This study is analyzed the implementation of medical aid for patients over 65 years of age who are among the discharged from hospitals with the capacity of over 100 beds. I have analyzed it with the data from an in-depth study of injury surveillance of discharged patients from hospitals done in a national project in 2004. After analyzing the results of the data from the beginning of this national project to the data collected in 2008, I could get the results that the rate of discharged patients over 65 years of age increased every year. Among them, the rate of discharged women was higher than that of the men, and the rate of deaths while at the hospital for patients over 65 years of age was higher than that of patients less than 65 years of age. The rate of operations done on patients over 65 was lower than that of patients under 65 years of age. The results of a diagnosis of popular symptoms showed that the rate of the diagnosis of cerebral infraction and structure of the heart at the circulatory organ was higher. In addition, the rate of the diagnosis of lung cancer, pneumonia, and chronic obstructive lung disease was higher, as well as the rates of gastric cancer, diabetes, liver cancer, and colorectal cancer. The results showed that the operation of the nerve system or cardiovascular system were higher. Therefore, according to this result, we should prioritize and allocate resources to the elderly people when setting up a management policy. And also, we should promote healthcare for elderly people after considering the characteristics of the implementation of medical aid in preparation of a super-aged society.
Background: Lung cancer continues to increase and one half of all cases of lung cancer occur in patients age 65 years and older. However, it seems that lung cancer is less treatable in elderly patients because of co-morbid illness or poor tolerance of surgery and chemotherapy. The intention of this study is to seek an adequate treatment approach for lung cancer in the elderly through an understanding of its characteristics. Method: The clinical data of 207 patients who were diagnosed with histologically proven lung cancer at the department of internal medicine in Seoul Municipal Boramae Hospital between September 1994 and August 1998 were retrospectively analyzed according to their age groups; group I$\geq$65 years(n=122) and group II<65 years(n=85). Results: The peak incidence of age was 7th decade(36.2%) and male age 65 years and older were 42% of all patients. Although dyspnea was more common in group I(26%) than in group II(11%)(p=.0l), there were no significant difference in other symptoms, stage, and histologic type between two groups. Group I significantly had more patients with poor performance(ECOG 3&4) than group II(35.2% vs.12.9%, p=.000). The percentage of patients with non-small cell carcinoma received supportive care only was significantly higher in group I than in group I(74% vs. 35%, p=.000). However, survival of patients who had curative intent treatment was similar between two groups(median survival 11.3 mos vs. 23 mos, p>.05). The histologic subtype, stage and performance status were significant prognostic factors affecting survival, but age itself was not. Conclusion : Lung cancer is prevalent in the elderly and aggressive diagnosis and treatment should be considered in elderly patients with good performance status.
This study analyzes the structural characteristics of rapid changes in Korean population, using the data of a census and other data on the changes in the population. Major findings are : 1. During 1950~1955, annual increase rate of population was about 1.02%, about 2.88% during 1955~1960, and 1.36% during 1980~1985. It is expected to decrease to 0.07% during 2015~2020. 2. Major Age Composition 0~14 is expected to reduce to 16.5% in 2020 from 41.2% in 1955, while the ratio of population aged 15~64 is expected to increase to 72.1% from 55.5%. Furthermore, for the population group of age 65 and over is expected to increase from a mere 3.3% to 11.4%. 3. The aging index of population is expected to increase to 69.5 in 2020 from 8.0 in 1955 and so the old dependency ratio is expected to 15.9 from 6.0 4. The median age is expected to 40.2 in 2020 from 19.0 in 1955. 5. In 2020 the child-woman ratio is expected to reduce to 22.3 from 64.7 in 1955. 6. In 2020 the age index of 0~4 is expected to 57.4 from 169.4 in 1955, and the age index of 65~69 to 261.7 from 95.6 on the contrary.
