발의 과도한 회내 상태가 슬관절 근력과 보행에 미치는 영향 (Effects of excessive Pronation of the Foot on Knee joint Strength and Gait)

  • BACKGROUND: This study aimed to determine the changes in muscle strength and walking ability in patients who complained of knee instability due to excessive pronation of the foot. METHODS: Twenty patients (ten men and ten women) who complained of instability of the knee joint due to excessive pronation of the foot participated in the experiment. In the experimental group, the internal rotation of the tibia caused by excessive adduction of the foot was maintained as external rotation, and the joint state was to recognize the movement of the joint position changed through maintenance of the muscle. This exercise was performed five times for each patient, and the muscle strength maintenance was performed for 20 seconds. In the control group, stretching and range of motion (ROM) exercises were performed. For the stretching exercise, one specific motion was performed for 20 second, and the ROM exercise was performed to confirm the change in muscle strength in the knee joint area and walking ability. RESULTS: The knee flexion and extension strength in the patients with excessive pronation of the foot differed significantly from those in the subjects from the control group (p<.05). Further, the before-after comparison of the step time and length in the evaluation of walking ability, which affects overall postural movement due to knee joint instability, revealed a significant difference between the experimental and control groups (p<.05). CONCLUSION: The patients that were subjected to manual therapy and ROM exercise for the knee joint showed improved knee joint muscle strength and walking ability compared to the subjects from the control group.

만성관절염 환자에 대한 관절염체조의 효과 (Effects of Ground Exercise for Arthritis Program in Person with Chronic Arthritis)

  • The purpose of this study was to explore the effects of GEAP on pain, joint function, activities of daily living(ADL) and fatigue in chronic arthritis patients. The GEAP was held twice a week for 6 weeks for chronic arthritis patients at one university hospital in Seoul, Korea. Thirty four subjects completed the program, who were recruited at four times from September, 1999 to September, 2000. The effect of GEAP were evaluated as follows: Pain severity and number of painful joints ADL, fatigue were measured before and after the GEAP. In order to examine the joint flexibility and strengthening, the followings were measured: the extent of the upward arm reach in both sides(flexibility of shoulder), the ability to touch fingertips of the both hands in back pat and rub(flexibility of arm), the degree of range of motion (ROM) of both ankles in their dorsiflexion(flexibility of ankle) and plantarflexion with standing with toe(strengthening of ankle), and the degree of knee extension, and the grip strength. Paired t-test and Wilcoxon signed rank test were used for data analysis and the significance of the differences in the variables was examined to compare the data obtained before and after the GEAP. After the GEAP, followings were found: 1. Pain severity and number of painful joints was significantly decreased. 2. The flexibility of both shoulders and arms, knee, both ankle were significantly improved. 3. The strengthening of both arms was significantly improved, but the strengthening of ankle was not changed. 4. ADL was significantly increased. 5. Fatigue was significantly decreased. In conclusion, GEAP used in this study was clearly proved to be an effective exercise program to reduce pain and fatigue, to enhance joint function and ADL in people with chronic arthritis. It is suggested that the GEAP should be recommended as one of the useful and appropriate nursing interventions for chronic arthritis patients.

PACE 프로그램이 퇴행성관절염 노인의 자기효능감과 통증 및 관절기능에 미치는 효과 - 미국이민 한국노인을 중심으로 - (The Effects of PACE Program on Self-efficacy, Pain and Joint Function in Korean Immigrant Elderly with Osteoarthritis.)

  • The PACE (People nth Arthritis Can Exercise) is an exercise program developed by the Arthritis Foundation to improve muscle strength and joint flexibility for patients with arthritis. The purpose of this study was to explore the effects of PACE program on self-efficacy, pain, and joint function in the Korean immigrant elderly. The PACE program was held twice a week for 6 weeks for Korean immigrant elderly who had osteoarthritis. Twenty four subjects completed the program, who were recruited in two places : 10 elderly in a senior residential apartment, and 14 elderly in a senior center supported by Congregated Meal Program for Korean Elderly. Self-efficacy(Sherer et al., 1982), pain severity(by using Visual Analogue Scale), and number of painful joints were measured before and after the PACE program. To examine the joint flexibility and strengthening, the followings were measured : the extent of the upward arm reach in both sides(flexibility of shoulder), the ability to touch fingertips of the both hands in back pat and rub(flexibility of arm), the degree of range of motion(ROM) of both ankles in their dorsiflexion(flexibility of ankle) and plantarflexion with standing with toe(strengthening of ankle), and the degree of knee extension. Wilcoxon signed rank test was used for data analysis and the significance of the differences in the variables was examined to compare the data obtained before and after the PACE program. It is suggested that the PACE program should be recommended as one of the useful and appropriate nursing interventions for elderly with osteoarthritis.

