목적 : 감각 및 운동기능에 대한 기능적 자기공명영상에서 데이터 분석 시 global scaling이 뇌 활성화 영상에 미치는 영향을 알아보고자 하였다. 대상 및 방법 : 신경학적 병력이 없는 정상 성인 피검자 4명을 대상으로 하였다. 운동기능은 오른쪽 상지를 구부렸다가 폈다가를 반복하는 운동을 시행하였고 청각기능은 1 KHz 순음자극을 시행하였다. 기능적 자기공명영상은 3.0T 자기공명영상기기(GE, Milwaukee, USA)에서 BOLD-EPI 기법을 사용하였고 데이터 분석은 SPM2를 사용하였다. 데이터 분석 시 움직임 보정, 통계적 유의 수준 등은 동일하게 한 상태에서 global scaling의 시행 전후의 뇌 활성화 영상을 획득하였다. 결과 : 오른쪽 상지운동에 대한 기능영상에서 global scaling 효과를 고려하지 않은 경우와 고려한 경우의 뇌 활성화 영상의 차이는 크지 않았다 (p<0.000001). 청각기능 검사에서는 global scaling 효과를 고려한 경우에서 고려하지 않은 경우에 비해 뇌 활성화 영상이 훨씬 크게 나타났다 (p<0.05). 결론 : 국소적 BOLD 신호의 변화가 작은 기능영상 검사에서는 데이터 분석 시 global scaling이 뇌 활성화 결과에 큰 영향을 미칠 수 있으므로 주의가 요구된다.
Presuming that firing neurons have motions inside the MRI magnet due to the interaction between the neuronal magnetic field and the main magnetic field, we applied motion encoding gradients to dissected snail ganglia to observe faster responding MRI signal than the BOLD signal. To activate the snail ganglia in synchronization with the MRI pulse sequence, we used electrical stimulation with the frequency of 30 Hz and the pulse width of 2s. To observe the fast responding signal, we used the volume selected MRI sequence. The magnetic resonance signal intensity, measured with 8 ms long motion encoding gradient with a 20mT/m gradient strength, decreased about $3.46{\pm}1.48%$ when the ganglia were activated by the electrical stimulation.

Objective : The purpose of this study is to determine whether the changes of contralateral sensorimotor cortical activation on functional magnetic resonance imaging (fMRI) can predict the neurological outcome among spinal cord injury (SCI) patients when the great toes are stimulated without notice. Methods : This study enrolled a total of 49 patients with SCI and investigated each patient's preoperative fMRI, postoperative fMRI, American Spinal Injury Association (ASIA) score, and neuropathic pain occurrence. Patients were classified into 3 groups according to the change of blood oxygenation level dependent (BOLD) response on perioperative fMRI during proprioceptive stimulation with repetitive passive toe movements : 1) patients with a response of contralateral sensorimotor cortical activation in fMRI were categorized; 2) patients with a response in other regions; and 3) patients with no response. Correlation between the result of fMRI and each parameter was analyzed. Results : In fMRI data, ASIA score was likely to show greater improvement in patients in group A compared to those belonging to group B or C (p<0.001). No statistical significance was observed between the result of fMRI and neuropathic pain (p=0.709). However, increase in neuropathic pain in response to the signal change of the ipsilateral frontal lobe on fMRI was statistically significant (p=0.030). Conclusion : When there was change of BOLD response at the contralateral sensorimotor cortex on perioperative fMRI after surgery, relief of neurological symptoms was highly likely for traumatic SCI patients. In addition, development of neuropathic pain was likely to occur when there was change of BOLD response at ipsilateral frontal lobe.
This study compared cerebral lateralization index based on the area of neural activation with that based on the intensity of neural activation. For this purpose, 8 right-handed male college students (the mean age - 23.5 years) and 10 right-handed male college students (the mean age - 25.1 years) participated respectively in researches on visuospatial and verbal task brain function. Functional brain images were taken from 3T MRI using the single-shot EPI method. The result of measuring cerebral lateralization index based on the area of neural activation suggested that the right hemisphere is dominant in visuospatial tasks and the left one is in verbal tasks. However, the dominance is not sufficient to locate the exact part of the brain for these tasks. When cerebral lateralization index was computed based on the intensity of neural activation, it was derived that the area of cerebral lateralization closely related to visuospatial tasks is the superior parietal lobe, and the area of cerebral lateralization closely related to verbal tasks is the inferior and middle frontal lobes. Thus, cerebral lateralization index by area based on the intensity of neural activation as proposed by this study can determine the dominance of the cerebrum by area, so is helpful for accurate and quantitative determination of cerebral lateralization.
