A Study on the Background and Architectural Characteristics of Unit-care System in Japan - Regarding with the New-Model of Welfare Facilities for Aged Japan - (일본 유니트케어의 도입배경과 건축적 특징에 관한 연구 - 일본 신형 특별양호노인홈의 사례를 중심으로 -)

  • The purpose of this study is to propose our direction of long-tenn care facility for elderly. The long-tenn care insurance plays an important role for human well-being. The functional and architectural changes of Japanese elderly care facilities have been analysed in order to predict changes in Korean welfare facilities. In Japan, Unit-care and private rooms are essential elements after reforming welfare facilities for aged. This study proposes components of unit-care through the analysis of 20 facilities for the aged. A unit is basically consist of private rooms, rest room, public living area, kitchen, health care service station, and bath room. Besides, smoking room, tea room, public terrace, guest room, court yard, and so on can be added. A unit can be connected with other unit by semi-public spaces or can be independently organized. This relationship has been classified into 2 types; Open unit type and Separated unit type.

Factors Influencing Housing Satisfaction in the New Middle-Aged Rural Returnee Generation

  • Purpose: The purpose of this study was to analyze the factors influencing the housing satisfaction of the new middle-aged who returned to farming to derive the implications of real estate policies to resolve regional imbalances. Research design, data and methodology: As a result, this study used secondary data, housing survey data, and identified factors influencing housing satisfaction through multiple regression analysis as well as basic analysis such as exploratory factor analysis, reliability analysis, technical statistics analysis, and correlation analysis. Previous studies, in particular, mainly focused on aging, retirement, economic activity, and retirement preparation, but this study was able to derive predictive factors that can increase the persistence of living after returning to farming by analyzing housing satisfaction for the new middle-aged, and as a result, suggest implications for resolving regional imbalances. Results: As a result of the analysis, it was found that housing structure status had the greatest influence among the three factors of housing-related predictors: housing structure status, facility accessibility, and surrounding environment, suggesting that policy alternatives that can improve the quality of the housing structure itself should be prepared rather than facility accessibility and the surrounding environment. Conclusions: Based on these results, we discussed implications for improving the housing structure to increase the continuity of living for the new middle-aged after returning to farming, and presented follow-up research directions on the need for complementary research in addition to individual analysis of influencing factors, limitations of secondary data, and the lighting of research on the new middle-aged and other generations who did not return to farming.

Health Status and Utilization of Long-term Care Facility in the Urban and Rural Aged (도시와 농촌 노인의 건강기능 상태 및 요양시설 이용의사)

  • Purpose: The purpose of this study was to identify differences in health status and the utilization of long-term care service between urban and rural aged residents in Korea. Methods: Through convenience sampling, 1,405 elders (829 from urban areas and 576 from rural areas) were selected during March 1 to May 31 in 2004. All the subjects agreed to participate and filled out the survey questionnaire after signing the consent form. The instruments utilized in this study were the impairment of physio-sensory function, ADL IADL, cognitive function, and psycho-social function scale. This instrument was developed by modifying the scab developed by Gurland & Wilder (1984). Data was analyzed using the SPSS Win program. Results: There were significant differences in economic status, duration of living and type of medical insurance between rural and urban elderly(p<.05). Physio-sensory functions (t=4.53. p<.001), ADL (t=3.61. p<.001), IADL (t=2.45, p=.014), cognitive functions (t=-2.63. p=.024) and psycho-social functions (t=3.69. p<.001) were significantly different between the two groups. The utilization of long-term care facility in the urban elderly was significantly higher than that in the rural elderly ($x^2=10.14$, p<.001). Conclusion: Considering these findings. the need for long-term care should be assessed by residence characteristics. Because of different utilization of long term care facility according to the elderly's needs, long-term care services should be considered the residence characteristics.

Analysis of periodontal pathogens in care facilities for the elderly with dementia (Real-time PCR을 이용한 요양보호시설 치매 노인의 구강 병원성 미생물 분석)

  • Objectives: The purpose of this research is to examine oral pathogen distribution among elderly with dementia in a care facility to understand the importance of preventively managing oral diseases in terms of preventively managing senile diseases. Methods: From 11th August 2015 to 11th October 2015, gingival crevicular fluid was collected from 130 subjects consisting of demented/non-demented elderly people aged above 65 in care facilities located in the regions of Busan/Gyeonggnam. Based on collected data, real-time PCR analysis on oral pathogen was conducted. Results: The demented elderly group consisting of female patients aged from 0 to 79 indicated higher ratios of T. denticola in comparison to the non-demented elderly group, and the demented elderly group consisting of female patients aged above 80 indicated a high ratio of S. mutans. It was confirmed that P. gingivalis and T. forsythensis categorized under the red complex are correlated, and that bacterial species categorized under the orange complex and bacterial species categorized under the red complex are correlated. Conclusions: Because eldery people with demntia are exposed to periodontal disease and dental caries, their oral environments are more vulnerable. In order to improve such environments, it is necessary to provide care facility personnel with an opportunity to receive education to become aware of the importance of oral health, and it is also necessary to compulsorily assign dental hygienists to care facilities so that they can serve as personnel specializing in oral health management.

