개인적 특성과 사회환경이 청소년의 흡연에 미치는 영향 (The Influence of Personal Characteristics and Social Environment on Adolescent's Smoking)

  • Objectives: This study identified how personal characteristics, family environment, governmental policy for the prevention and cessation of smoking might influence on adolescent smoking. Methods: This study used data from the 2006 Korea Youth Risk Behavior Web-based Survey of 71,404 middle school and high school students, giving a response rate of 90.9%. We selected 61,508 adolescents subjects of the final analysis without missing data on independent variables and dependent variables which are used in this study. This study used $\chi^2$ tests and logistic regression models. Variables were added to the regression model in three groups using a hierarchical approach.Results: Adolescents were significantly more likely to become current smokers if they were boys, were in a higher grade, and had lower academic achievement. Adolescents experiencing stress and depression were associated with increased risk of current smoking. Adolescents with single parents or students of non-living with parents comparing with students of living with parents showed the high possibility of smoking. Lower father's education was associated with increased likelihood of current smoking. Adolescents who were exposed to smoking at home were more likely to smoke. Adolescents without contacting with the antismoking media campaign was associated with increased likelihood of current smoking. Conclusion: Promoting antismoking media campaigns targeted at adolescent is required, and the smoking prevention education which are proper for subjects are required. Proper plans which could decrease the exposure of secondhand smoking should be established.

청소년들의 흡연경험 및 흡연빈도 증가에 영향을 미치는 요인 (Factors Affecting Cigarette Use and an Increase in Smoking Frequency among Adolescents in South Korea)

  • Purpose: Because it is important to prevent adolescents from becoming involved in smoking, this study was done to explore important factors influencing cigarette use and the increase in smoking frequency. Method: For this study the Korea Youth Panel Survey (KYPS) was analyzed. Because the KYPS is longitudinal, a fixed effect regression method was used to control for the effects of time-independent factors. More specifically, a logistic regression was used to explore factors affecting cigarette use, and a Poisson regression was used to explore smoking frequency. Result: As the adolescents got older, the number of male adolescents who tried smoking increased, while the number of female adolescents who tried smoking decreased. Also, the frequency of cigarette use among male and female smokers increased over time. Significant factors affecting cigarette use were friends who smoked, delinquent behavior, and loneliness at schools. Important factors affecting the increase in smoking frequency were grade (e.g., the 2nd- and 3rd-year of middle school), friends who smoked, delinquent behavior, monthly pocket money, expectation for the highest level of education, and attack tendency. Conclusions: To solve the problems linked to adolescent smoking, it is critical to develop intervention programs that target specific homogeneous subgroups of smokers, and that take into consideration gender difference in smoking and factors affecting levels of smoking behavior.

청소년 흡연과 관련된 요인 -서울시 일부 남녀 고등학생을 대상으로- (Factors related with the adolescent cigarette smoking)

  • The purpose of this study is to examine the associated risk factors for adolescent cigarette smoking. In February 1995, a total of 1793 students from 17 general high schools in Seoul were assessed with a self-completing questionnaire pertaining current smoking status and school.family.peer environmental factors. The results are as follows; 1. The overall proportion of current smoker among students in the study was 17.3%; 27.7% in males and 6.6% in females. 2. There was significant association between smoking status and all school environmental factors (ranks at school, satisfaction at school, study hours after school, extracurricular activity) examined. 3. Siblings smoking in males and family structure in females were significantly associated with the smoking status of students. 4. Functional aspect of family environmental factors, such as APGAR score, parental supervision, attachment to father or to mother were related to smoking status of students. 5. Association with friends who smoke was significantly associated with smoking status of students. 6. In males, association with friends who smoke, ranks at school, siblings smoking were significant positive predictors and APGAR score, parental supervision, attchment to mother were significant negative predictors. In females, association with friends whosmoke, ranks at school, satisfaction at school were significant positive predictors and parental supervision, attchment to father were significant negative predictors. These findings suggest that strategies that influence smoking behavior need to be directed not only to the individual student but also to their peer group, family and school environment.

궐련 흡연 청소년과 전자담배를 중복 사용하는 흡연 청소년의 금연시도 영향요인 비교 (Comparison of the Factors for Attempts to Quit Smoking by Adolescent using Tobacco Cigarettes only and those Adolescents using Tobacco Cigarettes Together with Electronic Cigarettes)

  • Purpose: This study was conducted to compare and analyze the smoking-related social-ecological factors affecting attempts to quit smoking by adolescents using tobacco cigarettes only and also those adolescents using tobacco cigarettes along with electronic cigarettes. Methods: This study, as secondary analysis research, used the raw data from the 14th Korea youth risk behavior survey 2018. The data was analyzed by frequency analysis, the Rao-scott χ2-test and logistic regression analysis when considering the complex sample's analysis. Results: On logistic regression analysis, during their first smoking period, intense physical activities and having friends who smoked were associated with significantly more attempts to quit smoking by cigarettes smokers, and their first smoking period and experiences of undergoing smoking cessation education were associated with significantly more attempts to quit smoking for dual smokers of both tobacco cigarettes and electronic ones. Conclusion: As a result of this study, we identified the need to differentiate different types of smoking cessation counseling and education according to the types of smoking in adolescents.

