2 Cases of Amyotrophic Lateral Sclerosis(ALS) with Oriental Medical Treatment Evaluated by K-ALSFRS-R and ALSSS (K-ALSFRS-R과 ALSSS로 평가한 근위축성 측삭 경화증 2례 증례보고)

  • Objectives : Amyotrophic Lateral Sclerosis(ALS) is a progressive disease that causes degeneration of the motor neurons of the brain and spinal cord. The purpose of this case study is to improvement of oriental mediacl treatment on ALS. Methods : The patients were treated by acupuncture, moxibustion, cupping therapy, herbal medication, physical treatment. To determine the effects of Oriental medical treatment, we evaluated weekly used by Korean version of Amyotrophic Lateral Sclerosis Functional Rating Scale-Revised(K-ALSFRS-R), Amyotrophic Lateral Sclerosis Severity Scale(ALSSS). Results : Symptoms such as cervical and shoulder pain, knee pain, cold sweating, insomnia etc were improved after above treatment. But K-ALSFRS-R and ALSSS were no improvement after above treatment. Conclusions : The Oriental medical treatment is effective on local symptoms of ALS, but there were no functional improvement of ALS in this case study. It is necessary to have more examination about ALS.

  • Objectives This case study was about a Taeyangin patient with Amyotrophic Lateral Sclerosis identified as Hae-Yeok pattern. In this study, we report the significant improvement of extremity weakness of this patient after Sasang Constitutional medicine treatment.Methods The patient was identified as Taeyangin Hae-Yeok pattern and treated with Ogapijangchuk-tang and acupuncture. Korean-Amyotrophic Lateral Sclerosis Functional Rating Scale-Revised (K-ALSFRS-R) was used to assess the overall function of the patient. And the Global Assessment Scale (GAS) was used to assess the change of upper and lower extremities weakness after the treatment.Result and Conclusion The K-ALSFRS-R total score decreased from 30 points to 26 points and extremity weakness showed significant improvement in GAS after 12 days treatment. Furthermore, walking distance was much longer after treatment. In conclusion, this study shows that Sasang constitutional medicine can be effective treatment for Taeyangin patient with Amyotrophic Lateral Sclerosis.

  • Objective : This study is a report on treatment effect of Korean medical treatment to ALS(Amyotrophic Lateral Sclerosis) patient. The purpose of this case study is to improvement of Korean medical treatment and evaluate prognosis and clinical course of ALS patients. Methods : The patient was treated by acupuncture, Su-Gi therapy, herbal medication, physical treatment. To determine the effects of Korean medical treatment, we evaluated every four days used by Korean version of Amyotrophic Lateral Sclerosis Functional Rating Scale-Revised (KALSFRS-R), Amyotrophic Lateral Sclerosis Severity Scale(ALSSS). Results : Before mental contraction caused by everyday life nursing management, symptoms such as lumbar pain, getting hoarse, lower limb powerless & spasticity, upper limbs powerless, neckache, tongue-tied etc. were improved after above treatment. K-ALSFRS-R and ALSSS scores were also increased till patient's mental contraction Conclusions : The Korean medical treatment is effective on local symptoms of ALS, and there is also functional improvement of ALS diagnosed by external examination. It is necessary to have more examination about ALS.

A pilot clinical study on the Traditional Korean Medicine treatment of Amyotrophic lateral sclerosis (근위축성 측삭경화증에 대한 한방 치료의 임상선행연구)

  • Objectives : This study was to investigate the effect of Oriental medical treatment on ALS. Methods : We investigated 12 ALS patients which were admitted to Gwang-Ju O.M. hospital from Oct. 14, 2008 to Nov. 14, 2008. All patients were treated by SAAM-acupuncture, herb medication, Bee venom Pharmacopuncture therapy, Needle-embedding therapy, etc. We evaluated patients using the Amyotrophic lateral sclerosis Functional Rating Scale-Revised(ALSFRS-R), Medical Research Council (MRC) Scale. Results : After 30 days, mean ALSFRS-R score of patients was improved from $28.42{\pm}7.83$ to $29.08{\pm}7.99$, and mean MRC Scale of patients was improved from $24.79{\pm}8.37$ to $25.34{\pm}8.45$. But in both cases, the variation was not statistically significant. After 30 days, mean ALSFRS-R score and mean MRC Scale of patients was more improved in subjects with bulbar-onset, onset age: 51-60yrs., disease duration: 24-48mo. And the results showed partially significant difference. Conclusions : We think that the results of this case be a pilot study that proves the effect of Oriental Medical treatment on ALS.

The follow-up study on patients of Amyotrophic lateral sclerosis after 1 year (근위축성 측삭경화증 환자 1년 후 추적 조사)

  • Objectives : This study was to investigate the effects of Oriental medical treatment on ALS. Results : After 1 month, mean ALSFRS-R score of patients was $29.08{\pm}7.99$, 2 months $28.70{\pm}7.17$, 3 months $28.16{\pm}8.23$, 1 year $21.33{\pm}9.93$ and mean MRC Scale of patients was $25.34{\pm}8.45$, 2 months $25.34{\pm}8.45$, 3months $21.56{\pm}9.20$. But in both cases, the variation was not statistically significant. Conclusions : We think that the results of this case be a pilot study that proves the effect of Oriental Medical treatment on ALS.

