Gastritis in a Dog Infected with Helicobacter heilmannii (Helicobacter heilmannii 균 감염이 확인된 개에서의 위장염 발생 증례)

  • A 3-year-old intact female korean pungsan dog showing chronic intermittent vomiting and anorexia was admitted to The Veterinary Medical leaching Hospital of Seoul National University. The patient had been received antiemetics, antacids or proton pump inhibitor for these symptoms but had not been recovered yet. All results of physical examination, complete blood count, serum chemical analysis, fecal test and radiography were normal. Gastric endoscopic examination was also performed and gastritis with hemorrhage on near antrum region was detected. In PCR assay of bioptic gastric tissues for detecting Helicobacter species infection, Helicobacter heilmannii infection was confirmed. After receiving triple therapy with amoxicillin, metronidazole and omeprazole for 21 days, all symptoms were recovered and had not been occurred again. Helicobacter heilmannii also were not detected.

Antiemetic Effect of Dolasetron Mesylate in the Prevention of Acute and Delayed Nausea and Vomiting due to Moderately Emetogenic Chemotherapy (악성종양환자에서 중등도 이상의 오심, 구토를 유발하는 항암화학요법 시급성 및 지연성 오심, 구토의 예방에 대한 Dolasetron의 효과)

  • Purpose: To evaluate the efficacy of dolasetron mesylate in controlling nausea and vomiting in the first 24 hours and to extend these comparisons over the next 4 days in patients receiving moderately emetogenic chemotherapy. Methods: This was a single center, open-labeled study with single arm. Dolasetron (1.8 mg/kg) was given intravenously (I.V.) prechemotherapy with 10 mg of dexamethasone IV, followed 24 hours later by oral dolasetron (200 mg once daily) for the subsequent 4 days. The frequency of vomiting, severity of nausea and the presence of rescue antiemetics were assessed daily. Results: Of 30 patients enrolled, 28 were eligible and evaluable for the efficacy. Four out of 28 patients had complete control of nausea and vomiting without any rescue antiemetics through 5 days. The complete control got better as time went by with the rates of 17.9/46.4/42.9/53.6/60.7% on days 1 to 5. Vomiting was better controlled than nausea in both cisplatin-containing and non-containing chemotherapy. The adverse events were mild to moderate degrees of headache, diarrhea and fever, but were recovered spontaneously. Conclusion: Dolasetron was effective and safe for the control of nausea and vomiting in the patients with moderately emetogenic chemotherapeutic agents.

  • Citrus unshiu Marc., a family of Rutaceae, has been used as antitussives, antiemetics, expectorants, spasmolytics, stomachics and digestants in oriental medicine. But there is few reports on the pharmacological effects of Citrus unshiu pericarpium. Present experiment was performed to observe the effects of the water extract of Citrus unshiu on isolated auricle and intestine in rabbits and blood pressure in rats and also to study on the active component on the cardiovascular system by means of thin layer chlomatography and spectrophotometer. 1. The water extract showed positive inotropic and positive chronotropic effect in the concentration of $1{\times}10^{-4}g/ml$, $5{\times}10^{-4}g/ml$, and $1{\times}10^{-3}g/ml$. 2. The motility of isolated rabbit intestine was significantly depressed by the water extract. 3. The blood pressure was markedly increased by the water extract. 4. The blood vessel of rabbit ear was constricted by perfusion of the ether extract. 5. The purified alkaloid of Citri pericarpium by ion exchange resin showed the same Rf value with l-synephrine on thin layer chromatography. 6. The purified alkaloid of Citri pericarpium by thin layer chromatography showed the same absorption curve with l-synephrine in the ultraviolet scanning.

Effects of the Nei-Guan Acupressure by Wrist Band on Postoperative Nausea and Vomiting after Middle Ear Surgery (손목밴드를 이용한 내관지압이 중이수술 후 오심과 구토에 미치는 효과)

  • Purpose: This study was to identify the effects of Nei-Guan accupressure using a wrist band on postoperative nausea and vomiting after middle ear surgery. Methods: The study design was a nonequivalent control group non-synchronized posttest design. Forty patients were assigned into two groups of 20 patients, the wrist band group and the control group. In the wristband group, acupressure was applied with a wrist band on the P6 point at both wrists from 30 min before to 24 hr after anesthesia. Results: The 1st hypothesis 'Applied wrist band groups will have a greater reduction in the nausea & vomiting grades by INVR than the control group' was accepted (t=2.303, p=.028). The 2nd hypothesis 'The Applied wrist band groups will have a greater reduction in the occurrence of nausea & vomiting than the control group' was partly accepted. The 3rd hypothesis 'The applied wrist band group will have a greater reduction in injection of antiemetics for 24 hr. post anesthesia than the control group' was rejected. Conclusion: Acupressure on the P6 point using a wrist band is effective to alleviate nausea for middle ear surgery patients.

Incidence and Factors Influencing Oral Mucositis in Patients with Hematopoietic Stem Cell Transplantation (조혈모세포이식 환자의 구강 점막염 발생실태와 영향요인)

  • Purpose: This study was done to examine the incidence of oral mucositis in hematopoietic stem cell transplantation patients and to identify factors influencing oral mucositis and patient outcomes according to severity. Methods: In this retrospective study, data were collected from electronic medical records of 222 patients who had received hematopoietic stem cell transplantation. Oral mucositis was evaluated using WHO's assessment scale. Data were analyzed using Chi-square test, Fisher exact test, Spearman's correlation, Ordinal logistic regression, ANOVA and Kruskal-Wallis test. Results: A total of 69.8% of the patients evaluated developed oral mucositis (grade II and over). As a results of ordinal regression, factors influencing oral mucositis severity were found to be diagnosis, type of transplantation, oxygen inhalation and the number of antiemetics administration before transplantation. The severity of oral mucositis was found to increase the days of hospitalization, days of TPN administration, days of using antibiotics and the number and dosage of analgesics. Conclusion: The results would help predict severity of oral mucositis in hematopoietic stem cell transplantation patients and suggest that provision of appropriate nursing assessment and oral care would improve patient outcomes.