우리나라 고등학생의 흡연과 우울증의 관련성 (Relationship between Cigarette Smoking and Depression Symptoms of High School Students)

  • 이 연구는 2009년 청소년 건강행태 온라인 조사를 완료한 고등학교 1학년부터 3학년까지의 재학생 36,657명(인문계 27,380명, 실업계 9,277명)을 대상으로 흡연과 우울증의 관련성을 파악하기 위한 목적으로 수행되었다. 혼란변수는 성, 학년, 학교급, 학업성적, 경제상태, 주관적 스트레스 인지, 주관적 수면 충족률, 주관적 행복 정도, 주관적 건강상태, 자살 생각 경험을 포함하였다. 연구 결과, 평생 흡연 경험이 없는 학생에 비해서 흡연 경험이 있는 학생은 우울증의 관련성이 약 1.4배(OR=1.43, 95% CI: 1.35-1.50) 더 유의미하게 높았다(p<0.01). 또한, 일일 평균 흡연량이 1개비 이하인 학생에 비해서 반 갑 이상 한 갑 미만인 학생은 우울증의 관련성이 약 1.5배(OR=1.48, 95% CI: 1.22-1.79), 한 갑 이상인 학생은 우울증의 관련성이 약 2배(OR=2.05, 95% CI: 1.59-2.64) 더 유의미하게 높았다(p<0.001). 본 연구의 결과는 청소년기 흡연이 우울증과 독립적으로 관련이 있을 가능성을 시사한다. 이 결과를 기초로 청소년기 정신 건강의 증진 및 유지를 위해서 흡연 행위의 적극적 관리가 요구된다.

사회 심리 이론에 근거한 학교 흡연 예방 프로그램의 메타분석: 미국 사례와 Explanatory Variables (A meta-analysis of adolescent psychosocial smoking prevention programs in the United States: Identifying factors associated with program effectiveness)

  • 청소년을 위한 학교 흡연예방 프로그램은 사회심리 이론에 근거한 프로그램이 대체로 성공적이었다고 알려져 있으나, 각 프로그램의 효과 정도에는 많은 차이가 있다. 이 연구는 다른 메타 분석처럼 전체적인 프로그램 효과도를 측정하여 일반적인 결론을 유도한 것이 아니라, 프로그램의 효과와 관계가 깊은 요인 (Explanatory Variables)을 자세히 파악하여 보건교육 담당자, 연구원, 또는 정책 결정자들에게 구체적인 가이드라인을 제공하는 데에 목적을 두고 있다. 주요한 연구결과는 다음과 같다. 1. 8-12학년 학생들보다는 초등학교에서 중등학교로 바뀌는 5-7학년 학생들에게 흡연예방 프로그램은 더 효과가 있었다. 2. 연구 방법론에 있어서는 experimental design, random assignment, 순수 비교그룹을 사용하였을 경우, implementation fidelity와 instrument reliability가 높은 경우, 또는 10% 미만의 attrition rates일 때 프로그램 효과도 (effect size)가 더 높게 나타났다. 3. 프로그램 실행 시 또래 리더를 사용하였을 경우, 알코올 등 다른 약물을 배제한 담배만을 중점적으로 다루었을 경우, 적어도 10회 이상 연속적으로 이루어지거나 프로그램 종료 후 일년 뒤에 추가 프로그램이 주어진 경우가 더욱 효과적이었다.

흡연 청소년의 암 예방행위 관련요인 (일개지역 금연학교 참여자를 대상으로) (The related factors of adolescent smoker's cancer preventive behaviors)

