The influence of phentermine hydrochloride on heart rate variability (비만 치료제 Phentermine Hydrochloride가 심박동 변이율(Heart Rate Variability)에 미치는 영향)

  • Background : Phentermine classified by "sympathomimmetic amie", is a stimulant of sympatheic tone But there has been no concrete study which presents the influence of phentermine on autonomic nervous system. Analysis of Heart rate variability is reliable, non-invasive and very useful for evaluating function of autonomic nervous system. We tried to elucidate the influence of phentermine on autonomic nervovs system by heart rate variability. MethodsAmong the 70 candidates who participated in the double-blind case control study whichwas designed in purpose of approving whether- Adipekⓓ is effective for treatment of obesity, 45persons were folled up. From April, 2005 to May 2005, HRV of the candidates who takes phentermine or placebo for 1month, was recorded using BFM-5000ⓓ(medi-core) for 5 minutes in resting state. HRV measures were assessed by time-domain and by frequency- domain analysis. Time domain parameters contain SDNN(Standard Deviation of NN intervals) and RMSSD(Root-Mean-Square of Successive Differences), etc and frequency domain Parameters contain Total Power(TP), Low frequency(LF'0.04-0.15Hz) power. High Frequency(HF:0.15-0.4Hz) power and LF/HF ratio etc. Results: Intakes of phentermine reduce HRV significant1y. SDNN & RMSSD, the main tine domain parameters of HRV, were decreased significantly(P=0.007. 0.016). PSI(Physical Stress Index of Pressure Index) was increase significantly(P=0.002)The main frequency domain parameters(TP, LF & VLF), also decreased significantly. (P=0.024,0.033, 0.015)Conclusion: The result showed that intakes of phentermine reduce heart rate variability and influence on most parameters of HRV. So phentermine not only accelerates sympathetic tone, but also inhibit the balance and function of autonomic nervous system.

  • Objective : The purpose of this experimental-controlled study was to investigate the characteristics of the autonomic nervous system in students with Keongke by using HRV(Heart rate variability) and PSV(Pupil size variability). Method : The study group was consisted of 11 students with self recognition as the experimental group, and 25 normal students as the control group. Informations on gender and age were obtained by medical charts and personal interviews. By using heart rate variability and pupil size variability, we measured the value of HRT(Heart rate), SDNN(Standard deviation of NN intervals), LFnorm(Low frequency normalization), HFnorm(High frequency normalization), LF/HF ratio, Pupil area, B.S.(Basic size), C.R.(Max Constriction Rate) and 1s.d.(1sec Dilation Rate). I compared the degrees of the sympathetic and parasympathetic activity. Result : 1. In the result of heart rate variability between experimental and control group, none of the parameters of experimental group were significantly different from control group. And even though there were no statistical significance, there were some numerical differences in SDNN, LF norm, HF norm. 2. In pupil size variability, C.R. and 1s.d. of the experimental group were increased compared to control group. Conclusion : The study results suggest that the group with Keongke has differences of autonomic nervous system as compared to those in normal state. Measurement value of PSV is a new technical approach to estimate the autonomic nervous system.

Effects of Self-stretching and Joint Mobilization to Upper Thoracic Vertebrae in Craniovertebral Angle and Autonomic System Function in Chronic Cervical Pain (만성 목통증에 대한 자가신장과 위등뼈 관절가동술이 머리척추각 및 자율신경계 기능에 미치는 효과)

