An Research Into The Reactive Safety Action Program for Promoting Aviation Safety Culture

  • Objective: The objective of this research is to inquire about safety information from the standpoint of its usefulness to suggest the significance of the Reactive Safety Action Program, which serves to promote aviation safety culture. Background: Safety information plays an important role in operating safety programs. Each organization learns lessons from safety information collected from aviation accidents and incidents. When an accident occurs, it is only through safety investigation and a close inquiry on the cause that we can come up with an appropriate countermeasure which would contribute to preventing the recurrence of the same or similar accident. However, the usefulness of safety information produced from unsatisfactory safety investigation is insufficient. Method: This research analyzed the characteristics of aviation accidents, the differences between safety investigations and legal accident investigations in systematic and operative perspectives, and safety culture as a measure to activate reporting systems (compulsory/voluntary). Results: This research defined the investigation scope and processes of safety investigations and legal accident investigations. It also suggested factors such as just culture based on trust, non-punitiveness, confidentiality, the participation of the entire staff through the use of inclusive reporting base, ensuring the independence of the operating organization as a way to promote safety through reporting systems. Conclusion: The organization's effort is the important aspect in obtaining exact and accurate safety information from accidents/incidents. The separate running of SIB (Safety Investigation Board) and AIB (Accident Investigation Board), the systematization of safety information reporting system, and prescribing (legislating) the composition of related organizations are some representative programs. Application: This research inquired experiences that contributed in promoting aviation safety culture in a reactive perspective, and will serve a role in spreading safety culture by enabling the use of application experiences of the aviation field in other domains.

  • ICAO Universal Safety Oversight Audit Programme is a comprehensive evaluation for aviation safety matters, including aircraft operation, licensing and aircraft incident/accident investigation. As this programme is measures aviation safety level of States, there is a need to meet international standards, especially in the field of aircraft incident/accident investigation. This research discusses points required to satisfy international standards, which are independence and autonomy of an accident investigation board, the lack of the enough number of investigators and efficient management of those personnel, the effective composition and use of an investigation report, an accident investigation itself to build big data and the creation of a cooperation system among States. This paper suggests various opinions in order to enhance aviation safety especially through aircraft accident investigations, using case studies from the US, UK and Japan.

시스템안전 관점에서의 사고 모형 고찰 - 항공기 사고를 중심으로 - (A Study on the Accident Model from the System Safety Perspective - Focused on Aircraft Accident -)

  • Many organizations apply reactive safety management to prevent the same or similar types of accidents by through investigation and analysis of the accident cases. Although research on investigation techniques has contributed a lot to the objective results of safety accidents and the preparation of countermeasures, many accident investigation techniques currently in use treat accidents from a linear perspective, revealing limitations in reflecting current systems dominated by complexity and uncertainty. In order to overcome these limitations, this study will review recent studies and concepts from a system safety perspective and predict future research trends through a case analysis of aviation accident. The models used in the analysis are STAMP, HFACS, and FRAM, and the characteristics of each technique are presented so that analysts who perform related tasks in the field can refer to them.

  • Aviation safety is increasingly important to secure the safety of the Republic of Korea Air Force (ROKAF). A critical activity for enhancing aviation safety is to analyze an accident throughly and to identify causes that can explain it reasonably. The results of such a systematic accident investigation can be effectively used for improving information displays, task procedures, and training systems as well as for reorganizing team structure and communication control system. However, the current practice of analyzing aviation accidents in ROKAF is too superficial and simple to diagnose them systematically. Additionally, the current practice does not give a full consideration to human factors that have been identified as main causes of most of the aviation accidents. With this issue in mind, this study aims to suggest a new approach to analyzing aviation accidents related to human factors.The proposed method is developed on the basis of several models and frameworks about system safety, human error, and human-system interaction. Its application to forty-two human factors-related accidents, which have occurred in ROKAF during the last ten years, showed that the proposed method could be a useful tool for analyzing aviation accidents caused by human factors.

