Sepsis is a major health problem worldwide, with an extremely high rate of morbidity and mortality, partly due to delayed diagnosis during early disease. Currently, sepsis diagnosis requires bacterial culturing of blood samples over several days, whereas PCR-based molecular diagnosis methods are faster but lack sensitivity. The use of biosensors containing nucleic acid aptamers that bind targets with high affinity and specificity could accelerate sepsis diagnosis. Previously, we used the systematic evolution of ligands by exponential enrichment technique to develop the aptamers Antibac1 and Antibac2, targeting the ubiquitous bacterial peptidoglycan. Here, we show that these aptamers bind to four gram-positive and seven gram-negative bacterial sepsis agents with high binding efficiency. Thus, these aptamers could be used in combination as biological recognition elements in the development of biosensors that are an alternative to rapid bacteria detection, since they could provide culture and amplification-free tests for rapid clinical sepsis diagnosis.
국내 콩에서 발생하는 세균병해인 불마름병, 들불병, 세균점무늬병, 세균갈색점무늬병의 다중 진단을 위한 PCR 방법을 요약하면 다음과 같다. 1. 콩에 발생하는 각각의 세균들은 서로 다른 박테리오신(bacteriocin) 이나 파이토톡신(phytotoxin)을 생산하는데 이와 관련한 유전자를 목적으로 하여 진단프라이머를 설계하였다. 2. 불마름병은 glycinecin A, 들불병은 tabtoxin, 세균점무늬병은 coronatine과 세균갈색점무늬병은 syringopeptin을 목적유전자로 하여 다중 진단프라이머 조합을 설계하였다. 3. 1차 선발로 각각의 균주에 대한 단일 진단 프라이머를 선발하였으며, 여기선 선발된 21개의 프라이머들을 조합하여 4종 다중진단프라이머 선발을 위한 2차 선발에 이용하였다. 최종적으로 280 bp의 불마름병, 355 bp의 세균갈색점무늬병, 563 bp의 들불병과 815 bp의 세균점무늬병으로 구성된 다중진단 프라이머 조합이 개발되었다. 4. 선발된 4종 다중 진단 프라이머 조합의 경우 다른 세균들과의 비특이적 반응이 있는지 확인하기 위한 3차 선발을 거쳐 그 특이성을 검증하였다.

Purpose: To identify the factors associated with differential diagnosis of neonatal bacterial meningitis at the earliest opportunities possible and to evaluate the value of the bacterial meningitis score especially in neonates. Methods: We conducted a single-center, retrospective study of neonates diagnosed meningitis at our hospital between January 2000 and March 2014. We compared the general characteristics, clinical manifestations, laboratory findings, bacterial meningitis scores between the bacterial group and the aseptic group. Results: Bacterial meningitis differs significantly from aseptic meningitis in platelet count, the cerebrospinal fluid polymorphonuclear leukocyte count, and the serum protein including also the albumin (P<0.05). Except two infants, the bacterial meningitis score over 2 accurately predict bacterial meningitis in the other 11 infants. Conclusion: The bacterial meningitis score appears highly useful to identify neonatal infants with bacterial meningitis. However, its diagnostic and prognostic value is just 'adjunctive', because low score cannot rule out bacterial meningitis.

Chronic recurrent multifocal osteomyelitis (CRMO) is one of the most severe form of chronic non-bacterial osteomyelitis (CNO), which could result in bone and related tissue damage. This autoinflammatory bone disorder (ABD) is very difficult for its clinical diagnosis because of no diagnostic criteria or biomarkers. CRMO in the jaw must be suspected in the differential diagnosis of chronic and recurrent bone pain in the jaw, and a bone biopsy should be considered in chronic and relapsing bone pain with swelling that is unresponsive to treatment. The early diagnosis of CRMO in the jaw will prevent unnecessary and prolonged antibiotic usage or unnecessary surgical intervention. The updated researches for the identification of genetic and molecular alterations in CNO/CRMO should be studied more for its correct pathophysiological causes and proper treatment guidelines. Although our trial consisted of reporting items from Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA), there are very few articles of randomized controlled trials. This article was summarized based on the author's diverse clinical experiences. This paper reviews the clinical presentation of CNO/CRMO with its own pathogenesis, epidemiology, recent research studies, and general medications. Treatment and monitoring of the jaw are essential for the clear diagnosis and management of CNO/CRMO patients in the field of dentistry and maxillofacial surgery.

