Nutrient intakes of infants with atopic dermatitis and relationship with feeding type

  • BACKGROUND/OBJECTIVES: The prevalence of atopic dermatitis in infants is increasing worldwide. However, the nutrient intake status of infants with atopic dermatitis has not been studied properly. This study was conducted to compare the nutrient intake status of infants in the weaning period with atopic dermatitis by feeding type. MATERIALS/METHODS: Feeding types, nutrient intake status and growth status of 98 infants with atopic dermatitis from age 6 to 12 months were investigated. Feeding types were surveyed using questionnaires, and daily intakes were recorded by mothers using the 24-hour recall method. Growth and iron status were also measured. RESULTS: The result showed that breastfed infants consumed less energy and 13 nutrients compared to formula-fed or mixed-fed infants (P < 0.001). The breastfed group showed a significantly lower intake rate to the Dietary Reference Intakes for Koreans than the other two groups (P < 0.001). In addition, they consumed less than 75% of the recommended intakes in all nutrients, except for protein and vitamin A, and in particular, iron intake was very low, showing just 18.7% of the recommended intake. There was no significant difference in growth by feeding type, but breastfed infants showed a significantly higher rate of iron deficiency anemia (P < 0.001). CONCLUSIONS: Continuous management programs should be prepared for breastfed infants with atopic dermatitis, who are in a period when rapid growth takes place and proper nutrient intake is essential.

학령기 아동의 아토피 피부염 예방 및 관리를 위한 영양교육 프로그램 개발 및 적용 (Development and Application of Nutrition Education Program for Prevention and Management of Atopic Dermatitis)

  • Atopic related skin symptoms must be controlled continually. Because it is involved with food, eating behaviors play an important role in the prevention and management of atopic dermatitis. The aim of this study was to develop and apply nutrition education program for the prevention and management of atopic dermatitis on elementary school students. After applying the nutrition education program, our research attempts to evaluate the effectiveness of such nutrition education program. This study examined the effects of a nutrition education program on atopic dermatitis-related snacking behaviors, eating attitudes, and food behaviors. Subjects were 3rd grade elementary school students in Daejeon. A 6-week nutrition education program was implemented to 97 children as the educated group and another 94 children were included in the study as the control group. A self-administered questionnaire was used to assess the effects of nutrition education program. As results, eating attitudes reached significant improvement by the nutrition education program(p<0.05). In a paired t-test, the control group did not show significant difference, but the educated group showed a significant difference. In snacking behaviors, the two groups did not show statistical difference except for some considering factors. Also, there were no significant differences of the two groups after nutrition education program, except the snacking place after the nutrition education group. However, in the educated group, the types of snacks made by mothers increased 17.5% from 40.2% to 57.7%, and in-house as snacking place increased 13.4% from 68% to 81.4%. In the results of before and after the nutrition education program, the program showed some effect on the prevention and management of atopic dermatitis. Therefore, these results showed foundation for nutritional education and various educational programs are needed afterward to increase the effect of nutrition education in the prevention and management policy related to atopic dermatitis.

아토피피부염 개인건강기록 앱 개발에 관한 연구 (Study on Development a Personal Health Record Application of Atopic Dermatitis in Korean Medicine)

  • Objective : The purpose of this study is to develop a personal health record(PHR) application of atopic dermatitis in Korean Medicine(KM). Methods : We have identified the items necessary to provide an PHR application that helps to record and manage the symptoms of an atopic dermatitis in KM. We also derived the symptom collection process and method and applied it to the application. Results : In this study, the types of symptoms collected for atopic dermatitis were derived. Symptoms include daily check, stool/urine/sleep, daily emotion, meal management, symptom photographs, SCORAD, quality of life, progress check, original symptom, pediatric health check, weakness check, and subjective symptoms. The recording cycle can be divided into the first, daily, weekly, specific day, and subjective. We developed the PHR application of atopic dermatitis in KM by deriving the type of symptoms and symptom recording process. The app organized menus into dashboards, checklists, daily checks, and health records. Conclusions : We developed a PHR application for atopic dermatitis by deriving symptom collection items of atopic dermatitis and developing symptom collection process and collection technique. The app does not make an accurate diagnosis of atopic dermatitis symptoms, but it helps facilitate symptom collection and helps to identify or predict a person's health condition. It can also be used for medical treatment through sharing symptoms with Korean medicine. Patients are able to communicate in both directions on a daily, weekly, self-aware basis, at the request of a doctor, to record their symptoms and use them for medical purposes. If the doctor asks for a progress check to refer to the next examination, it can be written through the system. This manages atopic dermatitis in daily life and can be used in the clinical field.