Mountain Ginseng Pharmacopuncture Treatment on Three Amyotrophic Lateral Sclerosis Patients (산삼약침 치료를 시행한 근위축성 측삭 경화증 3례에 대한 임상보고)

  • Objectives : The objective of this study is to report the change of progress in symptoms and various scales after treated with Mountain Ginseng Pharmacopuncture(MGP) on the patients of Amyotrophic Lateral Sclerosis(ALS). Methods : The three ALS patients who treated with MGP, were checked the change of progress by ALS Functional Rating Scale(ALSFRS), ALS Severity Score(ALSSS), grasping power on both arms and circumference of both thighs and calves. Results : After MGP treatment on three ALS patients, first case did not grow worse during MGP treated whereas she got worse rapidly during none treated period. Second case did not grow worse during treated period and third case got worse in progress by slow degrees despite of MGP treatment. Conclusions : Although MGP could not control the progress of ALS completely, MGP may help the improving of quality of life(QOL) in ALS patients and have the effect of delayed ALS progression.

Depression and Caregiving Burden in Families of Patients with Amyotrophic Lateral Sclerosis (근위축성측삭경화증 환자를 돌보는 가족의 우울과 간병 부담감)

  • Purpose: The purpose of this study was to describe depression, caregiving burden and the correlation of the two variables in the families of patients with amyotrophic lateral sclerosis (ALS) and to clarify factors predicting caregiving burden. Methods: A descriptive and cross-sectional study was conducted with 139 family members who provided care to patients with ALS. The characteristics of patients and families, Korean-Beck Depression Inventory (K-BDI), Korean version of Zarit Burden Interview (K-ZBI) and Korean-Amyotrophic Lateral Sclerosis Functional Rating Scale - Revised (K-ALSFRS-R) were used as study measures. Results: The mean score for K-BDI was 19.39 out of 63 suggesting sub-clinical depression and 38.2% of the family members exhibited depression. The mean score for K-ZBI was 66.03 out of 88. The predictors for K-ZBI were K-BDI, age of family member, length of time spent per day in caring, relationship to patient and K-ALSFRS-R. Conclusion: The results of this study suggest that levels of depression and caregiving burden are high among family members caring for patients with ALS. As depression is associated with caregiving burden, screening and emotional supports should be provided to reduce the burden of care for these family. Support programs to alleviate the care burden are also needed, considering family demographics, time per day in caring giving and K-ALSFRS-R.

  • Amyotrophic Lateral Sclerosis(ALS) is Motor Neuron Disease(MND) that reveals muscle relaxation, bulbar palsy, extremities weakness, Pneumonia, in severe case, leads to death. Objectives : Amyotrophic Lateral Sclerosis is one of the incurable disease. In Oriental medicine, Wei symptom is similar as Amyotrophic Lateral Sclerosis, so we diagnosed it as Wei symptom and treated in Oriental medical system. Methods : The patient was treated by acupunture, moxibustion, herb medication, physical treatment. The improvement of the patient was judged by Amyotrophic Lateral Sclerosis Functional Rating Scale (ALSFRS), Grasping power on right arm, circumference of thigh and calf. Result : The patient had better condition for a while but the sputum irritated breathing and the day before he discharged vital sign was not stable and could not breath well. Conclusion : It is necessary to have more examination about the incurable syndromes such as Amyotrophic Lateral Sclerosis, and keep the patient's life better and expand their lives.

The Clinical Case of Oriental Medical Treatment at Tender Point for Patient with Lower Back Pain Suggesting of Amyotrophic Lateral Sclerosis (요통을 호소하는 ALS환자에 대한 한의학적 치료 효과 증례 보고)

  • Objectives : This study was performed to report the effect of oriental medical treatment in the lower back pain(LBP) with amyotrophic lateral sclerosis(ALS). Methods : A 32-years-old man who diagnosed as ALS was admitted with LBP. We treated him by burning acupuncture, herbal medicine, acupuncture, electroacupuncture, moxibustion, cupping therapy and physical therapy from 11th May 2010 to 10th June 2010. Results : After treatment, there was improvement in LBP. Pain Intensity estimated by visual analog scale(VAS) as percentile Pain Intensity Difference(PID), Korean Owestry Disbility Index(KODI) was also improved. Korean version of Amyotrophic lateral sclerosis Functional rating scale-revised(K-ALSFRS-R) shows that the patient's physical ability has been improved. Conclusions : Our study suggests that oriental medical treatment are significantly effective in the LBP due to ALS. And further studies will be aid to identify underlying mechanism of treatment.

Quality of Life and the Factors Related to Family Caregivers Caring for Those with Amyotrophic Lateral Sclerosis (근위축성측삭경화증 환자를 돌보는 가족간호자의 삶의 질과 영향 요인)

  • Purpose: The purpose of this study was to investigate QOL and the factors related to family caregivers who care for Amyotrophic Lateral Sclerosis (ALS) patients. Methods: The subjects were 83 family caregivers caring for ALS patients who visited the neurology outpatient section of a tertiary hospital located in Seoul between January and July of 2008. Their demographic characteristics were assessed and data were collected using the amyotrophic lateral sclerosis functional rating scale (ALSFRS), the caregiver burden inventory (CBI), HADS, and SF-36. Results: The mean score of the physical health component (PHC) of the family caregiver's QOL was $62.6{\pm}24.4$, and the mean score of the mental health component (MHC) of their QOL was $57.7{\pm}22.4$. The mean score of caregiver burden was $76.5{\pm}30.7$. Anxious family caregivers amounted to 55.4% and depressed family caregivers accounted for 63.9% overall. In a multivariate analysis, the PHC of QOL was explained by caregiver burden (41.1%), depression (9.4%), caregiver gender (3.8%), and caregiver age (3.1%). Anxiety (39.8%), caregiver burden (10.9%), patient gender (5.4%) and depression (1.7%) were predictive factors of the MHC of QOL. Conclusion: Caregiver burden and depression affected both the PHC and the MHC. Caregiver burden affected the PHC more than it did the MHC of QOL, but anxiety affected the MHC more than it did the PHC of QOL.
