Voice Rehabilitation Other than Tracheo - Esophageal Shunt Method - (후두적출자의 음성재활 - 기관식도천자법 이외의 방법 -)

  • The problem of voice restoration after total laryngectomy has existed ever since Billroth's first total laryngectomy in 1873. Since then, all the efforts to restore the voice was tried to divert the tracheal air to the pharynx to produce voice, which became the tracheo-esophageal shunt voice currently used. With the intact pharyngoesophagus, however, there are two basic options for speech rehabilitation : the artificial larynx and esophageal voice. The artificial larynx is an electrically driven buzzer or a sound transducer and its most common type is placed against a supple point on patient's neck and introduces a mechanical sound into the tissues and air spaces of the neck. This sound, emanating form the mouth, is articulated by the intact structures of the remaining vocal tract as understandable speech. Esophageal voice is a commonly recommended method for alaryngeal speech rehabilitation, which can be successfully done by regurgitating the air stored in the esophagus. Successful esophageal voice is preferable to the artificial larynx but, most patients usually adapt only one of those methods according to their needs and feasibility to learn.

The Design of an Artificial Larynx Controlled by the EMG (근전위 제어형 인공후두의 설계)

  • In this Paper, we developed an electrolarynx controlled by the EMG(electromyography) of the sternohyoid muscle, and rested the property of the new electrolarynx while normal persons with his own larynx use it. for the examination of property of the developed electrolarynx, our researchers performed three different experiments. The first experiment was tested whether that on/off control of the vibrator of the electrolarynx is synchronized with the activity of the sternohyoid EMG. In the second experiment, it was tested when the lower amplitude of the sternohyoid EMG is produced, whether the higher pitch of the electrolarynx is produced, and vice versa. The third experiment was tested the probability that the electrolarynx can produce the voiceless sound. As the results, we found that the developed electrolarynx had the good produce of the on/off vibrator control and the pitch control of the electrolarynx. Also, we ascertained the possibility that it can produce the voiceless sound.

Development of Neck-Type Electrolarynx Blueton and Acoustic Characteristic Analysis (경부형 전기인공후두 Blueton의 개발과 음향학적 성능 분석)

  • Electrolarynx(EL), battery operated vibrators which are held against the neck by on-off button, has been widely used as a verbal communication method among post-laryngectomized patients. EL speech can produce easily without need of any additional surgery or special training and be used with any other methods. This institute developed a neck-typed EL named "Blueton" in commperation with EL Company Linkus, which consists of 3 parts : Vibrator part, Control part, Battery part. In this study we evaluated the acoustic characteristics of the produced voices by Blueton compared with Servox-inton using MDVP. Three EL users (2 full time users, 1 part time user) were participated. The results revelaed that NHR higher in Servox than Blueton and intensity is higher in Blueton than Servox. The spectra for vowels produced by EL speakers are mixed signals combined with talkers' vocal output and electrolarynx noise. The spectra pattern is similar with two ELs. High, SPI index and vowel spectra from MDVP demonstrated characteristics of both electrolarynxes related to noise signal. This finding suggests that Blueton helps to provide one of useful rehabilitation options in the post laryngectomy patients.

A Case Report of Huge Oral Verrucous Carcinoma in Oral Cavity

  • Verrucous carcinoma is a distinctive form of low-grade squamous cell carcinoma. The tumor has an exophytic, cauliflower-like appearance, and usually occurs in males and tends to affect individuals over 60 years of age. It typically involves the oral cavity, larynx, esophagus and skin. Most intraoral cases involve the mandibular vestibule, the buccal mucosa and the hard palate. The cause is unknown, but most verrucous carcinomas arise from the oral mucosa in people who chronically use chewing tobacco or snuff. The treatment of oral verrucous carcinoma remains controversial. Whenever possible, surgically total excision and skin or mucosal grafting is recommended. After total excision of huge verrucous carcinoma,instead of dermal or mucosal grafting, we used artificial dermis silicone membrane. We had a good result without recurrence and present this case.

  • Recently, several deep learning studies have been reported to automatically identify the location of diagnostic devices using endoscopic data. In previous studies, there was no design to determine whether the configuration of the dataset resulted in differences in the accuracy in which artificial intelligence models perform image classification. Studies that are based on large amounts of data are likely to have different results depending on the composition of the dataset or its proportion. In this study, we intended to determine the existence and extent of accuracy according to the composition of the dataset by compiling it into three main types using larynx, esophagus, gastroscopy, and laryngeal endoscopy images.

An acoustic study on the alaryngeal voice using the Multi-Speech (Multi-Speech를 통한 후두적출자의 발성에 대한 음향학적 분석)

  • The purpose of this study was to provide acoustic data on the voice of the laryngectomized patients for more scientific and efficient voice rehabilitation. The phonation of prolonged /a/ of 9 electronic artificial larynx(AL) users, 5 esophageal(EP) speech users, and 2 tracheo-esophageal(TEP) voice users were recorded and analyzed using Multi-Speech. Habitual f0, mean f0, sd f0, max f0, min f0, jitter, shimmer, and NHR were compared among groups of subjects using t-test. The EP and TEP groups exhibited higher f0 compared to the AL group. The AL and TEP groups showed more stable f0 than the EP group. In addition, the quality of TEP and EP voices were comparatively better in terms of jitter, shimmer, and NHR.
