  • Discoid lateral meniscus is a well-known entity of the knee whereas Wrisberg ligament-type discoid lateral meniscus is extremely rare. This is a report of a case of bilateral discoid lateral meniscus with peripheral detachment.

  • 고관절경을 시행하는 집도의사는 관절경을 통해 관찰되는 정상 고관절의 해부학적 구조물에 대한 정확한 지식과 삽입구 위치에 대한 숙지를 통해 환자의 증상과 비정상 관절경 소견을 임상적으로 연관 시키는 것이 중요하며, 적절한 환자의 선택과 고관절경의 술기에 대한 항상성을 유지할 수 있어야 최소 침습적인 술기의 장점을 가지고 있는 고관절경의 만족할 만한 치료 결과를 기대할 수 있다.

  • Several reports have described the increased perioperative risks as well as surgical techniques for performing various procedures in obese patients. However, few reports have addressed the effects of obesity on routine arthroscopic procedures known to be common and safe. Describing the case of a partial meniscectomy performed in a 17-year-old male with a body weight of 120 kg, we review literatures published to the present and provide technical problems and possible complications that can occur when performing routine knee arthroscopy in bariatric patients.

Use of Subtalar Arthroscopy in Intra-Articular Calcaneus Fractures (관절 내 종골 골절에서 거골하 관절경술의 이용)

  • Displaced intra-articular calcaneal fractures are difficult to treat because of complex anatomy and high soft tissue complications. Various surgical approaches have been introduced to solve these problems, but the treatment remains complex. Recently, clinically and radiographically superior results were reported using a subtalar arthroscopy in reducing the posterior facet in both percutaneous and open approaches. In the percutaneous approach, the arthroscopically assistant percutaneous approach must be selected carefully for mild-to-moderately displaced fractures because of the limited view. In the open approach, there is little evidence of the utility of subtalar arthroscopy. Therefore, intraoperative arthroscopy should always be used in conjunction with fluoroscopy to achieve reduction and assess the internal fixation placement.

The Role of Ankle Arthroscopy in Patients with Chronic Ankle Pain (만성 족관절통 환자에서의 족관절 관절경 소견)

  • The purpose of this study is to evaluate the significance of arthroscopic examination in patients with chronic ankle pain. Eighty eight arthroscopic procedures were conducted due to symptoms of chronic ankle pain. The pathology of the 88 ankles could be categorized into four groups ; 22 anterior impingement syndrome, 20 anterolateal impingement syndrome, 22 instability and 20 osteochondral lesion, 59 of patients(67.0%) had trauma history and 14(15.9%) of patients had injured chronic repetitive microtrauma. During the arthroscopic procedure, we found unexpected chondral lesions of high incidence(39.1%). These lesions were not detected both by the physical examination and radiologic examinantion. These pathologies were treated during the course of arthroscopic procedure. We concluded that ankle arthroscopy may be a very useful and therapeutic tool in the patients who have not reponded to the conservative treatment. And due to high incidence of intra-articular pathology in patients with chronic ankle pain, arthroscopic examination should be performed prior to final decision.
