Psychiatric Symptoms after Taking Oseltamivir in a Child and Its Causality Assessment (Oseltamivir 복용 이후 소아에서 발생한 이상행동 및 그 인과성 평가)

  • Oseltamivir is an antiviral medication prescribed to prevent and treat influenza A and B. A case from a community pharmacy in Korea was reported for an adverse event associated with oseltamivir administration. A 20-month-old boy had psychiatric symptoms after receiving 2 doses of oseltamivir. Therefore, an evaluation of whether the psychiatric symptoms were caused by oseltamivir was required. To determine whether the adverse event resulted from the administrated medication or other factors, three tools were used: the Naranjo scale, the Korean causality assessment algorithm (Ver.2), and the World Health Organization-Uppsala Monitoring Center (WHO-UMC) criteria. The psychiatric symptoms occurred after oseltamivir administration, and were attenuated after oseltamivir termination. A possible cause of the psychiatric symptoms is high fever, but information on the body temperature of the patient was not sufficient. Therefore, it was unclear whether there were other nonpharmacological causes of adverse drug reaction. For these reasons, in terms of causality, the results evaluated by the three tools represented, "possible", "probable", and "probable/likely", respectively.

Analysis of Breaking Research Trends in Korea (국내 브레이킹 연구동향 분석)

  • The purpose of this study is to identify trends in domestic breaking research to derive characteristics and implications, and further suggest future research directions. To this end, literature analysis (the timing of paper publication, research method, research topic) and keyword analysis of 50 papers related to breaking published in academic journals registered with the Korea Research Foundation (KCI) were conducted. The research results are as follows. First, the trend by thesis publication period was first published in 2006, showed a slight increase in 2012, and then increased rapidly in 2021. Second, domestic braking-related research has been mainly focused on qualitative research (60%). Third, looking at the research topic, it is divided into three categories: identity establishment, culture and arts field, and sports field, of which studies related to identity establishment accounted for more than 60%. Finally, looking at the keywords frequently used in breaking papers, the most frequently appeared word was 'hip-hop', followed by 'culture'. Based on these results, implications were drawn to establishing the identity of braking through academic and theoretical approaches, practical approaches through the development of standardized textbooks and curriculum, strengthening the characteristics and capabilities of the field through integrated approaches, and changing to sports.

Early Diagnosis of Burkitt Lymphoma on the Mandible: A Case Report (하악골에서 발생한 Burkitt 림프종의 조기발견과 진단)

  • Burkitt lymphoma (BL) is an aggressive form of non-Hodgkin's B-cell lymphoma found primarily in the pediatric population. In the oral cavity, this tumor can grow rapidly and often brings about facial swelling or development of an exophytic mass involving the jaws. A 5-year-old boy was referred for swelling and pain in the left mandibular area. The patient showed diffuse swelling on the left side of the mandible and firm-moderate tenderness upon palpation. An intraoral examination showed moderate mobility and sensitivity to percussion on the left primary first and second molars, without severe caries. A radiographic examination revealed complete loss of the lamina dura on the left primary second molar and permanent first molar. There was a radiolucent osteolytic lesion and destruction of the cortical bone of the left mandibular body. Based on the clinical, radiographic, and immunohistochemical findings, the patient was diagnosed with BL, and was referred to a pediatrician for systemic evaluation and intensive chemotherapy. Even before the completion of chemotherapy, the swelling resolved and the displaced teeth were relocated to a normal position. This patient showed a good prognosis due to prompt diagnosis and intensive chemotherapy. Early diagnosis and referral for treatment can prevent the development of BL.

The Effects of Oral Activity With Sensory Integration Intervention on Eating of a Child With Oral Defensiveness: A Single-Subject Research Design (구강 활동을 병행한 감각통합중재가 구강 방어 아동의 먹기에 미치는 효과: 단일 실험 설계 연구)

  • Objective : The purpose of this study was to investigate the effects of sensory integration intervention combined with oral activity on eating of child with sensitive oral senses. Methods : The study subject was 6 years old boy and diagnosed with developmental disorder. The experiment period was 15 sessions from April 21, 2017 to June 2, 2017. Experimental design used AB design in single-subject design, baseline (A) phase (sensory integration therapy) was 5 sessions and intervention phase (B) (sensory integration therapy combined oral activity) was 10 sessions. During the experiment, the child used the Goal Attainment Scale (GAS) to measure behavioral changes in eating. Results : The GAS score of voluntary eating of favorite foods was 0 on the baseline phase and increased to 1.9 on the intervention phase. The GAS score of voluntary eating of disliking foods was 0 on average during the baseline phase and increased to 1.7 on average during the intervention phase. Conclusion : This study suggests that sensory integration intervention combined with oral activity program may have a positive effect on eating of children with sensitive oral senses and limited food intake.

