학령전기와 학령기 아동에서의 아토피 피부염 유병률과 위험인자 (Prevalence and Risk Factors for Atopic Dermatitis in Pre-school and School Aged Children)

  • Purpose: This study was done to estimate the prevalence of atopic dermatitis (AD) and its risk factors for AD in children living in the community. Method: Random samples of 10,236 were selected from 43 kindergarten (1,418) and 57 elementary (8,718) students in K city. Data from 1,079 (kindergarten children) and 7,271 (elementary) students were used in the final analysis. The Korean-translated modified version of the International Study of Asthma and Allergies in Childhood (ISSAC) questionnaire was used in this cross-sectional survey. Parents answered the survey questionnaire. Results: The lifetime and last 12-month prevalence of AD were 40.15%; 30.86% in kindergarten children and 33.56%; 25.37% in elementary children. The lifetime and last 12-month prevalence of AD diagnosis were 40.8%; 18.68% in kindergarten children and 34.36%; 12.63% in elementary children. The lifetime prevalence of AD treatment was 25.93% in kindergarten children and 22.07% in elementary children. Multivariate logistic regression analysis revealed that risk factors for AD were age, allergic disease, age of house, carpets, pets. Conclusion: The study suggests that prevalence of AD has increased among children in the community. These data can be used to manage possible risk factors that are predictors of childhood AD.

BALB/c 마우스에서 생약복합제의 아토피 치료 효능 (Effects of Herbal Complex on Atopic Dermatitis in BALB/c Mice)

  • Atopic dermatitis (AD) is a common chronic inflammatory disease of preceding the development of allergic disorders. The incidence of AD is increasing and it causes problems with administrative costs. Therefore, no side effects, easyto- use development of AD treatment is needed. The aim of this study was to evaluate whether PBMCAT, a functional extract from herbal complex was effective to treat the AD mice. AD was induced by patching ovoalbumin on the backside in BALB/ c mouse and then mice were topically treated with PBMCAT. Elidel $cream^{(R)}$ (pimecrolimus, PL) was used as a control. Scratching counts (SC) and clinical skin severity (CSS) were measured, and total serum IgE level was also measured. After inducing AD, SC and CSS were increased. The total serum IgE level was also increased in AD-induced mice. Treatment with PBMCAT significantly decreased SC, CSS, and serum IgE concentration in mice. Especially, treatment of PBMCAT 0.1% in BALB/c mice more effected than PL. These results suggest that the ointment of PBMCAT may enhance the process of AD healing by influencing phase of allergic reacting.

Diosmetin and Its Glycoside, Diosmin, Improve Atopic Dermatitis-Like Lesions in 2,4-Dinitrochlorobenzene-Induced Murine Models

  • Naturally derived diosmetin and its glycoside diosmin are known to be effective in treating inflammatory disease. This study was performed to determine whether diosmin and diosmetin have the effect of improving atopic dermatitis in a 2,4-dinitrochlorobenzen (DNCB)-induced atopic dermatitis (AD) model. DNCB was used to establish AD model in hairless mice. Skin moisture, serum immunoglobulin E (IgE), interleukin 4 (IL-4), and histological analysis were performed to measure the effectiveness of diosmin and diosmetine to improve AD. IL-4 levels were also measured in RBL-2H3 cells. Administration of diosmetin or diosmin orally inhibited the progress of DNCB-induced AD-like lesions in murine models by inhibiting transdermal water loss (TEWL) and increasing skin hydration. Diosmetin or diosmin treatment also reduced IgE and IL-4 levels in AD-induced hairless mouse serum samples. However, in the in vitro assay, only diosmetin, not diosmin, reduced the expression level of IL-4 mRNA in RBL-2H3 cells. Diosmin and diosmetine alleviated the altered epidermal thickness and immune cell infiltration in AD. Diosmin is considered effective in the cure of AD and skin inflammatory diseases by being converted into diosmetin in the body by pharmacokinetic metabolism. Thus, oral administration of diosmetin and diosmin might be a useful agent for the treatment of AD and cutaneous inflammatory diseases.

DNCB로 유발된 아토피 피부염 동물 모델에 대한 곽향정기산(藿香正氣散) 경구투여의 효과 (The Effects of Orally Administrated Gwakhyangjeonggi-san on DNCB-induced Atopic Dermatitis like Mice Model)

