Diagnostic evaluation and educational intervention for learning disabilities (학습장애의 진단 평가와 교육학적 개입)

  • Learning disabilities (LD), also known as learning disorders, refers to cases in which an individual experiences lower academic ability as compared to the normal range of intelligence, visual or hearing impairment, or an inability to peform learning. Children and adolescents with learning disabilities often have emotional or behavioral problems or co-existing conditions, including depression, anxiety disorders, difficulties with peer relationships, family conflicts, and low self-esteem. In most cases, attention deficit and hyperactivity disorder coexists. As learning disabilities have the characteristics of a difficult heterogeneous disease group that cannot be attributed to a single root cause, they are diagnosed based on an interdisciplinary approach through medicine and education, such as mental health medicine, education, psychology, special education, and neurology. In addition, for the accurate diagnosis and treatment of learning disabilities, the diagnosis, prescription, treatment, and educational intervention should be conducted in cooperation with doctors, teachers, and psychologists. The treatment of learning disabilities requires a multimodal approach, including medical and educational intervention. It is suggested that educational interventions such as the Individualized Education Plan (IEP) and the Response to Invention (RTI) should be implemented.

Comorbid Psychiatric Symptom Associated With Oppositional Defiant Symptom in Community School-Age Children

  • Objectives: Oppositional defiant disorder (ODD) is often comorbid with other psychiatric disorders in childhood. This study aimed to investigate comorbid psychiatric symptoms and associated factors in elementary school children with symptoms of ODD. Methods: The participants consisted of 205 mother-offspring pairs. Psychiatric symptoms were measured using the Diagnostic Predictive Scales and Korean Child Behavior Checklist. Psychiatric comorbid symptoms were compared between children with ODD symptom and those without ODD symptom. Multivariate logistic regression analysis was used to estimate the odds ratio of psychiatric symptom on ODD. Results: ODD group had a significant association with internalizing and externalizing problem (p=0.001, p<0.001, respectively). ODD group were more comorbid with anxiety disorder, depressive disorder, attention-deficit/hyperactivity disorder, and conduct disorder. Among psychiatric disorder, generalized anxiety disorder (GAD) (adjusted odds ratio [AOR]=18.620, p<0.001) and conduct disorder (AOR=9.529, p=0.014) were associated with ODD symptom. Conclusion: These findings suggest that children with ODD symptom had significantly higher rates of comorbid psychiatric symptoms. And GAD and conduct disorder are related to ODD symptom.

Initial Impact of the COVID-19 Outbreak on ADHD Symptoms Among University Students in Japan

  • Objectives: The coronavirus disease 2019 pandemic and its associated societal responses are anticipated to have wide-ranging effects on youth development and mental health. Depression, anxiety, and attention deficit hyperactivity disorder (ADHD) are the three most common mental health problems among university students. Many factors that can threaten mental health during the outbreak affect these three conditions, as well as sleep conditions, in undergraduate students. Thus, determining how these abrupt changes in students' circumstances impact their mental health is very important from a public health perspective. Methods: We investigated the usual conditions and changes in ADHD symptoms during the outbreak, in relation to depressive and sleep-related symptoms among undergraduate students. A total of 252 students, primarily juniors, completed the online survey. Results: The results showed that 12% of the students exceeded the cut-off score of the ADHD questionnaire before the pandemic. Approximately 6%-21% of the university students, especially those with ADHD traits, rated their ADHD behaviors as worse during the outbreak than that before the outbreak. Conclusion: Female students and undergraduates with ADHD traits are more susceptible to experiencing further deterioration of ADHD (inattention) symptoms during the pandemic. In cases where it is difficult to intervene with ADHD symptoms, approaching circadian rhythm or depression will be of considerable clinical use.

  • This study was conducted to investigate the communicative intent between Reactive Attachment Disorder(RAD) and Developmental Language Disorder(DLD). The subject of this study were 20 27-51 monthold children(10 RAD Children, 10 DLD children) functioning at similar stage of language development. The communicative intent was investigated vertical and horizontal dimension. Rating of vertical communicative intent was based on the assessment scales devised by Wetherby and Prutting(1984). Horizontal dimension was measured by the assessment guidelines of Wetherby and Prizant(1989). All the data were rated by two rators independantly. 1) In communicative intent, vertical development level of DLD children was more sophiscated than that of RAD children. 2) DLD children expressed more horizontal communicative intent than RAD children. The percentage of the three major categories(behavioral regulation, social interaction, and joint attention) of communicative intent in DLD children was lined up social interaction>joint attention>behavioral regulation. On the contrary, RAD children displayed in order of behavioral regulation>social interaction>joint attention. In addition, DLD children showed diversely as compared with RAD children.