Importance of accurate molecular diagnosis and quantification of particular disease-related pathogenic microorganisms is highlighted as an introductory step to prevent and care for diseases. In this study, we designed a primer/probe set for quantitative real-time polymerase chain reaction (qRT-PCR) targeting rgpA gene, known as the specific virulence factor of periodontitis-related pathogenic bacteria 'Porphyromonas gingivalis', and evaluated its diagnostic efficiency by detecting and quantifying relative bacterial load of P. gingivalis within saliva samples collected from clinical subjects. As a result of qRT-PCR, we confirmed that relative bacterial load of P. gingivalis was detected and quantified within all samples of positive control and periodontitis groups. On the contrary, negative results were confirmed in both negative control and healthy groups. Additionally, as a result of comparison with next-generation sequencing (NGS)-based 16S metagenome profiling data, we confirmed relative bacterial load of P. gingivalis, which was not identified on bacterial classification table created through 16S microbiome analysis, in qRT-PCR results. It showed that an approach to quantifying specific microorganisms by applying qRT-PCR method could solve microbial misclassification issues at species level of an NGS-based 16S microbiome study. In this respect, we suggest that P. gingivalis-specific primer/probe set introduced in present study has efficient applicability in various oral healthcare industries, including periodontitis-related microbial molecular diagnosis field.
Background: As Mycoplasma pneumoniae pneumonia has increased in Korea, its relevance to infants, toddlers, and adolescents has magnified as well as. However, it is difficult to perform the serological test and PCR test routinely for diagnosis in actual clinical practice. Thus, the authors conducted this study to help clinicians do presumptive diagnosis of Mycoplasma pneumoniae pneumonia using clinical, radiological, and hematological findings. Methods: The study population consisted of 224 children between 1 month and 14 years old, hospitalized for radiographically confirmed pneumonia. Patients were divided into two groups of 100 children with Mycoplasma pneumoniae pneumonia, as diagnosed using the ELISA method. Groups with negative result in Mycoplasma IgM antibody test were classified into the viral group (98 patients with respiratory virus) and the bacterial group (46 patients with the bacteria detected in the blood sputum culture or antibiotic treatment except macrolide improved the patient's condition). These groups were compared and analyzed using clinical, hematological, and radiographic differences and scoring system. Results: Clinical, hematological, and radiographic characteristics of Mycoplasma pneumoniae pneumonia have shown the intermediate level results between bacterial pneumonia and viral pneumonia. In terms of scoring system, the mean score of Mycoplasma pneumoniae pneumonia was 4.23, which was the intermediate level between bacterial pneumonia (mean score=6.67) and viral pneumonia (mean score=1.48). Conclusion: Results suggest that the combination of the scoring system information can increase the accuracy in the diagnosis even if they may have difficulties on diagnosis, because clinical manifestations, hematological, and radiographic findings are nonspecific.
Synthetic biology builds programmed biological systems for a wide range of purposes such as improving human health, remedying the environment, and boosting the production of valuable chemical substances. In recent years, the rapid development of synthetic biology has enabled synthetic bacterium-based diagnoses and therapeutics superior to traditional methodologies by engaging bacterial sensing of and response to environmental signals inherent in these complex biological systems. Biosynthetic systems have opened a new avenue of disease diagnosis and treatment. In this review, we introduce designed synthetic bacterial systems acting as living therapeutics in the diagnosis and treatment of several diseases. We also discuss the safety and robustness of genetically modified synthetic bacteria inside the human body.
Although cardiac myxoma is one of the most common types of benign cardiac tumors, infected cardiac myxoma is very infrequent. The diagnosis of infected cardiac myxoma may be challenging because the presenting symptoms are non-specific and established management guidelines are lacking. This report describes a 39-year-old woman with a 5-month history of uncontrolled fever, chills, and myalgia who was diagnosed with myxoma and underwent mass excision. Although blood and urine cultures were negative for growing bacteria, a pathologic examination showed that the excised mass was a left atrial myxoma, with pan-bacterial polymerase chain reaction (PCR) of the surgical specimen revealing Haemophilus parainfluenzae at 99.87%, resulting in a diagnosis of infected cardiac myxoma. Laboratory tests, such as PCR, may supplement culture results in the diagnosis of infected cardiac myxoma.
우리나라에서 재배되고 있는 키위나무 잎에는 궤양병과 세균성점무늬병처럼 세균에 의한 병과 잿빛곰팡이병, 흰가루병, 과실곰보병을 비롯하여 여러 가지 형태 점무늬병 등 곰팡이에 의한 병이 발생하여 피해를 주고 있다. 이 총설에서는 키위나무잎에 발생하는 피해를 주는 주요 병에 대하여 병징과 발생특성을 요약하고 지난 30년간 수행한 연구 업적과 현장 경험 그리고 세계적인 주요 연구 산물들을 기초로 하여 키위나무 잎에 발생하는 주요 병에 의한 피해를 경감할 수 있도록 농가에서 실용적으로 사용할 수 있는 방제방법을 제시하고자 한다.
본 연구는 장티푸스 환자에 대한 혈청학적 시험법인 Vi-PHA, $SD^{(R)}$ kit, Widal 시험(O & H)에 대하여 평가를 위해 수행하였다. 2005년부터 2006년까지 수집된 36건의 혈청검체를 대상으로 하였으며, 9건은 배양검사를 통하여 장티푸스균이 검출된 확진환자의(Typhoid fever) 검체이며, 27건은 비장티푸스성 열성환자의(Non-typhoid fever) 검체이다. 시험결과 Vi-PHA는 환자 혈청 9건 중 양성 8건으로 민감도 88.9% (P<0.001; Fisher's exact test), 비장티푸스성 열성환자 27건 중 1건이 양성을 보여 특이도 96.3%로 나타났다. $SD^{(R)}$ kit는 민감도 100% (P<0.001), 특이도 92.6%로 나타났다. Widal (O & H) 시험의 민감도는 각각 88.9% (P=0.001), 100% (P<0.001), 특이도는 77.8%, 70.4%로 나타났다. 민감도는 $SD^{(R)}$ kit와 Widal H 검사법이 가장 높은 것으로 확인되었으며, 특이도는 Vi-PHA가 가장 높게 나타났다. 장티푸스의 효과적인 혈청학적 진단을 위하여 급성 또는 유행지역에서는 특이도가 높은 Vi-PHA로 검사를 수행하고, 장티푸스 비 유행지역과 고위험군에 대하여는 민감도가 높은 Widal H와 $SD^{(R)}$ kit를 적용하는 것이 진단의 유의수준을 높일 수 있을 것으로 판단된다.