  • Objectives : This study was conducted to investigate the effects of Gwakhyangjeonggi-san(GJS) on atopic dermatitis(AD) induced by 2,4-dinitrochlorobenzene(DNCB) in mice. Methods : The mice(Balb/c mice) were divided into three groups; normal Balb/c mice with oil treatment(Sham group), DNCB-induced AD mice(AD group), and GJS treated AD mice(GJS group). GJS group were orally administered GJS daily for 2 weeks. We observed changes of clinical skin severity score, the expression of thymic stromal lymphopoietin(TSLP), interleukin(IL)-4 and tumor necrosis factor(TNF)-${\alpha}$ in skin and mast cell infiltration. Also, serum immunoglobulinE(IgE), IL-4, $TNF-{\alpha}$ and IL-6 were evaluated. Results : The clinical skin severity score of GJS group was decreased compared to AD group. In hematoxylin and eosin staining results, GJS group showed a significant reduction of epithelial skin thickness. In addition, expression of TSLP and mast cell infiltration in skin were also reduced by GJS treatment compared to those of AD group. Thus, we evaluated expression of IL-4, Th2-dependent cytokine, and $TNF-{\alpha}$, pro-inflammatory cytokine in skin. GJS significantly reduced both IL-4 and $TNF-{\alpha}$ compared to AD mice. Moreover, levels of IgE, IL-4, $TNF-{\alpha}$ and IL-6 in plasma also significantly decreased by oral GJS treatment. Conclusion : The present study suggests that GJS can significantly reduced symptoms of AD, therefore it can be a promising candidate for anti-atopic dermatitis treatment.

융합 LED 광선치료가 아토피 피부염에 미치는 영향 (Therapeutic Effects of LED Fusion of Two Wavelength Bands on Atopic Dermatitis of NC/Nga Mice)

  • 아토피 피부염(Atopic dermatitis, AD)은 주로 5세 이전에 발병하여 심한 가려움을 동반하는 만성 염증성 피부질환이다. 본 연구의 목적은 NC/Nga 마우스를 AD 유사 증상에 대한 405 nm+ 850 nm LED 광선 치료의 효과를 조사하는 것이다. 마우스는 Normal (Vehicle), 아토피성 피부염 유발군 (CON), 405 nm+ 850 nm LED 광선 치료군 (LED)으로 나누어 난괴법을 이용하여 무작위 배치하여 실험을 진행하였다. LED 실험군은 하루 10분씩 405 nm+ 850 nm 파장의 LED 치료를 7일 간 실시하였다. LED 광선치료 연구는 Dermatics 점수의 개선을 측정하고 피부염으로 인한 표피조직 두께 감소를 관찰하였다. LED 광선 요법으로 인한 혈청 IL-1𝛽의 현저한 감소와 경피 수분 손실 및 혈청 IgE 농도 결과를 바탕으로 LED 광선 치료는 아토피 유발 생쥐의 정상적인 피부 상태 회복에 도움을 주었다. 본 연구 결과는 아토피 피부염 마우스 모델에서 적외선 영역의 광선과 blue light 영역의 광선의 동시 조사치료가 아토피 피부염 치료에 뛰어난 효능을 가지며, 두 파장의 LED의 동시 사용 가능성에 대하여 시사하였다.

Therapeutic potential of traditional Korean music, Daegeum Sanjo in atopic dermatitis-like murine model

  • Atopic dermatitis (AD) is an allergic and inflammatory skin. Recently, the limitations and side effects of drug therapy, and possibility of alternative therapies, such as music therapy are emerging in the treatment of AD. Thus, the present study determined whether traditional Korean music, Daegeum Sanjo, regulates AD symptoms by comparing the rhythm, Jinyangjo-jangdan and Jungmori-jangdan in an AD-like murine model. Jinyangjo-jangdan and Jungmori-jangdan of Daegeum Sanjo reduced the duration of scratching behavior increased by DNFB challenge. Jinyangjo-jangdan and Jungmori-jangdan of Daegeum Sanjo attenuated clinical symptoms. However, Jinyangjo-jangdan and Jungmori-jangdan of Daegeum Sanjo did not inhibit IgE, histamine, interleukin (IL)-4, IL-6, or thymic stromal lymphopoietin levels in serum or AD-like skin lesions. In conclusion, the present study suggests that it is possible for Jinyangjo-jangdan and Jungmori-jangdan of Daegeum Sanjo to ameliorate AD symptoms. However, further study is needed to clarify significant mechanisms of Jinyangjo-jangdan and Jungmori-jangdan of Daegeum Sanjo therapy for AD symptoms.

Anti-atopic dermatitis properties of Cordyceps militaris on TNFα/IFNγ-stimulated HaCaT cells and experimentally induced atopic dermatitis in mice

  • [Purpose] This study evaluated the anti-atopic dermatitis (AD) properties of Cordyceps militaris (CM) aqueous extract in keratinocytes in vitro and in vivo. We investigated the nutraceutical composition of the CM extract, including its protein, carbohydrate, and selected phytochemical content. [Methods] The expression of pathogenic cytokines in keratinocytes was assayed using an in vitro model. The CM extract downregulated extracellular signalregulated kinase 1/2 (ERK1/2) and p38 kinase expression in TNFα/IFNγ-stimulated HaCaT cells. We also established an in vivo AD model by repeatedly exposing the ears of mice to local Dermatophagoides farinae extract (DFE; house dust mite extract) and 2,4-dinitrochlorobenzene (DNCB). The epidermal and dermal ear thickness, mast cell infiltration, and serum immunoglobulin levels were measured following a 4-week oral administration of the CM extract. [Results] Histopathological examination showed reduced epidermal/dermal thickness and mast cell infiltration in mouse ears. The CM extract also suppressed serum immunoglobulin levels and gene expression of T helper (Th)1/Th2 cytokines in mouse ear tissue. [Conclusion] These results suggest that the CM extract may be useful for the treatment of AD-like skin lesions.

