PET/CT 검사 시 움직임 보정 기법의 유용성 평가 (The Correction Effect of Motion Artifacts in PET/CT Image using System)

본 연구에서는 의료기관에서 방사성동위원소를 이용하여 암을 비롯한 여러 질환을 검사하는 PET/CT 검사에서 환자의 움직임으로 인한 영상의 질 저하와 판독 오류를 발생할 수 있는 점을 보완하기 위해 AI 기반의 Algorithm을 이용하여 개발한 Mothion Free 소프트웨어를 이용하여 호흡으로 인한 움직임의 보정 정도를 확인하고 유용성을 평가하여 임상에서의 적용을 위한 연구를 하였다. 실험 방법은 RPM Phantom을 사용하여 방사성동위원소 18F-FDG를 진공 바이알과 서로 다른 크기 NEMA IEC body Phantom의 sphere에 방사성동위원소를 주입하고 이것의 움직임을 호흡 시 움직이는 병소로 연출하여 영상을 획득하였다. 진공 바이알은 서로 다른 위치에서 움직임 정도를 다르게 하였고, 서로 다른 크기 NEMA IEC body Phantom의 sphere는 서로 다른 병소의 크기를 연출 하였다. 획득한 영상을 통하여 병소의 체적, 최대 SUV, 평균 SUV를 각각 측정하여 Mothion Free가 움직임 보정 정도를 정량적 평가를 하였다. 움직임 정도를 크게 설정한 진공바이알 A의 평균 SUV는 23.36 %, 움직임 정도를 작게 설정한 진공 바이알 B는 29.3 % 오차율이 감소하였다. NEMA IEC body Phantom의 sphere 37 mm, 22 mm에서의 평균 SUV는 각각 29.3 %, 26.51 % 오차율이 감소하였다. 오차율을 산출한 네 가지 측정치의 평균 오차율 30.03 % 감소하여 보다 정확한 평균 SUV 값을 나타내었다. 이 연구에서는 2차원적인 움직임 만을 연출할 수 있었기에 보다 정확한 데이터를 얻기 위해서는 실제 인체의 호흡 운동을 구현할 수 있는 Phantom을 이용하고, 움직임의 범위의 다양성을 구성한다면 보다 정확한 유용성 평가를 할 수 있다고 연구된다.
The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. Our society has long been talking about necessity for innovation. Since companies in particular need to carry out business innovation in their overall processes, they have attempted to apply many innovation factors on sites and become to pay more attention to their innovation. In order to achieve this goal, companies has applied various information technologies (IT) on sites as a means of innovation, and consequently IT have been greatly developed. It is natural for the field of IT to have faced another revolution which is called cloud computing, which is expected to result in innovative changes in software application via the Internet, data storing, the use of devices, and their operations. As a vehicle of innovation, cloud computing is expected to lead the changes and advancement of our society and the business world. Although many scholars have researched on a variety of topics regarding the innovation via IT, few studies have dealt with the issue of could computing as IT. Thus, the purpose of this paper is to set the variables of innovation attributes based on the previous articles as the characteristic variables and clarify how these variables affect "Performance Expectancy" of companies and the intention of using cloud computing. The result from the analysis of data collected in this study is as follows. The study utilized a research model developed on the innovation diffusion theory to identify influences on the adaptation and spreading IT for cloud computing services. Second, this study summarized the characteristics of cloud computing services as a new concept that introduces innovation at its early stage of adaptation for companies. Third, a theoretical model is provided that relates to the future innovation by suggesting variables for innovation characteristics to adopt cloud computing services. Finally, this study identified the factors affecting expectation and the intention to use the cloud computing service for the companies that consider adopting the cloud computing service. As the parameter and dependent variable respectively, the study deploys the independent variables that are aligned with the characteristics of the cloud computing services based on the innovation diffusion model, and utilizes the expectation for performance and Intention to Use based on the UTAUT theory. Independent variables for the research model include Relative Advantage, Complexity, Compatibility, Cost Saving, Trialability, and Observability. In addition, 'Acceptance for Adaptation' is applied as an adjustment variable to verify the influences on the expected performances from the cloud computing service. The validity of the research model was secured by performing factor analysis and reliability analysis. After confirmatory factor analysis is conducted using AMOS 7.0, the 20 hypotheses are verified through the analysis of the structural equation model, accepting 12 hypotheses among 20. For