The Emotional Distress Prior to the Surgery and the Parenting Stress in Elementary School Children with Neurosurgical Diseases : The Preliminary Study (외과적 뇌질환을 가진 학령기 아동의 수술 전 정서적 불안정감과 어머니의 양육스트레스에 관한 예비연구)

  • Objectives: This study was conducted to compare emotional problems in children with neurosurgical diseases prior to surgery as well as levels of parenting stress experienced by mothers of children treated with surgery and those without surgery. The goals was to provide a basis on which comprehensive treatment interventions could be established. Methods: Subjects included 78 elementary school children who visited the Department of Pediatric Neurosurgery as well as their mothers. The Children's Depression Inventory (CDI) and the Revised Children Manifest Anxiety Scale (RCMAS) were administered to children. The Parenting Stress Indexs (PSI) and Beck Depression Inventory (BDI) were administered to their mothers. Results: The children with neurosurgical diseases showed higher levels of depression prior to surgery. In their mothers, parenting stress was relatively higher and parenting competence was lower. In particular, for those children who underwent surgery, depression was significantly higher. Specifically, they exhibited more negative expectations, and negative self-image. In mothers of children who underwent surgery, depression was significantly higher. Conclusion: Our results suggest higher levels of emotional distress and parental stress in children who undergo neurosurgical operations and their mothers, respectively. Based on this, the necessity for surgical and comprehensive psychological intervention is suggested.

Relationships between Treatment Belief, Personal Control, Depressive Mood and Health-related Quality of Life in Patients with Hemodialysis (혈액투석 환자의 치료에 대한 믿음, 질병 통제감과 우울, 건강 관련 삶의 질과의 관계)

  • Purpose: The purpose of this study was to examine the relationships between treatment belief, personal control, depressive mood, and health-related quality of life in patients with hemodialysis based on self-regulation theory. Methods: Data were collected from 220 patients at 27 local hemodialysis clinics in Seoul during 2013 and 2014. The Revised Illness Perception Questionnaire, the Hospital Anxiety and Depression Scale, and Medical Outcomes Study Short Form-12 were used to measure outcome variables. Data were analyzed using descriptive statistics, t-tests, ANOVA, Pearson correlation, and multiple regression using the 'enter' method. Results: Treatment belief and personal control scored 3.58 and 3.54 out of 5 points respectively, on average. Treatment belief and personal control of kidney disease were negatively correlated with depressive mood and positively correlated with health-related quality of life. According to the regression analysis, treatment belief, monthly income, and personal control were discovered to account for 21.8% of the variance in depressive mood, where as depressive mood, monthly income, treatment belief, and age were found out to account for 40.6% of the variance in health-related quality of life. Conclusion: Our study demonstrated significant positive relationships between treatment belief and illness outcome in hemodialysis patients. Interventions aimed to provide the necessary information and trust to maximize the effectiveness of treatment need be developed to improve patients outcomes.

An Verification of the Effect of Structured Cognitive Behavioral Intervention Program for Elementary School Students with High Risk Behavior (고위험 문제행동을 보이는 초등학생을 위한 구조화된 인지행동 중재 프로그램의 효과 검증)

  • The study is a preliminary study of the effectiveness of the structured cognitive behavioral arbitration program targeting elementary school students who perform high risk problems based on schools for convergence with local communities. The program is designed to be practiced by semi-trained professionals. To screen high-risk students, total 102 students were tested K-CBCL and those who scored over 60T on problem behavior syndrome scale were recruited. 32 students were selected and devided into intervention group and control group. Intervention group was received 90-minutes-sessions per week, for nine weeks. The program was done by graduate students and undergraduate students majoring counseling and supervised by school psychologist. The internalization group showed significantly lower score in Internalization, anxiety/depression, and externalization group showed externalization, aggressive behavior. but mixed group didn't showed significantly lower score. Implications of the results, limitations and suggestions for future study were mentioned.