A Systematic Review on Intervention of Interactive Metronome: Focus on Single-Subject Research Design in Korean Academic Journals (상호작용식 메트로놈(Interactive Metronome)의 중재에 대한 체계적 고찰: 국내 단일대상연구를 중심으로)

  • Objective : This study aimed to confirm the research characteristics and quality of single-subject literature among domestic studies that applied interactive metronome (IM) intervention. Methods : Regarding literature search, 11 single-subject studies using IM were selected from an online database from January 2011 to June 2022. Moreover, the general characteristics and quality of the research method were analyzed. Results : The qualitative level of the analyzed literature was above the moderate level. However, intervention blindness and reliability showed low compliance. The ABA design accounted for the largest proportion of methods. Most of the study participants had attention deficit hyperactivity disorder. Attention, balance, bilateral coordination, and timing were checked as dependent variables. The IM-SFT was used most frequently as an evaluation method. The mediation session applied more than 8-10 interventions for 3-11 weeks. The intervention results in all studies indicated functional improvement after intervention. Conclusion : It might be necessary to expand the application of IM interventions to diverse diseases. In addition, there is a need to study the effect on the participants' quality of life and changes in daily life along with dependent variables such as attention and balance

EFFECT OF METHYLPHENIDATE ON FUNCTIONAL CEREBRAL LOCALIZATION DURING ATTENTIONAL TASKS IN BOYS WITH ATTENTION-DEFICIT/HYPERACTIVITY DISORDER (주의력결핍 과잉운동장애 남아에서 중추신경자극제가 주의력과제 수행 중의 대뇌 기능적 국소화에 미치는 효과)

  • This study aims at investigating the effects of psychostimulant on functional cerebral localization during the attentional tasks in ADHD. Inclusional subjects were 13 boy between 6-12 years old who met the DSM-IV criteria for ADHD. In each patient, there was a drugfree period(without methyphenidate) and a drug-loaded period(with oral methylphenidate administration), and within each period there was a resting state and a stimulated state with TOVA. Comparisons were made by measuring the amplitudes of four bands(${\alpha}$, ${\beta}$, ${\delta}$, ${\theta}$) of quantitative EEG to see if there were any differences between the drug-free period(resting and stimulated) and the drug-loaded period(resting and stimulated). In the resting state, there was no difference between the drug-free and drug-loaded periods. In the stimulated state with TOVA, the presence of methylphenidate induced significant changes in the theta to beta ratio(${\theta}/{\beta}$) in the right frontal, right parieto-occipital, and left temporal-parietal areas in contrast to the drugf-ree period. These data suggest that methylphenidate shows electrophysiological influences on cerebral topographical activities during the attentional tasks in ADHD.

COMPLIANCE STUDY OF METHYLPHENIDATE IR IN THE TREATMENT OF ADHD (주의력결핍과잉행동장애 치료 약물 Methylphenidate IR의 순응도 연구)

  • Objectives : There have been very few studies on the compliance of methylphenidate-immediate releasing form(MPH-IR), which is the most frequently used drug in Korea, in Attention Deficit Hyperactivity Disorder(ADHD). This study was conducted to investigate the compliance rate and the related factors in the one year pharmacotherapy process via OPD for children with ADHD. Method : Total 100 ADHD patients were selected randomly among patients who have been treated with MPH-IR from September in 2002 to December in 2002. All the selected patients were diagnosed with DSM-IV-ADHD criteria and fulfilled the inclusion criteria. In March, 2003(at the time of 6 month treatment), all the patients and parents received the questionnaire for the compliance and satisfaction for MPH-IR treatment. In October 2003(at time of 1 year treatment), we, investigators evaluated the socio-demographic variables, developmental data, medical data, family data, comorbid disorders, treatment variables, and compliance rate. Through these very comprehensive data, The compliance rate at the time of mean 1 year treatment and the related factors were investigated. Result : 1) In the questionnaire for compliance and satisfaction for MPND treatment, the 60% of respondents(parents) reported more than moderate degree of satisfaction in the effectiveness of MPND. Their compliance rate for the morning prescription was 81%, but the rate of afternoon prescription was 43%. 2) In the evaluation at the time of 1 year treatment(October 2003), the 38% of parents were dropped out from the OPD treatment. The mean compliance rate for the 1 year treatment was 62%. the 38% of parents were dropped out from the OPD treatment. The mean compliance rate for the 1year treatment was 62%. 3) Compared with the noncompliant group(drop-out group), compliant group showed higher total, verbal and performance IQ scores. In the treatment variables, higher reposponder rate(clinician rating), higher medication dosage and more compliance rate in afternoon prescription were found in the compliant group compared with the noncompliant group. There were no statistical differences in the demographic variables(age, sex, SES, parental education level), medical data, developmental profiles and academic function. Conclusion : To our knowledge, this is the first report about the compliance rate of the MPH-IR treatment for the children with ADHD. The compliance rate at the time of mean 1year treatment was 62%, which was comparable with other studies performed in foreign countries, especially States. In this study, the compliance related factors were IQ score, clinical treatment response, dosage of MPH-IR, and early compliance for the afternoon prescription. These results suggest that clinician plan the strategies for the promotion of the early compliance for the after prescription and enhancement of overall treatment response.