A Study on the Supply Estimation and Allocation of Healthcare Facilities for the Elderly People in Seoul (서울시 노인의료복지시설의 공급과 배치에 관한 연구)

  • Recently, as the population of the elderly rapidly increases, the number of the special care facility for the elderly is a big social issue. Especially, special care facilities(nursing home, skilled nursing home, etc) for the elderly in Seoul are insufficient now. So this study quantitatively estimates nursing home beds needed in seoul in 2007, 2012 and 2017, and proposes an allocation of these facilities in Seoul. This study is to clarify problems in supply of long-term care facilities and present a solution for them. The main outcome of this study can be summarized into three parts; first, as architectural planning and supply estimation of long term care facility, an aged population of cities and countries in Seoul should be considered. Second, when the allocation of long term care facility in Seoul, the type of facilities, regional equilibrium etc. should be considered. Third, nursing home and residential care home are linked with the other area.

Distribution of Depressive Disorders among the Aged People by the Type of Residence (일개 중소도시의 거주형태별 노인 우울장애 분포 양상)

  • Objectives: We studied to find out the relevant relationship between the type of residence and the level of mental depression among aged people. Methods: The authors surveyed 156 people who are over 65 in a small city. Here are three groups: 50 persons who live in the facility for the aged. 72 persons who live with their family and 34 persons who live alone. We developed the questionnaire and the severity of depressive symptoms was measured using Korean Form of Geriatric Depression Scale (KGDS) score. Results: Positive rate of depressive disorders among the aged people was 43.6%. Positive rate of depressive disorders among the aged who live in the facility for the aged was 42.0% and that persons who live with their family was 9.7%. It's more likely to be their gender, the marital status, schooling, going out and pocket money that affect on their mental depression. According to the multiple logistic regression, the odds ratios of the persons who live in the facility for the aged versus persons who live with their family and pocket money in a month were significant (p<0.05). Conclusions: The research shows that the mental depression in the aged people is more serious in the facility for the aged and also significantly related to their pocket money.

  • In the 70s, facility called 'Sozialstation' was introduced in Germany, which is a supporting organisation for a systematic and efficient integrated health and social service system for the aged. As a theoretical approach, presented here could be a foundation for space planning for the development of an integrated health and social service system for the aged in Korea, this study examines design, concept, function, operation area and origin background etc. of the 'Sozialstation' through the case studies of space program.

  • The senior adults which are a central manpower of economic activity of the nation decreased and the elderly sustenance allowance already went over 10%. And the economic activity participation of the woman which are the supporter of the unpaid the elderly within the family is increasing. This big change is expected to support awareness. To respond to these changes, the improvement of social welfare system for elderly with the job of retirement lifestyles of the elderly, a figure that is needed for the ceremony. The elderly medical treatment facility that began in 2008 the elderly long-term medical treatment law enforcement because of the demand is expected to grow. It is forecast with the fact that the data which is fundamental is most important will become that old person medical treatment facility of the middle-aged layer which is a central role of the protector who decides the facility use of the preliminary consumer of the elderly care facility and currently the very the elderly and manhood ceremony and the preference to plan of the elderly welfare facility. The purpose of this study is to present the fundamental data about the elderly care facility for comparative analysis the awareness & perfernces of the elderly care facilities of the senior adults & the elderly.

  • Although the government financial aid is insufficient and the lack of programs and facilities prevent it from developing and activation in the rural local society, welfare centers for the aged are formed an ever-greater part of the leisure and welfare facility for the aged in rural areas. As a result, the objectives of this study are to investigate and identify the actual conditions of use by the aged and the degree of satisfaction for welfare centers for the aged in Uiseong district. The results of this study are as follows. In the survey sample characteristics based on a total of 303 survey, the percentage of women (55.2%) was much higher than men (44.8%), average age is 73.3 years old. According to the results of the analysis, most important motivation is for health and making friends, and major useful facilities and programs are related to physical activities, sing a song and rehabilitation. From the results of the overall satisfaction, positive promotions and sufficient aid are required for the aged in the rural local society. Further any programs developed internally are required to promote and activate the leisure activity programs for the aged.

  • Aging brings color-weakness to the eye due to yellowed eye sight. The 87% of information required for human behavioral decisions are visually perceived. Therefore, the color-weakness will affect behavior in many ways. Most of previous research regarding color planning for the aged focuses on discovering their preferred colors and reflecting the colors on facility color planning. But such research failed to identify the confused range of color cognitions due to the color weakness of the aged. The purpose of this study is to propose a design guide for fee-charging residential facilities for the elderly, reflecting upon color perception characteristics. An experimental simulation on aged vision using the UV filter and Y-2 filter, YA-3 filter alternating for general eyesight, 70 year-old and late 80 year-old was conducted. The results of this study are as follow : color perception is changed by crystalline lens aging process and yellowed eye sight. The recognition arrangement of colors was preferred to contrast harmony rather than similarity harmony and the dominant color is a warm color rather than a cool color.