지연발생근육통에 대한 테이핑 처치가 통증정도와 가동성 회복에 미치는 영향에 관한 사례연구 (A Case Study of Taping Therapy Effects on Range of Motion and Pain in Delayed Onset Muscle Soreness)

  • This case study was peformed to assess the effectiveness of taping therapy for the patient who have delayed onset muscle soreness(DOMS ). The results were as follows: 1. 드롭 착지와 착지 후 점프 시 충격흡수 기전의 차이 분석 (Analysis of the Differences of the Shock Absorption Strategy between Drop-Landing and Countermovement-Jump)

  • The aim of this study was to investigate and identify the differences in lower extremity energy dissipation strategies between drop-landing and countermovement-jump maneuvers. Fourteen recreational athletes(Age : $23.3{\pm}2.1years$, Height : $172.3{\pm}4.0cm$, Weight : $69.2{\pm}4.7kg$) were recruited and instructed to perform drop-landing from 45 cm height and countermovement-jump from 45 cm to 20 cm height. The landing phase was taken as the time between initial contact and peak knee flexion. A motion-capture system consisting of eight infra-red cameras was employed to collect kinematics data at a sampling rate of 200 Hz and a force-plate was used to collect GRF data at a sampling rate of 2000 Hz. Paired t-test was performed to determine the difference in kinematics and kinetics variables between each task. During the countermovement-jump task, all of lower extremity joint ROM and the hip joint eccentric moment were decreased and the ankle joint plantarflexion moment was increased than drop-landing task. In the eccentric work during countermovement-jump task, the ankle joint displayed greater while knee and hip joint showed lesser than drop-landing. Therefore, the knee joint acted as the key energy dissipater during drop-landing while the ankle joint contributed the most energy dissipation during countermovement-jump. Our findings collectively indicated that different energy dissipation strategies were adopted for drop-landing and countermovement-jump.

우리나라 연령별 보행분석 비교연구 (The Comparative Study on Age-associated Gait Analysis in Normal Korean)

  • Purpose: This study was done to establish reference data for temporo-spatial, kinematic and kinetic parameters for normal Koreans as they age. Methods: Normal adults and children without a previous history of musculoskeletal problems were enrolled in this study. The normal subjects were divided by age into three groups: Group I: children ($11.95{\pm}0.29$ years); Group II: young adults ($23.90{\pm}3.67$ years); Group III: older adults ($71.40{\pm}4.08$ years). The temporo-spatial and kinematic data were measured using 6 MX3 cameras while each subject walked through a 10 m walkway at a self-selected speed. The kinetic data were measured using 2 force plates and were calculated by inverse dynamics. Results: Motion patterns are typically associated with a specific phase of the gait cycle. Our results were as follows: 1. There were significant differences between the different age groups in temporo-spatial parameters such as cadence, double support, time of foot off, stride length, step length, and walking speed. 2. There were significant differences between the groups in kinematic parameters such as range of motion (ROM) of the hip, knee and ankle in the sagittal plane, ROM of the pelvis, hip and knee in the coronal plane and ROM of the pelvis, hip and ankle in the transverse plane. 3. There were significant differences between the groups in kinetic parameters such as joint moments of force, joint mechanical power generation or absorption and ground reaction forces. Conclusion: The results of this study can be utilized (a) as a reference for kinematic and kinetic data of gait analysis in normal Koreans, and (b) as an aide in evaluating and treating patients who have problems relating to gait.

