Factors Affecting Recovery of Spontaneous Circulation in Patients Before Cardiac Arrest in Emergency Department: 2012~2016 Focused on Medical Records Data (응급실 도착 전 심정지 환자의 자발순환 회복 여부에 미치는 요인 -2012~2016 의무기록 자료를 중심으로-)

  • The purpose of this study was to ascertain whether or not to recover the spontaneous circulation of patients with cardiac arrest before arrival in the emergency room for 5 years (2012 to 2016), and try to investigate the factors affecting this. In this research, we used the "raw material for acute cardiac arrest survey" conducted in "Disease management headquarters" from 2012 to 2016 for the whole country of our country as the main material. In this study, 136,212 cardiac arrest patients were analyzed in the study data of the cardiac arrest in the country during the 5 years from 2012 to 2016. We performed a Chi-square analysis to analyze the recovery of spontaneous circulation before arrival in the emergency room according to general characteristics, social·demographic characteristics, and developmental characteristics. and We performed a Binary logistic regression analysis to determine the factors affecting the recovery of spontaneous circulation. The analysis results of this study show that whether CPR sustained transport before endoplasmic reticulum arrival, whether to witness an acute cardiac arrest before arrival in the emergency room, the type of general cardiopulmonary resuscitation, the location of acute cardiac arrest, the acute heart Causes of stoppage cause factor of whether spontaneous circulation recovery recovers before arrival of the endoplasmic reticulum(P<0.001, P<0.01). Therefore, it is necessary to strengthen systematic government health policy implementation and dissemination and health education focusing on factors that affect recovery of spontaneous circulation of cardiac arrest patients.

The Effects of a Trauma Team Approach on the Management of Open Extremity Fractures in Polytrauma Patients: A Retrospective Comparative Study

  • Purpose: Open extremity fractures require prompt antibiotic medication and initial debridement surgery to reduce the infection rate and restore functional stabilization. We aimed to report the effects and positive outcomes of a trauma team approach on the management of open extremity fractures in polytrauma patients. Methods: This retrospective review included all polytrauma patients with open extremity fractures admitted between March 2009 and December 2019. Patients were divided into two groups according to whether they were treated before or after the implementation of the trauma team approach (March 2014). We analyzed the outcomes in each group with respect to the time interval until the doctor's arrival, total length of stay in the emergency department, the time interval until initial antibiotic treatment and operation, whether the initial operation was performed within 24 hours, and the rate of deep infections. Results: A total of 123 patients met the inclusion criteria. There were no statistically significant differences in demographic characteristics. The time interval until the doctor's arrival (64.12±49.2 minutes vs. 19.82±15.23 minutes; p=0.035) and initial antibiotic treatment (115.47±72.12 minutes vs. 48.78±30.12 minutes; p=0.023) significantly improved after implementing the trauma team approach. The union rate was not significantly different. However, the time interval until initial debridement, opportunity for initial debridement within 24 hours, and the rate of deep infections demonstrated better results. Conclusions: The reduced time interval until initial antibiotic treatment and debridement could be attributed to the positive effect of the trauma team approach on the management of open extremity fractures in polytrauma patients.

  • Various studies suggest that the initial operating form of UAM, which is being presented as a solution to the urban traffic problem, will be similar to VTOL aircraft among current aircraft. In a form similar to determining the direction of the runway where fixed-wing aircraft take off and land, the vertiport where take-off and landing of VTOL aircraft takes place determines the flight direction of departure and arrival in consideration of the direction of the wind. Unlike areas where airports are generally built, in the case of downtown areas, it is expected that the characteristics of wind may continuously change depending on the environment of changing terrain or obstacles such as the construction of new buildings. In this study, long-term actual observation data for reviewing the take-off and landing directions at the city center where the location of the vertiport is expected are compared using a wind speed map, and the characteristics of the ground wind and the possibility of change in the direction of the predominant wind depending on the observation period and observation location confirmed.

The Impact of Living Alone on the Transfer and Treatment Stages of Acute Ischemic Stroke in the Busan Metropolitan Area (부산권역 급성 허혈성 뇌졸중 환자 이송 및 치료단계에서 독거가 미치는 영향)

  • Background: This study aimed to analyze the prehospital process and reperfusion therapy process of acute ischemic stroke in Busan metropolitan area and examine the impact of living arrangement on the early management and functional outcomes of acute ischemic stroke (AIS). Methods: The patients who diagnosed with AIS and received reperfusion therapy at the Busan Regional Cardiovascular Center between September 2020 and May 2023 were selected. We investigated the patients' hospital arrival time (onset to door time) and utilization of 119 emergency ambulance services. Additionally, various time matrices related to reperfusion therapy after hospital were examined, along with the functional outcome at the 90-day after treatment. Results: Among the 753 AIS patients who underwent reperfusion therapy, 166 individuals (22.1%) were living alone. AIS patients living alone experienced significant delays in symptom detection (p<0.05) and hospital arrival compared to AIS patients with cohabitants (370.1 minutes vs. 210.2 minutes, p<0.001). There were no significant differences between the two groups in terms of 119 ambulance utilization and time metrics related with the reperfusion therapy. Independent predictors of prognosis in AIS patients were found to be age above 70, National Institutes of Health Stroke Scale score at admission, tissue plasminogen activator, living alone (odds ratio [OR], 1.785; 95% confidence interval [CI], 1.155-2.760) and interhospital transfer (OR, 1.898; 95% CI, 1.152-3.127). Delay in identification of AIS was shown significant correlation (OR, 2.440; 95% CI, 1.070-5.561) at living alone patients. Conclusion: This study revealed that AIS patients living alone in the Busan metropolitan region, requiring endovascular treatment, face challenges in the pre-hospital phase, which significantly impact their prognosis.

