1. Background and Purpose : I intended to investigate the effects of Taeumin Yuldahansotang and Jowisokmyungtang experimentally to hypertension and renal function and contrived to approach these diseases by constitutional medicine. 2. Methods : I adapted two groups, normal rats and spontaneously hypertensive rats, to the same environment for more than 2 weeks and adminstrated water extracts of Yuldahansotang and Jowisokmyungtang. After adminstration of water extracts, I observed significant changes of values of the blood pressure and the urinary excretion of creatininine, electrolytes, osmolarity, renin, atrial natriuretic peptide and the plasma level of aldosterone, triglyceride, phospholipid, cholesterol. 3. Results : Yuldahansotang and Jowisokmyungtang decrease the blood pressure in the spontaneous hypertensive rats, the one by increasing the plasma level of atrial natriuretic peptide and decreasing the plasma level of aldosterone, the other by decreasing the plasma level of aldosterone, from which we could detect the therapeutic difference between Yuldahansotang and Jowisokmyungtang.

Background Aldosterone has an important role in the pathophysiology of heart failure. Aldosterone promotes the retention of sodium, the loss of magnesium and potassium, sympathetic activation, parasympathetic inhibition, myocardial and vascular fibrosis, baroreceptor dysfunction, and vascular damage and impairs arterial compliance. Objectives We investigated the effects of additional spironolactone to angiotensin-converting enzyme inhibitor (ACEI) / angiotensin-II receptor blocker (ARB) in patients with heart failure. Adrenocortical carcinomas are rare and frequently aggressive tumors that may be functional (hormone-secreting) and may cause Cushing's syndrome or virilization, or non-functional and manifest as an abdominal mass. This paper reports the case of a 77-year-old woman with cortisol- and aldosterone-secreting adrenal carcinoma. The patient complained of general weakness, a moon face, and weight gain. She also had hypokalemia and hypertension. Her endocrinological data showed excessive aldosterone production and non-suppressible cortisol production in a low-dose dexamethasone suppresion test. Her abdominal CT showed a right adrenal mass. She underwent right adrenalectomy, and her histology revealed the presence of adrenocortical carcinoma. After adrenalectomy, her hypokalemia returned to normal and she is being treated with hydrocortisone.

Background: Increasing evidence supports interplay between aldosterone and parathyroid hormone (PTH), which may aggravate cardiovascular complications in various heart diseases. Negative structural cardiovascular remodeling by primary aldosteronism (PA) is also suspected to be associated with changes in calcium levels. However, to date, few clinical studies have examined how changes in calcium and PTH levels influence cardiovascular outcomes in PA patients. Therefore, we investigated the impact of altered calcium homeostasis caused by excessive aldosterone on cardiovascular parameters in patients with PA. Methods: Forty-two patients (mean age $48.8{\pm}10.9$ years; 1:1, male:female) whose plasma aldosterone concentration/plasma renin activity ratio was more than 30 were selected among those who had visited Severance Hospital from 2010 to 2014. All patients underwent adrenal venous sampling with complete access to both adrenal veins. Results: The prevalence of unilateral adrenal adenoma (54.8%) was similar to that of bilateral adrenal hyperplasia. Mean serum corrected calcium level was $8.9{\pm}0.3mg/dL$ (range, 8.3 to 9.9). The corrected calcium level had a negative linear correlation with left ventricular end-diastolic diameter (LVEDD, ${\rho}=-0.424$, P=0.031). Moreover, multivariable regression analysis showed that the corrected calcium level was marginally associated with the LVEDD and corrected QT (QTc) interval (${\beta}=-0.366$, P=0.068 and ${\beta}=-0.252$, P=0.070, respectively). Conclusion: Aldosterone-mediated hypercalciuria and subsequent hypocalcemia may be partly involved in the development of cardiac remodeling as well as a prolonged QTc interval, in subjects with PA, thereby triggering deleterious effects on target organs additively.

