Effects of a β-Adrenergic Agonist on Growth Performance and Protein Metabolism in Broilers Treated with or without an Antithyroid Substance

  • To determine the interrelationship between thyroid status and the reparitioning action of clenbuterol (CLE) in broilers, two-week-old female chickens were fed diets containing an antithyroid substance, propylthiouracil (PTU, 0 or 0.3%), CLE (0 to 1 mg/kg), or both for 18 days in a $2{\times}2$ factorial design experiment. Muscle weights (breast muscle, gastrocnemius and peroneus longus) increased only in the normal chickens fed CLE. As absolute mass, protein of leg muscle quantitatively increased in the CLE-fed normal birds. In contrast, inhibition of the CLE-induced protein accretion, especially of peroneus longus, occurred in the PTU group. A quantitative increase in DNA was observed in leg muscles of the normal chickens, but no DNA response to CLE was shown in the PTU-treated chickens. The decreased RNA in leg muscles of the PTU group was more reduced by CLE feeding. Although not statistically significant, the reduced degradation rate of whole muscle protein in normal chickens fed CLE was not confirmed in the PTU-fed group. The present study, therefore, concluded that metabolic action of thyroid hormones was a prerequisite for the hypertrophic effect of ${\beta}$-agonist in broilers.

  • Thyrotropin binding inhibitory immunoglobulin (TBII) and thyroid stimulating antibody (TSAb) activities were measured in the thyroidal and peripheral venous blood samples at the time of subtotal thyroidectomy from twenty one patients with Graves' disease prepared for surgery with antithyroid drugs. There was no difference in TBII and TSAb activities between thyroidal and peripheral blood samples. These findings were regarded that while intrathyroidal lymphocytes are major site of thyrotropin receptor antibody (TRAb) production, similar levels are found in thyroidal and peripheral veins and that this in vivo study cannot exactly ascertain the TRAb producing site.

  • Studing the binding of the 5-Iodopyrimdines by human serum albumin we obtained the following conclusions; 1. The more strong electron donating groups in the molecule of 5-Iodopyrimidines, the larger the binding force with human serum albumin. This trend seems to be attributed by increase of polarization of the electron donating groups in 5-Iodopyrimidines molecule. 2. The binding force of 5-Iodopyrimidines by human serum albumin is increased with the pH increasing could be occurred the configurational changes of human albumin molecule, and this new binding sites of human serum albumin molecule would form the intermolecular complex with 5-Iodopyrimidines molecule more strongly.

A Case of Hyperthyroidism in a Patient with Improved FT4 Levels Following Treatment with Traditional Korean Medicine (한의약 치료로 FT4 수치에 개선을 보인 갑상선기능항진증 환자 1례에 관한 증례보고)

  • Objective: This study evaluated the effects of traditional Korean medicine on a patient with hyperthyroidism, who could not use antithyroid medication due to an adverse reaction to methimazole. Methods: Herbal medicine was administered and acupuncture was carried out. A thyroid function test was used to evaluate the effects of the treatment. Results: After treatment with traditional Korean medicine and therapy, the patient showed significant improvements in symptoms and FT4 levels. Conclusion: This case proved that traditional Korean medicine can be used for patients who have adverse reactions to antithyroid medication; however, studies of larger populations are required in the future.

  • Hashimoto's encephalopathy (HE) is a rare autoimmune disease characterized by a high serum concentration of antithyroid antibodies without evidence of cerebral disease. Magnetic resonance imaging (MRI) findings in HE patients are nonspecific, although diffuse or focal white matter changes have been reported in several cases. We present a rare case involving a 79-year-old woman with elevated antithyroid antibody levels and abnormal imaging findings similar to meningoencephalitis. Serial MRI initially showed multiple T2 hyperintense lesions with diffuse leptomeningeal enhancement that disappeared after steroid therapy.

