Multimodal Treatment of Poorly Differentiated Tracheobronchial Carcinoma in a Persian Cat

  • A 14-year-old castrated male Persian cat presented with a 2-week history of respiratory difficulty. On physical examination, the patient showed intermittent open-mouth breathing and thoracic auscultation revealed wheezing. Thoracic radiographs revealed a narrowed upper airway and pulmonary infiltration. Computed tomography detected a mass occluding the lumen of the trachea at the level of the entrance to the thorax, a mass causing right main bronchus stenosis, and a nodule on the right caudal lung lobe. Bronchoalveolar lavage cytology tentatively diagnosed a carcinoma. Tracheal mass resection was performed through tracheostomy. Histopathology confirmed the presence of tracheobronchial carcinoma. The survival time after diagnosis was 10 months, during which time the cat underwent tracheostomy, debulking by endotracheal tube, and tracheal stent placement procedures in combination with toceranib phosphate adjuvant chemotherapy.

An Effect on Recovery of Post-operative Bowel Movement on Nursing Intervention of Meridian Acupressure (경혈지압 간호중재가 수술후 장유동 회복에 미치는 영향)

  • This study was conducted to identify the effectiveness of meridian acupressure for enhancement of post operative bowel movement to the patient with absolute bed rest who having surgery under general anesthesia. This study used a qusai experimental, nonequivalent control group post test only design. This subject were 44 patients, 22 for the experimental and 22 for the control group, who were admitted at KyungHee University hospital, neurosurgical unit A and B ward, assigned by matched sample by the name of operation who having microvascular decompression and laminectomy. Date were collected from May 1, 2001 to June 30, 2001 by auscultation, self report and by using 7 point face scale. The recovery of bowel sound were measured every 4 hours until gas out for 1 minute auscultation on lower abdomen after 4 hours having surgery. The time of gas out were measured by self report, the severity of nausea were measured every 4 hours for 7 point face scale and also the degree of satisfaction of nursing care were measured after 2 days having surgery with same scale. Data were analyzed with $X^2$, t-test, repeated measures ANOVA and ANOVA. The result of this study were as follows; 1. The experimental group which were implemented with meridian acupressure showed shorter time the recovery of bowel sound after having surgery than control group(t=-5.112, p=.0001). 2. The experimental group which were implemented with meridian acupressure showed shorter time of gas after having surgery than control group(t=-4.010, p=.0001) 3. The experimental group which were implemented with meridian acupressure showed decreased level of nausea score according to time interval than control group(F=21.995, p=.0001). 4. The experimental group which were implemented with meridian acupressure showed higher the degree of satisfaction of nursing care than control group(t=-4.010, p=.0001). These finding indicate that a meridian acupressure could be a effective nursing intervention for enhancement of post operative bowel movement to the patient with absolute bed rest who having surgery under general anesthesia.

  • For having good therapeutic value, putting the ideal of diagnosis and treatment based on overall analysis of symptoms and signs[辨證施治] into practice is very important, so the Eastern Medicine's peculiar diagnosis without interference by the western medical diagnosis is very important. The peculiar method of Eastern Medicine's diagnosis is four methods of diagnosis(四診法), a joint term for inspection, auscultation and olfaction, interrogation, pulse feeling and palpation. In the process of interrogation[問診], from analyzing the perspiration phase, doctor can get various physical information. Especially perspiration from particular region can be a clue for founding focus or cause of disease. This thesis divide the phase of perspiration into eight bodily region, perspiration from all body, head, face, back, chest and armpit, stomach, pubic region and lower part, hands and feet, and inquire the possible causes and principles of these perspirations. In conclusion, the regional perspiration can be a clue point out the origin of fever and condition of Gi(氣) flow. Perspiration from head, back, chest and armpit, hands and feet means that certain fever cannot extend to the outside of body, and at the same time, means the Gi(氣) flow of outside is being intercepted. So the perspirations from that region become an object of medical treatment.

  • Kidney diseases rates in modern Korean society, which is entering an aging society, are growing. These diseases diagnosed at early stage can be cured with the drug or dietary treatment. If, however, at worsening stage, kidney dialysis or transplant is needed. In this context, we analyzed the relationship between kidney and voice by measuring and analyzing the changing rate of the vocal chord and intensity according to hemodialysis. For this, we collected voice samples before and after hemodialysis, and compared and analyzed them through application of vocal analytical element. Namely, based on oriental medical diagnosis theory that voice is quieted down when kidney is abnormal, we collected voice samples before and after hemodialysis, and applied trembling and intensity analysis of vocal chord.

Class Determination Based on Kullback-Leibler Distance in Heart Sound Classification

  • Stethoscopic auscultation is still one of the primary tools for the diagnosis of heart diseases due to its easy accessibility and relatively low cost. It is, however, a difficult skill to acquire. Many research efforts have been done on the automatic classification of heart sound signals to support clinicians in heart sound diagnosis. Recently, hidden Markov models (HMMs) have been used quite successfully in the automatic classification of the heart sound signal. However, in the classification using HMMs, there are so many heart sound signal types that it is not reasonable to assign a new class to each of them. In this paper, rather than constructing an HMM for each signal type, we propose to build an HMM for a set of acoustically-similar signal types. To define the classes, we use the KL (Kullback-Leibler) distance between different signal types to determine if they should belong to the same class. From the classification experiments on the heart sound data consisting of 25 different types of signals, the proposed method proved to be quite efficient in determining the optimal set of classes. Also we found that the class determination approach produced better results than the heuristic class assignment method.

