시뮬레이션기반 교육이 간호대학생의 전문심폐소생술 지식과 수행능력에 미치는 효과 (Effects of Simulation-Based Training on Nursing Students' Knowledge and Ability to Perform Advanced Cardiovascular Life Support)

  • Purpose: The purpose of this study was to analyze the effects of simulation-based training on nursing students' knowledge and ability to perform advanced cardiovascular life support. Methods: A nonequivalent control group non-synchronized design was used. The participants were, in total, 59 junior college nursing students (experimental group: 30, control group: 29). The data were analyzed with a ${\chi}^2$-test, independent t-tests, and analysis of covariance (ANCOVA) using IBM SPSS Statistics 23 program. Results: The experimental group ($80.8{\pm}8.4$), who received simulation-based advanced cardiovascular life support education, showed a higher level of knowledge of advanced cardiovascular life support than the control group ($75.0{\pm}9.9$), who received lecture-based education. In addition, the experimental group ($106.4{\pm}11.9$) showed a higher level of ability to perform advanced cardiovascular life support than the control group ($86.5{\pm}9.9$). Conclusion: Simulation-based advanced cardiovascular life support education was more effective for increasing the knowledge and performance ability of college students than lecture-based education. Therefore, simulation-based training should be used to improve the knowledge and ability of nursing students and should be applied with reinforced systematic education programs.

  • 본 연구의 목적은 시뮬레이션 기반 교육이 간호대학생의 전문심폐소생술 지식, 수행에 대한 자신감, 비판적 사고성향, 임상실무능력에 영향을 미치는 효과를 파악하고자 실시된 단일군 사전-사후 실험연구이다. 연구대상자는 J지역에 소재한 간호학과 4학년 성인간호학교과목을 수강한 35명의 학생으로 자료는 설문지를 이용하여 시뮬레이션 실습교육 전 후에 수집되었다. 연구결과 전문심폐소생술 지식, 수행에 대한 자신감, 비판적 사고성향, 임상실무능력 모두가 통계적으로 유의미하게 향상되었다. 그러므로 시뮬레이션 기반 교육이 간호대학생의 전문심폐소생술 지식, 수행에 대한 자신감, 비판적 사고성향, 임상실무능력에 미치는 긍정적인 효과를 확인하였다. 전문심폐소생술 지식, 수행에 대한 자신감, 비판적 사고성향, 임상실무능력을 상승시키는 시뮬레이션 기반교육의 실습모듈 개발 및 교육효과를 검증할 수 있는 후속연구가 필요하다고 생각된다.

  • The aim of this article is to provide major announcements within the last decade or so about possible medical emergencies in dentistry. This would be helpful to improve the knowledge on first aid suitable for dental environment. Syncope was the most common medical emergencies in dentistry. Medical emergency situations can be divided into urgency and true emergency. Urgency situations can be solved well if proper treatment is taken. However, even in an urgency, if the correct treatment is not performed, it may soon turn into an emergency. With the joint efforts of the Korean Dental Anesthesiology Society and the Korean Cardiopulmonary Resuscitation Association, dental advanced life support (DALS) has been prepared since 2015. Through 17 training experiences until January 2020, scenarios and textbooks were developed. Dentists and dental care teams need to be prepared to strengthen their competence as professionals and to increase their team-level response capabilities..

  • As a dentist, we should be prepared for emergencies that can occur at any time in dental practice. In addition, we must be able to provide first aid to patients with the latest knowledge when an emergency actually occurs. It has been revised every five years since the American Heart Association published advanced cardiovascular life support (ACLS) guidelines. The current final guideline is the 2015 version. It is expected that the 2020 version will be released around the winter of 2020. Therefore, at this point, the latest version (2015 ver.) will be important. Many changes were made from 1995 to 2015. I would like to summarize the parts of the revised parts that remain important.

  • 연구목적: 본 연구는 미국심장협회 ACLS provider course의 시뮬레이션기반 전문심장소생술 교육이 간호학생의 지식 및 수행능력에 미치는 효과를 검증하기 위함이다. 연구방법: 연구대상은 D대학 졸업학년 간호학생 22명이다. 수집된 자료는 SPSS/WIN 18.0 program을 이용하여 분석하였다. 연구결과: 시뮬레이션기반 전문심장소생술 교육을 받은 후 교육 전보다 간호학생의 지식 및 수행능력 점수가 유의하게 높아졌다. 결론: 시뮬레이션기반 전문심장소생술 교육은 간호학생의 지식 및 수행능력을 향상시키는데 효과적임을 검증하였으므로 간호학생 대상의 표준화된 응급간호 교육프로그램 개발에 기초자료를 제공할 수 있다고 본다. 또한 전문심장소생술 교육에 다양한 학습방법을 적용한 후 비교분석하는 후속연구와 지식 및 수행능력 지속성 연구와 같은 종적연구를 제언한다.

Felder-Silverman 학습유형에 따른 전문심장소생술 시뮬레이션 교육의 지속효과 (Continuous effect of advanced cardiovascular life support simulation education according to Felder-Silverman learning style)

  • Purpose: The purpose of the study was to investigate the continuous effect of advanced cardiovascular life support (ACLS) simulation education according to Felder-Silverman learning style. Methods: A self-reported questionnaire was completed by 94 students of emergency medical technology and nursing. There were 50 female students (53.2%) and 88 students (93.6%) had basic life support certification. The study instruments included knowledge, performance, and confidence. Data were analyzed using SPSS v. 20.0. Results: The learning style consisted of reflective type (51.1%), sensory type (76.6%), visual type (63.8%), and sequential type (64.9%). There was a significant difference in continuous effect on performance by learning type. Conclusion: It is necessary to identify the learning style of students before simulation education in order to maintain continuous effect of ACLS education.