Purpose: To develop a new diffusion-based functional MRI (fMRI) sequence to generate apparent diffusion coefficient (ADC) maps in single excitation and evaluate the contribution of b0 signal on neuronal changes. Materials and Methods: A diffusion-based fMRI sequence was designed with single measurement that can acquire images of three directions at a time, obtaining $b=0s/mm^2$ during the first baseline condition (b0_b), followed by 107 diffusion-weighted imaging (DWI) with $b=600s/mm^2$ during the baseline and visual stimulation conditions, and another $b=0s/mm^2$ during the last activation condition (b0_a). ADC was mapped in three different ways: 1) using b0_b (ADC_b) for all time points, 2) using b0_a (ADC_a) for all time points, and 3) using b0_b and b0_a (ADC_ba) for baseline and stimulation scans, respectively. The fMRI studies were conducted on the brains of 16 young healthy volunteers using visual stimulations in a 3T MRI system. In addition, the blood oxygen level dependent (BOLD) fMRI was also acquired to compare it with diffusion-based fMRI. A sample t-test was used to investigate the voxel-wise average between the subjects. Results: The BOLD data consisted of only activated voxels. However, ADC_ba data was observed in both deactivated and activated voxels. There were no statistically significant activated or deactivated voxels for DWI, ADC_b, and ADC_a. Conclusions: With the new sequence, neuronal activations can be mapped with visual stimulation as compared to the baseline condition in several areas in the brain. We showed that ADC should be mapped using both DWI and b0 images acquired with the same conditions.
목적 : 시각자극을 이용하여 대뇌 후두엽에서 BOLD(blood oxygen level dependent)효과에 의한 기능적 자기공명영상과 양성자 자기공명분광법을 이용한 대사물질영상( metabolic i image)을 얻어 그 결과를 평가하고자 하였다. 대상 및 방법 : 24세에서 30세 사이의 8명의 남자와 2명의 여자를 포함하는 10명의 정상 성인 남녀를 대상으로 1.5T MRI에서 시각 지극을 이용하여 echo planar imaging펠스시퀀스를 사용하여 BOLD효과에 의한 영상들을 대뇌 후두엽에서 얻었다. 시각자극은 8Hz의 red flicker를 이용한 광 자극을 주였다. MR에서 얻어진 영상들을 workstation으로 전송한 뒤 자체 개발한 분석프로그램을 사용하여 상관계수방법을 이용하여 기능적 영상을 구하였다. 그리고 얻어진 영상의 활성화 부위를 선정한 뒤 양성자 자기공명 분광법을 사용하여 시각 자극에 의한 뇌 활성화시 대사물질의 변화를 분석했다. 그 후 자가공명분광영상법(magnetic resonance spectro-scopic imaging)를 실행하여 젖산(lactate)의 대사물질 영상을 얻었다. 결과 : 시각 자극에 따른 BOLD효과에 의한 뚜렷한 활성화 부위가 후두염 대뇌피질에 나타났다. 또한 이들 활성화 부위에서 자기공명분광법을 시행한 결과 휴식상태와 시각자극상태에서 NAA(N-acetyle aspartate)/Cr(creatine)이 $1.79{\pm}0.28$에서 $1.88{\pm}O.20$으로 거의 변화가 없는 것에 반해 lactate는 자극 전후 $9.48{\pm}4.38$배로 증가하였으며 이를 바탕으로 lactate에 대한 대사물질영상을 얻을 수 있었다. 결론 : 시각자극에 의한 뇌 활성화시 BOLD효과를 이용한 자기공명영상볍으로 활성화 부위를 확인할 수 있었고, 같은 부위에서 자기공명분광법을 이용하여 lactate의 증가를 확인하였다. 또한 lactate의 증가를 대사물질영상으로 표현함으로써 두 결과에 대한 상관관계 및 뇌 기능적영상에 대한 신뢰성을 높일 수 있게 되었다.