The Spectral properties of Knee Joint Sounds (슬관절 청진음의 주파수 특성에 대한 연구)

  • The aim of this study was to analyze the characteristics of knee joint sound in frequency domain and classify the knee joint diseases. The spectral analysis of knee joint sounds was performed using LPC(Linear Predictive Coding) and Wigner-Ville distribution. Ten normal subjects and 5 patients with meniscal tearing were enrolled. Each subject was seated on a chair and underwent active knee flexion and extension for 60 seconds. Sampling frequency was 10kHz and electronic stethoscope and electro-goniometer were applied during the knee motion for data collection. The spectral analysis showed 3 peaks in both groups and the difference energy distribution in time-frequency domain. These results suggest that the diagnosis of knee joint pathology using the auscultation could be easier and more correct.

  • In this study, we have evaluated and classified arthritic pathology using the auscultation of knee joint sound. Six normal persons and 11 patients with knee problem were enrolled. Six patients of Group 1 needed an orthopeadic surgery because of the ruptured wounds of meniscus or ACL(Anterior Cruciate Ligament) and 5 patients of Group 2 diagnosed as osteoarthritis. Subjects were taken knee flexion and extension being seated in a chair for 20 seconds which repeated 3 times. Also subjects stood up and sit down repeatedly in the same way. After the movement of knee was divided into 18 degrees, the pitch perturbation according to partial degrees was analyzed and the DTW(Dynamic Time Warping) method was applied for normalizing a time-axis and unpaired t-test was used for statistic results among groups. As a result, the amplitude and frequency perturbations of group 2 was higher than group 1(p<0.05) and showed a characteristic 'w-shape' in angle-amplitude graph. These results suggest that the analysis of knee joint sound might assist in early diagnosis of knee joint disease.

  • The conventional stethoscope can not store its stethoscopic sounds. Therefor a doctor diagnoses a patient with instantaneous stethoscopic sounds at that time, and he can not remember the state of the patient's stethoscopic sounds on the next. This prevent accurate and objective diagnosis. If the electronic stethoscope, which can store the stethoscopic sound, is developed, the auscultation will be greatly improved. This study describes an amplifier for electronic stethoscope system that can extract heart sounds of fetus as well as adult and alow us hear and record the sounds. Using the developed stethoscopic amplifier, clean heart sounds of fetus and adult can be heard in noisy environment, such as a consultation room of a university hospital, a laboratory of a university. Surprisingly, the heart sound of a 22-week fetus was heard through the developed electronic stethoscope. Pitch detection experiments using the detected heart sounds showed that the signal represents distinct periodicity. It can be expected that the developed electronic stethoscope can substitute for conventional stethoscopes and if proper analysis method for the stethoscopic signal is developed, a good electronic stethoscope system can be produced.

Surgical Repair of Left Ventricular Aneurysm and Postinfarction Ventricular Septal Defect with Myocardial Revascularization (A report of 6 cases] (심근경색후 발생한 좌심실류 및 심실중격결손의 외과적 치료 (6례 보고))

  • A clinical analysis was performed on 115 cases of -patent ductus arteriosus treated surgically during the period of 11 years from Aug. 1977 to Jul. 1988. at the Department of Thoracic and Cardiovascular Surgery, Chungnam National University Hospital. Among 115 cases, male was 38 and female was 77 and ages ranged 12 days to 27 years old with the average of 8 7/12 years. The major clinical symptoms on admission were frequent URI attack[77.4%], dyspnea on exertion[32.2%] and palpitation[13%]. On auscultation, continuous machinery murmurs were detected in 97 cases[84.3%] and loud systolic murmurs were detected in 18 cases[15.7%]. Preoperative electrocardiographic findings were as follows: LVH 59[51.3%], RVH 12[10.4%], BVH 16[13.9%] and WNL 28[24.3%]. Radiologically, there were increased pulmonary vascularity in 104[90.4%] and cardiomegaly 62[53.9%]. Cardiac catheterization were performed in 101 cases and mean systolic pulmonary arterial pressure was 49.84*29.7mmHg and mean Qp/Qs was 2.95k1.8. Methods of operation were multiple ligation in 96, division in 11 and transpulmonary arterial repair using cardiopulmonary bypass in 8. Complication were recannalization in 2, temporary hoarseness due to left recurrent laryngeal nerve paralysis in 3 and respiratory distress in 1 and overall mortality rate was 1.7%[2 cases].

Blood Pressure Measurement using the Modified Step-wise Deflation Method (개선된 계단 배기 방법을 이용한 혈압 측정)

  • In the automatic non-invasive blood pressure measurement device, the oscillometric method iswidely used. In the oscillometric method, the step-wise deflation has the advantage of the robustness for the motion artifacts than the linear deflation method. But it has the disadvantage of its longer measurement time because we need to detect two or more pulses in a certain cuff pressure step. In this study, we suggest the modified step-wise deflation method to overcome this limitation while maintaining the general concept of step-wise deflation. Using one valid pulse in each step and the deflating valve control during the diastolic period, the measurement time could be reduced. In order to verify the accuracy of the proposed algorithm, we compared the blood pressure values from the suggested method and the blood pressure values from the conventional auscultation method. The mean and standard deviation were -0.50${\pm}$5.3mmHg and 2.08${\pm}$4.75mmHg, for systolic and diastolic blood pressure respectively. The measurement time can be reduced up to the half of conventional step-wise deflation method.
