Background: Bertolotti's syndrome (BS), a form of lumbago in lumbosacral transitional vertebrae, is an important cause of low back pain in young patients. The purpose of this study was to assess the etiology of low back pain and the efficacy of treatment offered to patients with BS. Methods: All patients of BS Castellvi type1a during a period of 6 months were enrolled in the study. The patients underwent interventional pain procedures for diagnosis and pain relief. Response to the therapy was assessed based on VAS and ODI scores. A 50% decrease in VAS score or a VAS score less than 3 would be considered adequate pain relief. Results: All 20 patients diagnosed with BS during the 6-month observation period had scoliosis. Common causes of back pain were the ipsilateral L5-S1 facet joint, neoarticulation, the SI joint, and disc degeneration. Responses to various interventions for pain relief were different and inconsistent from patient to patient. In particular, responses to interventions for neoarticular pain were generally poor. Conclusions: Pain in patients with BS does not usually respond to interventional pain treatment. A very dynamic treatment approach must be pursued while managing BS patients, and the treatment plan must be individualized at various stages in order to obtain satisfactory pain relief.

Methods : This study was carried out on 40 patients with Low Back Pain and Sciatica who were hospitalized in Dongshin Univ. Oriental Hospital from January, 2003 to May, 2004. Group A of 20 patients were taken both common treatment and Traction treatment. And Group B of 20 patients were only taken common treatment. And at discharge day, we checked and compared VAS(Visual Analogue Scale) and ODI(Oswestry disability index) out of these two groups. SPSS(Statistical Program for Social Science) for Window was used for a statistical analysis and the independent T-test was performed to gauge the improvement of VAS(Visual Analogue Scale) and ODI(Oswestry disability index) out of these two groups, in which case, value of P below 0.05 is considered as useful. Results : The result of the VAS(Visual Analogue Scale) and ODI(Oswestry disability index) of comparison analysis between two groups at discharge day, shows that the improvement of Low Back Pain and Sciatica in cmmon treatment with Traction treatment Group is more effective than that in Common treatment.

Objective : The purpose of this report is to use more Shin Jong Gyuk of Ohaeng Acupuncture acupunture treatment clinically by comparing the Improvement of Back pain and Sciatica between Common Acupuncture treatment Group and Common Acupuncture with Shin Jong Gyuk of Ohaeng Acupuncture treatment Group. The roles of polypropylene (PP) beads and pH on membrane fouling and treatment efficiency were investigated in a hybrid advanced water treatment process of tubular carbon fiber membranes (ultrafiltration (UF) or microfiltration (MF)) and PP beads. The synthetic feed including humic acid and kaolin flowed inside the membrane, and the permeated contacted the PP beads fluidized in the space between the membrane and the module with UV irradiation and periodic water back-flushing. In the hybrid process of UF ($0.05{\mu}m$) and PP beads, final resistance of membrane fouling ($R_f$) after 180 min increased as PP beads increased. The turbidity treatment efficiency was the maximum at 30 g/L; however, that of dissolved organic matters (DOM) showed the highest at PP beads 50 g/L. The $R_f$ strengthened as pH of feed increased. It means that the membrane fouling could be inhibited at low alkali condition. The treatment efficiency of turbidity was almost constant independent of pH; however, that of DOM showed the maximum at pH 5. For MF ($0.1{\mu}m$), the final $R_f$ was the minimum at PP beads 40 g/L. The treatment efficiencies of turbidity and DOM were the maximum at PP beads 10 g/L.
Objective : The authors retrospectively analysed the factors affecting the favorable outcomes in the treatment of the Failed Back Surgery Syndrome(FBSS) or six years. Method : From March 1991 to December 1996, seventy-five patients were diagnosed as FBSS and its incidence was 3.4% of admitted patients with back pain in our department. We analysed the demographic data, etiologies, clinical menifestations, outcomes according to methods of operation, numbers of previous surgery, and time intervals between initial operation and final operation. Result : The most common etiology and previous surgical method of FBSS were insufficient surgery(54.7%) and partial laminectomy(53.3%). Patients were managed as followings ; Total laminectomy and spinal fusion with instrument in 41 cases(54.7%), total laminectomy without fusion in 23 cases(30.7%), and only partial laminectomy in 11 cases(14.7%). The overall treatment outcome was satisfactory in 48 cases(64%). Satisfactory results were observed in those patients who were treated by spinal fusion after complete laminectomy(34 cases), who had underwent only one operation previously(45 cases), and when the time interval between the initial operation and reoperation is shorter than 12 months(43 cases). Conclusion : The treatment outcome in FBSS was favorable in cases of the complete total laminectomy and spinal fusion with instrument, only one previous back surgery, and short time interval between initial and final operations.
In this study periodic water-back-flushing using permeate water was performed to minimize membrane fouling and to enhance permeate flux in advanced water treatment by tubular ceramic ultrafiltration membrane for Gongji stream in Chuncheon city. The back-flushing period (FT, filtration time) 2 min with periodic water-back-flushing of 15 sec showed the highest value of dimensionless permeate flux (permeate flux vs. initial permeate flux), and the lowest value of resistance of membrane fouling. Also in the results of BT effect at fixed FT 10 min, BT (back-flushing time) 20 sec showed the lowest value of resistance of membrane fouling and the highest value of dimensionless permeate flux, and we could be obtained the highest total permeate volume of 107.3 L. Consequently FT 10 min and BT 20 sec could be the optimal condition in Gongji stream water treatment, which was the exactly same results of our previous tubular alumina microfiltration. Then the average rejection rates of pollutants by our tubular ceramic UF system were 97.0% for turbidity, 32.1 % for chemical oxygen demand by manganese method, 28.8% for ammoniac nitrogen and 54.4% for T-P.

