Probiotic Properties of Pediococcus pentosaceus SH-10 Isolated from the Hard Clam Meretrix meretrix Shikhae (백합(Meretrix meretrix)식해에서 분리한 Pediococcus pentosaceus SH-10의 생균제적 특성)

  • This study examined the suitability of characteristics of potential strains of probiotic bacteria. Among 25 lactic acid bacteria isolated from Korean traditional fermented food, the Hard Clam Meretrix meretrix Shikhae, the SH-10 strain, which exhibited superior resistance to low pH and bile salts, was selected as a potential probiotic bacteria. By examining carbohydrate utilization, morphological properties, and the 16S rRNA gene sequence, the SH-10 strain was identified as Pediococcus pentosaceus (hereafter, P. pentosaceus SH-10). P. pentosaceus SH-10 was resistant to amikacin, cefotetan, ciprofloxacin, gentamicin, kanamycin, nalidixic acid, streptomycin, and vancomycin. Tests of antimicrobial activities against pathogens such as Bacillus cereus, Listeria monocytogenes, Salmonella choleraesuis, and Staphylococcus aureus, indicated that P. pentosaceus SH-10 inhibited the growth of pathogenic bacteria. These results suggest that P. pentosaceus SH-10 can be developed as a probiotic bacteria.

Drug Resistance and R-Plasmid Profile of Shigella Isolated in 1986-1988 (이질균의 약제 내성과 R-Plasmid)

  • These studies were made to assess the present stage of resistance of Shigella species to antibiotics and to find characteristics of R-plasmid of these bacteria. From 1986 to 1988, 125 strains of Shigella species were isolated from patients specimens collected in Chung Cheong-do Hospital, Kyunghee Medical Center, city an provincial health & environmental institutes. These specimens were tested for resistance to 12 kinds of antimicrobial agents by agar dilution method. Using Muller-Hinton agar for the assay of drug resistance and Trypticane Soy Broth as propagating medium for conjugation. All the strains (100%) were resistant to one or more antibiotics. Drug resistance patterns of isolated strains were found as the highest resistance to ampicillin (98%) in 1986, to tetracycline (98%) in 1987, to tetracycline (100%) in 1988, all strains were sensitive to gentamicin, amikacin, tobramycin. Chronologically, resistance decreased gradually as it was shown in relation to kanamycin, rifampicin in 1986, 1987 and 1988, (4%, 2%) (4%, 2%) (0%, 0%) respectively. But, resistance was increased year by year as it was shown in relation to tetracycline, nalidixic acid, streptomycin in 1986, 1987, 1988 (89%, 19%, 45%) (98%, 46%, 71%) and (100%, 58%, 88%). The resistance in correlation to more than 5 drugs, which was 13 strains among 47 strains in 1986, 38 strains among 87 strains in 1987, 23 strains among 26 strains in 1988, was increased gradually. In the transfer test of drug resistance by conjugation methods, the rate which was 3 strains (50%) in 1986, 8 strains (62%) in 1987, 3 strains (100%) in 1988, was increased gradually. When the donor strains were conjugated with the recipient strains, the conjugation rate was high in the multiple resistant strains. The relationships of transferring patterns of drug resistance and molecular weight of R-plasmid were variable. However, only a plasmid which has more than 35 Mgd was transferred.

Phytochemical isoflavones against diabetic foot bacteria

  • Wound swabs and pus samples were collected from diabetic foot ulcers, and control pus samples from non-diabetic cases. In 144 diabetic cases screened, Pseudomonas aeruginosa was isolated from 78 cases, in which 10.59% of the isolates were multidrug resistant (MDR), whereas the 60 control cases were not MDR. The isolated bacteria were decreasingly resistant to 6 clinically administrated antimicrobics such as ceftazidime, gentamicin, ciprofloxacin, tobramycin, piperacillin and amikacin. Therefore, it is demanded that new and more effective antimicrobials of phytochemical origins are sought after. Among 11 isoflavones (YS11-YS21) isolated from Sophora and Euchresta (Leguminosae; pea plant family), 2 (YS19 and YS21) prominently exhibited the high antibacterial activity both in vitro and in vivo. By the preliminary results, the object of this paper is to evaluate the in vitro antibacterial effect of YS19 and YS21 on the clinically isolated bactera of Ps. Aeruginosa in hospitals. All the isolates were sensitive to YS19 and YS21 and for both, minimum inhibitory concentration (MIC) values ranged from $2\;to\;50\;{\mu}g/mL$. The $MIC_{90}$ values of YS19 and YS21 were $50\;{\mu}g/mL$. It is suggested that these isoflavones might consist a basis phytochemical prevention and therapy for diabetic foot infections caused by pseudomonads.

