골격성 고정원을 이용한 상악 6전치 후방 견인시 힘의 적용점 변화에 따른 치아 이동 양상에 관한 유한 요소법적 분석 (Three-dimensional finite element analysis of initial tooth displacement according to force application point during maxillary six anterior teeth retraction using skeletal anchorage)

  • 본 연구에서는 활주법을 이용한 전치부 후방 견인시 micro-implant의 다양한 수직적 위치와 전치부에서 힘의 적용점에 따른 치아 이동 양상을 관찰하여 공간 폐쇄시 전치부의 설측 경사와 정출력을 방지할 수 있는 micro-implant의 위치와 전치부 힘의 적용점의 위치를 알아보고자 하였다. 유한 요소 모델을 이용하여 제1소구치가 발거된 상악 치열궁 형태를 제작하고 $.022"{\times}.028"$ 슬롯 브라켓을 모형화하여 치아에 부착시켰다. $.019"{\times}.025"$ stainless steel 선재를 3차원 beam모형으로 제작하고 상악 측절치와 견치 브라켓 사이의 선재 상에 $.032"{\times}.032"$ 크기의 stainless steel hook을 수직으로 8mm의 높이로 형성하였으며, 선재로부터 2mm높이에서 1mm간격으로 8mm까지 힘 적용점을 설정하였다. 지름 1.2mm,길이 6mm의 micro-implant를 제2소구치와 제1대구치 사이의 치조골에 선재로부터 4mm에서 10mm까지 2mm간격으로 4개를 위치시켰다 각각의 micro-implant와 전치부 hook에 150gm의 힘을 적용시켜 다양한 힘 적용점에 따른 치아의 초기 변위를 분석하여 다음의 결과를 얻을 수 있었다. 1. Micro-implant 높이가 4m일 경우 5mm이하의 전치부 hook 높이에서는 전치부 설측 경사 이동이 일어났으며 전치부 hook 높이가 6m이상 되었을 때 전치부 순측 경사 이동이 일어났다. 2. Micro-implant높이가 6mm일 경우 5mm이하의 전치부 hook높이에서 전치부 설측 경사 이동이 일어났으며 전치부 hook 높이가 6mm 이상 되었을 때 전치부 순측 경사 이동이 일어났다 이것은 4mm micro-implant에서의 실험결과와 유사한 이동 양상을 나타내었지만, micro-implant높이가 6mm일 때 전치부 설측 경사 이동이 좀더 감소하였다. 3. Micro-implant높이가 8m일 경우 전치부 hook높이가 2mm일 때 전치의 설측 경사 이동이 일어났으며 3mm이상의 전치부 hook 높이에서 순측 경사 이동이 비례적으로 증가하였다. 4. Micro-implant높이가 10mm일 경우 전치부 hook 높이가 2mm 이상에서 전치의 순측 경사 이동이 비례적ㅇ로 증가하였다. 5. 전치부 hook 높이가 증가할수록 전치의 순측 경사 이동이 증가되지만 선재의 뒤틀림에 의한 변형이 증가되므로 견치와 소구치 부위에서 정출력이 발생하는 바람직하지 못한 치아 이동 양상이 일어났다. 6. Micro-implant를 이용한 상악 5전치 후방 견인시 구치부의 이동은 선재와 브라켓 사이의 마찰력에 의해서 원심경사 이동이 발생하였다.

Relationship between tooth loss and carotid intima-media thickness in Korean adults

  • PURPOSE. The aim of this study was to examine the relationship between tooth loss and sub-clinical atherosclerosis in Korean adults. MATERIALS AND METHODS. The subjects were part of a cohort study conducted in Ansan city by the Korea University medical school as part of the Korean Genome project. 749 subjects over than 40 years old were evaluated. After taking panoramic radiography, the amount of tooth loss was calculated. The intima-media thickness (IMT) was assessed by using ultrasonography at the common carotid artery. Traditional cardiovascular risk factors for atherosclerosis were also evaluated. The relationship between tooth loss and the IMT was evaluated using ANOVA with Scheffe's multiple comparison method in univariate analysis. Multiple regression analysis was also performed to determine the significance between the IMT and tooth loss. RESULTS. With age, tooth loss increased, but there was no significant increase in other traditional cardiovascular risk factors. Univariate analysis revealed the IMT to be positively related with the amount of tooth loss. Regression analysis of the IMT in the anterior and posterior tooth loss revealed that only the posterior tooth loss was significantly related with the IMT at all sites of the common carotid artery (right far wall, P = .015; left far wall, P = .008; right near wall, P < .001; left near wall, P = .001). CONCLUSION. This study verified the positive relationship between the increased tooth loss at the posterior area and the accumulation of atheroma in arteries.