The Effects of Sensory Integrative Intervention on the Self-regulation Capacities of Children With Cerebral Palsy: Single-Subject Research Design (감각통합치료가 뇌성마비 아동의 자기조절 행동에 미치는 효과)

  • Objective : The purpose of the study was to investigate effects of sensory integration therapy on self-regulation capacities of children with cerebral palsy. Methods : Subjects of this study were four years old girl and six years old boy, living in P city. Both were diagnosed as diplegia. Research design used in this study is ABA' design for a single-subject research. The experimental period was 12 weeks with 22 sessions of treatment. It was divided into three phase: 6 sessions for baseline phase (A), 14 sessions for treatment phase (B), 2 sessions for baseline phase (A'). There was no treatment on the baseline phases, and sensory integration therapy focused vestibular and proprioceptive stimuli was conducted on the treatment phase. At each session, subjects' behaviors has been recorded with a Digital Camera for 10 minutes and then the occurrence frequency of problematic behavior was identified based on the operational definition of it. Results : In comparison with the baseline phase (A), the occurrence frequency was reduced during the treatment phase for three types of problematic behavior associated with the self-regulatory capacities. The tendency of reducement was maintained during baseline phase (A'). Conclusion : Based on the result, it was implied that sensory integration therapy has a positive effect on the self-regulation capacities in children with cerebral palsy.

A novel BTK gene mutation, c.82delC (p.Arg28 Alafs*5), in a Korean family with X-linked agammaglobulinemia

  • X-linked agammaglobulinemia (XLA) is a hereditary humoral immunodeficiency that results from Bruton's tyrosine kinase (BTK ) gene mutations. These mutations cause defects in B-cell development, resulting in the virtual absence of these lymphocytes from the peripheral circulation. Consequently, this absence leads to a profound deficiency of lg all isotypes, and an increased susceptibility to encapsulated bacterial infections. A 15-month-old Korean boy presented with recurrent sinusitis and otitis media after 6 months of age, and had a family history of 2 maternal uncles with XLA. Laboratory tests revealed a profound deficiency of Ig isotypes, and a decreased count of $CD19^+$ B cells in the peripheral circulation. Based on his family history and our laboratory test results, he was diagnosed with XLA. We performed BTK gene analysis of peripheral blood samples obtained from family members to confirm the diagnosis. Mutational analysis revealed a novel hemizygous frameshift mutation (c.82delC, p.Arg28Alafs*5), in the BTK gene. His mother and maternal grandmother were heterozygous carriers of this mutation and his two maternal uncles were hemizygous at the same position. After XLA diagnosis, intravenous immunoglobulin (400 mg/kg, monthly) treatment was initiated; recurrent sinusitis and otitis media were subsequently brought under control. To our knowledge, this is the first reported case of a Korean pedigree with a novel mutation in the BTK gene.

The effect that infant temperament has on the infant's sense of self-respect and peer competence (유아기질이 유아자아존중감과 또래유능성에 미치는 영향)

  • This study has an objective of suggesting the ways to improve infant's sense of self-respect and peer competence, by examining what effect infant temperament has on infant's sense of self-respect and peer competence. The notable study results through analysis are as follows; First, it was shown that, in regard with adaptability and activity of infant temperament, cognitive ability of the sense of self-respect and peer acceptance, the average score of day care centers was higher, while it was shown that, in regard with response of infant temperament, the sense of self-respect's mother acceptance, peer relationship, sociability of peer competence, the average score of kindergartens was higher, and the boy infants' physiological regularity was shown to be higher than that of girl infants. Second, as a result of looking into the difference of infant temperament, sense of self-respect, and peer competence according to infant's age, we found that, in case of 'response' of infant temperament, age 5 group was classified into 'group a', and age 3 and 4 group was classified into 'group b', and two groups showed the difference, and in case of 'physiological regularity', age 3 group had the biggest regularity while age 5 group had the relatively low regularity, thus two groups showed the difference. In regard with sense of self-respect and peer competence, it was shown that age 4 and 5 group had higher one than that of age 3 group.