드롭랜딩 시 높이 변화에 따른 인체 분절의 충격흡수 전략에 관한 연구 (The Study of Strategy for Energy Dissipation During Drop Landing from Different Heights)

  • The purpose of current study was to investigate the effects of the heights on the lower extremities, torso and neck segments for energy dissipation during single-leg drop landing from different heights. Twenty eight young healthy male subjects(age: $23.21{\pm}1.66yr$, height: $176.03{\pm}4.22cm$, weight: $68.93{\pm}5.36kg$) were participated in this study. The subjects performed the single-leg drop landing from the various height(30, 45 & 60 cm). Force plates and motion-capture system were used to capture ground reaction force and kinematics data, respectively. The results were as follows. First, the ROM at the ankle, knee, hip and trunk was increased with the increased heights but the ROM at the neck was increased in the 60cm. Second, the angular velocity, moment and eccentric work at the ankle, knee, hip, trunk, and neck was increased with the increased heights. Third, the contribution to total work at the knee joint was not significantly different, while the ankle joint rate was decreased and hip and neck rate was increased in the 60cm, and trunk rate was increased with the increased heights. Lastly, the increase in landing height was able to augment the level of energy dissipation not only at the lower extremities but also at the trunk and neck. The findings showed that drop landing affect trunk and neck with lower extremity joints. Therefore, we need to consider that trunk and neck strengthening including stability should be added to reduce sports injury during prevention training.

인솔의 아치높이 및 경도 선호도가 정적 아치 높이 및 발목 안정성에 미치는 영향 (Effects of Preferred Arch Height and Hardness of the Insole on Static Arch Height and Ankle Stability)

  • Objective: The purpose of this study was to investigate the differences in static arch height and ankle stability according to the preference for insole height and hardness in the arch area. Method: The study participants were 20 adult males (age: 22.7 ± 1.8 yrs., height: 175.3 ± 4.3 cm, body weight: 72.5 ± 7.7 kg). First, the arch heights of all subjects were measured in static postures (sitting and standing). The inversion and eversion movements of the ankle joint were analyzed during walking (1.3 m/s & 1.7 m/s) and running (2.7 m/s & 3.3 m/s). The variables (static arch height, and inversion and eversion angle of ankle joint) were compared by classifying groups according to the preference for the height and hardness of the arch of the insole. First, it was divided into a high arch insole preference group (HAG, n=8) and a low arch insole preference group (LAG, n=12) according to the preference for the arch height of the insole. Second, it was divided into a high hardness insole preference group (HHG, n=7), medium hardness insole preference group (MHG, n=7), and low hardness insole preference group (LHG, n=6), according to the preference for the arch hardness of the insole. Results: First, the range of motion (ROM) of inversion-eversion at the ankle joint during walking was statistically smaller in HAG than in LAG (p<.05). Second, the arch height change of HHG was statistically greater than that of MHG and LHG (p<.05). Conclusion: In the case of flexible flat feet with a large change in arch height, providing a high hardness arch insole that can disperse foot pressure can improve comfort. It was found that people with high medial and lateral sway of the ankle joint preferred a low arch insole, but it is necessary to differentiate and compare the insole heights of the arch part in detail. In addition, in the case of fast motion such as running, the preference for the arch height and hardness of the insole was not related to the static arch height and ankle stability.

관절 질환의 통증과 가동력에 파라핀 치료가 미치는 효과 (The Effect of Paraffin Therapy on Release of pain and Range of motion with Joint Disease Patients)

  • This study was conducted in order to determine the effect of Paraffin Therapy on release of pain, skin temperature and range of motion with joint diseases. This study was performed on 42 patients with general joint diseases and 21 patients with degenerative joint diseases. All the subjects were randomly assigned to three groups with 21 each. The experiment had been done from June, 1. 2002 to September, 30. 2002, in two general hospitals and a social welfare center using Digital Thermometer Kane-May KM330 and VAS(visual analogue scale). Each pint was measured in terms of skin temperature, pain scale and ROM(range of motion). The results of the experiment are as follows : 1. In application of three different heat modalities (Paraffin bath, Infra red, Hydro pack) on the Knee, Ankle, Elbow pints of the subjects for 30 minutes, the elevation of skin temperature and the reduction of pain scale were found to be significantly different. When checked right after the 30 minutes' application, the Infra red resulted in a higher degree elevation of skin temperature than two other modalities, while Paraffin bath resulted in the highest degree of the elevation of skin temperature when it was checked one hour after the application was over. It means that Paraffin bath lasted longer than the other two modalities(F=14.691, p<.000). 2. With Paraffin bath application, pint pain was reduced significantly both in 30 minutes and one hour checks(F=20.675, p<.001). 3. In terms of ROM(range of motion) increase, didn't seem to have any significant differences in 30 minutes and one hour. The results suggest that Paraffin therapy be helpful in maximizing th effects on pint diseases. I strongly suggest that further studies be done on other pints and muscle diseases using Paraffin bath in different methods of applying the therapy.
