Prevalence and Risk Factors of H. pylori from Dyspeptic Patients in Northwest Ethiopia: A Hospital Based Cross-sectional Study

  • Background: Gastric cancer is the second leading cause of cancer-related deaths worldwide and infection with H. pylori is considered essential for its development. Helicobacter pylori infects more than 50% of the world's population with higher prevalence in developing countries than developed countries. The prevalence of H. pylori varies in different societies and geographical locations. The objectives of this study were to estimate the seroprevalence and determine the risk factors of H. pylori infection in dyspeptic patents in Ethiopia. Materials and Methods: A cross-sectional study involving 209 dyspeptic patients was carried out from February 15 to April 30, 2013. Five to ten ml venous blood was collected from each dyspeptic patient and analyzed for detection of Helicobacter pylori immunoglobulin (IgG). The socio-demographic characteristic, hygienic practices, alcohol consumption, sources of drinking water and types of latrine were also obtained with a pre-tested questionnaire. Results: The overall seroprevalence of Helicobacter pylori was 72.2%. There was statistically significant difference in the prevalence of H. pylori among age groups (p=0.02). Seroprevalence of H. pylori was higher in those patients who used unprotected surface water (76.4%) than those with access to piped tap water (65.9%). There was also statistically significant differences in prevalence of H. pylori with the habit of hand washing before meal (p=0.01) and alcohol consumption (p=0.001). Conclusions: The prevalence of H. pylori was high in the study area and increased with age of dyspeptic patients. Alcohol consumption and the type of drinking water are risk factors that have associations with the prevalence of H. pylori. Molecular epidemiological techniques can show a true picture of H. pylori and improvement in the drinking water quality is recommended.

Alcohol consumption and gastric cancer risk in Korea: a case-control study

  • BACKGROUND/OBJECTIVES: The International Agency for Research on Cancer defined alcohol beverages and acetaldehyde derived from alcoholic beverages as a Group 1 carcinogen to humans. However, the association between alcohol consumption and gastric cancer risk has been controversial in Korean. We assessed the relationship between alcohol consumption and gastric cancer risk in Korea through a case-control study. SUBJECTS/METHODS: From 2 hospitals, a total of 316 cases with gastric cancer (208 men, 108 women) were selected and matched to 316 controls by sex and age (${\pm}5years$) during the same duration. The current status, frequency, and amount of alcohol consumption for a year three years ago were assessed by trained interviewers. RESULTS: Alcohol consumption status and frequency did not show any significant association with gastric cancer risk. However, high alcohol consumption (${\geq}20g/day$ for women or ${\geq}40g/day$ for men) significantly increased the risk of gastric cancer (odds ratio (OR) 1.73; 95% confidence interval (CI) 1.05-2.85). Gastric cancer risk was strongly positively associated with alcohol consumption of ${\geq}20g/day$, especially in women (OR 5.62; 95% CI 1.32-23.81). CONCLUSION: The results from this study suggest that excessive alcohol consumption rather than the current status or frequency of alcohol consumption contributes to the increased risk of gastric cancer, especially in women.

  • Objective: The purpose of this study was to show the effects Oriental medicine and treatment for aftereffects of Central pontine myelinolysis (CPM). Methods: The 52-year-old woman suffering from CPM showed symptoms of weakness of both extremities, dysarthria, dysphagia and gait disturbance. We treated the patient with oriental medical therapy. Results: After the treatments, overall symptoms of CPM were improved. Conclusions: This study suggests that oriental medical therapy is probably effective in the treatment of CPM.

