Symptoms of Posttraumatic Stress Disorder and Mental Health in Female Victims by Sex Trade : A Follow-Up Study in Shelter Samples (성매매 피해 여성의 외상 후 스트레스 증상과 정신건강-탈 성매매 후 추적 연구)

  • Objectives : Since prostitution is multitraumatic phenomena, it is known that most women in prostitution have symptoms of posttraumatic stress disorder (PTSD) and other mental illness. In Korea, new anti-prostitution law launched in the year of 2004 imposes protecting prostituted victims at government-supported shelter. The objective of this study was to compare the mental symptoms, especially PTSD features of women who escaped from prostitution with those of control subjects. Methods : We assessed 113 ex-prostituted women who live in shelter for 8 months on the average and 65 normal control subjects. Demographic data, questionnaire for sleep, physical illness, smoking, alcohol and drug use, Stress Response Inventory, Davidson Trauma Scale (DTS), Impact of Event Scale-Revised (IES-R) were used. Results : Ex-prostituted women had significantly higher scores of DTS, IES-R, stress related responses, sleep problems, smoking problems, and alcohol problems than control group. Age and duration of sex trade of the subjects were significantly correlated with the severity or frequency of PTSD symptoms. The degree of smoking, alcohol drinking and sleep problems were also significantly correlated with the PTSD symptoms. Conclusion : These results suggested mental symptoms did not disappear even after women escaped from prostitution.

Alcohol Craving in Subjects with Alcohol Use Disorders in Response to Alcohol Cues (알코올 사용 장애자의 알코올 단서에 의해 유발된 갈망 특성)

  • Initially, this study was to develop reliable and effective alcohol-associated visual cues. Alcoholic picture cues were chosen from the Normative Appetitive Picture System (NAPS). Additional images consisted of Korean beer and mild liquor was pre-tested to select the pictures that would induce craving most intensely. The images that recorded the highest scores on a scale were chosen through the pre-test. And then, the reliability and validity were examined for the selected alcohol cues from NAPS and pre-test via another psychometric test. Secondly, the study was to investigate differences in craving between subjects with alcohol use disorders (AUDs) and non-alcoholic control subjects when exposed to the alcohol visual cues. Alcohol abusers with AUD (n=9) and demographically similar non-abusers (n=9) participated in this study. After given 5cc of alcohol, subjects were exposed to different types of stimuli (i.e., alcohol, nonalcoholic beverage, and visual control pictures and one rest (cross-hair)). Craving levels were rated through self-report on a Likert scale immediately after the presentation of visual cues. Results showed statistically significant differences between the AUD group and the control group in the Obsessive Compulsive Drinking Scale (OCDS) scores, pre-sip and post-sip alcohol craving. Also, the AUD group showed significantly a higher level of craving during alcohol cues compared to the control group. In conclusion, alcohol craving induced by alcohol cues among subjects with AUD was found to be different from that of non-abusers.

Effect of an Ethanol Extract of Cassia obtusifolia Seeds on Alcohol-induced Memory Impairment (결명자 에탄올 추출물이 알코올로 유도로 유도한 기억 장애에 미치는 영향)

  • Heavy drinking disrupts the nervous system by activation of GABA receptors and inhibition of glutamate receptors, thereby preventing short-term memory formation. Degradation of cognition by alcohol induces blackouts, and it can lead to alcoholic dementia if repeated. Therefore, drugs need to be developed to prevent alcohol-induced blackout. In this study, we confirmed the effect of an ethanol extract of Cassia obtusifolia seeds (COE) on alcohol-induced memory impairment. The effects of COE and ethanol on cognitive functions mice were examined using the passive avoidance and Y-maze tests. The manner in which alcohol affects long-term potentiation (LTP) in relation to the learning and memory was confirmed by electrophysiology performed on mouse hippocampal slices. We also measured N-methyl-D-aspartate (NMDA) receptor-mediated field excitatory synapses (fEPSPs), which have a known association with cognitive impairment caused by ethanol. Ethanol caused memory impairments in passive avoidance and Y-maze tests. COE prevented these ethanol-induced memory impairments in these tests. Ethanol also blocked LTP induction in the mouse hippocampus, and COE prevented this ethanol-induced LTP deficit. Ethanol decreased NMDA receptor-mediated fEPSPs in the mouse hippocampus, and this decrease was prevented by COE. These results suggest that COE might be useful in preventing alcohol-induced neurological dysfunctions, including blackouts.