Comparison of Executive function in Children with ADHD, Asperger's Disorder, and Learning Disorder (주의력결핍과잉행동 장애, 아스퍼거 장애, 학습 장애 아동의 실행기능 비교)

  • Objectives : This study was conducted to investigate the deficits of executive function in children with ADHD, Asperger's Disorder(AD), and teaming disorder (LD), and to identify the differential characteristics of executive function deficits among three groups. Methods : The clinical group consisted of 46 children between the ages of 7 and 15 (16 ADHD, 16 LD, 14 AD). Neuropsychological tests for measuring cognitive function, attention and executive function were individually administered to children, and their performance scores were calculated based on the age norm for each test. Results : There was no significant difference in FSIQ, VIQ, and PIQ among the three groups. However, the AD group tended to show higher scores on the subtests of Information, Vocabulary and Digit Span, and lower score on Comprehension subtest than the ADHD and LD groups, while the LD group tended to show the lowest scores on the Information and Vocabulary subtests. On ADS, the ADHD group showed the highest omission and commission errors. All groups showed poor performances belonging to below 25 percentile ranks on executive function tests when compared to the age norms of normative group. The number of completed category on WCST was the smallest in the ADHD group, while the working memory score was the lowest in the LD group. Conclusion : These results suggest that ADHD, LD, and AD children have executive function deficit in common. However, the specific deficit areas in executive function are different for each group.

Evapotranspiration Measurements using an Eddy Covariance Technique in the Seolmacheon Catchment (에디 공분산으로 관측된 설마천 산림 유역의 증발산)

  • The importance of securing water resources and their efficient management has attracted more attention recently due to water deficit. In water budget analysis, however, evapotranspiration (${\lambda}E$) has been approximated as the residual in the water balance equation or estimated from empirical equations and assumptions. To minimize the uncertainties in these estimates, it is necessary to directly measure ${\lambda}E$. In this study, using the eddy covariance technique, we have measured ${\lambda}E$ in a mixed forest in the Seolmacheon catchment in Korea from September 2007 to December 2008. During the growing season (May - July), ${\lambda}E$ in this mixed forest averaged about 2.2 mm $d^{-1}$, whereas it was on average 0.5 mm $d^{-1}$ during the non-growing season in winter. The annual total ${\lambda}E$ in 2008 was 581 mm $y^{-1}$, which is about 1/3 of the annual precipitation of 1997 mm. Despite the differences in the amount and frequency of precipitation, the accumulated ${\lambda}E$ during the overlapping period (i.e., September to December) for 2007 and 2008 was both ${\sim}$ 110 mm, showing virtually no difference. The omega factor, which is a measure of decoupling between forest and the atmosphere, was on average 0.5, indicating that the contributions of equilibrium ${\lambda}E$ and imposed ${\lambda}E$ to the total ${\lambda}E$ were about the same. The results suggest that ${\lambda}E$in this mixed forest was controlled by various factors such as net radiation, vapor pressure deficit, and canopy conductance. In this study, based on the direct measurements of ${\lambda}E$, we have quantified the relative contribution of ${\lambda}E$in the water balance of a mixed forest in the Seolmacheon catchment. In combination with runoff data, the information on ${\lambda}E$ would greatly enhance the reliability of water budget analysis in this catchment.

INHIBITION, PLANNING, AND WORKING MEMORY IN CHILDREN WITH ATTENTION DEFICIT HYPERACTIVITY DISORDER (주의력 결핍 과잉행동 장애 아동의 억제능력, 계획능력, 그리고 작업기억 능력)

  • Objectives:The study was performed to compare the executive function(EF;Inhibition, Planning and Working memory) between ADHD and normal group, and to controll the effect of age and intelligence. And the function of inhibition was assessed in two dimensions(cognitive inhibition and motor inhibition). Methods:K-WISC III and EF test(Go-No-Go, Stroop test, Tower of Hanoi, Digit) was administered to both 25 children with ADHD and 25 normal control participants, all aged between 7 and 12. The results were analyzed after statistically controlled for age and intelligence. Results:Children in the ADHD group had significantly lower IQ score than those in the control group and consistent relations were found between the child's age and the study's major variables. Once IQ and age were controlled, results indicated that children with ADHD had deficit only cognitive inhibition and motor inhibition. There was no significant difference in planning and working memory. Conclusion:These results suggested that specific deficits in inhibition control rather than general EF deficits are associated with ADHD. So inhibition is the core problem of children with ADHD. Thus, the therapeutic approach focused on cognitive inhibition and motor inhibition is required.