  • In this paper, we propose a web GPS based logistics vehicle control management system using MVC design patterns. The proposed system is designed by applying design patterns of object oriented modeling called mini-architecture to enhance reliability of software as well as promote stability of overall system design. In addition, we can get a position information by means of the GPS embedded in PDA and communicate between client and monitoring server using CDMA network so that the position of client can be identified directly by the map service. The system provides an moving object indexing technique which extends the existing TB-tree to manage and retrieve a transporting trajectory of logistics efficiently. Finally, with development of the logistics vehicle control service called WG-LOGICS system, we can verify the usefulness of our system which is able for monitoring a vehicle preparation, allocating registration, loading a burden, transfer path, and destination arrival in real world.

  • The automated material handling systems today are playing ever more important roles in semiconductor/LCD fabrication facilities. Recently they became more flexible, intelligent, and speedy than in the past. The facilities have been fully automated because the size and weight of the unit loads used in the facilities were being increased beyond the limits that a human operator can handle. This research develops an efficient procedure to streamline the delivery of unit loads by the automated material handling system (AMHS). For this task, the research employs the event scheduling theory that has been successfully used in the both academia and industry. The developed procedure was applied to an actual LCD fabrication facility and improved the performance of an existing material handling system.

  • This study investigates two basic operations of mobility management of PCNs (Personal Communication Networks), i.e., the location update and the paging of the mobile terminal. From the realistic consideration that a user either moves through several cells consecutively or stays in a cell with long time, we model the mobility pattern by introducing two types of CRT (Cell Residence Time). Mobility patterns of the mobile terminal are classified Into various ways by using the ratios of two types of CRT. Cost analysis is performed for distance-based and movement-based location update schemes combined with blanket polling paging and selective paging scheme. It is demonstrated that in a certain condition of mobility pattern and call arrival pattern, 2-state CRT model produces different optimal threshold and so, is more effective than IID ( Independently-Identically-Distributed) CRT model. An analytical model for the new CRT model is compact and easily extendable to the other location update schemes.

  • Recently, owing to the increasing demand on the simplification of arrival and departure procedures, IMO's (International Maritime Organization) Facilitation Committee (FAL) is carrying out the standardization project of arrival and departure formalities and clearance form. Also, many port authorities of developed countries are making active researches for the smooth flow and efficiency of the information inbound and outbound ships by way of simplifying their formalities or through electronic means. However, this standardization project cannot be done by one country but by mutual cooperation among related nations. And to carry out this task, the first thing to be done is to standardize the formalities and document form, and to integrate information. To this end, this study has reviewed the model cases of advanced ports of developed countries with regard to their simplification and standardization efforts. And also we have analyzed the formalities and clearance form of the three countries Korea, China, and Japan. And then for the solution of common problems of three countries, this paper has suggested an ebXML-based Global Port B2B framework. Through this framework, we can reuse and automate the necessary information on the arrival and departure of ships, consequently realizing simplification, and laying a foundation for the introduction of e-commerce to the port industry.

A Study on the Heuristic-Based Yard Crane Scheduling Method Using Truck Arrival Information (트럭 도착 정보를 활용한 휴리스틱 기반 야드 크레인 스케줄링 방법)

  • Literatures have considered mathematical model that change the job order of shipper for improving the operation time of yard crane. However, on the real site, it is impossible to change the job order decided according to the shipper's arrival order. Therefore, operation managers have been utilized the relatively simple strategy that job control is better but the process time of yard crane is longer due to the growth of yard crane's interference time and empty drive time. This study proposed a new yard-crane scheduling approach that decided the job order before the shipper's truck arrived the yard terminal. We utilize the Container Pre-Information Notice estimating the arrival time of truck. We developed the container terminal simulation model for validation of the effect of proposed scheduling approach. The results show that the proposed scheduling reduced the interference delay time and empty moving time of yard crane and shipper's truck delay time.

Proposed STAR Procedure of Incheon International Airport Considering Safety and Efficiency (인천공항 도착항공기의 안전 및 효율 향상을 위한 표준접근절차 수정방안 연구)

  • Since continuous descent operations (CDO) is one of several tools available to aircraft operators and air navigation service providers (ANSPs) to increase safety, flight predictability, and airspace capacity while reducing noise, controller-pilot communications, fuel burn and emissions, widespread implementation of CDO would result in significant reductions in the environmental impact and aircraft operation costs in south korea as well. After analyzing each procedure from standard terminal arrival routes used for the Incheon international airport, it can be noticed that one of the procedures has a relatively high altitude constraint at initial approach fix than others, which lead the pilots to use unnecessary drag device in certain situations. Therefore we came to a conclusion that some arrival procedures need to be revised, so unnecessary procedure required during approach can be minimized, thereby reducing fuel consumption, noise and emissions compared to current approach procedures. And it is going to increase the safety margin significantly during approach phase due to reduced workload.