시각 장면은 중복적인 정보가 많이 포함되어 있다. 우리의 시각체계는 다양하고 중복적인 정보를 처리하기 위해 뇌 용적을 늘이기보다는 들어오는 외부 정보를 요약한다. 유사한 형태의 다양한 정보가 시각체계에 주어지면 시각체계는 정보의 통계적 특성을 추출해 낸다. 이런 통계적 표상의 대표적 형태가 바로 평균 표상이다. 평균 표상의 한 예로 시각 체계에서 계산해 내는 유사한 여러 크기들의 평균 크기를 들 수 있다. 평균 표상은 빠르고 정확하며 비교적 오랜 시간 지속되는 표상이고 평균 표상의 처리과정 또한 병렬적인 처리과정이다. 하지만 지금까지의 통계 표상에 관한 연구는 행동측정방법에 의한 연구였다. 따라서 본 연구는 기능적 자기 공명 영상 기법을 사용하여 통계 표상에 관한 신경기제를 찾고자 하였다. 사전 연구 결과들에 따르면 특정 자극을 연속하여 제시하였을 때 특정 자극을 담당하는 영역에서 자기 공명 영상 신호가 감소함을 알 수 있다. 본 연구에서는 이 반복 감소 현상을 사용하여 원들의 평균이 동일한 자극을 제시하였을 때 우측 후두 영역에서 유의미하게 자기 공명 영상 신호가 감소하는 것을 발견하였다. 이것은 우측 후두 영역이 시각자극에 대한 평균 표상을 처리하는 영역일 수 있음을 시사한다.
목적: 정상인에서 항혈소판제제인 클로피도그렐이 손운동기능 과제를 수행하는 동안 운동기능의 생리학적 반응에 대한 약리적 조절효과를 알아보고자 하였다. 대상과 방법: 10명의 오른손잡이 정상인을 대상으로 클로피도그렐 사용전, 최대 복용량 복용후, 정상상태 유지시로 세번에 걸쳐 뇌기능 자기공명영상 데이터를 획득하였다. 운동과제로는 주먹을 쥐었다 폈다하는 운동을 시행하였고 3.0 테슬라 자기공명영상기기에서 혈액산소의존성(BOLD) 대조도를 획득하였으며 이를 위하여 $T2^*$ 강조 EPI 영상기법을 사용하였다. 뇌기능 자기공명영상 데이터의 영상전처리 및 통계분석은 SPM2를 사용하였다. 결과: 이차수준 분석에서 주운동영역을 포함하는 편측 감각운동중추의 활성화가 나타났다. 클로피도그렐 사용전의 활성화 화소수는 173, 최대 복용량 복용후 활성화 화소수는 1049, 정상상태 유지시 활성화 화소수는 673 이었다. 최대 T값을 기준으로 측정한 BOLD 신호의 강도변화는 관찰되지 않았다. 결론: 본 연구결과는 클로피도그렐에 의해 대뇌 운동 활성이 조절된다는 사실과 또한 뇌기능 자기공명영상이 이러한 변화를 감지할수 있을만큼 높은 민감성을 가진다는 사실을 제안하고 있다.

Methods: The acupoint, GB34, located in the back of the knee, is known to be effective in recovering motor function after stroke. Using 3T MRI scanner, functional MR imaging of the whole brain was performed in 12 normal healthy subjects during two stimulation paradigms; acupuncture manipulation on GB 34 and sham points. This study investigates the activation of the motor cortex elicited by a soft and an intensified stimulation of GB 34. Three different paradigms were carried out to detect any possible modulation of the Blood Oxygenation Level Dependent (BOLD) response in the somatomortor area to motor stimulation through acupuncture. Results: Group analysis from seven individuals showed that bilateral sensorimotor areas (BA 3,4,6 and 7) showed stimulation related BOLD signal contrast of approximately 6% whereas very few areas were activated when sham stimulation is given. Conclusions: The present study shows that acupuncture fMRI study can be safely conducted in 3T MRI environment, and acupuncture stimulation in GB34 modulates the cortical activities of the soma- to motor area in human. The present findings may shed light on the CNS mechanism of motor function by acupuncture and form a basis for future investigations of motor modulation circuits in the stroke patients.
