로봇-보조 팔 훈련이 뇌졸중 환자의 팔에 근활성도와 체중지지에 미치는 영향 (Effects of Robot-Assisted Arm Training on Muscle Activity of Arm and Weight Bearing in Stroke Patients)

  • Background: This study investigated the effect of robot-assisted arm training on muscle activity of arm and weight bearing in stroke patients. Methods: The study subjects were selected 20 stroke patients who met the selection criteria. 10 people in the robot-assisted arm training group and 10 people in the task-oriented arm training group were randomly assigned. The experimental group performed robot-assisted arm training, and the control group performed task-oriented arm training for 6 weeks, 5 days a week, 30 minutes a day. The measurement tools included surface electromyography and smart insole system. Data were analyzed using independent sample t-test and the paired sample t-test. Results: Comparing the muscle activity of arm within the group, the experimental group and the control group showed significant differences in muscle activity in the biceps brachii, triceps brachii, anterior deltoid, upper trapezius, middle trapezius, and lower trapezius. Comparing the muscle activity of arms between the groups, the experimental group showed significant difference in all muscle activity of arm compared to the control group. Comparing the weight bearing within the groups, the experimental group showed significant difference in the affected side and non-affected side weight bearings and there were significant differences in anterior and posterior weight bearing. The control group showed significant difference only in the non-affected side weight bearing. Comparing the weight bearings between groups, the experimental group showed significant difference in the affected side and non-affected side weight bearings compared to the control group. Conclusion: This study confirmed that robot-assisted arm training applied to stroke patients for 6 weeks significantly improved muscle activity of arm and weight bearing. Based on these results, it is considered that robot-assisted arm training can be a useful treatment in clinical practice to improve the kinematic variables in chronic stroke patients.

Effect of Paretic Side and Non-paretic Side Arm Training on Trunk Control and Upper Limb Functions in Stroke Patients

  • Background: Stroke patients usually have arm weakness, which affects trunks and arms. Objective: To investigate the effects of paretic side and non-paretic side arm training on trunk control and upper limb functions. Design: Randomized Controlled Trial (single blind). Methods: Twenty patients with stroke in hospital were enrolled in the study. Twenty subjects were randomly assigned to paretic side arm training group (PATG, n = 10) or non-paretic side arm training group (NATG, n = 10). Trunk impairment scale (TIS) was used for trunk control, and box and block test (BBT) was used for upper limb function. Training was conducted for 4 weeks. Results: PATG showed significant difference in TIS (static balance, dynamic balance, coordination, total score) and BBT. NATG showed significant differences in static balance, and dynamic balance and total score except for coordination and BBT. PATG also showed a more significant difference in BBT and coordination and total score than NATG. Conclusions: The arm training performed on the paretic side are more effective than those performed on the non-paretic side in improving both upper limb function and trunk control in stroke patients.

Task-Oriented Approach for Improving Motor Function of the Affected Arm in Chronic Hemiparetic Stroke Patients

  • The purpose of this study was to assess the feasibility of task-oriented arm training for chronic hemiparetic stroke patients. The experimental design in this study was the pre-test and post-test with control group for 4-week intervention. Thirty patients with chronic hemiparetic stroke were recruited from 2 rehabilitation units. The subjects were divided randomly into experimental and control groups. The experimental group conducted task-oriented approach, involving 3 subparts of upper extremity activities, and the control group involved in the general upper extremity exercises. Functional movements of the upper extremities were assessed using clinical measures, including the Fugl-Meyer Assessment-Upper Extremity Section, Box and Block Test, and Action Research Arm Test. The score of Fugl-Meyer Assessment showed greater increases in the experimental group than in the control group after training. The improvement in Box and Block Test between pre-test and post-test measurements was significantly greater after task-oriented arm training compared to general upper extremity exercises. Action Research Arm Test scores also improved after task-oriented arm training compared to exercises in the control group. The task-oriented arm training improves the gross and fine motor activities and encouraging the use of the paretic arm through activity dependent intervention expedites the recovery of functional activities in the upper extremities for chronic hemiparetic stroke.