  • PURPOSE: This study aimed to examine the effects of self-stretching (SS) and joint mobilization (JM) on pain, craniovertebral angle (CVA), autonomic system function in chronic cervical pain patient with forward head posture (FHP). METHODS: A total of 30 male college students were selected as study subjects, and were divided into Group I (general physical therapy; GPT, n=10), Group II (GPT+SS, n=10), Group III (GPT+JM, n=10). All groups were evaluated three times a week for 4 weeks. Pain was measured by visual analogue scale (VAS), CVA was measured using digital goniometer and autonomic system function (heart rate; HR, skin conductivity; SC, LF norm, HF norm, LF/HF ratio) was measured by Biofeedback ProComp Infiniti. After 4 weeks, paired t-test was used to compare the changes within the group and one way ANOVA was used to compare those between the groups. RESULTS: In Group I, VAS was significantly decreased. In Group II and III was a significantly change in all items. In comparison between Group I and II was a difference in all items except HR. In comparison between Group I and III was a difference in all items. In comparison between Group II and III was a difference in VAS, LF norm and LF/HF ratio. CONCLUSION: This study showed that SS and JM can effectively reduce pain and normalize the autonomic system function.

Effects of an Aromatherapy Education Program on Stress, Self-efficacy, and Autonomic Nervous System Function of Female College Students Preparing for Employment (아로마테라피 교육 프로그램이 취업준비 여대생의 스트레스와 자기효능감 및 자율신경계에 미치는 영향)

  • Purpose : The stress of job-seeking college students is gradually worsening due to the continuing recession in the job market. Therefore, this study aims to verify whether intervention with an 'aromatherapy education program,' which combines theory and practice, has greater effect on stress, self-efficacy, and autonomic nervous system function than simple inhalation of essential oils in female college students preparing for employment. Methods : A single-blind, randomized controlled trial was conducted once a week for a total of 4 weeks with 29 female college students preparing for employment. Interventions included essential oil inhalation combined with an aromatherapy education program (experimental group(EG), n=14) and essential oil inhalation alone (control group(CG), n=15); measurements were taken before, immediately after, and 2 weeks after completion of the intervention. Evaluations were conducted using the job-seeking stress scale, stress VAS, self-efficacy scale, and a pulse-wave measuring instrument. Results : There were significant differences between the CG and EG groups with repeated measurements of job-seeking stress (p=.003), cognitive stress (p<.001), and self-efficacy (p<.001). There was a significant interaction between group and time. There were also significant differences between the CG and EG in terms of autonomic nervous system changes stress index(p=.047), total power(p=.029), high frequency(p=.036), and low frequency/high frequency ratio(p=.049). However, there was no significant difference in low frequency between the groups. Conclusion : The aromatherapy education program was more effective in improving job-seeking stress, cognitive stress, self-efficacy, and autonomic nervous system function than simple essential oil inhalation. For the aromatherapy education program to be useful as a complementary intervention for stress management in job seekers, it will be necessary to gather additional evidence of its efficacy by conducting follow-up studies with diverse participants and expanding the numbers, and durations, of the interventions.

A Preliminary Study on the Circadian Rhythm of Autonomic Nervous System in Patients Admitted to Intensive Care Unit, Using the Heart Rate Variability (심박변이도를 이용한 중환자실 입원 환자의 자율신경계 일중변동성에 대한 예비연구)

  • Objectives A normal circadian rhythm of autonomic nervous system function stands for the daily change of sympathetic and parasympathetic modulation, which can be measured by heart rate variability (HRV). Generally, patients in the intensive care unit (ICU) are prone to sleep-wake cycle dysregulation, therefore, it may have an influence on the circadian rhythm of autonomic nervous system. This study was designed to interpret possible dysregulation of autonomic nervous system in ICU patients by using HRV. Methods HRV was assessed every 3 hours in 21 ICU patients during a 7-minute period. The statistical differences of HRV features between the morning (AM 6 : 00-PM 12 : 00), and the afternoon (PM 12 : 00-PM 18 : 00) periods were evaluated in time domain and frequency domain. Results Patients showed significantly increased normalized power of low frequencey (nLF), absolute power of low frequencey (LF)/absolute power of high frequencey (HF) in the afternoon period as compared to the morning period. However, normalized power of high frequency (nHF) was significantly decreased in the afternoon period. There was no statistically significant difference between the morning period and the afternoon period in the time domain analysis. Conclusions The increased sympathetic tone in the afternoon period supports possible dysregulation in the circadian rhythm of autonomic nervous system in ICU patients. Future studies can help to interpret the association between autonomic dysregulation and negative outcomes of ICU patients.

