Epidemiology and Preventive Medicine in Times of New Technologies

  • Epidemiology and preventive medicine are changing together with Population and health and with ever expanding medical and non medical technologies. New technologies make epidemiology methodologically more sophisticated, but such advances risk overshadowing epidemiology's most important role: raising questions, providing answers, and helping the medical decision-making at ail levels of prevention. Epidemiology also plays a major role in the evaluation of new and other technologies whose effectiveness is poorly known. Epidemiological approaches, methods, techniques, and interpretations are widely used in new and rapidly expanding fields of medicine: research evaluation and synthesis (meta-analysis), establishment of guidelines for clinical preventive practices, new medical technology assessment, guidelines for national and international health policies, evidence-based medicine, outcomes research and disease management ('population-based' medicine and quality of care improvement). In the nearest future, infectious and noninfectious diseases may cease to be almost the sole subjects of epidemiology and they may share their place with other mass phenomena of the next millennium, such as medical practices and care, or political, social and economic actions and their consequences. Not only will primary, secondary, and tertiary Prevention will remain in the epidemiological mainstream, but health protection and health promotion will require perhaps a redefinition of epidemiology in these domains. Epidemiology and preventive medicine are both subjects of medical ethics and dilemma for right choices.

Human-ware Approach on the Difference between Industrial Relations of Korea and Japan -Based on the Identification of the Gene- (한국과 일본의 기업풍토 및 경영성과 차이에 대한 휴먼웨어적 접근 -동일 유전자 측면을 기초로 하여-)

  • In this paper, we would be searching for the complemental aspects in industrial relations of Korea compared with that of Japan. It is very reasonable that historical, geographical, and especially gene analysis data are almost identical, considering that the general evaluations to the motive of Japan's industry are $\ulcorner$the modernization of the tradition$\lrcorner$. Although it is reported Korea accomplished the high economic growth rate, the economic growth rate(efficiency) could not be continued. This reason looks like that Korean proper(eigen) traits(:Gene-ware) have been oppressed and concealed by social climate such as jealousy, stratagem and hatred, we can find one of this facts in the criminal and complaint numbers as social trust measurement. There are many cases that Korean industry was distorted by the superiority/inferiority relations, for examples, the loss of the native(eigen or proper) resources, a distorted introduction of the production method and the unfolding of the industry which be not related to the characteristics of region. In this reasons, Korean industry could not play the role of Human-ware function which activates Gene-ware or, Gene-Nascence. In this Human-ware aspects, Japanese characteristics, such as $\ulcorner$Getting the basic right$\lrcorner$ and $\ulcorner$Tendency to the origin$\lrcorner$, are to be useful in the Nascence of Gene-ware in Korean industry.

Ganglion Cyst of the Sternoclavicular Joint in an Adult

  • Ganglion cysts are most common on the dorsum of the hand or wrist, but they can occur in any part of the body. There have been few papers reporting ganglion cysts originating from the sternoclavicular joint, with most of these cases developing in children. A 76-year-old woman was referred to our department because of a painless mass over the right sternoclavicular joint. The mass was excised along with the portion of the sternoclavicular joint capsule surrounding the stalk. Histopathologic examination showed the cyst wall to be composed of compressed collagen fibers without evidence of an epithelial or synovial lining, which was consistent with ganglion cyst. To our knowledge, this is the first report of such a cyst in an adult. We consider this to be a useful report for surgeons that treat mass lesions occurring in almost any part of the body surface.

A Case of Tuberculous Pneumonitis With Continuous High Spiking Fever (지속적인 고열을 동반한 폐렴양 결핵병변 1예)

  • A 33-year old male was admitted due to continuous high spiking fever for 2 months via local clinic. He had been diagnosed pulmonary tuberculosis at local clinic. However, spiking fever had not been controlled by anti-tuberculous medications. Chest PA showed confluent consolidation on right upper & mid-lung field. 5 anti-tuberculous regimens(Streptomycin, Isoniazid, Rifampin, Ethambutol, Pyrazinamaide) were administered initially and steroid therapy was followed for relieving toxic symptoms Very slowly resolved chest X-ray lesion and continuous fever suggested the possibility of misdiagnosis. After 60th hospital day, the chest X-ray lesion was resolved gradually and fever subsided almost completely. He was discharged on 76th hospital day with anti-tuberculous drugs and steroid(prednisolon), without any other problems except sustained mild fever.

  • Housing Performance Rating Indication System was started in 2006. The aim of Housing Performance Rating Indication is to provide the guideline of housing performance. Repair Convenience is similar to Remodeling. Remodeling has been paid more attention as an alternative for reconstruction. Especially, now the actual house supply is facing almost 100%, it is time to keep and maintenance management of apartment buildings, and ultimately it will expand the Long life housing. Especially consumers has to have a right to select apartment buildings by comparing performance of house. For construction companies, they need a performance code that standardized by government, so they can provide a Certain performing house. This study examines data which has been obtained from the recent application of Housing Performance Rating Indication System and Repair Convenience category is main concern. Findings from the study will provide vital information in improving current Housing Performance Rating Indication System.