  • Salmonella spp. is of increasing public health concern as causative pathogens of food poisoning. The aim of this study was to investigate the serotypes and antimicrobial resistance pattern of Salmonella spp. isolated from duck farms in Daegu-Gyeongbuk province. Also, S. Enteritidis and S. Typhimurium isolates were further examined for plasmid analysis, phage typing and pulsed-field gel electrophoresis (PFGE). A total of 34 Salmonella spp. (16.4%) were isolated from duck farms and ten serotypes were identified. The predominant serotypes were S. Typhimurium (23.5%) S. Fyris (17.6%) and S. Haardt (11.8%), S. Agona and S. Enteritidis (respectively 8.8%). Of 34 Salmonella isolates, 15 (44.1%) isolates were resistant to at least one antimicrobial agent and multiple resistance (resistance to more than 4 drugs) was observed in 9 strains (26.5%). The high resistance was found to streptomycin (32.4%), tetracycline (29.4%), ampicillin, kanamycin and nalidixic acid (respectively, 26.5%), all Salmonella isolates were susceptible to cefoxitin, cefotaxime, gentamicin, amikacin and ciprofloxacin. All S. Enteritidis and S. Typhimurium isolates were found to contain only one plasmid (ca. 54 or 55kb, respectively). Among the S. Enteritidis isolates, two phage types were found, PT32a and PT1c, respectively, one isolates did not react with any of the phages used. Whereas, all S. Typhimurium isolates were RDNC (reacts but does not conform). PFGE showed to be a useful typing method better than plasmid analysis and phage typing for discrimination of isolates especially, S. Typhimurium isolates. Our results indicated that the serotypes of Salmonella isolates are widely distributed in duck farms, further epidemiological studies should be carried out.

Study on antimicrobial resistance of Escherichia coli isolated from domestic beef on sale (유통되는 쇠고기에서 분리한 대장균의 항생제 내성 조사.연구)

  • In this study, antimicrobial resistance of E coli isolated from domestic beef on sale in Busan and Gyeongnam province was investigated from March to October 2007. A total of 600 beef samples were collected for the monitoring of antimicrobial resistance, and 92 (15.3%) strains of E coli were isolated. Antimicrobial resistance test was carried out by agar disc diffusion method with 17 antimicrobials. In general, E coli isolates showed the highest antimicrobial resistance to doxycycline (73.9%), followed by tetracycline (70.7%) andcefazolin (63.0%). Then they showed higher resistance to several antimicrobials like norfloxacin (48.9%). However, They had low antimicrobial resistance to amikacin (4.3%), colistin (1.1%). Of 92 isolates, 82 (89.1%) were resistant to more than 2 antimicrobials. Among 17 antimicrobials examined, tetracyclines were the most resistant, followed by cephalosporins, quinolone. The resistance was seemed to be correlated to amounts of antimicrobial use. In the result of this study, we suggest that there be need to regulate the abuse of antimicrobial on food-producing animals in Korea because the concern on antimicrobial resistant is gradually increased worldwide.