example, Relative Advantage turned out to have the positive effect both on Individual Performance and on Strategic Performance from the verification of hypothesis, while it showed meaningful correlation to affect Intention to Use directly. This indicates that many articles on the diffusion related Relative Advantage as the most important factor to predict the rate to accept innovation. From the viewpoint of the influence on Performance Expectancy among Compatibility and Cost Saving, Compatibility has the positive effect on both Individual Performance and on Strategic Performance, while it showed meaningful correlation with Intention to Use. However, the topic of the cloud computing service has become a strategic issue for adoption in companies, Cost Saving turns out to affect Individual Performance without a significant influence on Intention to Use. This indicates that companies expect practical performances such as time and cost saving and financial improvements through the adoption of the cloud computing service in the environment of the budget squeezing from the global economic crisis from 2008. Likewise, this positively affects the strategic performance in companies. In terms of effects, Trialability is proved to give no effects on Performance Expectancy. This indicates that the participants of the survey are willing to afford the risk from the high uncertainty caused by innovation, because they positively pursue information about new ideas as innovators and early adopter. In addition, they believe it is unnecessary to test the cloud computing service before the adoption, because there are various types of the cloud computing service. However, Observability positively affected both Individual Performance and Strategic Performance. It also showed meaningful correlation with Intention to Use. From the analysis of the direct effects on Intention to Use by innovative characteristics for the cloud computing service except the parameters, the innovative characteristics for the cloud computing service showed the positive influence on Relative Advantage, Compatibility and Observability while Complexity, Cost saving and the likelihood for the attempt did not affect Intention to Use. While the practical verification that was believed to be the most important factor on Performance Expectancy by characteristics for cloud computing service, Relative Advantage, Compatibility and Observability showed significant correlation with the various causes and effect analysis. Cost Saving showed a significant relation with Strategic Performance in companies, which indicates that the cost to build and operate IT is the burden of the management. Thus, the cloud computing service reflected the expectation as an alternative to reduce the investment and operational cost for IT infrastructure due to the recent economic crisis. The cloud computing service is not pervasive in the business world, but it is rapidly spreading all over the world, because of its inherited merits and benefits. Moreover, results of this research regarding the diffusion innovation are more or less different from those of the existing articles. This seems to be caused by the fact that the cloud computing service has a strong innovative factor that results in a new paradigm shift while most IT that are based on the theory of innovation diffusion are limited to companies and organizations. In addition, the participants in this study are believed to play an important role as innovators and early adapters to introduce the cloud computing service and to have competency to afford higher uncertainty for innovation. In conclusion, the introduction of the cloud computing service is a critical issue in the business world.
인천 영종도와 영유도를 잇는 새로운 간척지에 건설되고 있는 인천국제공의 녹지 조경 식재 지반을 조성하는 데 필요한 기술적 지표와 효율적 관리 방안을 마련하기 위한 현지 연구가 1996년 6월부터 1998년 6월까지 24개월간 수행되었다. 인천국제공항 건설 부지 중 1995년 8월에 매립이 완료된 여객터미날 주차장 예정지에 8 ha의 시험포장을 조성하였다. 갯벌 지역에 대한 연구를 위하여 자생잔디 보호 구역 부근에 염생 식물 종자 채취 구역을 별도로 1.5ha 조성하였다. 조성된