소아치과의사를 위한 심폐소생술과 소아고급생명구조술 (Cardiopulmonary Resuscitation and Pediatric Advanced Life Support for Pediatric Dentist)

  • 심폐소생술(cardiopulmonary resuscitation, CPR)은 심장의 기능이 정지하거나 호흡이 멈추었을 때 인공적으로 혈액을 순환시키고 호흡을 보조해 주는 응급처치이다. 심폐소생술은 기본생명구조술(basic life support, BLS)과 전문소생술(advanced life support, ALS)로 나눌 수 있다. 기본생명구조술은 주요 조직으로 혈류 공급을 강제적으로 하기 위한 흉부압박과 호흡정지 환자에게 구조호흡(rescue breathing) 그리고 심실세동을 개선해 주기 위한 자동제세동기(automated external defibrillator, AED)이 포함된다. 전문소생술의 범주는 성인을 대상으로 하는 고급생명구조술(advanced cardiovascular life support, ACLS)과 소아를 대상으로 하는 소아고급생명구조술(pediatric advanced life support, PALS)이 있다. 치과 치료에 극심한 공포를 가지며, 여러 이유로 치과 진료에 협조를 얻기 어려운 소아를 대상으로 하는 치료환경에서는 약물을 이용한 진정법이 고려된다. 이는 심정지를 포함한 응급상황이 발생할 가능성이 증가하는 이유가 된다. 소아고급생명구조술은 기본생명구조술을 포함하며, 심정지에 이를 수 있는 원인을 호흡, 순환장애 그리고 심인성으로 나누어 대처하는 체계적인 방법을 제시한다. 소아는 성인에 비해 심인성의 원인은 낮지만, 해부학적인 약점으로 인해 호흡이나, 순환장애에 의한 심정지의 가능성은 높다. 따라서, 스트레스를 많이 받은 아이를 치료하거나 진정법을 시행하는 소아치과의사는 소아고급생명구조술을 익힘으로써 응급상황에 적절히 대처할 수 있는 역량을 갖출 수 있으리라 사료된다.

  • 진정법을 시행하는 치과의사는 반드시 발생할 수 있는 응급상황에 적절히 대처할 수 있는 능력과 환경을 갖추어야 한다. 2010년 치의학회에서 발간한 진정법 가이드라인에서 진정법을 준비하고 책임지는 치과의사는 기본생명구조술과 전문심장구조술 자격을 유지할 것을 요구하고 있다. 국내에서는 대한심폐소생협회가 2004년부터 기본생명구조술 및 전문심장구조술을 위한 Provider, Instructor 양성 과정을 진행하고 있다. 저자는2014년에 이 과정들을 이수하면서 과정에 대한 소개와 경험한 내용을 공유하고자 하였다. 기존 교육이 응급의료를 담당하는 사람들에게 초점이 맞추어 졌기에, 향후 치과의사의 특수성에 보다 적합한 교육 과정 개발이 필요하다고 사료된다.

심정지 환자 이송 시 구급대원의 자동제세동기 사용현황 (Use of Defibrillator(AED) on Prehospital Cardiac Arrests)

  • Purpose: There has been an increase in the number of prehospital cardiac arrests due to the increasing number of cardiovascular diseases and the increase in the average age of the population. We performed this study to identify the proper resuscitation technique and AED to be used to increase the survival rate in prehospital cardiac arrests. Methods: This studied 159 victims with prehospital cardiac arrests(VF or VT rhythm) by EMT's Reports from January to August, 2005. Results: 108 of 159 victims(67.9%) were shocked by AED. Eighty of 159 victims(50.9%) were recorded with AED shock in prehospital cardiac arrests. A number of shocks is averaged 2.19; 46.2% of one-shock and 86.1% of 1-3 shock. EMS first-tier response interval from time of dispatch to scene arrival was 5.88 minutes, from scene arrival to scene start was 7.36 minutes, from scene start to hospital admission was 9.91 minutes and from scene arrival to AED shock was 6.84 minutes. EMT provided advanced care to prehospital cardiac arrests: 97.5% in CPR, 10.1% in advanced airway management, 67.9% in AED shock. Conclusion: With the increase in cardiovascular disease and old age, the number of prehospital cardiac arrests has risen gradually. However, there were lack of CPR by bystander, defibrillation and advanced cardiac life support(ACLS) in prehospital stage. To improve the adequacy of basic life support and to increase the performance of ACLS, especially AED, we must create challenges to develop new protocols in prehospital care.

병원전 처치에서 자동제세동기의 사용 (Defibrillator(AED) in prehospital care)

  • Purpose: There has been an increase in the number of prehospital cardiac arrests due to increases in both cardiovascular diseases and the average age of the population, We performed this study to identify the proper resuscitation technique and AED to be used to increase the survival rate in prehospital cardiac arrest. Methods: We studied 57 victims with prehospilal cardiac arrest by EMT's Reports form January to December, 2004. Results: Fifty-seven of 92 victims were trasported with ECG in prehospital cardiac arrest. Ventricular fibrillation(VF)/ ventricular tachycardia was 56.2%, asystole was 33,3%, and pulseless electrical activity(PEA.) was 10.5% in initial rhythm analysis, There weren't attempt CPR in 14.0%. Twenty of the 32(62.5%) were shocked by AED for the adequacy method. There were no statistical differences the transportation time. Conclusion: With the increase in cardiovascular disease and old age, the number out-of-hospital cardiac arrests has risen accordingly. However, there were lack of CPR by bystander, defibrillation, advanced cardiac life support(ACLS) in prehospital stage. To improve the adequancy of basic life support and to increase the performance of ACLS, especially AED, we must create challenges to develop new protocols in prehospital care.