Previously, we showed that a chronic-low-dose nonylphenol (NP) exposure resulted in histological changes with sexually dimorphic pattern in rat adrenal glands. We hypothesized that such structural changes are closely related to the hormonal secretory patterns. To test this hypothesis, we developed the short-term adrenal incubation method, and measured the levels of catecholamines and cortical steroids using the high-performance liquid chromatography with electrochemical detection (HPLC-ECD) and specific enzyme-linked immunosorbent assay, respectively. The norepinephrine (NE) levels in media from NP-treated female adrenal, except 100 pM NP, were significantly increased [control (CTL) vs 1 nM NP, p<0.001; vs 10 nM NP, p<0.05; vs 100 nM NP, p<0.001; vs 1 µM NP, p<0.01]. The NE secretion from male adrenal was higher when treated with 100 nM and 1 µM NP (CTL vs 100 nM NP, p<0.05; vs 1 µM NP, p<0.05, respectively). The aldosterone level in the female adrenal media treated with 100 pM NP was significantly decreased, on the other hand, that of media treated with 10 nM NP was significantly increased (CTL vs 100 pM NP, p<0.05; vs 10 nM NP, p<0.01). In male adrenal media, the aldosterone levels of 10 nM, 100 nM and 1 µM NP-treated media were significantly declined (CTL vs 10 nM NP, p<0.001; vs 100 nM NP, p<0.001; vs 1 µM NP, p<0.001). These results showed the NP treatment altered secretory pattern of aldosterone from adrenals of both sexes, showing sexual dimorphism. It may be helpful for understanding possible adrenal pathophysiology, and endocrine disrupting chemicals-related sexually dimorphic phenomena in adrenals.

Objective : This study was performed to investigate the effects of aqua-acupuncture of Jibaikjihwangtang in two-kidney one clip Goldblatt hypertensive rats and spontaneously hypertensive rats. Methods : we injected aqua-acupuncture solution into Shin-Soo ($BL_{23}$) which corresponds to human acupuncture point in two-kidney one clip Goldblatt hypertensive rats and spontaneously hypertensive rats. Systolic blood pressure, renin activity, aldosterone and atrial natriuretic peptide (ANP) plasma levels were tested. Results : Systolic blood pressure decreased significantly after aqua-acupuncture of jibaikjihwangtang. Acupuncture group in two-kidney one clip Goldblatt hyper-tensive rats had deference with control group. In plasma levels of atrial natriuretic peptide, acupuncture group of spontaneously hypertensive rats increased meaningfully but to two-kidney one clip Goldblatt hypertensive rats it was decreased meaningfully. In Serum Aldosterone density, the acupuncture group of spontaneously hypertensive rats had significant alteration than control group, but the acupuncture group of two-kidney one clip Goldblatt hypertensive rats had decreased alteration than control group. Conclusion : According to these results, after Aqua-Acupuncture of Jibaikjihwangtang blood pressure decreased significantly and data suggest that blood pressure reduction activity connected with renin activity reduction in renal hypertensive rat.

This study was Investigated to prove the effect of Gamidojeok-san (Jiaweidaochi-san) on the hypertension. After administer Gamidojeok-san (Jiaweidaochi-san) extract to SHR(Spontaneous Hypertensive Rat) for 5 weeks, we examined changes of blood pressure, pulse rate, aldosterone and catecholamine in serum. Results : Following results were obtained; Gamidojeok-san(Jiaweidaochi-san) showed significant antihypertensive effect. Gamidojeok-san(Jiaweidaochi-san) decreased pulse rate but insignificant. Gamidojeok-san(Jiaweidaochi-san) showed a significant decrease in the aldosterone. Gamidojeok-san(Jiaweidaochi-san) was ineffective on the dopamine. Gamidojeok-san(Jiaweidaochi-san) decreased the norepinephrine but insignificant. Gamidojeok-san(Jiaweidaochi-san) showed a significant decrease in the epinephrine. These results indicate that Gamidojeok-san(Jiaweidaochi-san) can be used in hypertension. Further study will be needed about the functional mechanism and etc..