국내 개에서 발생한 아토피성 피부염에서 Dermatophagoides farinae의 항원 확인 (Identification of Allergens of Dermatophagoides farinae on Canine Atopic Dermatitis in Korea)

  • 개의 아토피성 피부염에서 Dermatophagoides farinae의 항원을 immunoblot을 통해 확인하였다. 실험은 D. farinae에 의해 아토피성 피부염이 유발된 16마리와 임상적으로 건강한 10마리 개의 혈청으로 수행하였으며, SDS-PAGE를 실시하여 D. farinae 추출물의 항원들이 15에서 220 kDa 범위 내에 존재하는 것을 관찰하였다. Immunoblot 결과, 임상적으로 건강한 10마리 개의 혈청에서는 단백띠가 보이지 않았으며, D. farinae에 의해 아토피성 피부염이 유발된 16마리 개의 혈청을 이용한 실험에서는 14마리에서 109 kDa, 13마리에서 44 kDa, 11마리에서 177 kDa, 7마리에서 27 kDa, 5마리에서 75 kDa, 3마리에서 15 kDa의 단백띠가 관찰되었다. 즉, D. farinae에 의해 아토피성 피부염이 유발된 개체 중 50% 이상에서 관찰되는 주요 항원은 177 kDa, 109 kDa, 44 kDa 항원이었다. 본 실험과 같이 주요 항원들을 확인하는 이러한 연구는 아토피성 피부염에 대한 새로운 면역 치료의 전략과 제제의 개발에 기여할 수 있을 것으로 생각된다.

Chamomile German 오일도포가 아토피성 피부염을 가진 NC/Nga 생쥐모델의 혈청 IgE와 IgG1양 변동에 미치는 영향 (Effect of Chamomile German Oil Application of IgG1 and IgE 1 to Atopic Dermatitis in NC/Nga Mice Model)

  • Atopic dermatitis is one of the most common skin disease in children and a characteristic chronically recurrent form of dermatitis with a hereditary tendency, affecting infants and may extend to the childhood or th the adult age. Environmental factors, stress, and foodstuffs such as milk, egg may cause atopic dermatitis. Nc/Nga mice were used as an animal model for human atopic dermatitis. The divided by 4group such as normal group, BMAC group, FK506 group, MR group for this study raised in conventional conditions. To investigate effect of Chamomile German on atopic dermatitis in NC/Nga mice, the serum IgE and IgG1 level were measured while the severity degree of the skin lesion was examined by the naked eyes of two volunteers who were unaware of the treatment status. The results were the followings. 1. The score on the severity degree of skin dermatitis in FK 506 and MR group was lower than that in control group. 2. The serum IgE level in control group was higher 25% than that in normal group. 3. The serum level of IgE in FK506 and MR group compares to control group was decreased. 4. The serum IgG1 level was decreased more than 3.5 times in FK506 compared to control group while MR group had significantly less the serum IgG1 than control group. From the above results, treatment of Chamomile German oil had the effect on atopic dermatitis in NC/Nga mice. If scientific researches on aroma oil are performed in various way, aroma oil will be used to cure skin dermatitis as a alternative therapy in the future.

한방치료를 통한 성인 전신성 아토피 피부염 치험 1례 - 오령산(五苓散) 사용 효과를 중점으로 (A case of Adult Whole-body Atopic Dermatitis treated by Traditional Korean Treatment - Focus on the effect of Oryeong-san)

  • Objetives : The purpose of this study is to report the effect of Oryeong-san on adult whole-body atopic dermatitis. Methods : We treated a 21-year-old male patient who have adult whole-body atopic dermatitis by Korean medicine. This treatment was enforced from 2019.5.25 to 2019.6.29. And we have confirmed the improvement of itching, rashes and oozing. We recorded photos that includes change of symptoms. Results : After the treatments was done, the grade of VAS and SCORAD was decreased. The VAS grade was decreased from grade 8 to grade 2. The SCORAD score was decreased from 72.1 to 52.1. And itching, rashes, oozing were clearly decreased. Conclusions : Oryeong-san has effect on decreasing itching, rashes and oozing. Also, sleeping time reduced due to itchness was normalized. Thus, It is able that Oryeong-san works on skin desease like atopic dermatitis.

Treatment of Atopic Dermatitis Associated with Malassezia sympodialis by Green Tea Extracts Bath Therapy: A Pilot Study

  • Multiple treatment modalities, including topical and systemic corticosteroid and phototherapy, have been used in treatment of patients with atopic dermatitis. However, long-term corticosteroid therapy may have various adverse effects. The purpose of this study was to investigate the therapeutic efficacy and safety of bath therapy using green tea extracts for treatment of patients with atopic dermatitis. A total of four patients with atopic dermatitis were enrolled in this study. A Malassezia multiplex detection kit was used in performance of multiplex PCR on clinical isolates, which confirmed Malassezia sympodialis. Subjects underwent treatment with bath therapy using green tea extracts three times per wk for a period of 4 wk. Assessment using the scoring atopic dermatitis (SCORAD) index, the visual analogue scale for pruritus, and transepidermal water loss was performed weekly. Laboratory tests were performed before and after treatment. All patients showed marked improvement on the mean SCORAD and visual analogue scale, and a significant decrease in the mean values of serum eosinophil counts was observed after treatment. Bath therapy with green tea extract is an effective, safe, and nonsteroidal therapy for treatment of patients with atopic dermatitis associated with Malassezia sympodialis.

