The Factors Associated with Depression in the Elderly Male: Based on the 5th Korea National Health and Nutrition Examination Survey (국민건강영양조사자료에 기초한 남성노인의 우울 영향 요인 분석)

  • Purpose: This study was conducted to identify factors influencing depression in the elderly male in Korea. Methods: The initial data were collected from the $5^{th}$ Korea national health and nutrition examination survey (KNHANES-V). The data of 1,210 adults with the age over 65 years were finally analysed using t or ${\chi}^2$ test, stepwise multiple logistic regression. Research variables utilized in this study were 29 factors including demographic and health-related characteristics, physical and economic activities, and life habits. Results: Seven factors were found to be associated with depression in the elderly male including stress level, uncontrolled drinking experience, physical discomfort days in recent two weeks, the level of activities in daily life, diabetes mellitus, economic activity status, and sleeping hours. Conclusion: These results can be used in developing appropriate depression prevention program considering the characteristics of the elderly male.

The Future Direction of Learning Objectives and National Nurse's Licensing Examination in Adult Health Nursing (성인간호학 교육의 학습목표와 국가고시 문제 개선방안)

  • Purpose: The purpose of this study was to identify a model based on f the learning objectives in adult health nursing curriculums. The model can be eventually reflected in Korean National Nurse's Licensing Examination (KNNLE). Methods: The model was developed through a series of practical analysis by the task force team from June to October, 2011. The research team conducted the data synthesis and analysis from the learning objectives of adult health nursing drawn from selected national and international universities, from the adult health nursing areas in KNNLE, and from the current disease prevalence from clinical data in the representative university hospitals in Korea. Results: The current disease prevalence and mortality rates are on the increase especially for malignant neoplasm, heart diseases, suicide and diabetes. The items on the KNNLE of adult nursing are categorized by body systems into digestive system (15%), introduction (13.8%), respiratory system (11.3%), and cardiac system (11.3%). While the current system-based learning objectives covers extensive areas of adult health nursing, the core items are required to be selected based on core competences and core learning objectives to restructure the items of KNNLE. The first revised model is to consolidate and restructure the items microscopically in the subjects of adult health nursing by system. The second proposed model from macroscopic perspective is to build a comprehensive scheme of nursing curriculum by encompassing 8 subjects under current KNNLE and adjust the number of items accordingly. Conclusion: The items of KNNLE need to be gradually redesigned by considering the intervention need at clinical practice and the modified learning objectives of adult health nursing. Based on the job analysis on core competences for newly employed nurses performed by the Korean Accreditation Board of Nursing, it is necessary to establish the core learning objectives of adult health nursing and set up standards for core items in KNNLE.

Pulmonary Langerhans Cell Histiocytosis in an Adult Male Presenting with Central Diabetes Insipidus and Diabetes Mellitus: A Case Report

  • Pulmonary Langerhans cell histiocytosis is an uncommon diffuse cystic lung disease in adults. In rare cases, it can involve extrapulmonary organs and lead to endocrine abnormalities such as central diabetes insipidus. A 42-year-old man presented with polyphagia and polydipsia, as well as a dry cough and dyspnea on exertion. Magnetic resonance imaging of the hypothalamic-pituitary system failed to show the posterior pituitary, which is a typical finding in patients with central diabetes insipidus. This condition was confirmed by a water deprivation test, and the patient was also found to have type 2 diabetes mellitus. Computed tomographic scanning of the lungs revealed multiple, irregularly shaped cystic lesions and small nodules bilaterally, with sparing of the costophrenic angles. Lung biopsy through video-assisted thoracoscopic surgery revealed pulmonary Langerhans cell histiocytosis. On a follow-up visit, only 1 year after the patient had quit smoking, clinical and radiological improvement was significant. Here, we report an uncommon case of pulmonary Langerhans cell histiocytosis that simultaneously presented with diabetes insipidus and diabetes mellitus.

