Objectives: Obesity is a globally prevalent public health issue. Hence, there is a need for the development of safer and more effective anti-obesity drugs. Lythrum salicaria, a traditional medicinal herb used for centuries, has been reported to improve lipid metabolism and fat accumulation. It also has a low toxicity profile. Therefore, its potential as a functional ingredient in health functional foods needs to be evaluated. Methods: In this randomized, double-blind, placebo-controlled clinical trial, 90 participants will be randomly assigned to either the experimental or control group. Each subject will orally receive L. salicaria extract (1,350 mg/day) (500 mg L. salicaria+850 mg lactose as vehicle) or lactose (1,350 mg/day) as a hard capsule formula for 84 days (12 weeks). The primary outcome will be body fat mass (kg), which will be assessed using dual-energy x-ray absorptiometry (DXA) (performed only at visits 2 and 4). Secondary outcomes include body mass index, body weight, waist-to-hip ratio, body fat percentage (%) measured using DXA, lean body mass (kg) measured using DXA (assessed only at visits 2 and 4), lipids (total cholesterol, triglyceride, high-density lipoprotein cholesterol, and calculated low-density lipoprotein cholesterol), free fatty acid, high sensitivity C-reactive protein, adiponectin, and leptin. Conclusions: This protocol will be implemented after approval of Institutional Review Board of Pusan National University Korean Medicine Hospital (approval number: PNUKHIRB-2022-08-002) and registration with the Korean National Clinical Research Information Service (CRIS) (CRIS-KCT0008060). The results of this trial will provide potential of L. salicaria as a new anti-obesity functional food with fat-reducing effects and low toxicity.
The hormone resistin is associated with type II diabetes mellitus in rodent model. Resistin impairs glucose tolerance and insulin action. A new class of anti-diabetic drugs were called thiazolidinediones (TZDs) downreguates a resistin. Resistin gene expression is induced during adipocyte differentiation and resistin polypeptide is secreted by adipocytes. But, the correlation between increased adiposity and resistin remains unknown. The objectives of this study was to clone a mouse resistin CDNA and to generate transgenic mice overexpressing mouse resistin gene. The pCMV-mus/resistin gene was prepared from previous recombinant pTargeT$^{TM}$-mus/resistin by digestion of Bgl II, and has used for microin- jection into pronuclei of one cell embryos. Mouse resistin expression was detected in transgenic F$_1$mice by RT-PCR. The transgenic mouse with resistin gene expression has heavier body weight which was measured higher level of plasma glucose than that of normal mouse. And in diet-induced experiments, in fasting group, resistin expression was higher than that of re-feeding group. This result demonstrates that the resistin gene overexpressing mice may be became to obesity and be useful as an animal disease model to be diabetes caused by insulin resistance of resistin.n.
BACKGROUND/OBJECTIVES: Obesity is a global health problem of significant importance which increases mortality. In place of anti-obesity drugs, natural products are being developed as alternative therapeutic materials. In this study, we investigated the effect of Brassica juncea L. leaf extract (BLE) on fat deposition and lipid profiles in high-fat, high-cholesterol diet (HFC)-induced obese rats. MATERIALS/METHODS: Male Sprague-Dawley rats were divided into four groups (n = 8 per group) according to diet: normal diet group (ND), high-fat/high-cholesterol diet group (HFC), HFC with 3% BLE diet group (HFC-A1), and HFC with 5% BLE diet group (HFC-A2). Each group was fed for 6 weeks. Rat body and adipose tissue weights, serum biochemical parameters, and tissue lipid contents were determined. The expression levels of mRNA and proteins involved in lipid and cholesterol metabolism were determined by reverse transcription polymerase chain reaction and western blot analysis, respectively. RESULTS: The HFC-A2 group showed significantly lower body weight gain and food efficiency ratio than the HFC group. BLE supplementation caused mesenteric, epididymal, and total adipose tissue weights to decrease. The serum levels of triglyceride, total cholesterol, and low-density lipoprotein cholesterol were significantly reduced, and high-density lipoprotein cholesterol was significantly increased in rats fed BLE. These results were related to lower glucose-6-phosphate dehydrogenase, acetyl-coA carboxylase, and fatty acid synthase mRNA expression, and to higher expression of the cholesterol $7{\alpha}$-hydroxylase and low density lipoprotein-receptor, as well as increased protein levels of peroxisome proliferator-activated receptor ${\alpha}$. Histological analysis of the liver revealed decreased lipid droplets in HFC rats treated with BLE. CONCLUSIONS: Supplementation of HFC with 3% or 5% BLE inhibited body fat accumulation, improved lipid profiles, and modulated lipogenesis- and cholesterol metabolism-related gene and protein expression.