Purpose: The purpose of this study is to identify risk factors of accidental falls by age in the Korean elderly. Methods: The study subjects included 10,242 community-dwelling elders aged 65 or over. The data were analyzed by logistic regression analysis. Results: The risk factors of accidental falls in the younger old group (aged 65~74) include fear of falling (OR=16.28, 95% CI: 12.03~22.03), depression (OR=1.24, 95% CI: 1.06~1.44), and hearing impairment (OR=1.20, 95% CI: 1.01~1.43). The risk factors of accidental falls in the older old group (aged 75~84) include fear of falling (OR=11.54, 95% CI: 7.04~18.93), muscle strength (OR=1.36, 95% CI: 1.15~1.60), number of chronic disease (OR=1.65, 95% CI: 1.09~2.50), and presence of spouse (OR=1.27, 95% CI: 1.06~1.51). The risk factors of accidental falls in the oldest old group (aged 85 years old or older) include fear of falling (OR=8.05, 95% CI: 1.85~34.98) and exercise (OR=2.91, 95% CI: 1.36~6.24). Fear of falling is defined as a common factor in the all age groups. Conclusion: Different strategies should be developed for age groups to prevent elderly falls with understanding of contributing factors of falls in each group.

Objective : Lumbar spinal stenosis is increasingly recognized as a common cause of low back pain in elderly patients. Conservative treatment has been initially applied to elderly patients, however, surgical treatment is sometimes indispensable to relieve severe pain. We retrospectively examine the age-related effects on the surgical risk, and results following general anesthesia and operative procedure in geriatric patients for two different age groups of at least 65years old. Methods : Consecutive 51 patients [${\ge}$ 65years], who underwent open surgical procedure for degenerative lumbar spinal stenosis, were selected in the study. Patients were divided into two groups. Group A included all patients who were between 65 and 69years of age at the time of surgery. Group B included all patients who were at least 70years of age at the time of surgery. We reviewed medical history including preoperative American Society of Anesthesiologists[ASA] classification of physical status, anesthetic risk factor, operative time, estimated blood loss, transfusion requirements, hospital stay, operated level, and clinical outcome to look for comparisons between two age groups [$65{\sim}69$ and over 70years]. Results : In preoperative evaluation, mean anesthetic risk factor of patients was numerically similar between the groups. The American Society of Anesthesiologists classification of physical status was similar between two groups. There was no difference in operated level, operative time, estimated blood loss, hospital stay, and anesthetic risk factor between the two groups. The clinical successful outcome showed 82.7% for Group A and 81.8% for group B. The overall postoperative complication rates were similar for both group A and B. Conclusion : We conclude that advanced age per se, did not increase the associated morbidity and mortality in surgical decompression for spinal stenosis.
The purpose of this study was to estimate coffee consumption and it's relationship to blood pressure and blood lipid profiles in Korean adult men and women. Antheropometrics, dietary intake by 24-hour recall, and serum lipid profiles were measured in 354 subjects. The average subject ages for the age ranges of $20{\sim}49$, $50{\sim}64$, and over 65 years, were 39.7, 57.5, and 70.6 years for the men, and 40.7, 56.9, and 70.9 years for the women, respectively. Mean height and weight were significantly higher in the men and decreased with increasing of age. Thirty percent and 28% of the men and women, respectively, consumed coffee. The proportions of coffee consuming subjects in the age rangers of $20{\sim}49$ years(34.9%) and $50{\sim}64$ years(35.6%) were significantly higher than in the group over 65 years(11.5%). The mean daily intakes of food and energy were showed the same results according to sex and age. Mean daily coffee intake was not significantly different between the men and women, but it was significantly lower in the over 65 age group. Systolic blood pressure was significantly higher in the men and increasing age. The atherogenic index was significantly higher in the $20{\sim}49$ year-old men, and in the women over 65 years. There were no significant correlations among coffee consumption, blood pressure, and the serum lipid profile. In conclusion, the daily coffee consumption by adult men and women, with the exception of the elderly, was $50{\sim}90m{\ell}$, and it was not significantly correlated to blood pressure or blood lipids.
The purpose of this study was 1) to investigate the food intake, dietary habits and life behavioral characteristics, and 2) to examine the relationship among these factors in a sample of adult Korean women. In October 2010, self-reported questionnaire survey was distributed to 200 women aged 30 to 65 in Gyunggi Province. Overall, 177 women participated and completed the survey. Most participants were married (97.2%) and housewives (83.6%). About 85% of respondents lived with their children. For comprehensive analysis, all respondents were divided into three age groups; 30~39, 40~49, and 50~65. The percent of intake of high protein foods such as meat and fish, eggs, and milk products was higher younger age groups (p<0.05). Moreover, age 30~39 group were more likely to eat potatoes (p<0.05) than age 50~65 group. However, age 50~65 women more frequently consumed seaweed than other age groups (p<0.01). Women who ate breakfast or dinner consumed a significantly higher frequency of vegetables, seaweed, potatoes, and kimchi (p<0.01). Additionally, women who ate breakfast with family more frequently consumed vegetables, fruit and kimchi. Furthermore, older women were likely to have less sleeping time than other groups. In conclusion, there were western dietary patterns among adult women and these dietary patterns were partly related to different life behavior and dietary habits. Considering that negative dietary habits of adult women can influence their family and health for the rest of their life, nutritional education programs based on changes in lifestyle should be developed.