  • Objective : To examine the relationship between cigarette smoking, alcohol and cancer mortality in men in the Kangwha cohort after 12 years and 10 months of follow up. Methods : The subjects consisted of 2,681 men in the Kangwha cohort aged over 55 in 1985. Number of deaths and the time to death front all cancers and other cause were measured and the data for the smoking and drinking habits were obtained from the baseline survey data in 1985. All subjects were categorized into four groups according to their smoking habits: non-smokers, ex-smokers, mode(ate-smokers (1-19 cigarettes per day), heavy-smokers ($\geq$20 cigarettes per day). In addition, they were also categorized according to their drinking habits: non-drinkers, light-drinkers ($\leq$1 drink per week), moderate-drinkers (<3 drinks per day), heavy-drinkers ($\geq$3 drinks per day). The cancer specific death rates were calculated according to their smoking and drinking status. The adjusted risk ratio for all cancer deaths according to their smoking and drinking status were estimated using the Cox's proportional hazard regression model. Results : Using nonsmokers as the reference category, the adjusted risk ratio for all cancer deaths were 1.573(95% CI=1.003-2.468) for heavy-smokers. For lung cancer deaths, the adjusted risk ratios were 3.540(95% CI=1.251-10.018) for moderate-smoker and 4.114(95% CI=1.275-13.271) for heavy-smokers. Compared to non-drinkers, the adjusted risk ratio for stomach cancer was 2.204(95% CI=1.114-4.361) for light-drinkers. Conclusion : Smoking is the most significant risk factor for cancer deaths particularly lung cancer.

대학생의 문제음주와 감각처리특성과의 상관관계 (The Correlation Between Problem Drinking and Sensory Processing Feature in College Students)

  • 목적 : 대학생의 문제음주와 감각처리특성과의 상관관계를 알아보고자 하였다. 연구방법 : 대학생 162명을 대상으로 2015년 4월부터 2015년 6월까지 문제음주와 감각처리특성 검사를 설문조사 방법으로 실시하였다. 연구도구는 한국형 알코올사용장애 선별검사와 청소년/성인 감각프로파일을 사용하였다. 문제음주와 감각처리특성과의 관계를 알아보기 위하여 상관분석을 사용하였다. 결과 : 감각처리특성에서 문제음주의 유무에 따른 대상자 집단 간에는 통계학적으로 유의한 차이가 없었다. 문제음주군에서 한국형 알코올사용장애 선별검사점수와 감각예민에서 양의 상관관계를 보였다. 결론 : 문제음주는 감각처리특성 중 감각예민과 낮은 상관성이 있었다. 대학생의 문제음주를 치료하기 위해 그들의 감각처리특성을 이해하고 적절한 평가방법과 치료프로그램 개발이 필요하다.

춘천지역 대학생의 문제음주에 영향을 미치는 요인 (Factors Affecting Problematic Drinking by University Students in Chuncheon Area)

  • 본 연구는 춘천지역 대학생 429명을 대상으로 일반적 특성 및 사회심리적 요인이 대학생의 문제음주정도에 어떻게 영향을 미치는지 분석하였으며 그 결과는 다음과 같다. 조사대상자의 평생음주율은 98.1%이었으며 일반적 특성에 따른 문제음주정도에서 전체 학년 모두 '위험음주'가 가장 많았다(P<0.001). AUDIT의 기준점을 8점으로 문제음주를 분류하였을 때 1학년(P<0.001)과 동아리 활동을 하고 있는 학생들(P<0.01)에서 문제음주자가 더 많았으며, 인지된 건강상태는 본인이 건강하다고 인지할수록 문제음주정도가 낮았다(P<0.001). 또한 어머니의 음주정도(P<0.05)가 많을수록, 음주시작연령이 빠를수록(P<0.001) 문제음주정도가 커지는 것으로 나타났다. 사회심리적 특성에 따른 문제음주정도에서 음주결과에 대한 긍정적 기대(P<0.001)와 음주대처동기(P<0.001)는 문제음주정도가 커질수록 높고, 음주문화에 대한 인식도(P<0.001)는 바람직하지 않은 것으로 나타났다. 각 변인들 간의 상호관련성을 알아본 결과 본인의 인지된 건강상태가 낮고, 음주시작연령이 빠를수록, 음주결과에 대한 긍정적 기대와 음주대처동기가 높을수록, 음주문화에 대한 인식도가 바람직하지 않을수록 AUDIT에 의한 문제음주정도가 커지는 것으로 나타났다. 그리고 음주결과에 대한 긍정적 기대가 높을수록 음주대처동기가 높고, 음주문화에 대한 인식도가 바람직하지 않으며, 자기효능감이 낮은 것으로 나타났다. 또한 음주대처동기가 높을수록 음주문화에 대한 인식도가 바람직하지 않고, 자기효능감이 낮은 것으로 나타났다. 성별, 동아리 활동여부, 인지된 건강상태, 대학생활 만족도, 음주시작연령, 음주결과에 대한 긍정적 기대, 음주대처동기, 음주문화인식도, 자기효능감의 변인들이 대학생의 문제음주에 미치는 영향을 알아보고자 다중회귀분석을 실시한 결과 독립변인들이 대학생의 문제음주정도를 27.0% 설명해 주고 있으며, 음주대처동기(${\beta}=0.283$, P<0.001)가 문제음주에 영향을 미치는 가장 중요한 변인으로 작용하였다. 따라서 본 연구결과를 토대로 대학생의 문제음주를 바람직한 방향으로 개선하기 위한 예방적 차원의 대책을 제시하고자 한다. 첫째, 대학생들이 건전한 음주문화를 조성할 수 있도록 대학 내 음주환경을 변화시킬 수 있는 접근이 요구된다. 대학생들의 높은 음주율과 특히 1학년과 동아리 활동을 하고 있는 학생들에서 문제음주자가 높게 나타난 만큼 대학 당국에서는 신입생과 재학생을 대상으로 한 절주 캠페인 및 교육을 실시하고 절주 동아리가 구성 운영될 수 있도록 적극적으로 지원하며, 대학 내 음주관련 환경적 통제가 함께 이루어질 수 있도록 노력해야 할 것이다. 둘째, 음주대처동기가 과음 및 음주관련 문제행동을 증가시키고 알코올 의존으로 발전하게 하는 핵심요인으로 나타났다. 따라서 대학생의 음주관련문제를 예방하기 위해서는 부정적 정서 또는 사건을 경험했을 때 대처수단으로 음주를 선택하는 자신의 음주습관을 통찰해 보고, 음주가 아닌 다른 대처자원을 활용할 수 있는 실천적 방법들이 개발 교육되어 문제성 음주를 변화시킬 수 있는 개입이 필요하겠다.