비만의 원인은 유전적, 대사적 및 환경적 요인 등 다양한 가운데 특정 세균 및 바이러스 감염에 의해 비만이 유도되는 것이 확인되며 이러한 비만은 감염성 비만이라고 한다. 또한 감염성 비만을 유발하는 병원체 가운데 아데노바이러스 36이 밝혀졌다. 현재 비만 예방 및 치료제 개발에 대한 연구가 활발히 진행되는 가운데 감염성 비만의 예방 및 치료제 개발에 대한 연구는 불충불한 실정이다. 이와 관련하여 본 연구에서는 제주에 자생하는 조록나무 잎을 이용하여 분획물을 제조하고, 비만을 유도하는 사람아데노바이러스 36 증식에 미치는 영향을 평가하고자 한다. A549세포에 사람아데노바이러스 36을 감염시키고, 조록나무 잎 에틸 아세테이트 분획물을 처리하여 바이러스 역가를 측정하였다. 그 결과, 조록나무 잎 에틸 아세테이트 분획물을 처리한군에서 농도 의존적으로 바이러스 증식 억제능을 확인하였다. 또한 조록나무 에틸 아세테이트 분획물의 물질 분리를 한 결과, quercitrin이 확인되었으며, 표준물질 quercitrin의 사람아데노바이러스 36에 대한 증식 억제능을 확인하였다. 이러한 결과 조록나무 잎 에틸 아세테이트 분획물은 감염성 비만 치료제 개발을 위한 효과적인 후보 물질로 사료되며, 본 연구는 감염성 비만 치료제 개발을 위한 기초 자료로 유용할 것으로 보인다.
최근 연구에서 활성 산소 및 비만에 대한 유전자 발현이 비만과 노화를 촉진시키는 원인으로 주목 받고 있으며, 이를 통해 노화와 비만의 억제 체계를 규명하려는 다양한 방법들이 연구되고 있다. 본 연구에서는 우리나라 전국 각지의 산야지에 자생하며 농가에서 재배하기도 하는 국화과에 속하는 식물인 참취(Aster scaber Thunb.) 메탄올 추출물의 항산화력과 3T3-L1 전구지방세포의 지방세포형성과정에 미치는 영향을 확인하였다. 참취 메탄올 추출물에 존재하는 총 폴리페놀 및 플라보노이드 함량은 각각 $57.07{\pm}2.17$, $54.62{\pm}2.24{\mu}g/mg$으로 나타났다. 시료의 DPPH, ABTS radical 소거 활성을 측정한 결과 $RC_{50}$값이 각각 $22.24{\pm}0.40$, $53.19{\pm}1.61{\mu}g/mL$로 우수한 항산화 효과를 나타냈다. 참취의 메탄올 추출물이 3T3-L1 전구지방세포에서 분화유도 물질(IBMX, DEXA, Insulin)과 함께 처리했을 때 지방구의 형성을 농도 의존적으로 감소시켰다. 또한 이들은 지방세포의 증식 및 분화되는 과정에서 발현되는 adipogenic transcription factor 및 관련 유전자의 단백질 발현과 mRNA 발현을 유의적으로 감소시키는 것을 확인하였다. 이상의 결과로 참취 메탄올 추출물을 이용하여 항비만에 우수한 효능을 가지는 기능성 식품 소재로서의 개발이 가능할 것으로 생각된다.