Anatomy: Facet joint syndrome most often affects the lower back and neck and refers to pain that occurs in the facet joints, which are the connections between the vertebrae in the spine that enable the spine to bend and twist. Many physicians have believed that the usual lesion of facet syndrome was an anatomical impairments of facet joint itself.. Facet joint injection using local anesthetics is a reliable method for the diagnosis and treatment for facet syndrome. Etiology: One of many possible causes is imbalances that can occur in stress levels, hormone levels, and nutritional levels. These imbalances can adversely affect posture, which can lead to neck and back pain. The common disorder called facet syndrome exhibits lower back pain, with or without, radiating pain to buttock and thigh due to facet joint arthropathy. Pain in the facet joint is supposedly the secondary effect of narrowing of joint space by sustained muscle contracture around joints. Syndrome: Facet joint syndrome tends to produce pain or tenderness in the lower back that increases with twisting or arching the body, as well as pain that moves to the buttocks or the back of the thighs. Other symptoms include stiffness or difficulty standing up straight or getting out of a chair. Pain can be felt in other areas such as the shoulders or mid-back area. Treatment: Non-drug treatments include hot packs, ultrasound, electrical stimulation, and therapeutic exercises. Stimulating blood flow using massage or a hot tub may also help. Alternative treatments include yoga and relaxation therapy. If your pain persists after trying these treatments, a surgical procedure called radiofrequency rhizotomy, which destroys the sensory nerves of the joint, may bring relief. Facet joint injection has been helpful in diagnosis and therapy for this facet syndrome. Radiofrequency thermocoagulation of medial branches is known to be an effective method of relieving pain caused by facet joint problems. We conclude that spasmolytic treatment of muscles connecting the two vertebral articular space would be better for treatment and diagnosis of facet syndrome rather than facet block with local anesthetic and steroid only.
This study was performed to investigate the effects on the general features of low back pain. The study subjects were low back pain patients treated with physical therapy at general hospitals located in Gyeonggi-do from August 2, 2000 to September 10. Total number of distributed questionnaires were 300, and 281 questionnaires were collected and analysed finally. The results were follows: 1. 76.3$\%$ of male answers that participate in sports, 60.6$\%$ of female answers that participate in sports and there are significant differences of two group(p < 0.01). 2. In case of male, preferential sports were running, tennis, golf, the others, but in case of female, preferential sports were walking, ascent of a mountain, swimming(p < 0.01). 3. In case of male, the most frequency preferred to participate in sports by patients was 30minutes daily, but in case of female, 3 times a week(p < 0.05). 4. The onset time of low back pain was 28.3$\%$ in the morning, 23.7$\%$ in the afternoon, 23.2$\%$ in motion according. 5. The onset conditions of low back pain was 48.5% during exercise, 44.4$\%$ after exercise. 6. Regardless of gender, low back pain patients want to go to a hospital for treatment. Otherwise, male preferred conventional treatment than a hospital, but female preferred a hospital than conventional treatment(p < 0.05). 7. Regardless of gender, 97.5$\%$ of patients is answered that the most preferential treatment method was physical therapy, 8. 92.2$\%$ of patients is answered that physical therapy was effective.

Background: The purpose of this study was to assess the effectiveness of transcutaneous electrical nerve stimulation (TENS) in chronic low back pain and neuropathic pain. Methods: Seventy-four patients aged 18-65 with chronic low back pain were included in the study. Baseline measurements were performed, and patients were randomized into three groups. The first group received burst TENS (bTENS), the second group conventional TENS (cTENS), and the third group placebo TENS (pTENS), all over 15 sessions. Patients' visual analogue scale (VAS) scores were evaluated before treatment (preT), immediately after treatment (postT), and in the third month after treatment (postT3). Douleur Neuropathique 4 Questions (DN4), the Modified Oswestry Low Back Pain Disability Questionnaire (MOS), the Beck Depression Inventory (BDI), and sympathetic skin response (SSR) values were also evaluated preT and postT3. Results: A statistically significant improvement was observed in mean VAS scores postT compared to preT in all three groups. Intergroup comparison revealed a significant difference between preT and postT values, that difference being assessed in favor of bTENS at multiple comparison analysis. Although significant improvement was determined in neuropathic pain DN4 scores measured at postT3 compared to preT in all groups, there was no significant difference between the groups. No statistically significant difference was also observed between the groups in terms of MOS, BDI, or SSR values at postT3 (P > 0.05). Conclusions: bTENS therapy in patients with low back pain is an effective and safe method that can be employed in short-term pain control.

Purpose: When pregnant women get symptom like pain, because of fear of using analgesic drugs and X-ray test during pregnancy, pain is nevertheless often left. This study was to report the clinical application and safety of oriental treatment to low back pain and pelvic pain caused by traffic accident during pregnancy. Methods: We experienced seven patients who suffered from low back pain and pelvic pain caused by traffic accident during pregnancy. During their admission, we treated with acupuncture, cupping therapy and herbal medicine. We checked the symptoms during admission at progress note, and follow up by phone call. Results: The degree of low back pain and pelvic pain were decreased or resolved. After treatment, no serious adverse effects were found on the patients, and there were no adverse effects at all on the infants. Conclusion: The results indicate that oriental treatment can be effective for the management of low back pain and pelvic pain caused by traffic accident during pregnancy. And these treatments are safe to both patients and infants.
