Respiratory Depression After the Second Dose of Intravenous Tramadol in a Rectal Cancer Patient with Normal Renal Function: A Clinical Case Report (정상 신기능 직장암 환자에서 Intravenous Tramadol 이회 투여 후 호흡곤란 발생 증례 보고)

  • We report an unusual case of respiratory depression and prolonged apnea after the second dose of 100-mg intravenous tramadol. Due to continuous pain at foley catheter keep site, intravenous tramadol was administered to the patient. Soon after the second dose of tramadol injection, the patient became apneic. The patient did not respond to verbal command and started exhibiting oxygen desaturation. The patient was quickly treated with 100% oxygen, and it took 4 hours for the spontaneous respiration to return to regular. This case report demonstrates that even two doses of tramadol administered intravenously may manifest as sudden and prolonged apnea. Respiratory depression with tramadol has been reported in patients with impaired renal functions and Cytochrome P(CYP) 2D6 gene duplication.

A Study on the Detection of Obstructive Sleep Apnea Using ECG (ECG를 이용한 수면 무호흡 검출에 관한 연구)

  • Obstructive Sleep Apnea(OSA) is a representative symptom of sleep disorder which is caused by airway obstruction. OSA is usually diagnosed through the laboratory based Polysomnography(PSG) which is uncomfortable and expensive. In this paper, the detection method for OSA events, using ECG, has been developed. The proposed method uses the ECG data sets provided from Physionet. The features for OSA events detection are the average and standard deviation of 1 minute R-R interval, power spectrum of R-R interval and S-pulse amplitude from data sets. These features are applied to the input of Neural Network. To evaluate the method, we used the another ECG data sets. And we achieved sensitivity of 89.66%, specificity of 95.25%. So, we can know that the features proposed in this paper are important to detect OSA.

  • 폐쇄성수면무호흡증(Obstructive sleep apnea syndrome : OSAS)은 신경근육계, 호흡계, 심혈관계의 복합적인 질환으로서 사회적, 생리학적으로 심각한 문제를 발생시킬 수 있는 질병이다. 수술적치료를 결정하기에 앞서 환자의 증상 및 징후, 신체 상태와 습관 등을 정밀검사하고, 기도폐쇄가 일상생활 및 정신건강에 미치는 영향을 고려하여 수술여부를 결정하여야 한다. 폐쇄성수면무호흡증후군의 진단에 유용한 검사인 수면다원검사와 Videofluoroscopy의 검사결과를 바탕으로 수술소견을 비교분석하고자 하였다.

  • 폐쇄성 수면 무호흡증 환자 중 가장 위험한 2~8세 아동들의 지속적인 수면 상태를 모니터링하는 것은 중요하며, 야간에 수면 상태를 모니터링 하는 것은 보호자에게 스트레스와 일상생활에 지장을 초래할 수 있다. 본 연구에서는 아동들의 수면 상태를 측정하기 위한 스마트폰 기반의 실시간 호흡 모니터링 시스템을 구현하였다. 구현된 시스템은 아동의 수면 중 자세 변화로 인한 잦은 폐쇄성 수면 무호흡을 지속적으로 모니터링 함으로써 수면 상태 모니터링 및 위험 상황을 인지할 수 있으며, 보호자에게 편안한 수면을 제공한다.

  • Changes in breathing pattern and apnea both can be !he result of sleep disorders. The focus of this paper is to develop methodologies to monitor the breathing pattern and to detect apnea. An accurate recording of the respiratory phase can be carried out with different methods. One of these methods is the use of a thermocouple, which reacts to the variation in air temperature, placed in the nose and mouth of the patient. The K-type thermocouple was used because it has high reliability, thermo-stability, and good corrosion resistance. And also, it has a considerable long time constant that gives a low cut-off frequency, well below the respiratory frequency and thereby causing a large phase difference. The result showed that timing of respiration was accurately obtained with the AD595, amplifier for K-type thermocouple.

  • As the obesity patients increase recently, breath disease such an OSA(obstructive sleep apnea) is also increasing. When the breath disease occurs, the risk comes to be high. Dependence degree the PAP(positive airway pressure) is also coming to be high. The mechanical simulator is composed cylinder, valve, ball screw and the motor that they correspond to the lung and airway, the diaphragm of the human. In order to confirm the characteristic of the motor and the valve, it accomplished an test. The simulator traces breath pattern against the normal breath and the OSA.

  • 본 연구에서는 폐쇄성 수면 무호흡증 환자를 대상으로 착용의 불편함을 최소화하고 수면 중 지속적인 모니터링이 가능한 안대형 폐쇄성 수면 무호흡증 검출시스템을 구현하였다. 이를 위하여 숙면을 돕기 위해서 착용하는 안대의 코 부근에 온도센서를 부착하여 실제 호흡에 따른 온도 변화를 감지하였다. 폐쇄성 수면 무호흡증 환자의 경우 수면 중 불안정한 호흡이 온도의 변화로 반영되기 때문에 이를 검출하기 위함이다. 또한 검출된 온도 변화는 안대에 내장된 제어부 및 블루투스 모듈을 통해 스마트폰으로 전송되어진다. 전송된 데이터는 안드로이드 기반의 어플리케이션을 구현하여 실시간으로 모니터링이 가능하며, 구현된 어플리케이션은 위험상황 인지 및 알림, 일월별 관리 기능을 포함하고 있다. 구현된 시스템의 성능 평가를 위하여 대학생 5명을 대상으로 임의의 호흡 변화에 대한 실험 프로토콜을 작성하여 실험을 수행하였으며, 그 결과 호흡을 중단하였을 시 온도 변화를 통해 검출이 가능함을 확인하였다.

  • Sleep disorders such as insomnia, obstructive sleep apnea (OSA), and restless legs syndrome (RLS) are very common disorders and may cause significant burden in terms of individual as well as societal aspects. Sleep insufficiency from such sleep disorders may cause deleterious effects on daily work life and may be associated with other major medical or psychiatric disorders including cardiovascular disease, diabetes mellitus, depression, and anxiety disorder. Various motor or occupational accident may result from the sleep problems. In addition, recent researches provide the method to evaluate the lost productivity time in terms of absenteeism and presenteeism. Moreover, several studies on cost-effectiveness of treatment of sleep disorders show that it is cost-effective.

  • Sleep disorders are relatively common occurrence after traumatic brain injury. Sleep disturbances often resulted in difficulties in sleep onset and sleep maintenance, nonrestorative after sleep, poor daytime performances and poor individual sense of wellbeing. Unfortunately, there has been minimal attention paid to this common and disabling sequela of brain injury. Better undertanding about problem, pathophysiology and treatment of sleep disorder after traumatic brain injury will improve the cognitive function, social adjustment and rehabilitation for injured patients. Also it may be helpful to reduce traumatic brain injury in patients with sleep apnea.

  • Considering psychosocial needs of patients, it is not surprising that surgery-first approach (SFA) is becoming more popular than ever. Although the concept of SFA was introduced a few decades ago, the limitation of analysis method based on two-dimensional images makes surgeons reluctant to choose SFA. Recently, the advancement of three-dimensional technology allows us to perform SFA even without minimal pre-surgical orthodontic treatment, and the prediction of surgical outcome became more accurate, especially in obstructive sleep apnea (OSA) patients to whom the advantages of SFA should be more significant. Here, we describe the current trend of SFA and its implication in OSA patients.