2021년에도 의료와 관련된 많은 판결들이 있었는데, 그 중 본 논문에서 검토한 판결들은 다음과 같다. 먼저 진료기록 부실기재 및 변조 등과 주의의무위반 관련 판결은 의료과실 유무 등에 관한 일차적 판단자료인 진료기록이 사후에 수정된 사례에 관한 것으로 그 수정내용 및 수정시기에 비추어 사후에 수정된 진료기록 내용은 고려하지 않고 최초 작성된 진료기록을 토대로 과실 유무 판단을 하였다. 다음으로 비만치료약 처방 등에 대한 손해배상책임을 묻는 사례에 관한 판결은 처방과 관련한 과실을 인정하였으나 상당인과관계를 부정하여 재산상 손해배상책임을 부정하고, 설명의무위반에 따른 위자료만 인정하였다. 또한, 환자의 가해자에 대한 기왕치료비 손해배상채권을 대위하는 국민건강보험공단의 대위범위에 관한 전원합의체판결은 '과실상계 후 공제방식'을 취해온 기존 판례를 변경하여 '공제 후 과실상계방식'으로 대위 범위를 판단하여 피해자 보호를 도모하였다. 그리고 과실 유무에 관해 진료기록감정회신결과와 달리 판단한 판결은 과실유무 판단을 함에 있어 진료기록감정결과에 구속되는 것은 아니고 자유심증에 따라 판단한다는 입장에 따라 규범적으로 판단하였다. 마지막으로 국민건강보험공단의 요양급여비용환수처분과 관련해서는 비의료인이 개설한 의료기관에 대한 환수처분을 함에 있어서도 재량권을 행사해야 한다는 판결과 시설 및 인력을 공동이용한 의료기관에 대한 환수처분의 경우 그 환수범위를 세부적으로 판단해야 한다는 판결을 검토하였다.
본 연구에서는 고지방식이 투여로 비만 및 고지혈증을 유도한 동물에서 L. plantarum CECT 7527, 7528과 7529 혼합물이 비만 및 고지혈증에 미치는 영향을 조사하고자 하였다. 흰쥐에 8주 동안 고지방식이를 급여하면서 L. plantarum 혼합물을 $0.6{\sim}2.4{\times}10^9CFU/day$를 경구 투여하였고 동물을 희생하여 혈청 및 간조직 내의 지질 함량을 조사하였다. L. plantarum 혼합물 투여에 의해 체중증가량, 간과 부고환 지방 무게가 현저히 감소하였다. 또한 비만과 관련 있는 호르몬인 혈청 렙틴의 함량이 L. plantarum 혼합물 투여에 의해 감소하였다. L. plantarum 혼합물 투여가 혈청 중성지방과 HDL-콜레스테롤 함량 및 혈당에는 영향을 미치지 않았으나 혈청 총콜레스테롤 함량 및 LDL-콜레스테롤 함량을 현저히 감소하였다. 이와 더불어 L. plantarum 혼합물 투여가 동맥경화지수와 심혈관위험지수를 유의적으로 감소하였다. 간조직 내의 총 지질 함량, 중성지방 함량 및 총콜레스테롤 함량은 L. plantarum 혼합물 투여에 의해 유의적으로 감소하였다. 이상의 결과는 L. plantarum CECT 7527, 7528과 7529가 혼합된 L. plantarum 혼합물이 체지방 감소 및 고지혈증 개선 효과를 나타냄을 보여준다. 향후 더 많은 연구가 필요하나 본 연구는 L. plantarum 혼합물을 비만 및 고지혈증 개선제로 개발할 수 있는 가능성을 제시한다.