The Influence of Psoas Muscle Contracture on Autonomic Nervous System Activity (요근 긴장이 자율신경계 활성도 변화에 미치는 영향)

  • Objectives : This study was performed to examine the hypothesis that the structural imbalance affect cardiac function and autonomic reflex system and to investigate the possibility of the chiropractic care for cardiovascular system. Methods : 78 of Dong-Guk University students with structural imbalance were recruited for the investigation from March to June 2007. Heart rate variability, Buss and Durkee Hostility inventory(BDHI) and physical examinations to evaluate psoas muscle contracture were performed. Results : Left psoas muscle contracture was associated with decrease of LF/HF ratio(p=0.048). Conclusion : Left side contracture of psoas muscle showed a tendency to decrease sympathetic activity.

  • Purpose : To research the Skin Resistance Variability(SRV) of premature ovarian failure(POF) patients by Autonomic Bioelectric Response Recorder(ABR-2000 system, Meridian, Korea) and report the specific results in SRV of POF patients. Methods : We measured SRV of 17 POF Patients who came to Conmaul Oriental Medical Hospital during August 2005 ${\sim}$ July 2006 by ABR-2000 system. We analyzed the results which height of graph Part was converted into 0${\sim}$10, and the readings of Low/Normal/High. Results : The mean value of graph height on each(1, 2, 3) part is lower than normal range(4-6) in POF patients. The distribution ratio of Low/Normal/High on each(1, 2, 3) part shows that there are much more Low proportion in POF patients. Conclusion : The low graph height of POF patients on 1, 2, 3 part means that conductivity & capacitance of POF patients is low, especially on head.

  • The study investigated the effect of multi-functional fabric on the autonomic nervous function and psychological variables of 20 students. The experimental group exhibited lower values in anxiety, depression, fatigue and stress level and higher emotional level. This study reveals that multi-functional fabrics reduced the low frequency/high frequency power ratio of heart rate variability. These results support the multi-functional fabrics increases cardiac parasympathetic tone. In addition, experimental group were found to have lower heart rate compared with controls. This augmented heart rate in experiment provides support for stablizing autonomic nervous system. In conclusion, multi-functional fabrics may stabilize the autonomic nervous system and psychological symptoms.

  • The R-R interval analyzer was developed to measure the autonomic nervous system function using microcomputer. The system based on 8 bit microcomputer including bandpass filter, R-wave detector and clock generator in order to obtain the mean value, standard deviation, total time, CV value, maximum value and minimum value in the specific view point of R-R interval variation. The pattern of R-R interval change after resting, voluntary standing and deep breathing can be analysed in normal subjects and diabetics with autonomic nervous dysfunction. The amplitude of the R-R interval variation showed sensitive pattern for normal subjects at resting, standing and deep breathing. On the contrary, the periodicities of amplitude for abnormal subjects with autonomic nervous dysfunction showed dull pattern. It was suggested that R-R interval analyzer is a good detection method for dysfunction of autonomic nervous system.

  • Measuring heart rate variability by deep breathing and using this to evaluate the autonomic nervous system is a well-known method widely used in various clinical fields. In hospitals, expensive equipment is installed and managed by experts for this purpose. This paper discusses an app we developed that can easily test the autonomic nervous system by deep breathing anytime, anywhere using a smartphone, and then reviews experiments performed to verify the results. The experiments were conducted in the clinical room of the Department of Neurology at Soonchunhyang University Cheonan Hospital on eight volunteers. We tested the autonomic nervous systems of the volunteers first with an electromyography device installed in the hospital and then with the app under identical conditions. We performed a correlation analysis on the results of these two methods using the Pearson method, and this yielded a very high correlation of 0.98.