Effect of robot arm reach training on upper extremity functional movement in chronic stroke survivors: a preliminary study

  • Objective: The purpose of this study was to investigate the effect of robot arm reach training on upper extremity functional movement in chronic stroke survivors. Design: One group pretest-posttest design. Methods: Thirteen chronic stroke survivors participated in this study. Robot arm reach training was performed with a Whole Arm Manipulator (WAM) and a 120-inch projective display to provide visual and auditory feedback. During the robotic arm reach training, WAM provided gravity compensation and assist-as-needed (AAN) force according to the robot control mode. When a participant could not move the arm toward the target for more than 2 seconds, WAM provided AAN force to reach the desired targets. All patients participated in the training for 40 minutes per day, 3 times a week, for 4 weeks. Main outcome measures were the Fugl-Meyer Assessment (FMA), Action Research Arm Test (ARAT) and Box and Block Test (BBT) to assess upper extremity functional movement. Results: After 4 weeks, significant improvement was observed in upper extremity functional movement (FMA: 42.15 to 46.23, BBT: 12.23 to 14.00, p<0.05). In the subscore analysis of the FMA upper extremity motor function domains, significant improvement was observed in upper extremity and coordination/speed units (p<0.05). However, there were no significant differences in the ARAT. Conclusions: This study showed the positive effects of robot arm reach training on upper extremity functional movement in chronic stroke survivors. In particular, we confirmed that robot arm reach training could have a positive influence by leading to improvement of motor recovery of the proximal upper extremity.

마네킹 모델과 컴퓨터 시뮬레이터를 이용한 정맥주사 실습교육의 효과 비교 (Comparison of Training Effectiveness for IV Injections: Intravenous (IV) Arm Model versus Computer Simulator)

  • Purpose: The purpose of this study was to compare the effectiveness of training using an intravenous (IV) arm model versus a computer simulator for IV injections. Method: Study was a quasi-experimental study conducted with 106 nursing students. Participants were divided into two groups: the IV Arm Group using a mannequin arm model (control group) and the Computer Simulator Group using the Virtual IV demonstration (experimental group). Theoretical lectures and video presentations on IV injections were given to both groups. Each group went through the training practice using the IV arm or computer simulator. After the completion of training, questionnaires were given to the students to evaluate their learning attitudes and experiences, self-confidence in IV injection, and satisfaction with the training materials. Results: Student satisfaction with the training materials including the reality, usefulness, and educational effects showed notable differences between the two groups with the Computer Simulator group reporting more positive effects that the IV Arm group. However, there was no statistical difference between the two groups in the categories of learning attitude, learning experience, or self-confidence. Conclusion: While there was a differences in strengths and weaknesses of the two methods, both methods should be considered for practice and further study needs to be done on educational effectiveness.

동작관찰훈련이 뇌졸중 환자의 상지 기능에 미치는 영향 (The Effect of Action Observational Training on Arm Function in People With Stroke)

  • The aim of this study was to determine the effect of action-observation training on arm function in people with stroke. Fourteen chronic stroke patients participated in action-observation training. Initially, they were asked to watch video that illustrated arm actions used in daily activities; this was followed by repetitive practice of the observed actions for 3 times a week for 3 weeks. Each training session lasted 30 min. All subject participated 12 training session on 9 consecutive training days. For the evaluation of the clinical status of standard functional scales, Wolf motor function test was carried out at before and after the training and at 2 weeks after the training. Friedman test and Wilcoxon signed rank test was used to analyze the results of the clinical test. There was a significant improvement in the upper arm functions after the 3-week action-observation training, as compared to that before training. The improvement was sustained even at two weeks after the training. This result suggest that action observation training has a positive additional impact on recovery of stroke-induced motor dysfunctions through the action observation-action execution matching system, which includes in the mirror neuron system.

PNF를 이용한 과제지향훈련이 만성뇌졸중환자의 상지기능과 일상생활수행능력에 미치는 영향 (The Effects of Task-oriented Training Using the PNF in Upper Arm Function and Activities of Daily Living with Chronic Stroke Patients)

  • Purpose : The aim of this study was to compare the effect of task-oriented training using the PNF(proprioceptive neuromuscular facilitation) and task-oriented training with chronic stroke patients. Methods : Sixteen chronic stroke patients participated. Participants were randomly assigned to the control and experimental group( 8 experimental, 8 control). All of participants were in-patients at local hospital and had been received training appropriate to the purpose of each group program. Experimental period was carried out 30 minutes/day, five days/week, during four weeks. The experimental group trained the task-oriented training using the PNF and control group trained the task-oriented training. Fugl-meyer assessment scale and Modified Barthel Index was measured to compare the upper arm function and activities of daily living. We were measured before and after the training. Results : The results of the study were as follow: Fugl-meyer assessment scale was significantly increased both groups(p<.05), and significant between groups(p<.05). Modified Barthel index was significantly increased both groups(p<.05) and between groups(p<.05). Conclusion : PNF can be effective in improving upper arm function and ability to perform daily life of chronic stroke patients.