The Effect of the Polygonum tinctoria Niram on Atopic Dermatitis in Dinitrochlorobenzene-Induced BALB/c Mice

  • In the present study, we investigated the effects of Polygonum tinctoria Niram (PTN) on atopic dermatitis (AD) in BALB/c mice induced by 2,4-dinitrochlorobenzene (DNCB). They were divided into four groups; Control, DNCB, DNCB+1%PTN (1% PTN extract) and DNCB+5%PTN (5% PTN extract), for evaluating the change of appearance of skin surface, skin hydration, thickness of epidermis and mast cell numbers during 4 weeks. PTN suppressed symptoms of AD in appearance of skin and increased skin hydration for DNCB+1%PTN and DNCB+5%PTN. Treatment with PTN significantly decreased the levels of eosinophils. In histopathological examination, DNCB+1%PTN and DNCB+5%PTN significantly reduced the thickness of epidermis and number of mast cell in dermis. These results suggested that the PTN improved symptoms of AD in BALB/c mice.

두시(豆豉) 추출물을 이용한 하태독법(下胎毒法)이 NC/Nga 생쥐에서 유발된 아토피 유도 피부염에 미치는 항염증 효과 (The Anti-inflammatory Effects of Hataedock Taken Douchi Extracts on Atopic Dermatitis-like Skin Lesion of NC/Nga Mouse)

  • Objectives Hataedock is a Korean herbal medical oral treatment that removes fetal toxic heat and meconium from new born babies. The purpose of this study is to evaluate whether Hataedock treatment of Duchi extracts has anti-inflammation effects on AD (Atopic Dermatitis)-induced NC/Nga mice. Methods After Hataedock treatment of Duchi extracts on days 0, 3-week-old NC/Nga mice were sensitized on days 28, 35, 42 by exposure of DNFB (dinitrochlorobenzene) and were induced to have AD. Immunohistochemistry of NF-${\kappa}B$ p65, iNOS, COX-2 and TUNEL assay of apoptotic body was used to identify changes of skin damages and anti-inflammation effects. Results The alleviate effect of the skin damage and angiogenesis was observed in DT group. The damage of stratum corneum, hyperplasia, edema, infiltration of lymphocytes and distribution of capillary were decreased in DT group. Also, the study results suggested that Hataedock treatment of Duchi extracts in DT group remarkably downregulated levels of NF-${\kappa}B$ p65 by 70% (p < 0.001), as well as COX-2 by 51%, iNOS by 62% (p < 0.001). Additionally, Hataedock treatment of Duchi extracts in DT group up-regulated apoptosis of inflammatory cells by 68% in atopic dermatitis-like skin lesion. Conclusions From the study results, we observed that Hataedock treatment of Duchi extracts alleviates AD through diminishing various inflammatory cytokines in the skin lesions, which are involved in the initial steps of AD development. It is anticipated to have potential applications for prevention and treatment of atopic dermatitis.

한방병원에서 입원치료를 시행한 중증 아토피피부염 환자의 예후지표 탐색 (Prognostic Factors of Inpatients with Severe Atopic Dermatitis Treated in a Traditional Korean Medicine Hospital)

  • Objectives : This study was aimed to find significant prognostic factor of inpatients with severe atopic dermatitis (AD) who treated in a Traditional Korean Medicine (TKM) hospital. Methods : A retrospective review of medical records of 39 patients with severe AD who hospitalized for more than 7days was performed. All patients were treated with acupuncture, herbal decoction and herbal wet wrap dressing. Therapeutic effect was assessed by difference of objective SCORing of Atopic Dermatitis index (${\Delta}OSI$) at admission and at discharge. Patients were divided into two groups: improvement group (${\Delta}OSI{\leq}5$) and no-improvement group (${\Delta}OSI>5$), and clinical characteristics, severity, infectious complication, serum total IgE, eosinophil counts, adherence and concomitant treatment were investigated. Results : There were 7 cases in no-improvement group. There was no significant difference in OSI or infectious complication between the two groups. Also, there was no significant difference in total serum IgE and eosinophil counts, which are known to be related to severity of AD. However, the proportion of adherence group was significantly lower in no-improvement group than improvement group. Conclusions : This study suggest that treatment adherence is a significant prognostic factor for treatment outcome in inpatients with severe atopic dermatitis. If the adherence to treatment is improved, the therapeutic effect of hospitalization would increase in spite of severe AD or infectious complication.