But the connection between increased adiposity and resistin remains unknown. The objectives of this study was to clone a mouse resistin cDNA and to generate transgenic mice overexpressing mouse resistin gene. The 555 bp of mouse resistin was amplified from mob cDNAS by polymerase chain reaction (PCR) and cloned into pCR$\^$(R)/ 2.1 TOPO T-vector. Mouse resistin mRNA on the basis of Genbank sequence (acession no. AF323080). Then, the PCR product was cloned into pTargeT$\^$TM/ mammalian expression vector that has pCMV promoter and chimeric intron. Restriction enzyme analysis with BamH I and Not I was carried out to determine an orientation of the insert in the vector. The pCMV-mus/resistin gene was prepared from previous recombinant pTargeT$\^$TM/-mus/resistin by digestion of Bgl II, and has used for microinjection into pronuclei of one cell embryos. The microinjected embryos were transfered to pseudopregnant foster-mother. Mouse resistin expression was detected in transgenic F1 mice by Reverse Transcriptase- Polymerase Chain Reaction (RT-PCR). Resistin gene expression mouse has heavier body weight which was measured higher level of plasma glucose than that of normal mouse. And in diet-induced experiments, the abdominal fat pads were isolated from each 24h starvation and re-feeding after fasting group mice that were assessed by RT-PCR analysis. In fasting group mice, resistin expression was higher than that of re-feeding group mice. This result suggests that the resistin gene overexpressing mice may be became to obesity and be useful as an animal disease model to be diabetes mellitus caused by insulin resistance of resistin.
Phentermine (PT) and phenmetrazine (PM) have been widely used as anti-obesity drugs. These drugs should be used with caution due to its close relation to amphetamine in its structure and toxicity. PT and PM, amphetamine-type anorectics, have recently been considered as alternatives for methamphetamine abuse in Korea. In addition, the misuse and abuse of PT and PM obtained by illegal sources such as the internet become a serious social problem. In the present study, a simultaneous detection and quantification method for determining PT and PM in human urine was developed and validated according to the international guidelines. The urine samples were screened using a fluorescence polarization immunooassay and analyzed by gas chromatography mass spectrometry (GC-MS) after extraction using automatic solid phase extraction (SPE) with a mixed-mode cation exchange cartridge and derivatization with pentafluoropropionic anhydride (PFPA). The validation results for selectivity, linearity, limits of detection (LOD) and quantification (LOQ), intra- and inter-assay precision and accuracy and recovery were satisfactory. The validated method was successfully applied to authentic urine samples collected from 38 drug abuse suspects. PT and/or PM were identified with or without methamphetamine in urine samples. Abuse of PT and PM have increased continuously in Korea, therefore, closer supervision of the inappropriate use of anoretics is necessary.

F. prausnitzii는 next-generation probiotic로서 건강한 성인의 경우 장내미생물 균총의 약 5%를 차지하며 성인 장 건강의 지표이다. F. prausnitzii는 butyrate-producing bacterium로서 항염증 효과를 나타내며, 면역 질환과 당뇨병 치료를 위한 next-generation probiotics로 기대된다. Postbiotics는 probiotics에 의해 분비되는 세포 상층액에 포함된 대사산물의 복잡한 혼합물이다. 반면, parabiotics는 probiotics를 불활성화 시킨 미생물 세포이다. Paraprobiotics와 postbiotics는 probiotics에 비해 명확한 화학구조(clear chemical structures)와 안전한 투여 용량(safety dose parameters), 장기간의 유통 기간(longer shelf life) 등 많은 장점을 가지고 있어서 probiotics를 대체할 수 있는 잠재력을 가지고 있다. 장 생태계의 불균형을 정상적으로 복원하기 위해서 장내 공생 박테리아(commensal bacteria) 중 next-generation probiotics (NGPs)를 사용하는 것이 가장 자연스런 방법이다. 따라서 next-generation probiotics 를 대상으로 parabiotics와 postbiotics와 같은 새로운 식품이나 약품으로 개발하는 것이 필요하다.