Purpose: Although the importance of life satisfaction in patients with chronic diseases has been emphasized, limited studies have been performed on the health-related quality of life (HRQoL) among elderly patients with chronic diseases. This study examined the HRQoL among Korean patients with diabetes aged 65 years or older. Methods: This study used data from the Korea National Health and Nutrition Examination Survey 2008-2016. The analysis included data of 2,353 elderly patients with diabetes aged 65 years or older who were aware that they had type 2 diabetes. Each participant was categorized into one of three age groups (65-69 years, 70-74 years, and ≥ 75 years). The EuroQol-5 Dimensions (EQ-5D) scale was computed to estimate the HRQoL. Multivariate linear regression analysis was used to compare the demographic characteristics, lifestyle characteristics, nutrient intake, and EQ-5D scores between the age groups. Results: The intake levels of most nutrients were similar in the three age groups, but the total energy intake levels were significantly lower, and the sodium intake levels were significantly higher in the older age groups (p < 0.001). In addition, all items of the EQ-5D index, except for anxiety/depression, tended to decrease with age. In particular, the oldest age group (≥ 75 years) showed significantly lower levels on most items and the EQ-5D index than those in the other age groups (p < 0.001). Conclusion: Elderly patients with type 2 diabetes may have low levels of life satisfaction, which worsens with increasing age. Therefore, it is important to develop and manage personalized guidelines and medical systems so that elderly patients have a higher quality of life for the remainder of their lives. The results of this study are expected to be used as basic data for the systematic management of diabetes in elderly Korean patients.
The purpose of this study is to analyze the factors affecting service use intention of long-term care among the disabled. This study conducts a longitudinal study using the modified Andersen model by the disabled age groups of the 50-64 group and the 65 over group. This study examined random effects panel logit analysis for the 50-64 group and the 65 over group by age variations. The results have shown that there are different factors Influencing factors for each specific age group of the disabled(50-64, 65 over). The results were as follows: there are differences of factors between the 50-64 group and the 65 over group. This study found that predisposing factors of the service use intention of long-term care were significantly related to age, residential area, education status, existence of spouses. Enabling factors of the service use intention of long-term care were significantly related to long-term care service recognition, saving, personal salary income level, housing status. Need factors of the service use intention of long-term care were significantly related to chronic diseases, psychological health, IADL. So, there are a need for a policy considerations such as service for the mid-old age disabled by age groups. Finally, implications and future research directions were discussed based on the finding of the study.
The purpose of this study was to estimate sugar intake and relation between it and blood lipid profiles in Korean adult according to age. The 346 subjects were measured anthropometric assessment, dietary intake using 24-hour recall method, and serum lipid profiles. The average age of the subjects of 20-49, 50-64, over 65 years were 40.8, 57.0, 70.2 years, respectively. The mean height and weight were significantly decreased with increment of age(p<0.001, p<0.01). The mean energy, food, and sugar intakes of the age groups were decreased with increment of age(p<0.05, p<0.001, p<0.001), that is, 1649.8 ㎉/1253.6 g/36.1 g for 20~49 years, 1555.4 ㎉/1157.0 g/31.4 g for 50~64 years, 1404.4 ㎉/893.5 g/17.1 g for over 65 years. The major foods consumed sugar of the age groups were watermelon, sugar, ice cream for 20-49 years, watermelon, sugar, peach for 50-64 years, watermelon, sugar, kimchi for over 65 years. Carbohydrate intake of the subject adjusted age provided significantly negative correlation with serum HDL-cholesterol(p<0.05). However, there was not significant correlation between sugar intake and serum lipid profile. Based on these results, further studies on effects on blood lipid of sugar and carbohydrate intakes were needed for proper carbohydrate intake.