Esthetic evaluation of maxillary single-tooth implants in the esthetic zone

  • Purpose: The aim of this study is to assess the influence exerted by the observer's dental specialization and compare patients' opinion with observers' opinion of the esthetics of maxillary single-tooth implants in the esthetic zone. Methods: Forty-one adult patients, who were treated with a single implant in the esthetic zone, were enrolled in this study. Eight observers (2 periodontists, 2 prosthodontists, 2 orthodontists and 2 senior dental students) applied the pink esthetic score (PES)/white esthetic score (WES) to 41 implant-supported single restorations twice with an interval of 4 weeks. We used a visual analog scale (VAS) to assess the patient's satisfaction with the treatment outcome from an esthetic point of view. Results: In the PES/WES, very good and moderate intraobserver agreements were noted between the first and second rating. The mean total PES/WES was $11.19{\pm}3.59$. The mean PES was $5.17{\pm}2.29$ and mean WES was $6.02{\pm}1.96$. In the total PES/WES, the difference between the groups was not significant. However, in the WES, the difference between the groups was significant and prosthodontists were found to have assigned poorer ratings than the other groups. Periodontists gave higher ratings than prosthodontists and senior dental students. Orthodontists were clearly more critical than the other observers. The statistical analysis revealed statistically significant correlation between patients' esthetic perception and dentists' perception of the anterior tooth. However, the correlation between the total PES/WES and the VAS score for the first premolar was not statistically significant. Conclusions: The PES/WES is an objective tool in rating the esthetics of implant supported single crowns and adjacent soft tissues. Orthodontists were the most critical observers, while periodontists were more generous than other observers. The statistical analysis revealed a statistically significant correlation between patients' esthetic perception and dentists' perception of the anterior tooth.

Evaluation of Tooth Movement and Arch Dimension Change in the Mandible Using a New Three-dimensional Indirect Superimposition Method

  • Purpose: To analyze the amount and pattern of tooth movement and the changes in arch dimension of mandibular dentition after orthodontic treatment using a new three-dimensional (3D)-indirect superimposition method. Materials and Methods: The samples consisted of fifteen adult patients with class I bialveolar protrusion and minimal anterior crowding, treated by extraction of four first premolars with conventional sliding mechanics. After superimposition of 3D-virtual maxillary models before and after treatment using best-fit method, 3D-virtual mandibular model at each stage was placed into a common coordinate of superimposition using 3D-bite information, which resulted in 3D-indirect superimposition for mandibular dentition. The changes in mandibular dental and arch dimensional variables were measured with Rapidform 2006 (INUS Technology). Paired t-test was used for statistical analysis. Result: The anterior teeth moved backward, displaced laterally, and inclined lingually. The posterior teeth showed statistically significant contraction toward midsagittal plane. The amounts of backward movement of anterior teeth and forward movement of posterior teeth showed a ratio of 6 : 1. Although the inter-canine width increased slightly (0.8 mm, P<0.05), the inter-second premolar, inter-first molar, and inter-second molar widths decreased significantly with similar amounts (2.2 mm, P<0.05; 2.3 mm, P<0.01; 2.3 mm, P<0.001). The molar depth decreased (6.7 mm, P<0.001) but canine depth did not change. Conclusion: A new 3D-indirect superimposition of the mandibular dentitions using best-fit method and 3D-bite information can present a guideline for virtual treatment planning in terms of tooth position and arch dimension.