이중탕(理中湯)이 IgE가 과잉생성되고 피부염이 유발된 NC/Nga생쥐에서 $IFN-{\gamma}$ 반응에 의한 Th2 세포분화 억제에 미치는 영향 (The Effect of Li Zhong Tang on the Suppression of Th2 Differentiation by $IFN-{\gamma}$ Response in IgE Hyperproduction and Atopic Dermatitis-like Skin Lesions Induced NC/Nga Mouse)

  • Objectives The purpose of this study is to investigate the effect of Li Zhong Tang(LZH-T) on atopic dermatitis by using NC/Nga atopic dermatitis mouse. Methods First, in vitro, we isolated B cells from NC/Nga mouse which induced atopic dermatitis-like skin for 18 weeks. We analyzed FACS by intracellular staining of $IFN-{\gamma}$, GATA-3+ and also analyzed cytokines by using real-time PCR. Secondly, in vivo, after administration of LZH-T to the 12 weeks old mouse with atopic dermatitis. We analyzed serum IgE, $IFN-{\gamma}$ level and observed the changes of activated cell. Results In vitro, LZH-T decreased the levels of CD4+/$IFN-{\gamma}$ and increased the levels of CD4+/GATA3+. In vivo, serum IgE levels were decreased and $IFN-{\gamma}$ levels were increased in LZH-T group compared to the control group. In PBMCs, the percentage of activated cell - granulocytes, CD3+, CD3+CD4+, B220+CD23+, and CCR3+ were decreased and CD19+, CD3+CD8+ were increased in LZH-T group compared to the control group. Conclusions This study demonstrates immunological activity of GPJST on atopic dermatitis-like model mice.

양혈제습탕(凉血除濕湯)이 DNCB로 유도된 NC/Nga mice의 아토피 피부염에 미치는 영향 (Effect of Yanghyeoljeseuptang (YHJST) Composition on Atopic Dermatitis NC/Nga Mice Induced by DNCB(dinitrochlorobenzene))

  • Objectives The purpose of this study is to investigate the effect of YHJST on atopic dermatitis in an experiment using an NC/Nga mice induced by DNCB, which has histological and clinical similarities to the condition in humans. Methods To investigate the effect of YHJST on atopic dermatitis(AD), we evaluated atopic dermatitis-like skin lesions by clinical skin index and analyzed immunological parameters in peripheral blood mononuclear cells(PBMCs) and performed skin histology in ears and dorsal skin of NC/Nga ato-mouse. Results YHJST medicines decreased Serum level of IgE, IL-6, TNF-$\alpha$. Also total number of $CD69^+$, $CD3^+$ in PBMCs, absolute cell number of $CCR3^+CD3^+$, $CD11b^+Gr-1^+$ in Dorsal skin tissue, Serum IgG1, IgM, IgG2a and IgG2b decreased significantly. Furthermore YHJST is extremely effective to histological symptoms; dermal and epidermal thickening, hyperkeratosis and inflammatory cell infiltration and suppressed histologic infiltration of $CD4^+$ & $CCR3^+$ in ear and dorsal skin lesions significantly. YHJST decreased gene-expression of IL-6, TNF-$\alpha$, CCR3, Eotaxin mRNA than that of control group. Conclusions YHJST on atopic dermatitis to atopic dermatitis NC/Nga mouse induced DNCB was incredibly effective.

내소화중탕가미방(內消和中湯加味方)이 아토피피부염 환아의 혈중 Cytokine 분비에 미치는 영향 (Effects of NaesowhajungtangGamibang of Serum Cytokines of patients with Atopic Dermatitis)

  • Objective : Based on the Th1/Th2 inbalance hypothesis, we aimed to evaluate the effects of NaesowhajungtongGamibang on serum $interferon{\gamma}$, interleukin-4, interleukin-5, of 19 patients with atopic dermatitis. Materials & Methods: The subjects are consisted of 19 patients with atopic dermatitis, and had been treated with NaesowhajungtongGamibang decoction for 45 days. Serum $IFN-{\gamma}$, IL-4, IL-5 level were checked before and 45 days after treatments. Results: The serum $IFN-{\gamma}$, IL-5, level in patients with atopic dermatitis were slightly decreased compared with normal control group, but had no statistical significance. The serum IL-4, level in patients with atopic dermatitis was slightly increased compared with normal control group, but had no statistical significance. After treatment with NaesowhajungtongGamibang for 45 days, serum $IFN-{\gamma}$, IL-5, level were increased, but had no statistical significance. After treatment with NaesowhajungtongGamibang for 45 days, serum IL-4 level were decreased, but had no statistical significance. Conclusion: This study shows that NaesowhajungtongGamibang decoction had effects were not correspond with Th1/Th2 inbalance hypothesis. Therefore study for other mechanism of NaesowhajungtongGamibang on atopic dermatitis is required.