Psychiatric Characteristics According to Tinnitus Severity (이명 심각도에 따른 정신의학적 특징)

  • Objectives The purpose of this study was to examine the psychiatric characteristics of outpatients with tinnitus. Methods A total of 88 subjects were included in this study. According to Tinnitus Handicap Inventory (THI) scores, the subjects were classified into two group ; a mild tinnitus symptoms (mild-tinnitus) group and a severe tinnitus symptoms (severe-tinnitus) group. A questionnaire was used for an assessment of demographic characteristics, and the THI, the Visual Analogue Scale (VAS) about tinnitus, the Beck Depression Inventory (BDI), and the Beck Anxiety Inventory (BAI) are applied for evaluation of other clinical psychiatric characteristics. Results Higher THI scores were positively correlated with tinnitus course, the number of accompanying symptoms, and the VAS. BDI total scores, BDI factors, and BAI total scores were significantly higher in the severe-tinnitus group than in the mild-tinnitus group. Also suicidal ideation, interpersonal problems, sleep problems, occupational impairment, and fatigue were significantly higher in the severe-tinnitus group than in the mild-tinnitus group. Conclusions Tinnitus is a common disorder of hearing which is associated frequently with psychiatric problems. This study suggests that psychiatric interventions should be taken into consideration in the treatment of patients suffering from tinnitus.

No Association between Genetic Polymorphism of Tryptophan Hydroxylase A218C and Serotonin Transporter Linked Polymorphic Region and Panic Disorder (한국인 공황장애 환자의 트립토판 가수분해 효소와 세로토닌 전달체 유전자 다형성)

  • Objectives:Genetic variations of the tryptophan hydroxylase(TPH) gene and the serotonin transporter linked polymorphic region(5-HTTLPR) polymorphism have been associated with its functional capacity. The authors investigated whether the allelic constitution of the TPH gene and 5-HTTLPR are associated in Korean panic patients. Methods:244 Korean patients with panic disorder and 227 normal healthy controls were tested for a genetic polymorphism of TPH A218C and 5-HTTLPR polymorphism. To assess the severity of panic disorder during the last one month, anticipatory anxiety, panic difficulty, panic distress, agoraphobic difficulty and agoraphobic distress were measured with visual analogue scale(VAS) score, STAI-S & T, BDI, SCL-90-R, ASI-R, CGI, PDSS, and HAMD. Results:There was no significant difference in genotype and allele frequencies of TPH A218C and 5-HTTLPR polymorphism between panic patients and controls. Although we observed some differences in genotype and allele frequencies of TPH A218C polymorphism among male subjects, these differences disappeared after Bonferroni correction. And there were no significant differences in clinical variables. Conclusion:Our results suggested that there are no association between the genetic polymorphism of TPH gene and 5-HTTLPR with panic disorder.

A Prospective Observational Study of Herbal Medicines on Depressive Disorder (우울장애 한약치료에 대한 전향적 관찰연구)

  • Objectives: The purpose of this study was to verify the clinical effects of herbal medicines on patients with depressive disorder. Methods: A prospective observational study on patients with depressive disorder was conducted. The patients were treated with herbal medicines for eight weeks. The Korean Hamilton Depression Rating Scale (K-HDRS) was evaluated as the primary outcome and the Korean Beck's Depression Inventory (K-BDI), Instrument on Pattern Identification for Depression, Korean Symptom Checklist-95 (KSCL-95), Insomnia Severity Index (ISI), Korean State-Trait Anxiety Inventory (STAI-K), Korean State-Trait Anger Expression Inventory (STAXI-K), EuroQol five-dimensional Questionnaire (EQ-5D), Minnesota Multiphasic Personality Inventory-2 (MMPI-2), Heart Rate Variability (HRV) were measured as the secondary outcome. Adverse events were assessed for safety. Results: A total of 35 participants were observed for 12 weeks. The K-HDRS improved significantly in four, eight, and 12 weeks and the K-BDI showed significant differences in four, and eight weeks. There were no significant improvements in other evaluation scales. Gyejibongnyeong-hwan was the most frequently and continuously prescribed, showing significant improvements of K-HDRS after eight weeks of treatment. Conclusions: In our observational study, herbal medicine was effective for relieving the depression of patients with depressive disorder and Gyejibongnyeong-hwan showed the most significant effectiveness.

Korean Immigrant Elderly's Generativity and Care-giving Experiences of Their Grandchildren (재미한인노인의 생성감과 손자녀 양육경험)

  • This study examined Korean immigrant elderly's generativity and care-giving experiences of their grandchildren according to their level of generativity. In-depth interview and participant observation method were used. First, this study used Loyola Generativity Scale in order to find Korean immigrant elderly's generativity level. 4 grandparents were selected as a group of low generativity level and 4 grandparents as high level. Grandparents with low generativity level showed non-voluntary care-giving, severe care-giving stress, guilty feeling for their adult children and shame for their ancestors, anxiety for the unstable future, lack of self-care, and lack of social assistance. Grandparents with high generativity level showed voluntary care-giving, positive reaction for the reversed parental care, feeling of usefulness for care-giving, hope for the future, existence of self-care, and existence of social assistance. Further discussion and suggestions were made from the study results.

