The spring back taking place after the coiling process of steam generator tube leads to the dimensional inaccuracy. In order to reduce the spring back, tension force was applied to the one end of the tube during forming. In this work, parametric study using FEM was performed to find the appropriate magnitude of tension force. The force that induces minimum spring back was found by simultaneously taking account if spring back amount, cross-sectional ovality, and thickness of the tube wall after deformation. In addition, stress relieving by heat treatment was also simulated as an alternative to the former method. The latter was found to be more effective under the given constraints.
Treatment and management of chronic low back pain (CLBP) should be tailored to the patient's individual context. However, there are limited resources available in which to find and manage the causes and mechanisms for each patient. In this study, we designed and developed a personalized context awareness system that uses machine learning techniques to understand the relationship between a patient's lower back pain and the surrounding environment. A pilot study was conducted to verify the context awareness model. The performance of the lower back pain prediction model was successful enough to be practically usable. It was possible to use the information from the model to understand how the variables influence the occurrence of lower back pain.
Chronic lower back pain is a leading cause of disability in musculoskeletal system. Degenerative disc disease is one of the main contributing factor of chronic back pain in the aging population in the world. It is postulated that sinuvertebral nerve and basivertebral nerve main mediator of the nociceptive response in degenerative disc disease as a result of neurotization of sinuvertebral and basivertebral nerve. A review in literature is done on the pathoanatomy, pathophysiology and pain generation pathway in degenerative disc disease and chronic back pain and management strategy is discussed in this review to aid understanding of sinuvertebral and basivertebral neuropathy treatment strategies.
Objectives : To report the clinical application and safety of Traditional Korean Medical treatment for patients with low back pain during pregnancy. Methods : Three patients who suffered from low back pain during pregnancy were treated with acupuncture, pharmacopuncture, moxibustion, and herbal medicine for 12 to 21 days. Numeric rating scale (NRS) of low back pain and Oswestry Disability Index (ODI) were checked to measure the outcome. A liver function and renal function test was carried out to identify any drug-induced liver or renal injury, and any adverse events were monitored thoroughly until delivery. Results : Low back pain reduced on NRS in all cases, and no serious adverse effects were found in the patients or infants. Conclusion : This study suggests that Traditional Korean Medical treatment is a candidate therapy for relieving low back pain during pregnancy, and also is safe for both patients and infants.
Objectives : This study was designed to evaluate the effect of Moxa Pellet therapy on chronic low back pain. Methods: Subjects were voluntarily recruited by newspapers and internet. The Moxa Pellet therapy was performed for 4 weeks. Patients were randomized to three groups: Moxa Pellets, Control-l and Control-2 group. Moxa Pellets group administered with Moxa Pellets, Control-l group administered with moxa pellet-shaped moxa pellet without available components and Control-2 group administered with round bonded fabric that could not give pressing stimulation. Acupuncture points used in this study were $GV_3$, $BL_{23}$, $BL_{24}$, $BL_{25}$ and $BL_{22}$. The patient's symptoms were assessed before and after 4 weeks treatments by VAS, SF-MPQ and SF- 36. Results : The results are follows; 1. VAS score showed significant difference in Moxa Pellet group and Control-l group after treatment. 2. SF-MPQ score showed significant difference only in Moxa Pellet group. 3. Moxa Pellet group showed significant difference in PF, RE, MR, and BP of SF- 36 score after treatment; Control-l group showed no significant difference; Control-2 group showed significant difference in BP. There were no significant difference among the groups. Conclusions : This study suggests that Moxa Pellet therapy can be applicable to improve symptoms in the patients with chronic low back pain. Further studies on the Moxa Pellet therapy and other treatment in the patients with chronic low back pain is recommended.
Purpose : This study was to find the effects of conservative therapy and sling therapy program of strength exercise and Gymnic Ball therapy program of stabilizer exercise and to find the effects of pain relieve through visual analogue scale(VAS) in chronic low back pain patients (male; 18, female;22). Methods : The measurement of pain was performed to both group by VAS from pre treatment to one to seven weeks. Results : 1. By the result of one-way ANOVA, Both group was the statistical significance in VAS during seven weeks(p<.05). 2. By the result of paired t-test, Both group was not statistical significance between the values of pre treatment and treatment after six and seven weeks but was statistical significance one week to seven weeks(p<.05). Conclusion : It was concluded that low back pain would be relieved by the progressive exercise and education for low back pain, and that the interventions would be helpful to prevent additional injuries by increasing the flexibility and muscular endurance.