2019년 3월 미세먼지 비상저감조치가 일주일 동안 발령되면서, 미세먼지로 인한 국민의 불안감은 점차 가중되고 있다. 본 연구는 공기정화식물이 적용된 바이오필터의 다중이용시설 내 적용성 평가를 위해 입자상 오염원의 실내 연속방출환경을 조성하여 오염원 저감효과에 대한 측정방법을 제안하고, 시스템의 실내공기질 개선 여부를 확인할 수 있는 기초연구를 진행하였다. 강의실을 대상으로 춘절기에 모니터링 1시간 전 모기향을 오염원으로 배경농도를 조성한 후, 스케줄에 따라 2시간 관수, 1시간 송풍하여 미세먼지의 저감능을 확인하였으며, 바이오필터 2m 전방에 PM10, PM2.5 및 온습도 센서를 설치하고, 3개 송풍구 중 중앙에 풍속 프로브를 설치하여 시계열 모니터링을 수행하였다. 바이오필터에 구비된 총 3개소의 송풍구 평균 면풍속은 0.38±0.16 m/s로 댐퍼 면적이 제외된 송풍구별 면적 0.29m×0.65m을 적용한 총 공조풍량이 776.89±320.16㎥/h로 산출되었다. 시스템 가동으로 평균온도 21.5~22.3℃, 평균상대습도 63.79~73.6%를 유지하여, 선행연구의 다양한 조건별 온습도 범위에 부합하는 것으로 판단된다. 시스템 공조부 구동을 통해 급격하게 상대습도를 상승시키는 효과를 효율적으로 운용할 경우, 계절에 따른 실내 미세먼지 저감과 적정한 상대습도 확보도 가능할 것으로 판단된다. 미세먼지 농도는 바이오필터 시스템 가동 전의 모든 주기에서 상승 현상이 동일하게 집계되었으며, 시스템 가동 후 1주기 송풍구간(B-1, β=-3.83, β=-2.45)에서 미세먼지(PM10)는 최대 28.8% 수준인 560.3㎍/㎥, 초미세 먼지(PM2.5)는 최대 28.0% 수준인 350.0㎍/㎥까지 저감되었다. 이후 미세먼지(PM10, PM2.5)의 농도는 2주기 송풍구간 감소(B-2, β=-5.50, β=-3.30)로 각각 최대 32.6% 수준인 647.0㎍/㎥, 32.4% 수준인 401.3㎍/㎥까지 저감되었고, 3주기 송풍구간감소(B-3, β=5.48, β=-3.51)로 최대 30.8% 수준인 732.7㎍/㎥, 31.0% 수준인 459.3㎍/㎥까지 저감된 것으로 확인되었다. 본 연구는 식생 바이오필터의 다중이용시설 내 설치와 유관한 관련 표준 및 규정을 참조하여, 객관적인 성능평가환경의 구축 방안을 제시할 수 있었다. 이를 통해 일반 강의실 환경 내에 보다 객관화된 모니터링 인프라를 조성하여, 상대적으로 신뢰성 있는 데이터 확보가 가능했던 것으로 판단된다.

Nursing of today has as one of its objectives the solving of problems related to human needs arising from the demands of a rapidly changing society. This nursing objective, I believe, can he attained by the appropriate application of scientific principles in the giving of comprehensive nursing care. Comprehensive nursing care may be defined as nursing care which meets all of the patient's needs. the needs of patients are said to fall into five broad categories: physical needs, psychological needs, environmental needs, socio-economic needs, and teaching needs. Most people who become ill have adjustment problems related to their new situation. Because patient teaching is one of the most important functions of professional nursing, the success of this teaching may be used as a gauge for evaluating comprehensive nursing care. This represents a challenge foe the future. A questionnaire consisting of 67 items was distributed to 200 professional nurses working ill direct patient care at Yonsei University Medical Center in Seoul, Korea. 160 (80,0%) nurses of the total sample returned completed questionnaires 81 (50.6%) nurses were graduates of 3 fear diploma courser 79 (49.4%) nurses were graduates of 4 year collegiate nursing schools in Korea 141 (88,1%) nurses had under 5 years of clinical experience in a medical center, while 19 (11.9%) nurses had more than 5years of clinical experience. Conclusions of this study are as follow: Before attempting the explanation, of the results, the questionnaire will he explained. The questionnaire contained 67 questions divided into 9 sections. These sections were: concept, content, time, prior preparation, method, purpose, condition, evaluation, and recommendations for patient teaching. 1. The nurse's concept of patient teaching: Most of the nurses had high levels of concepts and knowledge toward patient teaching. Though nursing service was task-centered at the turn of the century, the emphasis today is put on patient-centered nursing. But we find some of the nurses (39.4%) still are task-centered. After, patient teaching, only a few of the nurses (14.4%) checked this as “normal teaching.”It seems therefore that patient teaching is often done unconsciously. Accordingly it would he desirable to have correct concepts and knowledge of teaching taught in schools of nursing. 