Comparison of removal torques of SLActive® implant and blasted, laser-treated titanium implant in rabbit tibia bone healed with concentrated growth factor application

  • PURPOSE. The purpose of this study was to compare the removal torques of a chemically modified SLActive implant and a blasted, laser-treated (BLT) implant, which were soaked in saline for 2 weeks after their surface modifications. The removal torques of the two implants were measured 4 weeks after their implantation into the bone defect area in rabbit tibias with concentrated growth factor (CGF) application. MATERIALS AND METHODS. To make artificial bone defects in the cortical layers of both tibias, an 8-mm diameter trephine bur was used. Then, prepared CGF was applied to the bony defect of the left tibia, and the bony defect of the right tibia was left unfilled. Four weeks later, the surgical sites of 16 rabbits were re-exposed. For 8 rabbits, the SLActive implants (Straumann, Switzerland) were inserted in the left tibia, and the BLT implants (CSM implant, Daegu, Korea) were inserted in the right tibia. For other rabbits, the BLT implants were inserted in the left tibia, and the SLActive implants were inserted in the right. Four weeks afger the insertion, torque removal was measured from 4 rabbits exterminated via $CO_2$ inhalation. RESULTS. No significant difference was observed between removal torques of the BLT implant and the SLActive implant (P>.05). CONCLUSION. It was found that BLT surface modification exhibited excellent osseointegration. In addition, CGF application did not affect the insertion and removal torque of the implants.

  • Tooth restoration with implant placement have been interested in modern veterinary dentistry. It is important to reduce the interval between the tooth extraction and the insertion of the implants, to determine the restoration time after implant placement in dogs The aim of the this study was to compare the bone mineral apposition rate of immediately loaded implants with an unloaded control during the early healing state in the artificial extraction mandible. In four Beagles, two premolar sockets (PM2 and PM3) of the both sides of the mandible were installed with a one stage titanium mini-implant with a length of 11 mm and a diameter of 3.37 mm immediately after tooth extraction. Provisional restroration was given to implants, 3 weeks after implant placement in the left side (immediate loading) and 12 weeks in the right side (delayed loading), respectively. Radiographic and histologic examinations were performed. Radiographic evaluation revealed no significant difference between two groups. Bone-implant contact was $47.1{\pm}8.9$ in immediate loading and $50.2{\pm}8.2$ in delayed loading, interthread bone density was $78.7{\pm}10.9$ in immediate loading and $73.1{\pm}15.5$ in delayed loading, and peri-implant bone density was $95.4{\pm}7.1$ in immediate loading and $95.0{\pm}5.2$ in delayed loading, respectively. Three histologic analyses showed no significant difference between delayed and immediate loading. Followed by this study, the immediate loading of implants insterted into fresh extraction sockets after tooth extraction could be considered in veterinary dentistry.

Transfer learning in a deep convolutional neural network for implant fixture classification: A pilot study

  • Purpose: This study aimed to evaluate the performance of transfer learning in a deep convolutional neural network for classifying implant fixtures. Materials and Methods: Periapical radiographs of implant fixtures obtained using the Superline (Dentium Co. Ltd., Seoul, Korea), TS III(Osstem Implant Co. Ltd., Seoul, Korea), and Bone Level Implant(Institut Straumann AG, Basel, Switzerland) systems were selected from patients who underwent dental implant treatment. All 355 implant fixtures comprised the total dataset and were annotated with the name of the system. The total dataset was split into a training dataset and a test dataset at a ratio of 8 to 2, respectively. YOLOv3 (You Only Look Once version 3, available at https://pjreddie.com/darknet/yolo/), a deep convolutional neural network that has been pretrained with a large image dataset of objects, was used to train the model to classify fixtures in periapical images, in a process called transfer learning. This network was trained with the training dataset for 100, 200, and 300 epochs. Using the test dataset, the performance of the network was evaluated in terms of sensitivity, specificity, and accuracy. Results: When YOLOv3 was trained for 200 epochs, the sensitivity, specificity, accuracy, and confidence score were the highest for all systems, with overall results of 94.4%, 97.9%, 96.7%, and 0.75, respectively. The network showed the best performance in classifying Bone Level Implant fixtures, with 100.0% sensitivity, specificity, and accuracy. Conclusion: Through transfer learning, high performance could be achieved with YOLOv3, even using a small amount of data.