Differences in Prevalence and Risk Factors of the Metabolic Syndrome by Gender in Type 2 Diabetic Patients (제 2형 당뇨병환자 성별 대사증후군 유병률과 위험요인 차이)

  • Purpose: The purpose of this study was to assess the difference between prevalence and risk factors of the metabolic syndrome(MS) by gender in type2 diabetic patients. Method: 108 participants(males 69, females 39) were recruited from the endocrinology outpatient department of a tertiary care hospital in an urban city. MS was defined by a third report of the national cholesterol education program (NCEP) expert panel on detection, evaluation, and treatment of high blood cholesterol in adults(Adult Treatment Panel III)(ATP III), and abdominal obesity was determined by Asia-Pacific criteria in waist circumference. Results: The prevalence of MS by definition of NCEP-ATP III and the Asia-Pacific criteria in waist circumference was 39.3% in males and 66.7% in females type2 diabetic patients. The abdominal obesity prevalence was seen in 44.9% of males and in 79.5% of females patients. The prevalence of low HDL-cholesterolemia in serum was 26.2% in male, 52.8% in female type2 diabetic patients. Conclusion: These results show that nurses should focus on female diabetic patients for preventing MS.

Complementary and alternative therapies for obesity

  • Obesity is a serious medical disorder because it can cause a myriad of health problems, such as heart disease, hypertension, and adult-onset diabetes. While conventional medical treatment for obesity has limitations, there is enormous public enthusiasm for complementary and alternative treatments of obesity. This article reviews currently commonly used complementary and alternative therapies for obesity, such as medicinal herbs, nutritional supplements and acupuncture. Beneficial effects and possible adverse effects associated with these treatment are discussed.

고지혈증의 원인과 치료 - 고지혈증과 식사요법

  • 고지혈증의 치료를 위해서는 올바른 식사와 생활습관의 수정이 필요하다. 미국 당뇨병학회(American Diabetics Association, ADA)는 당뇨병환자의 고질혈증의 치료를 위해 생활습관 조정, 신체 활동 증가, 체중 감소, 금연과 함께 개인별 상황을 고려한 영양적 중재를 권하고 있고, 국립 콜레스테롤 교육 프로그램(National Cholesterol Education Program, NCEP)에서 제정한 고지혈증 치료지침서(Adult Treatment Panel III, ATP III)에서는 비약물 요법의 중요성을 강조하면서 식사요법 및 생활습관의 개선(therapeutic lifestyle change, TLC)으로 고지혈증의 치료를 시작하여 6주 이상 실시한 후에 효과가 없을 때는 약물치료를 하도록 권하고 있다. 따라서 고지혈증 개선을 위해 바람직한 식사 원칙에 대하여 자세히 알아보고자 한다.

Effectiveness of Internet-based Interventions on HbA1c Levels in Adult Patients with Diabetes: A Meta-Analysis of Randomized Controlled Trials (인터넷 기반 중재프로그램을 통한 성인 당뇨 환자의 HbA1c 중재효과: 메타분석)

  • Objectives: This study aimed to verify the effectiveness of Internet-based intervention programs for adults with diabetes by conducting a meta-analysis of studies conducted since 2000. Methods: We conducted a systematic review of research papers published in domestic and overseas journals from January 2000 to December 2015, and selected 9 papers that met the analysis criteria. Data analysis was performed using the open source statistical software R 3.5.0, to analyze the effectiveness of Internet-based interventions on experimental and control groups. Results: The analysis showed that intervention programs for controlling HbA1c levels in adult patients with diabetes most commonly comprised 7 sessions on Internet-based management (77.8%), and the most common frequency of application of intervention programs was 4 session in 6 months (33.4%). The present meta-analysis revealed statistically significant effects of Internet-based intervention activities (SMD = 0.92, 95% CI 0.45-1.40). The analysis of the effect size according to the intervention period showed that the 3-month, 6-month, and 12-month interventions reported in eight studies (89%) had a high effect on the Internet-based intervention group. Conclusions: The results of this study confirm the effectiveness of Internet-based intervention programs for adult patients with diabetes. The need for research on the utilization of Internet-based intervention programs for the steady management of diabetes, a chronic disease; for the development of specific guidelines for intervention activities; and for establishing appropriate protocols are acknowledged.

