치주질환으로 인해 유발된 하악의 만성 화농성 골수염의 치험 일례 (Chronic suppuraive osteomyelitis of the mandible caused by periodontal disease;a case report)

  • Osteomyelitis is an exhaustive disease whose main feature is an inflammation of inner part of bone, bone marrow. In oral and maxillofacial area, we have maxillary and mandibular osteomyelitis and the latter is dominant because of its impaired blood supply. The main cause of osteomyelitis is a bacterial infection and the ways of infections are by periapical odontogenic infection, fracture, post-operative complication, and periodontal disease. The predominant etiologic factor is periapical odontogenic infection mostly caused by advanced dental caries. It is generally believed that periodontal disease could be a cause of osteomyelitis. But periodontal disease is usually confined to the alveolar bone area and not extends to the underlying bone marrow. Accordingly periodontal infection per se rarely cause produce oseomyelitis. Even though osteomyeltis could be occurred by periodontal disease, its virulence of infection is milder than periapical odontogenic infection. So it usually provokes sclerosing or hyperplastic osteomyelitis rather than suppurative type. We had a case of suppurative osteomyelitis caused by periodontal disease and treated it with periodontal and oral and maxillofacial surgical method.

  • Purpose: The purpose of this clinical trial was to evaluate the fixation method for treating alveolar fractures in an emergency department. Methods: The efficiency of using the fixation method was judged on the basis of clinical criteria. Stability, occlusion state, bleeding amount after fixation, operation time, and difficulties during procedural operation were recorded. Results: Eight patients were enrolled in this study. In all instances, the fixation method was effective in bleeding control. Each patient had a noticeable decrement in bleeding. A wire was used for four of the eight patients, and nylon strings was used for the others. The average operation time was 6.3 minutes for the wire patients and 2.8 minutes for the Nylon string patients. No specific problem was identified during the procedural operation. However, the difference in the fixation material influenced the effectiveness of the procedure, the operation time, and the satisfaction of the doctor. Conclusion: In the emergency department, the fixation method using wire or nylon string in the treatment of alveolar fractures is effective in bleeding control

  • Purpose: To compare the case of widespread maxillary osteomyelitis treated through different approach. Method: We report the two case of osteomyelitis. In a case, we had gradually removed the small amount of sequestrum for several times. In the other case, intended Le Fort I fracture was done to approach the lesion. Result: In the gradual sequestrectomy case, bone formations were found after 4 months. In the other case, after intended Le Fort I down fracture, sequestrectomy of Lt. maxilla was done with the labiobuccal alveolar bone preserved for the prosthodontic treatment.

  • This study evaluated whether there is a difference between a conventional titanium distraction device and a TiN-coated device in terms of implant osseointegration after horizontal distraction osteogenesis in narrow alveolar bone. Four adult mongrel dogs, each weighing $9{\sim}10kg$, were used in this study. The lower premolars were extracted and horizontal distraction was performed with conventional titanium distraction devices (group 1) or TiN-coated devices (group 2). After an 8-week consolidation period, the implants were installed, and the dogs were sacrificed after another 8 weeks. The osseointegration around the implants was evaluated histomorphologically. There was one failure in experimental group 1 because of fracture of the device. Direct bone contact was achieved and there were no significant differences between the control group and experimental groups 1 and 2 in terms of osseointegration around the implants at 8 weeks. In summary, intraoral distraction osteogenesis is a good option for augmenting severely atrophied alveolar ridges, and the TiN-coated device may be useful because of its advantages, which include surface hardness, corrosion resistance, and reduced bacteria.

Healing pattern of the mucous membrane after tooth extraction in the maxillary sinus

  • Purpose: To investigate the healing pattern of the mucous membrane after tooth extraction necessitated by periodontal disease in the maxillary sinus. Methods: One hundred and three patients with 119 maxillary sinuses were investigated. Before implant placement, cone-beam computed tomography (CT) scanning was performed. The causes of extraction, the time elapsed since extraction, smoking, periodontal disease in adjacent teeth, and gender were recorded. In addition, the thickness of the mucous membrane of the maxillary sinus and the height of residual alveolar bone at the extracted area were calculated from CT images. Results: The thickness of the mucous membrane in the periodontal disease group ($3.05{\pm}2.71\;mm$) was greater than that of the pulp disease group ($1.92{\pm}1.78\;mm$) and the tooth fracture group ($1.35{\pm}0.55\;mm$; P<0.05). The causes of extraction, the time elapsed since extraction, and gender had relationships with a thickening of the mucous membrane of the maxillary sinus (P<0.05). In contrast, the height of the residual alveolar bone at the extracted area, periodontal disease in adjacent teeth, and smoking did not show any relation to the thickening of the mucous membrane of the maxillary sinus. Conclusions: The present study revealed distinct differences in healing patterns according to the causes of extraction in the maxillary sinus, especially periodontal disease, which resulted in more severe thickening of the mucous membrane.