Sol-Gel법으로 HA코팅된 치과용 임플란트 합금의 표면특성 (Surface Characteristics of HA Coated Dental Implant Alloy by Sol-Gel Method)

  • Surface characteristics of HA(hydroxyapatite) coated dental implant alloy by Sol-Gel method were investigated using potentiostat, ICP, SEM, EDX, EPMA and surface roughness tester. Surface roughness of HA coated specimen by Sol-Gel showed higher than that of PVD coated specimen. Corrosion resistance increased in the order of $1\%$ lactic acid, artificial saliva, $0.5\%$ HCI and $0.9\%$ NaCl solution. Amount of Ca element release was higher than that of V and P in the $0.5\%$ HCI and $0.9\%$ NaCl solution.

  • This study was to analyze the stress distribution of implant and supporting tissue in $Br{\aa}nemark$ osseointegration implant. The analysis has been conducted by using the axisymmetric finite element method and type of model according to crown material. Tests have been performed at 1 kg load on central fossa of crown portion. Each type of model was designed differently according to crown material. 1) Porcelain fused to metal crown(Model A) 2) Composite resin veneered crown(Model B) 3) Acrylic resin veneered crown(Model C) 4) Type III gold crown(Model D) The displacements and stresses of implant and supporting structures were analyzed to investigate the influence of the type of crown material. The results were obtained as follows : 1. Displacement of implant was shown uniformly downward displacement in all models and abutments were observed distally downward displacement. 2. In supporting tissues, stress was concentrated on the crest of compact bone and the spongy bone below implant. 3. The PFM and the type III gold crown showed the largest concentration of stress at the crest of compact bone and the spongy bone below implant, respectively. Acrylic resin artificial teeth and composite resin veneered crown indicated almost the same distribution of stress. 4. The gold screw, the abutment screw and the top of abutment showed the concentration of stress in implants of every model.

The reliability of Anycheck device related to healing abutment diameter

  • PURPOSE. The purpose of this in vitro study was to examine the reliability of the Anycheck device and the effect of the healing abutment diameter on the Anycheck values (implant stability test, IST). MATERIALS AND METHODS. Thirty implants were placed into three artificial bone blocks with 10 Ncm, 15 Ncm, and 35 Ncm insertion torque value (ITV), respectively (n = 10). (1) The implant stability was measured with three different kinds of devices (Periotest M, Osstell ISQ Mentor, and Anycheck). (2) Five different diameters (4.0, 4.5, 4.8, 5.5, and 6.0 mm) of healing abutments of the same height were connected to the implants and the implant stability was measured four times in different directions with Anycheck. The measured mean values were statistically analyzed. RESULTS. The correlation coefficient between the mean implant stability quotient (ISQ) and IST value was 0.981 (P<.01) and the correlation coefficient between the meant periotest value (PTV) and IST value was -0.931 (P<.01). There were no statistically significant differences among the IST values with different healing abutment diameters. CONCLUSION. There was a strong correlation between the Periotest M and Anycheck values and between the ISQ and IST. The diameter of the healing abutment had no effect on the Anycheck values.