The Effects of Music Therapy on Anxiety, Nausea and Vomiting in Patients Receiving Total Body Irradiation for Hematopoietic Stem Cell Transplantation (음악요법이 조혈모세포 이식 전 전신 방사선조사 시 불안, 오심과 구토에 미치는 효과)

  • Purpose: The present study was to investigate the effect of music therapy on anxiety, nausea, and vomiting in patients undergoing total body irradiation (TBI) for hematopoietic stem cell transplantation (HSCT). Method: A untreated control group with pretest and posttest was used in this study. A total of 35 patients receiving TBI for HSCT were recruited from a medical center at a university in Korea. The music therapy taking for 15-20 minutes per a time was performed twice a day for three days while the patients in the experimental group were undergoing the TBI. Results: The scores of state anxiety, nausea and vomiting tended to increase in the control group as compare with those in the experimental group. The patients in the control group had been administerd antiemetics more often than those in the experimental group. The levels of systolic and diastolic blood pressure and the pulse rates in the experimental group tended to decrease after the experiment. However, there was no statistically significant differences in anxiety, nausea, vomiting, blood pressure, and pulse between the groups. Conclusion: It is suggested to repeat this study with a enough sample size.

Flurbiprofen toxicity in 2 dogs (두 마리 개에서의 flurbiprofen 중독 2례)

  • Two dogs were presented with melena, vomiting and depression after accidental swallowing of candy form of Strepsils (flurbiprofen), which is one of non-steroidal anti-inflammatory drugs used in human medicine for controlling a sore throat. These dogs had common signs of anemia induced by gastrointestinal ulceration and hemorrhage with azotemia and leukocytosis. The dogs were treated with blood transfusion, fluid therapy, proton-pump inhibitor, antiemetics, mucus protectant and antibiotic. Although most of clinical signs of two dogs were resolved, azotemic problem with evidence of renal injury have remained.

  • Epidural morphine provides excellent analgesia for the management of postoperative pain, but nausea and vomiting are a commonly reported side effect. Scopolamine, a belladona alkaloid, is an effective antiemetic when nausea is induced by morphine. Transdermal scopolamine patches have the advantage of delivering a constant low dosage of the drug over a prolonged period. To evaluate the efficacy of prophylacitic transdermal scopolamine in reducing nausea or vomiting associated with postoperative epidural morphine analgesia, I studied 60 healthy adult patients. The patients were divided into 3 groups, each group consisting of 20 patients. Group 1; no scopolamine for control Group 2; transdermal scopolamine placebo patch Group 3; transdermal scopolamine patch All patients were anesthetized by epidural injection of 2% lidocaine 15 ml and 0.5% bupivacaine 10 ml with morphine 4 mg. A Comparison with the control group, the placebo group, and Group 3, indicated, that the transdermal scopolamine reduced the incidence of nausea or vomiting associated with postoperative epidural morphine analgesia (group 1; 35%, group 2; 25%, group 3; 10%). However there were no statistically significant differences between groups at a level of p>0.05.

  • Objectives: Pregnant women often experience the symptoms of Hyperemesis Gravidarium. Many pregnant women think it is a passing symptom, so, they usually don't receive aggressive treatment. But if there is a heavy degree may lead to dangerous situations, so, if necessary, should receive aggressive treatment. Methods: The treatment of Hyperemesis Gravidarium, there are several ways. We provided herbal medicine and acupuncture, observed changes of subjective complaint. Results: In this report, we gained significant effects with oriental medical treatment. To this Hyperemesis Gravidarium patient, oriental medical treatment rather than conservative treatment using fluid therapy and antiemetics was effective. Conclusions: This patient was mainly treated with herbal medicine and acupuncture of oriental medical treatment. But, to the patient of Hyperemesis Gravidarium, you may consider other treatments and we suggest that you should take advantage of various therapies.

Factors Associated with Discontinuation of Postoperative Intravenous Patient Controlled Analgesia (수술 후 자가통증조절요법 중단 관련요인)

  • Purpose: The purpose of this study was to identify the factors associated with the discontinuation of patient controlled analgesia (PCA) after surgery. Methods: The data of 1,092 adult patients that were over 20 years of age and underwent PCA after surgery in the Gachon University Hospital from May 1 to June 30, 2017, were collected through the patients' Electronic Medical Record (EMR). The collected data was analyzed via the use of the Chi-test, t-test and multivariate logistic regression analysis using SPSS 18.0 program. Results: The postoperative PCA discontinuation rate was 26.1%. It was associated with various symptoms, such as those of nausea, dizziness, and headache. The PCA discontinuation was also related with female (odds ratio, OR= 1.75; confidence interval, CI= 1.09-2.82), nausea (OR= 105.27; CI= 61.03-181.58), total intravenous anesthesia (TIVA) of the thyroidectomy (OR= 10.43; CI= 5.01-21.70). Conclusion: It is necessary to provide additional medication and nursing interventions to reduce nausea, which is the symptom associated with PCA discontinuation, especially in the operation of female subjects and thyroidectomy under TIVA. That is, those who are at a high risk for PCA discontinuation should be able to administer additional antiemetics or reduce non medication nursing interventions.