Outcomes and Use of Therapeutic Drug Monitoring in Multidrug-Resistant Tuberculosis Patients Treated in Virginia, 2009-2014

  • Background: Reports of therapeutic drug monitoring (TDM) for second-line medications to treat multidrug-resistant tuberculosis (MDR-TB) remain limited. Methods: A retrospective cohort from the Virginia state tuberculosis (TB) registry, 2009-2014, was analyzed for TDM usage in MDR-TB. Drug concentrations, measured at time of estimated peak ($C_{max}$), were compared to expected ranges. Results: Of 10 patients with MDR-TB, 8 (80%) had TDM for at least one drug (maximum 6 drugs). Second-line drugs tested were cycloserine in seven patients (mean $C_{2hr}$, $16.6{\pm}10.2{\mu}g/mL$; 4 [57%] below expected range); moxifloxacin in five (mean $C_{2hr}$, $3.2{\pm}1.5{\mu}g/mL$; 1 [20%] below); capreomycin in five (mean $C_{2hr}$, $21.5{\pm}14.0{\mu}g/mL$; 3 [60%] below); para-aminosalicylic acid in five (mean $C_{6hr}$, $65.0{\pm}29.1{\mu}g/mL$; all within or above); linezolid in three (mean $C_{2hr}$, $11.4{\pm}4.1{\mu}g/mL$, 1 [33%] below); amikacin in two (mean $C_{2hr}$, $35.3{\pm}3.7{\mu}g/mL$; 1 [50%] below); ethionamide in one ($C_{2hr}$, $1.49{\mu}g/mL$, within expected). Two patients died: a 38-year-old woman with human immunodeficiency virus/acquired immune deficiency syndrome and TB meningitis without TDM, and a 76-year-old man with fluoroquinolone-resistant (pre-extensively drug-resistant) pulmonary TB and low linezolid and capreomycin concentrations. Conclusion: Individual pharmacokinetic variability was common. A more standardized approach to TDM for MDR-TB may limit over-testing and maximize therapeutic gain.

Aminoglycoside Dosage in Neutropenic Fever Patients after Tranplantation of Blood Stem Cells (조혈모 세포 이식후 neutropenic fever환자에서의 aminoglycoside dosage에 관한 검토)

  • Pharmacokinetic parameters and dosage of aminoglycosides (AGs) were studied retrospectively in 36 patients with neutropenic fever after stem cell transplantation in Seoul National University Hospital from July 1996 to June 2001. AGs pharmacokinetic parameters were calculated with steady-state peak and trough serum drug concentrations by the method of Sawchuk and Zaske et at. The calculated aminoglycosides volume of distribution and clearance were greater than population value $(0.36\pm0.06\;L/kg,\;116\pm32\;ml/min/1.73\;m^2,\;respectively)$. The average dosage of aminoglycosides required to maintain optimal serum AGs concentration was also greater than recommended dose in insert paper. The average dosage of amikacin was $11\pm2.1$ mg/kg every 12 hours (In case of tobramycin, $2.09\pm0.37$ mg/kg every 8 hours or $2.59\pm0.20$ mg/kg every 12 hours). The relationship between AGs volume of distribution and sex, serum albumin (g/dl), body mass index $(kg/m^2)$, body weight change $(\%)$, the amount of fluid inpu (ml/kg/day), the degree of hematocrit decrease $(\%)$ were studied respectively. Univariate anlysis revealed that body mass index $(kg/m^2)$, the amount of fluid input (ml/kg/day) and the degree of hematocrit decrease $(\%)$ had significant correlation with aminoglycosides volume of distribution. But sex, serum albumin, body weight change $(\%)$ had no significant correlation with aminoglycosides volume of distribution.

One-Stage Treatment of Chronic Calcaneal Osteomyelitis with Bone Morphogenetic Protein 2 and Local Antibiotic Delivery in a Cat