자연감 있는 임플란트 보철물 (Naturalization of Implant Prostheses)

  • 임플란트 술식을 이용하여 소실된 구강조직을 수복하는 방법은 보철적 수복 술식중 중요한 부분을 차지하고 있다. 구강수복의 목적은 손상된 구강조직의 기능성, 심미, 발음 등을 회복하는 데 있다. 최근 환자, 치과의사, 기공사들의 심미성에 대한 요구가 증가하는 추세이므로 임플란트 보철수복의 자연미에 대한 관심 또한 고조되고 있다. 최종 보철물의 심미적인 결과에는 형태, 색조, 비율, 잇몸의 형태 등등의 많은 요소가 관여한다. 자연감과 생동감이 있는 인공보철물을 환자에게 제공해 주기 위하여 주의 깊은 진단 및 치료계획과 이를 뒷받침하는 기공사의 역할이 필수적이다. 자연감 있게 보이는 임플란트 보철물을 제공하기 위하여 재료, 잇몸의 재현, 형태 및 발음, 보철물의 design 등에 관하여 screw type, cement type, 그리고 overdentuure 등의 몇 가지 증례를 통하여 살펴보고자 한다.

임플란트용 실링 어버트먼트의 개발 및 구조해석을 통한 성능분석 (Development of a Sealing-Type Abutment for Implant and the Performance Evaluation via Structural Analysis)

  • Currently, dental implants are widely used as artificial teeth due to their good chewing performance and long life cycle. Generally, a dental implant consists of an abutment as the upper part and a fixture as the lower part. When chewing forces are repeatedly applied to a dental implant, a gap is often generated at the interfacial surface between the abutment and the fixture, and it results in some deterioration such as loosening of the fastening screw, dental retraction and fixture fracture. To enhance the sealing performance for coping with such problems, this study proposes a new sealing-type abutment having a number of grooves along the conical surface circumference, and it carries out finite element analysis in consideration of the external chewing force and pretension between the abutment and the fixture. The result shows that the proposed sealing-type abutment shows an enhanced sealing performance in terms of the contact pressure in comparison with a conventional abutment.

임플란트 healing abutment 높이와 타진각도에 따른 타진방식 임플란트 안정성 측정기기의 수치 차이 (Differences in percussion-type measurements of implant stability according to height of healing abutments and measurement angle)

  • 목적: 인조골에 동일한 조건으로 식립한 임플란트에 대하여 타진방식의 임플란트 안정성 측정기기인 Periotest와 AnyCheck을 사용하여 안정성 수치를 측정함에 있어서, healing abutment의 높이와 타진각도의 변화에 따른 각 측정기기의 안정성 수치 차이를 조사하는 것이다. 재료 및 방법: 총 60개의 임플란트를 주문제작한 인조골 블록에 식립하고 2, 3, 4, 5 mm 높이의 healing abutment를 각각 15개씩 네 그룹으로 나누어 체결하였으며, 각각 식립 토크, implant stability test, Periotest value를 측정했다. 임플란트 식립 시 45 - 55 Ncm의 토크를 유지하였다. 임플란트 장축에 수직인 지면을 기준으로, implant stability test는 AnyCheck을 사용하여 $0^{\circ}$, $30^{\circ}$의 기울기로 측정했으며 Periotest value는 Periotest M을 사용하여 $0^{\circ}$, $30^{\circ}$의 기울기로 측정하였다. 측정값은 통계적으로 비교 분석하였다. 결과: 식립 토크는 그룹 간 통계적 유의차가 없었다. AnyCheck과 Periotest M으로 측정했을 때, healing abutment의 높이가 증가할수록 안정성 수치가 낮게 측정되었다. 또한 AnyCheck과 Periotest M을 $0^{\circ}$$30^{\circ}$ 기울기로 측정 시 $0^{\circ}$로 측정한 그룹에서 안정성 수치가 더 낮게 측정되었다. 결론: 임플란트의 안정성 수치를 타진방식으로 측정할 때에는 healing abutment 높이와 타진각도의 변화에 따른 차이를 고려하여 측정해야 표준화된 평가를 할 수 있으며, 이에 대한 더욱 세밀한 사용법을 인지할 필요가 있다고 사료되었다.