The natural history and prognostic factors of Graves' disease in Korean children and adolescents (한국 소아 및 청소년 Graves병의 자연 경과 및 예후 인자)

  • Purpose : Graves' disease is the most common cause of hyperthyroidism in children and adolescents. In this study, we investigated the natural course and the prognostic factors of Graves' disease in Korean children and adolescents. Methods : One-hundred thirteen (88 girls and 25 boys) patients were included in this study. A retrospective analysis was made of all patients who were diagnosed with Graves' disease. The following parameters were recorded and analyzed: patient's sex, age at diagnosis, duration of disease, laboratory findings, symptoms and signs, and family history of autoimmune thyroid disease. Results : All patients were initially treated with antithyroid drugs, either methimazole (93.8%) or propylthiouracil (6.2%). Antithyroid drugs had been discontinued in 75 (66.4%) of 113 patients. Of these 75 patients, 23 (20.4%) relapsed after $25.5{\pm}33.7$ months. Thirteen (11.5%) of 23 patients, who experienced the first relapse, showed a second remission. However, 2 (1.8%) of 13 patients relapsed again. Euthyroid state could not be achieved by antithyroid drugs in 1 patient, and radioactive iodine therapy was performed. The older the patient at diagnosis, the greater the likelihood of remission ($P$=0.034). Conclusion : Age at diagnosis seems to be a prognostic factor in Korean children and adolescents with Graves' disease, and should be taken into account in treatment plan determination.

Analysis of 2-Mercapro-1-Methylimidazole in Controlling Drug of Thyroid by Differential Pulse Polarography (펄스차이 폴라로그래피를 이용한 갑상선기능조절 약제 중의 2-mercapto-1-methyl-imidazole 분석)

  • The determination of 2-mercapto-1-methyl-imidazole (MMI) in 5.0${\times}10^{-2}$ M lithium perchlorate suporting electrolyte has been investigated by the differential pulse polarography. The optimum condition of MMI analysis was as follows; -0.9 volts initial potential, 0.08 mV pulse height, 2 mV/sec scan rate, and medium mercury drop size. Standard calibration curve showed a good linearlity in the range of 1.0${\times}10^{-7}M\;to\;8.0{\times}10^{-5}$ M and the detection limit has been (2.2${\pm}0.1){\times}0.1^{-9}$ M. This method was applicated for the determination of MMI in antithyroid drug without interference of additives.

  • Objective : Graves' disease, the most common cause of hyperthyroidism, is an autoimmune disorder associated with autoantibodies to the TSH receptor. The clinical features of Graves' disease are goiter and hypermetabolic symptoms induced by excessive hormones. Antithyroid drug therapy is the first-line treatment for Graves' disease in Korea, Japan and European countries. Yet in spite of a long period and high-dose of treatment, it is hard to achieve remission because of adverse effects, frequent recurrence and resistance to antithyroid drugs. Recently, it has been reported that the abnormal thyroid hormone and clinical symptoms of Graves' disease were reduced by Ahnjeonbaekho-tang (AJBHT). Methods : To investigate the effectiveness and action mechanism of AJBHT, we studied the influence of AJBHT on FRTL-5 thyroid cell proliferation, DNA synthesis and expression of T4, TSH, cAMP, Tg and TPO mRNA. Results : AJBHT significantly inhibited the FRTL-5 cell proliferation, DNA synthesis, T4 synthesis, cAMP production and the expression of Tg mRNA in comparison with control and MMI. Conclusions : These results suggest that AJBHT may inhibit the cell proliferation and DNA synthesis by regulating the cAMP, and suppress the T4 synthesis by modulating Tg mRNA expression and cAMP synthesis, and that it may be useful agent for treating the goiter and hormone abnormality of Graves' disease.

  • Acute supraglottitis is characterized by an inflammation and edema of the supraglottic region and a potential life-threatening condition because of its risk for sudden upper airway compromise. Prompt diagnosis, administration of broad spectrum antibiotics, and airway management is pivotal for reducing serious complications. In the immunocompromised host, microorganisms are more likely to elicit mucosal inflammations, thus clinicians should pay attention to those patients for prompt removal of the causes of immune disruption. Here we report a case of acute adult supraglottitis with neutropenia caused by anti-thyroid drug with a review of the related literatures.