Influence of the anterior arch shape and root position on root angulation in the maxillary esthetic area

  • Purpose: This study was conducted to characterize the relationship of the angulation between the tooth root axis and alveolar bone axis with anterior alveolar(AA) arch forms and sagittal root position (SRP) in the anterior esthetic region using cone-beam computed tomography (CBCT) images. Materials and Methods: CBCT images that met the inclusion and exclusion criteria were categorized using a recent classification of AA arch forms and a SRP classification. Then, the angulation of the root axis and the alveolar bone axis was measured using mid-sagittal CBCT images of each tooth. The relationships of the angulation with each AA arch form and SRP classification were evaluated using 1-way analysis of variance and a linear regression model. Results: Ninety-eight CBCT images were included in this study. SRP had a greater influence than the AA arch form on the angulation of the root axis and the alveolar bone axis(P<0.05). However, the combination of AA arch form and SRP was more predictive of the angulation of the root axis and the alveolar bone axis than either parameter individually. Conclusion: The angulation of the root axis and alveolar bone axis demonstrated a relationship with the AA arch form and SRP in teeth in the anterior esthetic region. The influence of SRP was greater, but the combination of both parameters was more predictive of root-to-bone angulation than either parameter individually, implying that clinicians should account for both the AA arch form and SRP when planning implant placement procedures in this region.

Validation of three-dimensional digital model superimpositions based on palatal structures in patients with maximum anterior tooth retraction following premolar extraction

  • Objective: This study aimed to evaluate the superimposition accuracy of digital modes for measuring tooth movement in patients requiring anterior retraction after premolar extraction based on the proposed reference regions. Methods: Forty patients treated with bilateral maxillary first premolar extraction were divided into two groups: moderate retraction (< 7.0 mm) and maximum retraction (≥ 7.0 mm). Central incisor displacement was measured using cephalometric superimpositions and three-dimensional (3D) digital superimpositions with the 3rd or 4th ruga as the reference point. The Wilcoxon signed-rank test and linear regression analyses were performed to test the significance of the differences and relationships between the two measurement techniques. Results: In the moderate retraction group, the central incisor anteroposterior displacement values did not differ significantly between 3D digital and cephalometric superimpositions. However, in the maximum-retraction group, significant differences were observed between the anteroposterior displacement evaluated by the 3rd ruga superimposition and cephalometric methods (p < 0.05). Conclusions: This study demonstrated that 3D digital superimpositions were clinically as reliable as cephalometric superimpositions in assessing tooth movements in patients requiring moderate retraction. However, the reference point should be carefully examined in patients who require maximum retraction.

Prevalence of dental implant positioning errors: A cross-sectional study

  • Purpose: This study evaluated the prevalence of dental implant positioning errors and the most frequently affected oral regions. Materials and Methods: A sample was obtained of CBCT images of 590 dental implants from 230 individuals who underwent diagnosis at a radiology center using cone-beam computed tomography from 2017 to 2020. The following variables were considered: thread exposure, violation of the minimum distance between 2 adjacent implants and between the implant and tooth, and implant contact with anatomical structures. Descriptive data analysis and the Pearson chi-square test(P<0.05) were performed to compare findings according to mouth regions. Results: Most (74.4%) of the 590 implants were poorly positioned, with the posterior region of the maxilla being the region most frequently affected by errors. Among the variables analyzed, the most prevalent was thread exposure (54.7%), followed by implant contact with anatomical structures, violation of the recommended distance between 2 implants and violation of the recommended distance between the implant and teeth. Thread exposure was significantly associated with the anterior region of the mandible (P<0.05). The anterior region of the maxilla was associated with violation of the recommended tooth-implant distance (P<0.05) and the recommended distance between 2 adjacent implants(P<0.05). Implant contact with anatomical structures was significantly more likely to occur in the posterior region of the maxilla (P<0.05). Conclusion: Many implants were poorly positioned in the posterior region of the maxilla. Thread exposure was particularly frequent and was significantly associated with the anterior region of the mandible.

Esthetic improvement in the patient with one missing maxillary central incisor restored with porcelain laminate veneers

  • This article describes esthetic improvement in a patient with a missing maxillary left central incisor. Space analysis of the anterior dentition showed that minor tooth rearrangement was needed. Optimal space distribution for restorations was attained by orthodontic treatment. Through transforming tooth shape with porcelain laminate veneers, the maxillary left lateral incisor was transformed into central incisor and the maxillary left canine into a lateral incisor. The maxillary right central incisor was also restored for esthetic improvement. In a case of changing a tooth shape with porcelain laminate veneers, pre-treatment evaluation, space analysis and diagnostic wax-up are important factors.