The Effects of Adult Attachment and Covert Narcissism on Smartphone Addiction in College Students (대학생의 성인 애착과 내현적 자기애가 스마트폰 중독에 미치는 영향)

  • Smartphones, now a necessity for modern people, have become more than just an appliance, but an object of obsession for many. This has resulted in smartphone addiction in many modern people. However, the phenomenon of object addiction can lie beyond the problem of the object itself, varying according to the psychological characteristics of the subjects who use it. For this reason, this study aimed to examine adult attachment and covert narcissism as factors influencing smartphone addiction in college students. In order to do this subjects' level of smartphone addiction, adult attachment and covert narcissism needed to be measured on a scale of smartphone addiction, adult attachment and covert narcissism. Regression analysis was conducted with smartphone addiction set as the dependent variable and adult attachment and covert narcissism as independent variables. As a result, we observed anxiety attachment related to adult attachment and hypersensitivity/weakness of covert narcissism played roles in smartphone addiction. These findings suggest that in order to understand smartphone addiction in college students, it is important to know the psychological characteristics of our subjects.

The Correlation of Voice Characteristics and Depression Index Analysis in Accordance with Menstrual Cycle (월경주기에 따른 우울지수 정도와 음성특성과의 상관관계 분석)

  • This study investigated the differences between emotional parameters BDI, VHI, STAI-X-I and STAI-X-II according to the menstrual cycles of the female and the relation between changes of the depression index and voice characteristics (jitter, shimmer, CPP, HNR, $pF0{\cdot}F1{\cdot}F2{\cdot}F3$, sF0, sF4, sB1, $H1_{c/u}$, $A1_u$, $A3_c$, $H1A3_{c/u}$, $H1A1_u$). Twenty three females ($30{\pm}4.4$ years old) living in Seoul and Gyeonggi Province were participated in this study to answer the questionnaires and record their voice. The participants prolonged /a/ vowel for 5 seconds in a natural condition for their voice recording. Voice data were analyzed using the Matlab and Praat program. A t-test and a correlation analysis were conducted by using SPSS for the statistical analysis. The results are as follows. First, the BDI is significantly higher in group I (lurear phase contrast the menstrual period) and group II (follicular phase against the menstrual period) than group III (luteal phase for follicular phase) (p<.05). Second, shimmer, CPP, pF0 showed a statistically high correlation regarding the BDI in group I (lurear phase contrast the menstrual period). Voice parameters may be useful as supplement in evaluating the emotional change in the phase of menstrual cycle.

Clinical and Neuropsychological Characteristics of Adult Alcohol Use Disorder Patients with or without Attention-Deficit Hyperactivity Symptom (성인 알코올사용장애 환자에서 주의력결핍 과잉행동 증상 유무에 따른 임상적 및 신경심리학적 특성)

  • Objectives : Alcohol use disorder (AUD) is often comorbid with adult attention-deficit hyperactivity disorder (ADHD) and other psychiatric illnesses. When associated with other mental problems, the prognosis of the AUD can be more serious. This study shows research on the clinical and neuropsychological characteristics according to whether or not ADHD symptoms and AUD were comorbid. Methods : A total of 64 adult inpatients who completed AUD scales about adult ADHD, alcohol dependence, depression, anxiety, and impulsiveness. They also completed neuropsychological tests about attention and executive function. According to the Adult ADHD Self-Report Scale score, patients were categorized into two groups (ADHD symptom positive/negative group). Results : Fourteen among the 64 subjects were part of the ADHD symptom positive group (21.9%). They had statistically significant shorter abstinence periods and a higher rate of history of 'rule violation during school' and 'physical damage in childhood' compared to the ADHD symptom negative group. Conclusion : The comorbidity rate with ADHD symptom in adult AUD is higher than the general population. The ADHD symptom positive group suffered from more severe and refractory AUD. Considering the more frequent history of rule violation during school in the ADHD symptom positive group, the association between ADHD and AUD may be mediated by conduct disorder. Therefore we suggest the necessity of careful evaluation and intervention in children and adolescents with ADHD and conduct disorder.