An Integrative Review of Cognitive Behavioral Therapy for Patients with Alcohol Use Disorder (알코올 사용장애 환자를 대상으로 시행된 인지행동치료에 대한 연구의 통합적 문헌고찰)

  • Purpose: The present study aimed to review the characteristics of cognitive behavioral therapy (CBT) programs for alcohol use disorders and to examine the outcomes of such programs. Methods: We searched for domestic and foreign studies that implemented a CBT program for alcohol use disorders, published from 2006 to 2017. Studies that met inclusion/exclusion criteria were selected and a quality assessment was performed using the Risk of Bias (RoB) and Risk of Bias Assessment Tool for Nonrandomized Studies (RoBANS) instruments. We utilized a five-stage analysis process, through which nine experimental studies were selected. Results: Of these nine studies, four were randomized controlled trials, four used a quasi-experimental design, and one used a qualitative design. The CBT involved the three domains of cognitive reconstruction, problem solving, and coping. Coping with drinking situations and communication constituted most of the sessions. The outcomes of such interventions showed that CBT had a positive effect on alcohol use and self-evaluation. Conclusion: CBT is an effective way to change alcohol use behaviors, motivation, interpersonal relationships, emotional control, and self-efficacy among patients with alcohol use disorders. This study provides evidence for the effectiveness of CBT-based programs for the treatment of alcohol use disorders.

Alcohol Use Disorder, Stress, Mental Health and Suicide among Seoul Citizens (서울시 지역주민의 알코올장애와 스트레스, 정신건강 및 자살)

  • Objectives: This study furthers the currents understanding of alcohol use disorders, stress, mental health status and suicide among Seoul citizens. This study is to identify differences according to socio-demographic variables and variables which affect stress, mental status and suicide for developing mental health programs. Methods: The study subjects using cluster-stratified sampling method were 1234 adults over 19 years old from 17 dong, S-Ku in Seoul City. A cross-sectional study with face-to-face interview was used to collect data. A questionnaire measuring socio-demographic variables, Symptom Checklist-90-Revised (SCL-90-R) and psychosocial wellbeing index-short form (PWI-SF) was utilized. Results: Three findings in this study were discussed: (1) The prevalence of problem drinking and alcohol dependence and AUDIT score were significantly higher among male, younger age, and high level of education and income; (2) AUDIT score were related with stress, all 9 symptoms of SCL-R and suicide plan; (3) Problem drinkers were more likely to have 2.5 times of anxiety, 3.0 times of hostility, and 2.4 times psychoticism than non alcohol problem drinkers. Alcohol dependence drinkers were more likely to have 1.9 times of high risk group of stress, 4.1 times of obsessive-compulsive, 4.7 times of anxiety, 5.5 times of hostility, and 2.3 times psychoticism than non alcohol dependence drinkers. Conclusion: The findings suggest that the future intervention of Mental Health programs should be carefully designed and tailored by socio-demographic variables.

Health and risk taking behaviors of freshmen in college (대학교 신입생들의 건강위험행태)

  • Purpose : This study was conducted to survey the preliminary data on risk behaviors and to identify the factors that prevent risk-behaviors in late adolescence. Methods : Freshmen(n=1,297) beginning the first semester in Korea University, Seoul, Korea completed self-administered risk behavior questionnaires, comprising 5 domains : demographics, smoking, drinking, drug abuse and sexual behavior. Results : The rate of smoking experience was higher in people having friends who smoke and the predictors of transition to current smoking were male gender, urban residence, friends' smoking, and nicotine dependence. The rate of high risk drinking was higher among students who are male and who had experience of heavy episodic drinking. The study group showed a low prevalence of narcotic users, but two-thirds of students could get medicine easily without prescriptions. The prevalence of sexual experience was 6.5 percent, and the sexual education was not a predictor of contraceptive behavior. The prevalence of homosexuality was 1.6 percent, and the rate of mostly heterosexuality was higher in female students. Conclusion : The main targets of youth health education should be campaigns aimed at atcessation of reinforcing risk behaviors and the development of a surveillance system for the prevention of chronic disease. These results can be used to find risk factors of health-risk behaviors among late adolescents.

The Effects of Alcohol Use, Depression, and Self-esteem on Suicidal Ideation of the Aged (노인의 알코올 사용, 우울 및 자존감이 자살생각에 미치는 영향)

  • The purpose of this study was to examine the effects of alcohol use, depression, and self-esteem on suicidal ideation of older adults. Community-dwelling older adults aged 65 years and older were recruited from three welfare centers. A total of 245 older adults was assessed with the Alcohol Use Disorder Identification Test-Korean (AUDIT-K), Geriatric Depression Scale Short Form-Korea Version (GDSSF-K), Self-Esteem Scale (SES), and Scale for Suicidal Ideation (SSI). Descriptive statistics, Pearson's correlation, independent t-test, one-way ANOVA, and multiple regression analysis were performed using the SPSS WIN 19.0 program. Higher depression, lower self-esteem, no religion, and poorly perceived health were statistically significant determinants of higher level of suicidal ideation in older adults. To prevent or reduce suicidal ideation of older adults, health care providers should consider the determinants identified in this study.