  • 본 연구는 흡연청소년의 암 예방행위와 관련요인을 규명하기 위한 서술적 조사연구이다. 대상자는 B광역시소재 W병원에서 운영하는 금연학교에 참석 중인 흡연 청소년 200명으로, 자료수집기간은 2012년 9월 1일부터 2013년 3월 31까지였다. 연구결과, 대상자의 암 예방행위 정도는 학년, 종교, 유익성, 장애성, 건강상태, 일상생활, 음주, 운동, 니코틴의존도, 흡연시작 나이, 흡연기간 및 금연의도에 따라 유의한 차이가 있었다. 암 예방행위 정도는 암에 대한 지식(r=-.166, p<.05)과 미약한 음의 상관관계를, 태도(r=.212, p<.01)와는 미약한 양의 상관관계를 나타내었으며 통계적으로 유의하였다. 암 예방행위를 설명하는 유의한 변수는 일상생활, 장애성, 학년, 운동, 유익성, 암에 대한 지식, 금연의도의 순이었고, 전체 설명력은 30.6%였으며, 일상생활(${\beta}=25.8$)이 가장 설명력이 많은 것으로 나타났다. 따라서 흡연청소년들의 암 예방행위를 높이기 위해서는 일상생활과 학교생활에서 흡연을 대체할 수 있는 건전한 취미와 운동 등을 보급하고 암 예방행위의 유익성을 강조하고 장애성을 감소시킬 수 있는 보건교육과 학년과 금연의도 등의 학생들의 특성을 고려한 맞춤형 암 예방교육프로그램의 개발이 필요하다.

청소년의 흡연과 건강행위 및 건강수준과의 관련성: 궐련담배와 전자담배 비교 (Association between Adolescent Smoking and Health Behavior and Status: Comparison between Cigarette and E-cigarette)

  • Purpose: This study examined whether there is a relationship between smoking and health behavior and the status of cigarette or e-cigarette smoking adolescents. Methods: This study, as secondary analysis research, used the raw data from the 14th Korea youth risk behavior survey 2018. The data were analyzed by frequency analysis, Rao-Scott 𝑥2-test, and logistic regression analysis. Results: Differences in gender, economic levels, and the educational levels of mothers were observed between cigarette smoking and e-cigarette smoking adolescents. The relationship between smoking and health behavior and status was examined by comparing cigarette and e-cigarette smoking. The results showed that the ratio of e-cigarettes was relatively higher in adolescents who have substance experiences. In contrast, cigarette smoking was related to drinking and perceived lack of sleep. Conclusion: This study suggests that it is necessary to develop healthy behavior that is strongly related to the smoking types of adolescents and health promotion programs.

청소년 흡연자를 위한 금연건강캠프의 효과 (The Effects of a Smoking Cessations Health Camp among Adolescent Smokers)

  • Purpose: This study was aimed at evaluating the effects of a smoking cessations health camp on levels of expiratory Carbon Monoxide concentration, self-efficacy, smoking knowledge and nicotine dependence among adolescents smokers. Methods: This research uitlized a one group pre-test post-test design. The smoking cessations health camp was developed after considering cognitive and behavioral aspects and was provided for two days and one night. The number of subjects was 31 high school students. Data were analyzed using descriptive analysis and paired t-test. Results: As compared to a pre-test, the average score of expiratory CO concentration and nicotine dependence were significantly lower and the average score of smoking knowledge was significantly higher in a post-test. An increase in the mean score of self-efficacy was not statistically significant. Conclusion: These results suggest that this smoking cessations health camp can provide teen smokers with the insightful antismoking information and an opportunity for anti-smoking practices. Therefore, this smoking cessations health camp can be effective and useful for adolescent smokers in other school settings.

지역 특성과 친구의 흡연이 청소년의 습관적 흡연 시작에 미치는 영향 (The Effect of Neighborhood Characteristics and Friends' Smoking Status on the Habitual Smoking Onset in Adolescents)

  • Purpose: The aim of this study was threefold, to longitudinally examine the risk of habitual smoking onset in adolescents, to delineate the effects of neighborhood characteristics and friends' smoking status on the habitual smoking onset, and to investigate whether the association between friends' smoking status and habitual smoking onset was moderated by neighborhood characteristics. Methods: This study conducted multilevel discrete-time survival analysis, using cohort data from the 3rd to 6th waves of the Korean Child and Youth Panel Survey, which excluded habitual smokers, matched with 2010 census data on respondents' residence. Results: Habitual smoking onset risk increased from the 8th to the 11th grade, and then slightly decreased from the 11th to the 12th grade. Friends' smoking status (B = 0.60, p < .001), smoking rate (B = 0.06, p = .038), and the number of tobacco outlets in the respondents' neighborhood (B = 0.51, p = .003) were positively associated with habitual smoking onset risk. Furthermore, the association between friends' smoking status and habitual smoking onset risk was moderated by the number of tobacco outlets in the neighborhood. Specifically, the association was stronger in neighborhoods with more tobacco outlets (B = 0.58, p = .048). Conclusion: Friends' smoking status and living in neighborhoods that are more susceptible to smoking increase the risk of habitual smoking. The number of tobacco outlets in the neighborhood enhances the peer effect of adolescent's smoking behavior. Therefore, policies or interventions designed to reduce youth's tobacco use should focus on not only on reducing peer smoking, but also restricting smoking by adults and the number of neighborhood tobacco outlets.