Objectives : The purpose of this study is to know that Beck Depression Inventory(BDI) score to effect on improvement degree of low back pain after oriental medical treatment. Methods : We analyzed charts of 33 inpatients who were treated in Ja-Seng Hospital of Oriental Medicine and Bu-Cheon Ja-seng Hospital of Oriental Medicine. Subject is divided into Group A(BDI<10) and Group B(BDI${\geqq}$10). Numerical Rating Scale(NRS) and Oswestry Disability Index(ODI) were checked on admission, after 1 week, NRS and ODI were rechecked to estimate the efficacy of treatments. Results : 1. In both groups, NRS was significantly decreased(p<0.05). Analyzed result of the interaction between the two groups didn't show significant difference(p=0.197). 2. In Group A, ODI was significantly decresed(p<0.05). In Group B, ODI wasn't significantly decreased(p=0.058). The interaction analysis between the two groups didn't show significant difference(p=0.173). 3. Analyzing patients with chronic back pain longer than 6 months by the presence of depression tendency and NRS, ODI decrease after 1 week of ambulation, while the NRS reduction showed relevancy by the depression tendency (p<0.05), the ODI reduction didn't(p=0.272). Conclusions : 1. Although improvement of pain before and after the treatment and degree of functional recovery in high-level BDI group was less than low-level BDI group, but it was stastically insignificant. 2. High BDI score of patients with chronic low back pain is the factor of disturbing pain improvement.
Objectives The purpose of this study is to find a relation between hospitalization time and the overall outcome of treatment, and suggest an optimal hospitalization date. Methods We analyzed the medical records of patients who received admission treatment at Mokhuri Neck and Back Hospital in April, 2018. Results By analyzing the difference in visual analog scale (VAS) scores according to the hospitalization date, it was shown in this study that when hospitalization was held out by the fourth day of accident there was a significant difference in the improvement of symptoms than any other day. There were no statistically significant differences in initial VAS scores and admission periods. Conclusions It is the most effective for a traffic accident patient to receive hospitalization within the first four days of accident for the most effective reduction in overall pain.
Background: Epiduroscopic laser neural decompression (ELND) has been performed as a treatment tool for chronic refractory low back pain and/or radicular pain. There are some studies about the usefulness of epiduroscopy for post lumbar surgery syndrome, however, few studies about the effectiveness of epiduroscopy for patients without back surgery. We compared the satisfaction of patients who underwent ELND for chronic low back pain and/or radicular pain after back surgery and for the same symptoms without surgery. Methods: We compared the degree of satisfaction of patients after ELND between who had underwent the lumbar spine surgery and who had not retrospectively by chart reviewing. We divided 39 patients who had received ELND into two groups, one is the group of patients who got the lumbar surgery (group 1), and the other is the group of patients who did not (group 2). Their medical records including age, sex, previous treatment, duration of illness, degree of symptom relief were investigated. We compared each items between two groups. Results: The number of patients in group 1 was 17, and group 2 was 22. In group 1, 16 patients (94.1%) showed more than 'Acceptable', and 19 patients (86.4%) showed more than 'Acceptable' in group 2. There is no significant differences statistically in percentage of patients who showed more than 'Acceptable' in the satisfaction after ELND between two groups. Conclusions: ELND provided satisfaction (more than 85%) for patients with chronic low back pain and/or leg pain regardless of previous back surgery history.
Purpose : The aim of this study was to investigate the change of the respiratory function of chronic low back pain patients after the thoracic manipulation and the trunk stability exercise on chronic low back pain patients. Methods : For this study, the group of the experiment was consisted of 44 patients suffering from the low back pain chronically. The participants were divided into two groups: the control group, which was assigned for core stabilization exercise (CSE), and the experimental group, which was randomly assigned for core stabilization exercise after thoracic manipulation (CSE+TM). Both groups carried out each assigned treatment on three times a week for 8 weeks. To study the change related to measurement variable from each test groups before and after intervention, paired t-test was performed. Further, the statistics for an intergroup comparison was analyzed by covariance analysis, ANCOVA. The measurement was conducted by the respiratory function, the respiratory function was measured by forced vital capacity (FVC) and forced expiratory volume in one second (FEV1). Results : As a result, forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) showed significant differences not only by each groups but also between those groups before and after the test. Conclusion : In conclusion, both experiments, the core stabilization exercise and the core stabilization exercise conducted after thoracic manipulation, on chronic low back pain patients resulted in the significant level of difference in the respiratory function. This result indicates that the thoracic manipulation is an efficient treatment for improving the respiratory function for chronic low back pain patients.