Analysis of the Prevalence of Diabetes Mellitus and Related Factors for the Local Seniors (국내 거주노인의 당뇨 유병현황 및 관련요인 분석)

  • Recently, the Diabetes Mellitus is the major cause of death in Korea. According to the results of v 2011, 34% of adult(over 19 years) are diagnosed as a DM and 21.5% of the aging(over 65 years) are also DM. So in this study, we find out the status of DM prevalence among old people and related factors. Using the results, we will suggest several ways to prevent DM. We use Korean Longitudinal Study of Ageing(2006, version1.2)that those diagnosed with Diabetes Mellitus. We carried out X2 test and Binary Logistic Regression by IBM SPSS 19. 65~79 ages (p=0.006, OR=1.482), Urban people (p=0.008, OR=1.366), People diagnosed with Hypertension (p<0.001, OR=1.237), have high probability of Diabetes Mellitus. However, Subjective health condition(good) (p=0.005, OR=0.648), Subjective health condition(normal) (p<0.001, OR=0.335), and People diagnosed with Chronic lung disease (p=0.002, OR=0.778) have low probability of diabetes. Based on the study results, politic proposals were disccused.

Effect of the New Diabetic Formula on Streptozotocin-induced Diabetic Rats (Streptozotocin 당뇨모델에 대한 신당뇨방의 효과)

  • The present study evaluated the beneficial effect of new diabetic formula(NDF) in diabetic rats. Adult Sprague-Dawley rats weighing 250-300 g were used. Diabetes mellitus was induced by intramuscular injections of streptozotocin(STZ, 50㎎/㎏). The extracts of NDF were orally administered at low or high dose two times a day to fasted diabetic rats for 3 weeks. Adminstration of NDF alliviated a significant reduction in weight gain in rats with STZ-induced diabetes. Following acute and repeated treatment, low dose of NDF suppressed the blood glucose concentrations of fasted diabetic rats. Repeated adminstration of NDF for 21 days improved liver and kidney functions in diabetic rats, as indicated by decline of serum alanine aminotransferase(AL T), alkaline phosphatase(ALP), blood urea nitrogen(BUN), creatinine level and kidney weights. The present study showed that NDF exerted antihyperglycemic effects and alliviated liver and renal damages caused by streptozotocin-induced diabetes.

Factors Influencing Self Management Behavior for Patients with Type 2 Diabetes: Comparison of Difference between the Elderly and Adults (제2형 당뇨병 환자의 자기관리행위 예측 요인: 노인과 성인의 비교)

  • Purpose: The purpose of this study was to investigate the level of self management behavior of patients with type 2 diabetes and to compare the factors influencing self management behavior between the elderly and adults. Methods: The participants consisted of 105 adults and 100 elderly who visited the outpatient department of a hospital from August to December 2011. Data was collected by questionnaires and analyzed using the SPSS/WIN 18.0. Results: The elderly showed higher mean scores of subcategory of self management behavior than adults. In stepwise multiple regression analysis, a total of 42% (adults), 52% (elderly) respectively of variance in self management behavior were accounted for by self-efficacy, perceived barriers, perceived benefits, yes or no smoking and regular exercise in adults and readiness, self efficacy, perceived benefits in the elderly. Conclusion: Therefore, a diabetics intervention program should be designed and provided as a tailored one for adults and elderly separately in Korea. In the strategy for adults with type 2 diabetes in the promotion of self efficacy, perceived benefits and reduction of perceived barriers, and correction of life habits such as smoking and exercise are needed, whereas in the elderly promotion of readiness, self efficacy, perceived benefits are required.
