후의 신경이나 혈관 손상 등의 문제점이나 합병증 등은 발생하지 않았으며 기기 자체의 문제들도 발생하지 않았다. 평균 6개월간의 추시 결과 상에서는 재탈구 등의 증상은 나타나지 않았다. 결론: 유연성 골 천공기를 사용한 Bankart 병변의 봉합시 관절경적 수술이면서도 골 관통식 봉합이 가능하여 관절와순의 부착면적의 증대 효과 뿐 아니라 손쉬운 활주 결찰이 이루어 질 수 있음을 보여주었다.
목적: 견관절의 전방 불안정성이 있는 환자에서 관절경하 생체흡수형 knotless anchor를 이용한 치료 결과를 객관적 및 주관적 평가를 통하여 알아보는데 있다. 대상 및 방법: 2003년 1월부터 2003년 6월까지 본원에서 외상성 견관절 전방 불안정성에 대하여 관절경하 생체흡수형 knotless anchor를 이용하여 Bankart복원술을 시행하고 평균 12개월 추시된 15예를 대상으로 하였다. 환자 구성은 남자가 14명, 여자가 1명이었으며, 평균 연령은 24세 (16세${\sim}$42세)였다. 평균 추시 기간은 14개월(12개월${\sim}$18개월)이었다. 수술시간은 같은 저자에 의해 2002년 1월부터 2002년 10월까지 suture anchor 술기를 이용하여 관절경하 Bankart 복원술을 시행한 20예와 비교하였다. 결과: 수술 후 탈구의 재발이나 아탈구의 발생은 없었으며 Rowe등에 의한 기능적 평가에서 평균 89.4점이었고 환자의 주관적 만족도는 평균 87.5였다. 최종 추시시의 견관절 운동 범위는 각각 굴곡이 평균 171도, 외회전이 평균 54도로 측정되었고 전 예에서 결과에 만족하였으나 3예에서 운동시 불안감각이 남아 있었다. 수술시간은 suture anchor 술기를 이용한 Bankart 복원술에서 1개의 매듭 당 평균 25.5분이 소요되었지만, 생체흡수형 knotless anchor를 이용시 1개의 anchor당 소요 시간은 평균 16.5분으로 의미 있게 감소됨을 알 수 있었다(P<0.05).결론: 견관절 전방 불안정성의 원인인 Bankart병변의 치료에 있어 knotless anchor 술식은 관절낭의 적절한 긴장을 획득하고 수술 시간을 단축하는 장점이 있으며 탈구나 아탈구의 재발없이 견관절 불안정성을 치료할 수 있는 유용한 방법으로 사료된다.
목적: 본 연구의 목적은 관절경하 Bankart 수복술에서 금속 봉합나사못과 생체흡수성 봉합나사못을 사용 시의 결과를 비교하는데 있다. 대상 및 방법: 2006년 10월에서 2007년 3월까지 견관절 전방 불안정에 대해 관절경하 Bankart 수복술을 시행한 26명을 대상으로 하였고, 금속 봉합나사못 군(GI)은 15명, 생체흡수성 봉합나사못 군(GII)은 11명이었다. 조사대상의 평균연령은 27.4세였고, 평균 추시기간은 14개월이었다. 기능평가는 KSSI 점수, Rowe 점수, ASES점수, UCLA 점수를 사용하여 평가하였다. 결과: 금속 봉합나사못 군(GI)에서 KSSI, ASES점수, UCLA점수, Rowe 점수들의 평균값의 상승은 각각 20.6, 24.0, 4.0, 45.5이었고, GII에서는 각각 21.0, 23.6, 4.6, 48.9였다. 최종 추시 시 두 군에서 기능평가상 유의한 차이를 보이지 않았지만(p>.05), GII에서 금속돌출로 인한 문제와 외상에 의한 재탈구가 각각 1예씩 있었다. 결론: 두 군 모두 만족스런 결과를 보였으며, 생체흡수성 봉합나사못을 사용할 경우 금속으로 인한 문제를 피할 수 있는 이점이 있어 추천할 만한 술식으로 생각되었다.
초기 탈구에서 시행한 조기 안정화 술식이 재발성 탈구의 Bankart 복원술 보다 좋은 결과를 나타내었으며 이는 관절경상 관찰되어진 관절낭 인대의 이완이나 관절와 연의 마멸이 결과에 영향을 미칠 수 있을 것으로 추정되었다. 또한 초기 탈구에서 발생된 Bankart 병변이 술기상으로 보다 쉽게 복원될 수 있으며 치유도 잘 되어 재발율이 감소될 것으로 생각되었고, 조기 수술적 치료를 통해 정상적인 일상 생활 및 스포츠 활동이 가능하리라 생각되었다. 따라서 활동력이 왕성하고 운동을 좋아하는 젊은 층이나 운동 선수에서는 문헌상에서 보고되고 있는 초기 탈구 후의 높은 재발율을 고려하여 초기 탈구 발생시 조기 안정화 술식이 유용할 것으로 사료되었다. 그러나 초기 탈구시 조기 안정화 술식을 시행할 지에 대해서는 보다 엄격한 적응이 요할 것으로 생각된다.
목적: Bankart병변에서 봉합나사못을 이용한 관절경적 치료는 관절와 가장자리에 점고정(point fixation)을 하여, 관절와 골 결손시에 적절한 부착부 족문(foot print)치유를 이루지 못한다는 한계점이 있다. 이에 본 저자들은 골성 Bankart병변에서 나사못 고정으로 인한 점접촉의 단점을 피하고 관절순의 교차압박을 통해 기계적 안정성을 개선시킬수 있는 경골-유사 교량형 봉합술식을 소개하고자 한다. 수술 술기: 본 술식은 교량형 봉합술식을 통한 경 골-유사 회전근개 봉합술에서 응용되었으며, 이는 부착부 족문과 건 사이 평균압력과 압력이 가해지는 접촉면적을 개선시킨다. 골성 Bankart 병변의 경우 관절와 부착부의 돌출된 골병변은 제거하거나 다듬어주며, 골편이 가동성이 있는 경우 관절순과 함께 분리시킨다. 이후 2개의 나사못(3.0 mm Biofastak, $Arthrex^{(R)}$, Naples, FL)을 골성 Bankart병변 위, 아래 부위의 내측 연에 삽입한다. 관절와를 적당한 두께로 덮을수 있도록 하 상완관절와인대 부근 관절낭순의 내측에 봉합갈고리를 이용하여 봉합사를 각각 통과시켜 mattress봉합을 시행한다. 교량형 봉합을 위해서, 전방 관절와연의 관절측 가장자리에 3.5 mm pushloc 나사못 ($Arthrex^{(R)}$, Naples, FL) 구멍을 만든 후 먼저 원위 교량형 봉합을 시행하고, 이어 근위 교량형 봉합을 시행함으로써 근위방향으로 관절순을 가동시킬 수 있다. 본 술식은 나사못 고정으로 인한 점 접촉의 기술적 단점을 피하고 관절순 교차압박을 통해 간격형성을 감소시킬수 있는 이점이 있다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