A case of mosaic ring chromosome 13 syndrome (13번 환염색체의 모자이크 증후군)

  • The clinical features of ring chromosome 13 include mental and growth retardation, CNS anomalies, facial dysmorphism, cardiac defects, genital malformations, limb anomalies, skeletal deformities and anal malformations. Although many cases of ring chromosome 13 have been reported worldwide, only 6 cases have been reported in Korea, and the latter cases were not mosaic but pure ring chromosome 13. Here we report a case with mosaic ring chromosome 13. The baby boy was born at 37 weeks of gestation by induced vaginal delivery due to intrauterine growth restriction (IUGR). He was the second baby of a 28-year-old hepatitis B carrier mother and a 32-year-old father. There was no family history of chromosomal anomalies. The baby was a symmetric IUGR with a birth weight of 1,860 g, length of 44.8 cm, and head circumference of 29.4 cm. The physical examination revealed microcephaly, trigonocephaly, flat occiput, large ears, short neck and dysmorphic facial features, including microophthalmia, hypertelorism, antimongoloid slanting palpebral fissures, a flat nasal bridge, and micrognathia. The karyotype of this patient performed by peripheral blood lymphocytes was 46,XY,r(13)(p13q34)/45,XY,-13/46,XY,dic r(13;13)(p13q34;p13q34). The baby showed failure to thrive, hypotonia, and developmental delay. We report the first case of mosaic ring chromosome 13 in a male baby in Korea and compare this case with other Korean cases of non-mosaic ring chromosome 13.

The Effects of Sensory Integrative Therapy on Vestibulo-Proprioceptive Sensory Processing of Children With Asperger Syndrome (감각통합치료가 아스퍼거 아동의 전정.고유감각 처리능력에 미치는 효과)

  • Objective : This study verifies the effects of sensory integrative (SI) therapy on vestibular- and proprioceptive sensory (BPS) processing ability of a child with Asperger Syndrome (AS). Method : A boy who is 11 years and 2 months old took the Functional Independence Measure for Children (Wee-FIM), Canadian Occupational Performance Measure (COPM), Short Sensory Profile, Bruininks-Oserestky Test of Motor Proficiency-2 (BOT-2), and Test of Playfulness (ToP) for the baseline. The child participated in 3 evaluation sessions and 8 therapy sessions based on the AB research design. Duration of each session is 50 min and the therapy session is divided into 40 minutes for treatment and 10 minutes for evaluation. Since the vestibular sense and proprioception build up one's ability of postural control, several tests were employed to evaluate the child' postural control as outcome measure; distance from front leg of chair to heel of the child with sitting (C-H distance), angle between trunk and thigh (hip joint angle) with sitting, and the 'prone-extension posture' which is a subtest of Clinical Observation of Motor and Postural Skill (COMPS) to examine postural control embodied with integration of reflex and BPS processing. Result : During the therapy, average data of the C-H distance is decreased from 27.33cm to 11.69cm, average data of the hip joint angle is also decreased from $43.3^{\circ}$ to $20^{\circ}$, and average time for the prone-extension posture is increased from 13.15seconds to 24.84seconds. Conclusion : This result indicates that the ability to postural control in sitting and to maintain the prone-extension posture can be improved by sensory integrative therapy, with enhanced BPS processing.

Upper Garment Sizing System for Obese School Boys Based on Somatotype Analysis (학령후기 비만 남아의 체형 분석에 따른 plus-size 남자 아동복 상의 치수 규격 제안)

  • The increasing rate of obesity in school aged children has become a conspicuous social phenomenon in Korea. This has been linked to greater economic growth, increasingly westernized dietary habits, and a consumer driven society. Given that obesity can lead to social exclusion or unfavorable attention by other students in a school setting, the design of plus-size garments have become important for effective appearance management skills. This research aimed to establish a somatotype database for obese school boys, aged 10 to 12, in order to develop a sizing system for plus-size upper garments. In order to measure somatotype of average and obese school boys, five categories were recorded; height, obesity, length of trunk, thickness of neck and chest. For obese boys, subcutaneous fat thickness and position of B.P/shoulder point factors were recorded. Obesity factor was subdivided into overall and specific ones, and while the deviation of obese body types was severe compared to the average type. Obese body type showed significantly higher measurements in width, girth, thickness. This is linked to the fact that the frequency ratio of obesity increases with age. Stature and chest were chosen as control dimensions for boys' wear. As crosstabulation of stature(5cm interval) and chest girth(2, 3 and 4cm), and stature(5cm interval)/chest girth(3cm interval) sizing system showed, the most effective cover ratio and adaptability to the data distribution $25{\sim}75$ quartile. Based on the findings, 10 sizes were formulated for average body type, while 18 sizes were formulated for obese type, whose size cover ratios were 48% and 62.9%, respectively. The primary ranges of stature were $145cm{\sim}150cm$, while those of chest girth were $79{\sim}82cm$. Each size was declared as "chest-somatotype{A(average)/O(obesity)-stature". This study proposed a plus-size upper garment sizing systems for obese boys, accompanied with reference measurements for suit, casual wear and underwear. The finding showed that the two systems were totally separate and not overlapping, meaning that plus-size sizing system is essential for obese school boys. The obesity type system had more size and wider range specs.