  • An age-unknown, 4.8 kg, male, wild, domestic short-hair cat was presented for left hindlimb lameness. A physical examination revealed a draining tract which was suspected of bite on left calcaneal bone. The left tarsal joint was markedly swollen and exudates were observed around the draining tract. Sequestrum at left calcaneus bone, and osteolysis were identified by radiography. The sequestrum and its surrounding exudative tissue were debrided during surgery and the tissue was submitted for bacterial culture and sensitivity test. The debridement caused a bone defect ($1.5cm{\times}0.5cm{\times}0.5cm$) on the medial left calcaneal bone. Plate and screw fixation was performed to the calcaneus bone as buttress plate. Recombinant human bone morphogenetic protein-2 (rhBMP-2) loaded hydroxyapatite was implanted in the bone defect. Furthermore, Amikacin-impregnated collagen sponges were also placed around bone plate to deliver local antibiotics. A systemic antibiotic treatment regimen based on bacterial culture and sensitivity test results was administered for 4 weeks. The wound properly healed without any signs of infection, and the bone healing was confirmed by radiography. The patient showed normal weight bearing ambulation at 18 weeks after surgery. The use of rhBMP-2 and local antibiotic delivery system is a good surgical option for the one-stage treatment of chronic osteomyelitis.

Antibiotic Sensitivity Patterns in Children with Urinary Tract Infection: Retrospective Study Over 8 Years in a Single Center

  • Purpose: We studied the pathogens and trends in antibiotic sensitivity pattern in children with urinary tract infection (UTI) over 8 years in order to evaluate adequate treatment. Methods: We performed a retrospective review of medical records of children with UTI from January 2009 to December 2016 in Daegu Fatima Hospital. Uropathogens and antibiotic sensitivity patterns were selected. Only 1 bacterial species with a colony count of ${\geq}105CFU/mL$ was considered a positive result. We compared 2 periods group (A: 2009~2012, B: 2013~2016) to investigate trends of antibiotic sensitivity pattern. Results: During the 8 year period, 589 cases are identified (E. coli was cultured in 509 cases, 86.4%). Among all patients, this study investigated the antibiotic sensitivity of E. coli. Antimicrobial susceptibility to ampicillin was steadily low for both periods (A: 32.6%, B: 40.1%, P=0.125), and to amikacin was consistently high for both periods (A: 99.4%, B: 99.3%, P=1.000). Antibiotic sensitivity to third-generation cephalosporin decreased from period A to B (A: 91.7%, B: 75.5%, P=0.000). Antibiotic sensitivity to quinolone significantly decreased from A to B (A: 88.4%, B: 78.2%, P=0.003). The prevalence of extended-spectrum ${\beta}$-lactamase-producing E. coli increased from period A to B (A: 6.1%, B: 17.1%, P=0.000). Conclusion: This study showed that conventional antibiotic therapy for the treatment of pediatric UTI needs to be reevaluated. A careful choice of antibiotic is required due to the change in antibiotic sensitivity and the emergence of antibiotic-resistant bacteria.

Treatment of Mycobacterium avium Complex Pulmonary Disease

  • The pathogen Mycobacterium avium complex (MAC) is the most common cause of nontuberculous mycobacterial pulmonary disease worldwide. The decision to initiate long-term antibiotic treatment is difficult for the physician due to inconsistent disease progression and adverse effects associated with the antibiotic treatment. The prognostic factors for the progression of MAC pulmonary disease are low body mass index, poor nutritional status, presence of cavitary lesion(s), extensive disease, and a positive acid-fast bacilli smear. A regimen consisting of macrolides (clarithromycin or azithromycin) with rifampin and ethambutol has been recommended; this regimen significantly improves the treatment of MAC pulmonary disease and should be maintained for at least 12 months after negative sputum culture conversion. However, the rates of default and disease recurrence after treatment completion are still high. Moreover, treatment failure or macrolide resistance can occur, although in some refractory cases, surgical lung resection can improve treatment outcomes. However, surgical resection should be carefully performed in a well-equipped center and be based on a rigorous risk-benefit analysis in a multidisciplinary setting. New therapies, including clofazimine, inhaled amikacin, and bedaquiline, have shown promising results for the treatment of MAC pulmonary disease, especially in patients with treatment failure or macrolide-resistant MAC pulmonary disease. However, further evidence of the efficacy and safety of these new treatment regimens is needed. Also, a new consensus is needed for treatment outcome definitions as widespread use of these definitions could increase the quality of evidence for the treatment of MAC pulmonary disease.