The Study on the Somatotype and Spinal Deformity among Middle and High School Girls (중·고 여학생의 체형분류 및 척추변형에 관한 연구)

  • The purpose of this study was to classify female students' somatotypes, to investigate the relationship between five somatotypes obtained from anthropometric measurements and to examine the relationship between the spinal deformity and somatotypes. The subjects for anthropometric measurements were 844 female middle and high school students. The data were analyzed by cluster analysis, correspondence analysis, Duncan test, and T-test. The results were as follows. Cluster analysis on the factors extracted from directly measured items classified subjects into five types (middle height and a little slim type (39.6%), low height and a little slim type (13.1%), middle height and a little fat type (18.0%), tall and very fat type (7.7%), and very tall and standard type (21.6%). In examining the relationship between the normal people and those of the spinal deformity, the normal showed higher mean value in almost all the items of the girth, the depth, and the length. High school students had more spinal deformity in number than middle school students. The degree of deformity was higher in the right spinal deformity than in the left spinal deformity as the age increased. In terms of the distribution of the spinal deformity classified by the somatotype, Type 1 (middle height and a little slim type) and Type 5 (very tall and standard type) appeared the highest.

  • The purpose of this study is to investigate possible correlation between the dental anomalies and site of cleft in cleft lip and palate. In this study, 142 patients who had cleft lip and/or cleft palate were examined. The results are as follows. 1. The incidence of missing tooth was high in the permanent dentition as compared to the incidence in the deciduous dentition. 2. There was not much difference of incidence of supernumerary tooth between deciduous and permanent dentition in the group of patients who had cleft lip and jaw with or without cleft palate. 3. In the group of patients who had cleft lip and jaw with or without cleft palate, the frequency of incidence of cleft sides was higer in unilateral than bilateral cases. And, incidence of left sides was higher than right sides. 4. The type of cleft between central incisor and canine with missing lateral incisor was most frequent in permanent dentition and the type of cleft between central and lateral incisor was most frequent in deciduous dentition. 5. The type of cleft associated with tooth position in deciduous dentition was not almost the same in the succeeding permanent dentition.

A RADIOLOGIC STUDY OF DENTIGEROUS CYSTS (함치성 낭종에 대한 방사선학적 연구)

  • A radiologic study of 139 dentigerous cysts (128 patients) indicated the followings: 1. There was higher incidence in males (62.99%) than in females (37.01%). The most common age group was 11-20 years and the average age was 23.6 years. 2. The most common clinical symptoms was swelling of the jaws (49 cases, 33.09%), and no symptoms was presented in 26 cases (18.71%). 3. Dentigerous cysts were found to be slightly more common in the maxilla (53.24%) than in the mandible (46.76%) and there was almost no difference in occurance between right and left side. The maxillary supernumerary tooth was the most frequent site of the dentigerous cyst and no dentigerous cyst was found to be related to a deciduous tooth. 4. The most common radiographic findings was root resorption of the adjacent teeth (33.09%), and the central dentigerous cysts were 78 cases (56.12%), the lateral dentigerous cysts were 61 cases (43.88%). 5. The increased radiolucency at the crown portion of the tooth in a cystic cavity was seen in 44 cases (31.65%), and a case of dentigerous cyst was found in a edentulous patient, and 16 cases of multiple dentigerous cysts were found in 5 patients. 6. The ameloblastic changes in 8 cases, the keratinization of the cyst wall in 2 cases, and a case of epidermoid carcinoma were confirmed microscopically.

Pulmonary Adenocarcinoma with a Micropapillary Pattern Detected by Bronchial Washing - A Brief Case Report - (폐의 미세유두상 패턴을 보이는 선암종의 기관지세척 세포소견 - 짧은 증례보고 -)

  • Adenocarcinomas with micropapillary patterns are generally aggressive and show lymphotropism. Only a few reports on pulmonary adenocarcinoma with micropapillary patterns have described cytologic findings. A 70-year-old Korean woman was admitted to the hospital because of intermittent dry cough and chest pain. Cytology after bronchial washing showed neoplastic cells in small, angulated, cohesive clusters consisting of 3-20 cells without a fibrovascular core. The resected right middle lobe showed a tumor occupying almost the entire lobe. Histologically, about 90% of neoplastic cells proliferated with micro papillary morphology in the background of bronchioloalveolar carcinoma. Cytologic smears of a bronchial washing showing tumor cells in small, cohesive clusters without a fibrovascular core may indicate an adenocarcinoma with micropapillary pattern.

  • Surely fuel economy(F.E.) label is needed to meet consumer's right to know. Korea has developed and adopted F.E. correction equation similar to the feeling, because consumer complain about F.E. label higher than they feel. Recently, through continuous research and development high-efficiency & performance green car like HEV, PHEV and EV are being sold. In this situation, it is needed to know whether the current equation can reflect a part of their improved technic or not. In this paper, to review current equation through test using hybrid vehicles on 5-cycle SOC, correction equation and F.E were discussed. The result show HEV didn't meet the SOC error standard on US06. Also, HEV has bigger F.E. difference between FTP-75 and 5-cycle than conventional vehicles. However, the correction equation that include HEV almost same with current one.
