The author has studied on 910 roentgenograms of lower second molars, which were taken by intraoral technic, and obtained the following results. 1. The development of crown of lower second molars was completed 8.25 years. 2. The formation of mesial and distal roots in full length on lower second molars was completed as follows: a. mesial roots 15. 07 years b. distal roots 15. 53 years 3. The formation of apical foramina of mesial and distal roots of lower second molars was closured as follows: a. apical foramen of mesial root 18.00 years b. apical foramen of distal root 18.79 years 4. As a general rule, the mesial roots were developed more earlier than distal roots.
The final preparation (MAF) size in infected root canals is still controversial. Nonetheless, recent studies demonstrated that larger apical preparation sizes produces a greater reduction in remaining bacteria and dentinal debris as compared to smaller apical preparation sizes. Therefore, clinicians should be practiced with treatment strategies guided by evidence-based information, especially in infected/failed root canals.
Forty extracted human mandibular second molars with C-shaped canal were chosen to study the anatomy of the root canal. The experimental teeth were injected with china ink, decalcified and cleared with Winter green oil, in vitro, to study the number of root, root canal, canal per root, frequency and location of lateral canal and transverse anastomosis. 1. All teedth had one root. 2. Mesial roots with two canals were 25%, and mesial roots with one canal were 75%. All distal roots had one canal. 3. In the roots with two canals, the common apical foramen appeared in 20% and the separte apical foramen appeared in 80%. 4. The frequence of lateral canal was 33.3% and the most frequent region was middle 1/3. 5. All teeth had the transverse anastomosis in all region of the roots.

The aim of this study was to evaluate the microleakage of teeth according to root canal preparation with & without apical enlargement in various size of apical foramen. 60 extracted one canal roots were cross-cutted at 5 mm from root apex and divided into two groups according to their apical foramen size of large (L) and small (S). Each group was subdivided into two groups accordance with their cross-sectional configuration at 5 mm from apex, round (R) and ovoid (O); SR Group, SO Group LR Group, LO Group. Each group was shaped in .02 taper by Quantec series Nickel-Titanium (NiTi) rotary file, obturated by lateral condensation method. Leakage was measured using a fluid transport model under 40 $cmH_2O$ pressure. After the leakage test, blocks which had showed the leakage retreated with .04 taper and ,06 taper and evaluated the degree of fluid filtration in each group. The data was analysed statistically using chi-square test and fisher's exact test. The results obtained were as follows : 1. Significant difference in leakage was found in groups which had different apical foramen size in .02 taper instrumentation (p < 0.05), but not in .04 taper instrumentation (p > 0.05) 2. The difference in microleakage according to the shape of canal was not evident at 5 mm from apex (p > 0.05). 3. There was correlation between .02 taper instrumentation and .04 taper instrumentation in LR group, LO group (p < 0.05).
This study was done to evaluate transportation of the apical foramen after 0.5 mm overinstrumentation by ProFile, ProTaper and $K^3$ in simulated resin root canal. Sixty simulated resin root canal with a curvature of J and S-shape were divided into two groups. Each group consisted of three subgroups with 10 blocks according to the instruments used: $ProFile^{(R)},\;ProTaper^{TM},\;and\;K^{3TM}$. Simulated resin root canal was prepared by ProFile, ProTaper and $K^3$ with 300 rpm by the crown-down preparation technique. Pre- and post-instrumentation apical foramen images were overlapped and recorded with Image-analyzing microscope 100X (Camcope, Sometech Inc, Korea). The amounts of difference in width and dimension on overlapped images were measured after reference points were determined by Image Analysis program ($Image-Pro^{(R)}$ Express, Media Cybernetic, USA). Data were analyzed using Kruskal-Wallis and Mann-Whitney U-test. The results suggest that ProFile showed significantly less canal transportation and maintained original apical foramen shape better than $K^3$ and ProTaper.
Purpose: The mental foramen (MF) is an important anatomical structure during local anesthesia and surgical procedures in terms of achieving effective mental nerve blocks and avoiding injuries to the neurovascular bundles. Thus, understanding the anatomic features of the mandibular canal and accessory mental foramen in Korean could contribute to the surgical anatomic assessment. This study was to elucidate frequency, position and course of AMF (accessory mental foramen) in Korean using 3D cone beam computed tomography. Materials and Methods: The CBCT (Conbeam computed tomography) DICOM data (Alphard, Asahi, Japan) from 540 patients in korean were analyzed. We investigated images of 3D CBCT using Ondemand (CyberMed, Korea) software program on the incidence and anatomical characteristics of accessory foramen. Results: The accessory mental foramina were found in 17 patients. Accessory mental foramina exist predominantly in the apical area of the second premolar and posteroinferior area of the mental foramen. The accessory branches of the mandibular canal showed common characteristics in the course of gently sloping posterosuperior direction in the buccal surface area. The size of most AMF was obviously smaller than that of MF. Conclusion: We could identify frequency, position and course of AMF (accessory mental foramen) by the anatomical study of the accessory mental foramen using 3D cone beam CT in Korean.
To compare calcium hydroxide with Vitapex paste in the effect of apical closure, apexification was done with two medicaments in the nine teeth of seven patients visiting at SNUDH, and then, after radiographic and clinical observation of at least six months, the author concluded the followings: 1. The significant differences between calcium hydroxide and Vitapex paste were not observed in the effect of apical closure. 2. In calcium hydroxide group, the resorption of the paste within the canal was not observed, but in Vitapex group, observed. 3. In both calcium hydroxide group and Vitapex group, the progressive healing of apical lesion was observed. 4. The progressive resorption of Vitapex paste out of the apical foramen was observed.