  • Current limitation of Aviation airworthiness manage system and text based Aviation safety report data, lack of Data Manage system for aviation parts failure and lack of continuing airworthiness related to task linking system for Inspection/Report/Improvement(AD ; Airworthiness Directive) have been apprehended to suggest direction of realizing improved operating system by applying aviation airworthiness manage system by using standardization and safety performance index based managing and safety performance index based data analyzing.

  • Structured Checklist of System Aviation Safety Oversight had been categorized by HFACS. In this Study, Findings of Aviation Safety oversight, have been categorized by HFACS. Thus, those two categorized results have difference in incongruity clause that can be concluded that Structured Checklist has lack of elements. Therefore, we are able to identify validity and suitability of checklist that need to be restructured and redeveloped by performing test analysis through this study.

국내 항공사고조사를 위한 항공사고 통합 데이터 관리시스템의 프로토 타입 개발 (Development of Integrated Data Management Prototype System for Aviation Accident and Incident Investigation)

  • 본 논문에서 국내 항공사고조사를 위한 통합 데이터 관리 시스템 프로토타입 개발을 제안한다. 최근 항공사고 조사 장비의 발전에 따라 사고조사 시스템은 다양한 형태의 jpg, avi, 및 wav 파일 자료들을 수집 및 관리해야한다. 하지만 ECCAIRS 시스템의 경우 항공사고 조사 시 발생하는 다양한 자료를 관리하기 위한 별도의 데이터베이스를 구축하고 있지 못한 상황이며, 국내 항공사고 관리시스템 역시 동일한 문제점을 가지고 있다. 따라서 본 논문에서는 해외 주요 국가의 항공사고보고서 시스템을 분석하고 국내 환경에 적용할 수 있는 방안을 마련한다. 통합 데이터 관리시스템 프로토타입을 통하여 기존 자료와 최근 조사한 자료를 입력을 통하여 성능을 확인하였다. 이 결과를 이용하여 최종 통합 데이터 관리시스템의 완성을 위한 기초 자료로 활용할 예정이다.

항공영어구술능력증명시험 개선방안 연구 (Research for Improvement of EPTA(English Proficiency Test for Aviation))

  • ICAO(International Civil Aviation Organization) established ICAO LPRs(Language Proficiency Requirements) which is an international standard of English for aviation to promote the aviation safety. The Republic of Korea government amended the aviation law on the 8th of November, 2005 and made EPTA(English Proficiency Test for Aviation) as a follow up action. Purpose of this research is analysing the problem of EPTA and improving it. This investigation takes a look at the current ICAO LPRs and compares the difference between EPTA and other global English proficiency requirements for aviation. The result indicated that there is a discordance between the standards of ICAO and EPTA. Also, the global English proficiency reqirements for aviation and EPTA had differences which required improvements. The study suggests to replace more job relevant questions in the EPTA and reform the level system of EPTA.

Domestic Helicopter Accident Analysis using HFACS & Dirty Dozen

  • Safety can be defined as being maintained or reduced to a level below which the possibility of human or physical harm can be tolerated through continuous identification of risks and safety risk management. FAA, EASA, IATA and Boeing, major organizations that conduct research and analysis for aviation safety around the world, report that about 70 percent of aviation accidents are caused by human factors, which have led to a surge in interest in human factors-induced accident prevention activities around the world. As part of this purpose, the FAA in the U.S. is raising awareness among aviation workers by publicizing the 12 human errors (Boeing, 2016), which account for the largest part of aviation accidents under the theme of Dirty Dozen, to prevent aviation accidents. Therefore, based on the domestic helicopter accidents reported to the Air Railroad Accident Investigation Committee from 2007 until recently, this study aims to use HFACS to extract human factors for the six recent helicopter accidents in Korea, analyze the extracted human factors in conjunction with the Dirty Dozen concept, and then present measures to prevent accidents by item.