Guinea pig에서 alcohol과 paraquat에 의한 간독성에 미치는 selenium의 방어 효과 (Protective effects of selenium on alcohol and/or paraquat-induced hepatotoxicity in guinea pigs)

  • Experiments were undertaken to examine the ability of selenium to protect against alcohol and/or paraquat-induced hepatotoxicity and to examine the additive effect between alcohol and paraquat. Protective effect against hepatotoxic functions was measured in serum from alcohol(15% v/v), paraquat(200ppm), alcohol and paraquat, and combination of sodium selenite(4ppm) in drinking water-fed guinea pigs ad libitum for 4 weeks. A total of 68 healthy 7-weeks-old male animals were assigned at random to 8 treatment groups(9~13 animals/group). Body and liver weight losses, and high serum concentrations in aspartate aminotransferase(AST), alanine aminotransferase(ALT, in only paraquat group), $\gamma$-glutamyltranspeptidase($\gamma$-GTP), cholesterol(Cho), creatinine, blood urea nitrogen(BUN), total bilirubin(TB), direct bilirubin(DB), total protein(TP), albumin and globulin as well as low values in alkaline phosphatase(ALP) and glucose were produced in a groups of alcohol or paraquat-fed. These values were not potentiated in a group given the combination of alcohol plus paraquat. Morphological changes in the liver were also observed in the alcohol or paraquat-fed group. Lipid droplet and cell swelling in the hepatocytes were observed in alcohol-fed guinea pig, especially Mallory's hyaline arounded hepatic vein. In the paraquat-fed guinea pig, lipid droplet, pyknosis and karyolysis were observed. When alcohol or paraquat was combined with selenium-fed, hyperplasia of Kupffer cell in liver were observed. However, the mean ALT, $\gamma$-GTP, Cho, BUN, TB, TP, albumin and globulin values were lower in groups given the combination of alcohol and/or paraquat plus selenium, compared with groups given alcohol and/or paraquat. Also, the ratio of liver weight to body weight and ALP values(exception of paraquat plus selenium group) were increased by selenium. These results suggest that an adequate selenium confers marked protection against alcohol and paraquat-induced hepatotoxicity.