In order to investigate the relationship between plasma renin activity and metabolism of Ca and Na in blood pressure, the habitual dietary intakes of Na and Ca urinary excretion of Ca, Na and K, and plasma level of renin activity, aldosterone, and indices of Ca metabolism were measured in 27 untreated hypertensive women and 30 age-matched normal women on a free diet. Hypertensive subjects were classified into high renin hypertensive (HH), medium renin hypertensive(HM) and low renin showed no significant difference among normotensive, LH, MH and HM groups. It appeared that 25-(OH) Vit D3 level of HH group was significantly higher than LH group(p<0.05). There was significant difference in habitual intake of Ca between normotensive and LH groups. However, habitual intake of Na showed no significant difference among normotensive, LH, MH and HH group. Positive correlation of systolic and diastolic blood pressure with PTH(r=0.324, r=0.375) and urinary Ca(r=0.496, r=0.278) and a negative correlation of systolic blood pressure with habitual Ca intake(r=-0.371) existed(p<0.05). A relative magnitude of factors affecting hypertension was analyzed by multiple regression analysis. Overall results about relative influence of independent variables to dependent variable (systolic blood pressure) indicated that urinary Ca was the higher correlation in all subjects(p<0.0001), followed by age and aldosterone. PTH showed a significant correlation for relative influence on diastolic blood pressure in all subjects. The above results indicated that renin-aldosterone system and Ca regulating hormone had a mutual relationship in hypertension.

The aim of the experiments was to investigate the effect of Semen Cuscutae aqua-acupuncture at the meridian point BL 20(脾兪) and BL 23(賢兪) on the blood pressure, plasma renin activity, plasma levels of aldosterone and atrial natriuretic peptide (ANP) in spontaneously hypertensive rats (SHR). The results of this study were as follows: 1. Systolic blood pressure was decreased significantly after Semen Cuscutae aqua-acupuncture at the meridian point BL 20, BL 23. 2. Plasma renin activity was increased significantly after Semen Cuscutae aqua-acupuncture at meridian point BL 23, BL 20. 3. Plasma levels of aldosterone was increased significantly after Semen Cuscutae aqua-acupuncture at the meridian point BL 20. 4. Plasma levels of atrial natriuretic peptide (ANP) was increased significantly after Semen Cuscutae aqua-acupuncture at the meridian point BL 23, but it was decreased significantly after Semen Cuscutae aqua-acupuncture at the meridian point BL 20. These results suggest that the changes of the depressor response after Semen Cuscutae aqua-acupuncture at the meridian point BL 20, BL 23 are related to the changes of the plasma renin activity, plasma levels of atrial natriuretic peptide (ANP) and aldosterone.

The study was performed to investigate the effects of administration of aluminum compound in Kidney metabolism and plasma hormone of rats. Seventy frve male Sprague-Dawley strains rats were divided into five groups consisting of the control, 250ppm AlCl$_3$group, 500ppm AlCl$_3$ group, 250ppm $Al_2$(SO$_4$)$_3$group, 500ppm $Al_2$(SO$_4$)$_3$group and kept on the diet for 2 weeks. The body weight gain was increased by the administration of AlCl$_3$ but decreased by the administration of $Al_2$(SO$_4$)$_3$as compared to the control. The urinary excretion of sodium and creatinine were increased and free water clearance and urine volume were decreased significantly after AlCl$_3$adminstration group as compared to the control. The water balance, free water clearance, excretion of sodium and creatinine were increased and the excretion of chlorine was decreased after $Al_2$(SO$_4$)$_3$ administration as compared to the control. Plasma renin activity was increased and plasma aldosterone content was compared to the control. Plasma renin activity was increased and plasma aldosterone content was significantly decreased after adminstration of aluminum compounds as compared to the control.