2. Contents of patient teaching: Most nurses (97.5%) had good information about content of patient teaching. They teach their patients during admission about their diseases, tests, treatments, and before discharge give nurses instruction about simple nursing care, personal hygiene, special diets, rest and sleep, elimination etc. 3. Time of patient teaching: Teaching can be accomplished even if there is no time set aside specifically for it. -a large part of the nurse's teaching can be done while she is giving nursing care. If she believes she has to wait for time free from other activities, she may miss many teaching opportunities. But generally proper time for patient teaching is in the midmorning or midafternoon since one and a half or two hours required. Nurses meet their patients in all stages of health: often tile patient is in a condition in which learning is impossible-pain, mental confusion, debilitation, loss of sensory perception, fear and anxiety-any of these conditions may preclude the possibility of successful teaching. 4. Prior preparation for patient teaching: The teaching aids, nurses use are charts (53.1%), periodicals (23.8%), and books (7.0%) Some of the respondents (28.1%) reported that they had had good preparation for the teaching which they were doing, others (27.5%) reported adequate preparation, and others (43.8%) reported that their preparation for teaching was inadequate. If nurses have advance preparation for normal teaching and are aware of their objectives in teaching patients, they can do effective teaching. 5. Method of patient teaching: The methods of individual patient teaching, the nurses in this study used, were conversation (55.6%) and individual discussion (19.2%) . And the methods of group patient teaching they used were demonstration (42.3%) and lecture (26.2%) They should also he prepared to use pamphlet and simple audio-visual aids for their teaching. 6. Purposes of patient teaching: The purposes of patient teaching is to help the patient recover completely, but the majority of the respondents (40.6%) don't know this. So it is necessary for them to understand correctly the purpose of patient teaching and nursing care. 7. Condition of patient teaching: The majority of respondents (75.0%) reported there were some troubles in teaching uncooperative patients. It would seem that the nurse's leaching would be improved if, in her preparation, she was given a better understanding of the patient and communication skills. The majority of respondents in the total group, felt teaching is their responsibility and they should teach their patient's family as well as the patient. The place for teaching is most often at the patient's bedside (95.6%) but the conference room (3.1%) is also used. It is important that privacy be provided in learning situations with involve personal matters. 8. Evaluation of patient teaching: The majority of respondents (76.3%,) felt leaching is a highly systematic and organized function requiring special preparation in a college or university, they have the idea that teaching is a continuous and ever-present activity of all people throughout their lives. The suggestion mentioned the most frequently for improving preparation was a course in patient teaching included in the basic nursing program. 9. Recommendations: 1) It is recommended, that in clinical nursing, patient teaching be emphasized. 2) It is recommended, that insertive education the concepts and purposes of patient teaching he renewed for all nurses. In addition to this new knowledge, methods and materials which can be applied to patient teaching should be given also. 3) It is recommended, in group patient teaching, we try to embark on team teaching.
본원에서는 비심장 수술환자의 수술 전후 심장사건의 위험도 평가를 위해 심근관류 SPECT를 시행하고 있다. 암환자의 경우 수술 전에 전신 뼈 검사 혹은 전신 PET 검사로 암 전이 여부 확인 후 심근관류 SPECT를 시행하여 불필요한 검사 처방을 막고 있다. 하지만 단기 병동 입원 환자의 경우 재원 일수를 줄이고자 전신 뼈 검사 후 최소 16시간의 간격을 두고
UltraSPECT사의 Wide Beam Reconstruction (WBR)은 노이즈(Noise)와 조준기의 광속 확산 함수 효과(Beam spread function effect)를 제거하고 환자와의 거리를 자동적으로 보상하여 높은 해상도와 대조도를 제공할 수 있어 영상 획득 시간을 짧게 할 수 있고 상당한 영상 질 향상에 도움을 준다고 보고되고 있다. 이에 본 연구에서는 핵의학 분야에서 가장 흔히 이용되는 전신 뼈 스캔에 대해 WBR의 임상적 적용에 대한 유용성을 알아보고자 한다. XpressBone (WBR)의 성능 실험을 위하여 NEMA에서 제공하는 방법에 의하여 선원(Line source)과 SPECT Phantom을 이용하여 공간 분해능을 측정 분석하였다. 실험방법은 선원의 총 계수치를 200 kcps에서 300 kcps로 변화시켜 측정하였으며, SPECT Phantom은 매트릭스 크기를 변화시켜 측정하여 공간분해능에 대한 분석을 하였다. 또한 2009년 1월부터 2009년 9월까지 본원을 내원하여 뼈 스캔을 시행 받은 환자 40명을 두 군으로 나누어 임상 연구를 시행하였다. 1군은