A Study on Legal and Institutional Improvement Measures for the Effective Implementation of SMS -Focusing on Aircraft Accident Investigation-

  • 가장 진보된 항공기술의 발전에도 불구하고 항공기사고는 지속적으로 발생하고 있는 반면 승객수송 항공교통량은 향후 15년간 갑절로 증폭할 것으로 예상되고 있다. 항공기사고 발생 후에 안전조치로 수행되는 항공기사고조사만으로는 항공안전을 확보할 수 없기 때문에, 선제적이고 예측적인 사고예방대책의 필요성에 대한 공감대가 형성되었다. 이러한 명목으로 항공안전관리시스템(SMS)이 2008년부터 도입되어 2011년부터 본격적으로 수행되고 있다. SMS는 선제적이고 예측적인 항공기사고 예방대책으로서, 항공안전과 관련된 기술적 요소, 인적요소를 넘어 조직적 요소에 접근함으로써 근원적인 위험요인을 차단할 수 있는 메커니즘이다. 방법론적으로는 항공기운항에 필요한 모든 현장에서 잠재되어 있는 위험요소를 수집하여 데이터베이스를 구축하고, 위험을 분석하여, 위험을 관리함으로써, 위험을 수용가능하거나 그 이하로 유지하는 방법이다. 따라서 SMS의 부적절한 이행은 항공기사고 예방의 미흡함을 나타내며 항공기사고와 직결된다. SMS에 있어 자신의 실수를 포함하여 업무상 발생하는 위험요소의 보고가 필수적이고, 가장 중요한 요소로 간주된다. 이를 위하여 자발적 보고에 대한 공정문화의 정책 하에, 정보제공자의 익명성, 비처벌 및 비문책 보장이 기본적인 것으로 되어있으나, 조직에 대한 신뢰의 부족으로 보고는 미미한 수준으로 정체되어 있는 상황이다. 최고관리자가 고위관리자와 더불어 자신의 조직에 대한 안전과 수익의 균형을 유지할 수 있는 안전의식을 갖고, 공정문화가 주축이 된 안전문화의 주도적 역할이 필요하다. 이에 대한 법적 제도적 근거는 국토교통부 훈령인 "항공안전관리시스템 인 및 운영지침"에 최고관리자가 및 고위관리자가 받아야할 교육이 명시되어있으나, 법적 구속력이 없는 상태이다. 따라서 법적 구속력이 있는 고시인 "국가항공안전프로그램"의 항공안전관리시스템 승인신청서의 구비서류에, 최고관리자 및 고위관리자의 SMS 교육이수증명서가 추가되어야 할 필요가 있다. 또한 항공기사고조사에 SMS항목이 누락되어 있어 안전문화와 관련된 조직적 요소 및 위험관리 부분에 대한 조사가 수행되지 않고 있다. 이는 근원적인 원인에 대한 규명이 불가능하여 향후 사고예방에 장애요소로 작용된다. ICAO가 발행한 항공기사고조사매뉴얼에는 SMS조사가 포함되어 있지만, 국제민간항공조약 부속서 13의 최종보고서 양식에는 포함되어있지 않다. 또한 전 세계적으로 항공기사고조사의 실질적 표본이 되고 미국교통안전위원회가 SMS조사에 미온적인 것으로 나타나고 있다. 이러한 이유들로 부속서 13에 의거 조사를 수행하고 있는 조사기구들은 SMS조사를 조사항목에 포함시키지 않고 있는 상황이며, 항공기사고 조사관들은 SMS 조사방법이나 기법에 노출되어 있지 않다. 이러한 맥락에서 부속서 13의 최종보고서 양식 중 조직 및 관리정보 목에 SMS조사를 포함시킬 필요가 있다. 국내적으로는 항공 철도사고조사위원회의 운영규정 중 최종보고서양식에 동일하게 SMS항목을 추가되어야 한다. 이러한 법적 제도적 개선방법이 보완되면 SMS의 이행이 효율적으로 이행되어 향후 항공안전증진에 기여하리라고 기대한다.