The Comparision of Lifestyle Patterns between Allergy and Non-Allergy People - Food intake, Exercise, Smoking, Alcohol intake- (알레르기 질환 유무에 따른 생활습관 차이 -식이, 운동, 흡연, 음주습관을 중심으로-)

  • Purpose. The purpose of this study was to compare the total lifestyle patterns. of allergy and non-allergy group. Method. The study was carried out to access the lifestyle patterns such as food intake, smoking, alcohol intake and exercise habits by questionnairing the no allergy people and 205 non-allergy people living in Seoul area. Results. 1. The proportion of eating stimulatory food materials-mainly hot and salty-of allergy group were significantly higher than those of non-allergy group. 2. In allergy group, the proportion of meat intake was significantly higher than that of non-allergy. 3. In allergy group, exercise habit was higher than that of non-allergy, but not significant. 4. In allergy group, smoking was lower than that of non-allergy, but not significant. 5. In allergy group, alcohol intake was lower than that of non-allergy, but not significant. Conclusion. Lifestyle patterens such as food intake(salt and meat) were significantly different by prevalence of allergy and allergy type, which suggested that lifestyle patterns of allergy people need to be studied to develop preventive stratagies for allergic reaction.

Effects of Injinoryung-San on Alcoholic Hepatitis (인진오령산의 알콜성 간염에 미치는 영향)

  • This study was performed to investigate the effect of the extracts from Injinoryng-San on liver injury patients induced by alcohol. Injinoryung-San was recorded the drug for jaundice at Hwang-Je-Nae-Kyung. Twenty-seven patients who have intaked alcohol above 40 g, every day, 5 years, twenty-seventy ages were selected. After take a written consent, we divide them into experimental groups and control groups by double blind test. Each groups was evaluated every 2 weeks(2,4,6 weeks). After 2 weeks, Injinoryung-San groups significantly decreased GGT in comparison with control groups(p<0.05). After 4 weeks, Injinoryung-San groups significantly decreased MCV, GGT in comparison with control groups(p<0,05). After 6 weeks, Injinoryung-San groups significantly decreased MCV in comparison with control groups(p<0.05). Reviewing these experimental results, it appears that extracts of Injinoryung-San have pharmaceutical efficacy in GGT, MCV on liver injury patients induced by alcohol. But it has not efficacy in AST, ALT. According to drinking alcohol has becomes social problems, we should effort to make efficacious oriental medicines on alcoholic hepatitis.

The Clinical Study of Risk and Lifestyle Factors in Stroke - 419 Case Control Study - (뇌졸중 발생 위험인자 및 생활습관에 관한 임상적 고찰 - 환자 대조군 연구 419례 -)

  • Objective : Stroke is a serious disease despite of the recent improvement in medical treatment. Hence, identification of modifiable risk factors for stroke is important. This case-control study was done to demonstrate that relation between risk factors(familial history of stroke, hypertension, diabetes mellitus and heart disease), lifestyle factors(alcohol drinking, psychologic stress, exercise, liking salty and hot foods) and the incidence of stroke. Methods : We interviewed 214 stroke patients(116 in men, 98 in women) as case group and 205 non-stroke patients(118 in men, 87 in women) as control group from January 1, 2001 to July 31, 2001 in oriental hospital of Kyung Hee medical center. We investigated risk factors, lifestyle factors of stroke in all patients and stroke-subtype of case group. Stroke-subtype consists of cerebral infarction and cerebral hemorrhage. Results : The percentage of familial history in the case group was significantly higher than that of control group, especially in the patients of cerebral infarction. The most ordinary preceding disease was hypertension. And the preceding disease of hypertension, diabetes mellitus was associated positively with the risk of stroke. The percentage of exercise was significantly higher than that of control group. The percentage of alcohol drinking, psychologic stress within 1 week and liking salty and hot foods was higher in the case group. However, there was no significant difference between case and control group. Conclusion : According to the above results, we found that familial history of stroke, preceding disease(hypertension, diabetes mellitus) affected to the incidence of stroke and exercise might be predisposing risk factors for stroke.