A CLINICAL STUDY OF THE ORAL AND MAXILLOFACIAL FRACTURE

  • With today's social and cultural personal interactions, greater leisure time and participation in sports activities, and growing traffic volume, the risk of physical trauma has increased markedly. This is a clinical and retrospective study of patients exposed to oral and maxillofacial trauma. We clinically observed 72 patients with trauma in the Department of Oral and Maxillofacial Surgery, Kyunghee University Dental Hospital, from June 2006 through November 2007. The following data was obtained: 1. The male:female ratio of patients having experienced physical trauma was 6.2:1, with most patients in their twenties. 2. Traffic accident (37.5%) was the most common cause of trauma. 3. The highest incidence of fracture occurred to the zygomatic arch(22.1%) among mid-facial fractures and angle(37.5%), symphysis(35.4%) in mandible fractures. 4. Open reduction (88.9%) was the most frequently used form of treatment. Closed reduction was performed on the remaining 11.1% of cases. 5. Teeth and alveolar bone damage occurred in 23.6% of all cases. 6. Other injuries that were related to mid-face fracture occurred in 27.8% of all cases. 7. Post-operative complications occurred in 31.9% of cases, and the highest complication was the nerve injury.

하악 유전치부 치조골 골절시 resin-wire open cap splint를 이용한 고정 (IMMOBILIZATION OF LOWER MANDIBULAR ALVEOLAR BONE FRACTURE USING RESIN-WIRE OPEN CAP SPLINT)

  • 치아 및 치조골은 복합적인 구조이므로 치아의 함입이나 측방 탈구와 같은 치조와의 변위가 있는 다수 치아를 포함한 외상에서 치조골 골절이 동반될 수 있다. 치조골이 골절되면 치주인대 및 치수로의 혈행이 단절되어 치아의 합병증을 유발할 수 있고, 특히 유치열기 외상은 후속 영구치배에 손상을 줄 가능성이 있으므로 장기간의 관찰이 필요하다. 치아 및 치조골 골절의 치료 시에는 환자의 연령과 외상의 위치 및 범위, 유치의 변위 정도 및 방향을 고려해야하며, 골절편을 재위치 시키고 고정하기 위하여 아치바(arch bar), 레진-강선 고정, 교정용 밴드, 아크릴 또는 금속 캡 스플린트(acrylic or metal cap splint), 화이버 스플린트(fiber-splint) 등의 다양한 방법이 고안되었다. 본 증례는 외상으로 인하여 치조골이 골절되어 내원한 1세 11개월 된 환아로서 모형상에서 레진-강선 오픈 캡 스플린트(resin-wire open cap splint)를 제작하여 고정한 후 양호한 치료 결과를 얻었다. 이 방법은 부가적인 기공 과정이 필요하지만, 유치열기에서 사용할 수 있고, 시술시간이 짧아 비협조적인 환아에게 진정요법 없이 적용할 수 있으며, 일반적인 아크릴 캡 스플린트에 비해 부피가 작아 불편감이 적고, 교합을 방해하지 않으며, 접착시 시멘트가 빠져나갈 공간이 있어서 스플린트의 정확한 안착이 가능하다. 또한 비교적 통증이 적으며 침습적이지 않아 출혈이 없으므로 의과적문제가 있는 경우에도 사용할 수 있다.

치은 연하로 파절된 상악 전치부의 수복 (Restoration of Subgingivally Fractured Upper Incisor)

  • The subgingival fracture near the alveolar bone is difficult to treat. This fractured tooth will be treated by many methods. First approach is to preserve the fractured tooth. Periodontal surgery has been used to lengthen the clinical crown, thereby allowing the tooth to be restored. Another method is erupting the tooth with orthodontic eruption (forced eruption) or surgical extrusion. Second approach is the restoration after extraction of the subgingivally fractured tooth. This is restorative with conventional fixed partial denture or implant. This article presents the variable restorative approach of subgingivally fractured upper incisor.

  • The authors have observed clinically and histopathologically on two cases of rhabdomyosarcomas occurred in the oral cavity. The results were as follow: 1. A case of adult pleomorphic rhabdomyosarcoma, involving the right mandibular third molar area of 52 year old male, revealed pathological fracture of the right mandibular angle area. Microscopically, sections of this tumor showed cross striations by staining of phosphotungstic acid hematoxylin. 2. A case of embryonal alveolar rhabdomyosarcoma, involving the right buccal area of one year and ten months old female, revealed severe bone destruction on the right mandibular body area. Microscopically, sections of this tumor did not show cross striations.

Missing teeth after maxillofacial trauma: a case report and management protocol

  • Management of maxillofacial trauma includes primary care, in which diagnosis and management of dentoalveolar injury play a vital role. Due to the impact sustained during a maxillofacial injury (whether direct or indirect), dentoalveolar injuries can occur, leading to fracture and displacement of teeth and associated alveolar bone into the surrounding soft tissues and associated structures, such as the maxillary sinus, nasal cavity, upper respiratory tract, tracheobronchial tree, or gastrointestinal tract. Undiagnosed displaced teeth may cause complications such as airway obstruction. This paper reports a case of displaced teeth in the nasal cavity and gastrointestinal tract and highlights the management protocol for displaced teeth secondary to maxillofacial trauma.