대학생의 폭음 행동 모형 (Construction of the Structural Equation Model on Binge Drinking among Korean Undergraduate Students)

  • Purpose: The purpose of this study was to construct a structural equation model (SEM) that would describe the binge drinking among Korean undergraduate students. Methods: Model construction was based on the theory of planned behavior and prototype/willingness model, using the variables; intention, attitude, descriptive norm, and prototype perception of binge drinking, alcohol use disorders identification test (AUDIT), and binge drinking behavior. The analysis of data was done with both SPSS 20.0 for descriptive statistics and AMOS 20.0 for SEM. Results: The binge drinking intention was found to have a significantly direct effect in influencing binge drinking behavior. In addition, attitude, descriptive norm, and prototype perception of binge drinking were found to have a significantly direct effect in influencing binge drinking intention. The final modified model yielded $x^2$=374.2 (p<.001), df=174, $x^2/df$=2.15, GFI=.87, AGFI=.82, NFI=.89, PNFI=.73 RMSEA=.07, and CFI=.94. Conclusion: This study constructed a model that addressed the factors related to binge drinking and described the relationship of these factors in influencing binge drinking among Korean undergraduate students. Findings from this study can contribute to designing appropriate prevention strategies to reduce problem related binge drinking in undergraduate students.

간호대 여학생의 음주실태, 음주동기와 문제음주의 상태 비교 (Comparison of Female Nursing College Students in the State of Drinking, Drinking Motives, and Problem Drinking)

  • 본 연구는 간호대학 여학생의 음주실태, 음주동기와 문제음주의 상태 비교를 하고자 실시하였다. 연구대상자는 일부 지역 간호 대학생을 대상으로 하였고, 자료수집기간은 2018년 4월부터 5월까지이며, 분석대상은 181명 이었다. 자료 분석은 정상음주 군과 문제 음주 군과의 차이를 알아보기 위하여 빈도 분석을 사용하였고, 문제음주와의 상관관계는 Pearson's correlation을 이용하였으며, 문제음주 관련요인을 분석하기 위해 로지스틱회귀분석(Logistic regression)을 실시하였다. 그 결과 음주횟수, 마시는 속도, 음주동기, 생활스트레스, 음주문제발생경험이 문제음주에 대한 위험비가 높았다. 구체적으로 살펴보면, 음주횟수로 볼 때 음주횟수가 일 년에 1-4회인 군보다 일주일에 2-3회인 군에서 문제음주에 대한 위험비가 21.53배 높았으며, 음주동기는 음주동기가 낮은 군 보다 음주동기가 높은 군에서 5.96배, 음주문제발생경험은 음주문제발생경험이 낮은 군에 비해 음주문제발생경험이 높은 군에서 8.80배 문제음주에 대한 위험비가 높게 나타났다. 결론적으로, 여대생의 문제음주 관련 요인은 음주횟수, 마시는 속도, 음주동기, 생활스트레스, 음주문제발생경험이 관련성이 높게 나타났다. 따라서 대학생들의 음주실태와 위험요인을 파악하고, 주기적인 절주교육, 문제음주선별과 예방프로그램을 통해 문제음주가 될 위험성을 낮추고, 건전음주문화 정착을 위해 노력해야 할 것이다.

여자 청소년의 초경시기에 따른 흡연 및 음주행위 (Smoking and Drinking Behaviors of Korean Female Adolescent according to Early Menarche)

  • Objectives: The aim of this study was to identify differential impacts on smoking and drinking behaviors according to early menarche. Methods: 31,277 Korean female adolescents who had undergone menarche were included. Among them, 1,822 participants(5.8%) who experienced menarche in elementary school under 4th grade were assigned to early menarche group and others were was assigned normal menarche group. Using 11th(2015) Korea Youth Risk Behavior Web-based Survey, logistic regression analysis was conducted. Results: Adjusted confounding variables were analyzed and it was found that both drinking and smoking behaviors were higher in early menarche group. The participants with early menarche showed higher risk of non-daily smoking(OR=1.39, 95%CI=1.18-1.63), light daily smoking(OR=1.64, 95%CI=1.14-2.35) and moderate-to-heavy daily smoking(OR=3.15, 95%CI=2.02-4.92). Also, in the drinking behaviors, the risk of light and heavy drinking was higher than those of normal menarche group. Conclusions: Finding of this study showed Early menarche adolescents were associated with heavy smoking and drinking behaviors compared to normal menarche. Further studies are needed more evaluations.