성인 상악 전치 형태에 따른 치은의 임상적 소견 (Clinical features of the gingiva according to maxillary anterior teeth form in adult)

  • It has been suggested that morphologic characteristics of the periodontium are partly related to the shape and form of the teeth. Furthermore, the severity of symptoms of periodontal disease have been proposed to differ among these various morphologic entities or "biotypes". The purpose of the present study was to examine the relationship between the form of the crowns in the maxillary anterior tooth segment and (1) a group of morphological characteristics and (2) the thickness of the gingiva. The thickness of gingiva was measured by ultrasonic device(SDM). 100 subjects devoid of symptoms of destructive periodontal disease were examined regarding, e.g., probing depth, gingival recession, width of keratinized gingiva, thickness of the keratinized gingiva. From maxillary study cast, the width(at the apical third-CW) and the length(CL) of the crowns of the 6 anterior teeth were determined. A CW/CL-ratio was calculated for each tooth and averaged for each tooth region. The individual mean CW/CL-ratio values for the central incisors were ranked. The 10 subjects ranked highest and the 10 ranked lowest were selected as having either a long-narrow(group N) or a short-wide(group W) form of the crown of the tooth. The data for each of the examined parameters were averaged for each tooth region in each subject and mean values for subjects in groups W and N were compared using the Student t-test. Stepwise multiple regression analysis, including data from the whole sample, was performed for each tooth region with the thickness of the free gingiva as the dependent variable. The results from the analyses demonstrated that individuals with a long-narrow form of the central incisors displayed, compared to individuals with a short-wide crown, form (l) a narrow zone of keratinized gingiva, (2) a pronounced "scalloped" contour of the gingival margin. There was no significant difference between groups N and W with respect to the thickness of the keratinized gingiva. The CW/CL-ratio data revealed that a certain form of the crowns in the central incisors was accompanied by a similar form in the lateral incisors and canine tooth region. The regression analyses demonstrated that the thickness of the keratinized gingiva in central, lateral incisors and canines was significantly related to the width of the keratinized gingiva.

한국성인 정상교합자의 치아크기와 비율에 관한 연구 (A study of Korean Norm about tooth size and ratio in Korean adults with normal occlusion)

  • 대한치과교정학회 부정교합백서 발간위원회에서 보관중인 한국성인 정상교합자의 표본(남 22명, 여 55명)과 연세대학교 치과대학 교정과에서 부정 교합 백서 발간을 위하여 수집한 표본(남자: 21명)을 이용한 한국성인 정상교합자의 치아크기의 계측과 비율에 관한 연구를 통해, 한국성인 정상교합자의 치아크기의 평균치를 구하고, 조화로운 교합관계와 심미성의 획득에 기여하는 치아크기의 비율을 구하여 다음과 같은 결론을 얻었다. 1. 한국성인 정상교합자의 남녀 치아크기의 평균치를 구하였다. 2. 남녀간 치아크기의 평균치 비교에서 상하악 제1대구치를 제외한 치아에서 통계적으로 유의하게 남자의 치아크기가 여자의 치아크기에 비하여 크게 계측되었다(p<0.05). 3. 계측된 한국성인 정상교합자의 치아크기 평균치를 바탕으로 교정치료시 전치부에서의 적절한 수직피개도와 수평피개도, 구치부에서의 적절한 교합관계를 예측할 수 있게 해주는 치아비율을 산출하였다. 1) Sum of incisors = 4 : 2.97 2) Neff anterior coefficient = 1.28 3) Bolton의 anterior ratio = 78.29%, overall ratio = 91.14% 4. 혼합치열기에서 맹출된 4전치 폭경의 합과 상하악 편측 미맹출 견치와 소구치 폭경의 합과의 상관관계를 조사한 결과 양의 상관관계를 나타내었으며, 이 상관관계에 의해 상하악 편측 미맹출 소구치 폭경의 합을 예측할 수 있는 회귀방정식을 구하였다. 1) 미맹출 편측 상악 견치와 소구치 폭경의 합 예측 회귀방정식 = 10.435018 + 0.513346 ${\times}$ (하악 4전치의 합) 2) 미맹출 편측 하악 견치와 소구치 폭경의 합 예측 회귀방정식 = 9.654002 + 0.502565 ${\times}$ (하악 4전치의 합)