양측성 상지 활동이 만성 뇌졸중 환자의 일상생활활동에 미치는 영향 (The Effect of Bilateral Arm Training for the Chronic Phase After Stroke in Activities of Daily Living)

  • 목적 : 양측성 상지 활동이 만성 뇌졸중 환자의 일상생활활동의 독립적 수행에 미치는 영향을 알아보고자 하였다. 연구방법 : 대구에 소재한 F병원의 연구 뇌졸중 환자 20명을 무작위로 실험군과 대조군으로 10명씩 나누었다. 실험군은 양측성 상지 활동과 작업치료 중재를 각각 1일 30분씩, 대조군은 작업치료 중재만 1일 30분씩 6주 동안 받았다. 두 집단 모두 실험시작 전과 후에 FIM을 통한 일상생활활동을 평가하였다. 결과 : 양측성 상지 활동 전 후의 일상생활활동은 유의한 차이를 보였으며 항목별 변화는 자조활동, 이동성, 보행, 사회적인지 영역의 모든 항목에서 유의한 차이를 보였다. 양측성 상지 활동(중재후-중재전)과 작업치료 중재(중재후-중재전)에 의한 일상생활활동 항목별 변화를 비교해 보면 자조활동 영역의 전 항목, 보행 영역의 계단 오르고 내리기 항목, 사회적인지 영역의 문제해결능력 항목에서 유의한 차이를 보였다. 결론 : 양측성 상지 활동을 적용한 만성 뇌졸중 환자에서 일상생활활동 수행 능력이 향상된 것을 알 수 있었다. 따라서 양측성 상지 활동이 만성 뇌졸중 환자의 기능적인 회복을 위한 치료방법으로써 임상에서 활용할 수 있을 것으로 사료되며 앞으로 일상생활활동과 관련된 다양한 양측성 상지 활동에 대한 연구가 이루어져야 할 것이다.

상지 이용 유무와 훈련 기간이 무용 회전 동작의 기능에 미치는 영향 (The Effect of Upper Extremity Usage and Length of Training to the Function of Dance Turn)

  • The first purpose of this study was to compare kinematic variables during spinning motion with or without upper extremity and identify the most effective spinning method. The second purpose of this study was to compare functional difference between novice and elite dancers with the term of training. Ten experienced female dancers and ten novices were recruited as subjects for this study. Elite group was asked to perform turn motion with three types of upper extremity. Novice group has taken training of spotting technique for five weeks. Four Falcon HiRES cameras were used to analyze kinematic variables including head angular velocity and CG displacement during spinning. These data were sampled before training, after 3-week, and 5-week of training. Eight different events in two consecutive turns were defined for statistical comparison. One-way ANOVA was performed to compare among the kinematics of turning motion with three types of upper extremity. Independent t-test also used to compare kinematics between elite and novice at three different length of training. As results, spinning with both arm increased angular velocity and stability compared to the turning motion with one arm or with arm strapped and found out that the turn with both arm was the most effective way of spin. Also, for novice dancers, three weeks of training were needed to complete spinning motion.

선 자세에서 상지 훈련이 만성 뇌졸중 환자의 균형과 보행 능력에 미치는 영향 (The Effect of Arm Training in Standing Position on Balance and Walking Ability in Patients with Chronic Stroke)

  • PURPOSE: The purpose of this study was to determine the effects of arm training in standing position on balance and walking ability in chronic stroke patients. METHODS: Sixteen chronic stroke patients were allocated equally and randomly to an experimental group (n=8) or a control group (n=8). All participants received 60 minutes of comprehensive rehabilitation treatment, the experimental group additionally received an arm training in standing position for 30 minutes, while the control group additionally performed a treadmill training for 30 minutes. These 30-minute training sessions were held three times per week for six weeks. Upper extremity function was assessed using Fugle-Meyer motor assessment function upper extremity (FMA-UE), balance was assessed using Berg balance scale (BBS), and walking ability (gait speed, cadence, step length, and double limb support period) was assessed using the GAITRite system. RESULTS: Improvement on all outcome measures was identified from pre-to-post intervention for both groups (p<.05). Post-intervention, there was a significant between-group difference on BBS, gait speed, cadence, step length, and double limp support period (p<.05). The experimental group exhibited greater improvement in the BBS (p=.01; z=-2.48), gait speed (p=.01; z=-3.26), cadence (p=.02; z=-2.31), step length (p=.01; z=-3.36), and double limb support period (p=.03; z=-2.84) compared to the control group. CONCLUSION: The findings of this study suggest that arm training in standing position may be beneficial for improving balance and walking ability of patients with chronic stroke.
