  • Purpose: The aim of this study is to investigate the colonization rate of Methicillin-resistant Staphylococcus aureus (MRSA) in neonates by different clinical characteristics, to presume the origin of MRSA acquisition, and to identify the risk factors associated with MRSA colonization. Methods: We retrospectively reviewed the medical records of 1,733 neonates admitted to Seoul Eulji hospital Neonatal Intensive Care Unit between January 2008 and December 2011. Nasal, inguinal and rectal swab specimens were obtained upon admission and each week until discharge. We classified the route of MRSA acquisition as; hospital associated (HA-MRSA) and community associated (CA-MRSA) according to the case definition. Results: Among 1,733 neonates, 415 (23.9%) were colonized with MRSA. Gestational age, birth weight, delivery type, maternal antibiotics usage before delivery, birth place and care place before admission were influencing factors in colonization of MRSA. The colonization rate was significantly high in neonates without maternal prophylactic antibiotics use before delivery than in the other group (relative risk 2.77, 95% CI 1.88-4.07; P<0.01), and outborns showed higher MRSA colonization rate compared to inborns (relative risk 2.28, 95% CI 1.17-4.42; P=0.015). Conclusion: We identified the neonatal MRSA colonization rate to be 23.9%. We estimated HA-MRSA colonization rate to be 10% (51/511) and CA-MRSA colonization rate to be 36% (309/858). We ascertained that risk factors in MRSA colonization in neonates were prophylactic use of antibiotics in mothers and the birth place.

The subjects for this study were 277 family members those who were giving care for the adult patients during hospitalized in general wards at Seoul National University Hospital in Seoul. Data were collected through interviews with the questionnaire from September second to September twentieth in 1989. The instrument used for this study was the family problems scale which was developed by the researcher. Analysis of data was done by frequency, percent, mean, t-test, ANOVA, Pearson-Correlation Coefficients, and Stepwise Multiple Regression Analysis. The results of this study are summarized as follows: 1. General characteristics of the care-giver in family. The average age of care-givers was 37.9 years, and the $26.4\%$ of monthly Income of family was 310,000-500,000 won group. The $93.5\%$ of family had taken the responsibility of caring for the patients instead of hiring the care-givers, and the $12.3\%$of the care-givers complained weakning of health status during care giving for the patients. The spouse took the largest part of responsibility of the care-giving services to the patient among the family members. 2. General characteristics of the patients. The average age of patient was 47 years, and the $80.9\%$ of patient was married status. The $39\%$ of patient was father in the position of family, and the $41.5\%$ had the responsibility to support their family before hospitalization. The average hospitalization period of patient was 24.3 day and the $50.9\%$ had admission experience. 3. The factors of family problems which were faced by the family were classified into six problems. The factors of family problems were ranked as follows; the first rank problem was related to care-giving for the patients. the second problem was resulted from the patients diseases, the theirds problem was related with adaptation to the hospital enviroments, the fourth problem was related to the arisen conflicts with medical team. the fifth problem was related to the change of family function. and the sixth problem was the financial problem. 4. The relationship between the family problems and the general charateristics of the care-givers showed that the nuclear type family was higher the family problems, that the admission period of patients became longer, and that the family who had the worse condition of health status of the care givers during care giving for the patients. From the above results, it was confirmed that the family care giving for patients was faced with some problems resulted from patient's illness, relation to the medical team, adaptation to the hospital enviroment, financial problem. change of family function, and care-giving for patients.

  • Purpose: On November 15, 2012, sales of OTC (Over-The-Counter) drugs began at convenience stores, which changed the accessibility of some drugs. As a result, the exposure and access patterns of these drugs could have changed. In this study, we reviewed the changes in the characteristics of drug poisoning patients because of the reposition of nonprescription drugs according to the revised Pharmaceutical Affairs Act. Methods: A retrospective study was conducted to evaluate changes in characteristics of drug poisoning patients between 2008 and 2016. A registry was developed by an emergency medical center in a local tertiary teaching hospital, and patients who visited the center were enrolled in this registry. We compared two periods, from 2008 to 2012 (Pre OTC) and from 2013 to 2016 (Post OTC), for type of intoxicant, time from poisoning to visiting the emergency center, intention, psychiatric history, previous suicidal attempt, alcohol status, and emergency room outcomes. The primary outcome was the number of patients who took acetaminophen and NSAIDs (nonsteroidal anti-inflammatory drugs). Secondary outcomes were ICU admission rate, mortality rate, and number of patients who visited the ER when the pharmacy was closed after taking acetaminophen and NSAIDs (nonsteroidal anti-inflammatory drugs). Results: Among 1,564 patients, 945 and 619 patients visited the emergency room during pre and post OTC periods. The number of patients with acetaminophen and NSAIDs poisoning decreased from 9.2% to 6.1% (p=0.016). The ICU admission rate and mortality rate in the emergency room did not show significant results in the relevant patient groups, and so was the number of patients visiting ER when the pharmacy was closed taking acetaminophen and NSAIDs. Conclusion: Despite the sales of nonprescription drugs at convenience stores, the number of acetaminophen and NSAIDs poisoning patients decreased.

  • This research is to prevent the infection of maternity in the hospital by examining the microbes contaminations in maternity through airbone microbes and those who are engaged in the ward of O.B. & G.Y. and to furnish the basic data available to hospital management. The bacterial growth of airbone microbes contaminations in nosocomial air and who thor the nasal cavity of passers by (doctors, nurses, parturient women) who went to the ward of O.B. & G.Y. contaminated or not were examined in“E”Univ. Hospital from July to August, 1979 by using thioglycollate broths and agar plates. The following results were obtained: 1. The average colony number of airborne microbes revealed as follows the pediatric ward (36 colonies), the internal ward (33 colonies), the ward of O.B. & G.Y. (30 colonies), the ward of surgery (24 colonies), delivery-waiting room (11 colonies), and the delivery room (3 colonies). 2. The bacterial growth beforenoon differed from that of afternoon. Namely, the latter (24 colonies) was higher than the former (21 colonies). 3. The type of strains isolated from the air of the ward revealed staphylococci (82%), Gram negative bacilli (18%), fungi (17%), Gram positive diplococci (13%), and Bacillus subtilis (2.8%). 4. The strains isolated in the delivery-waiting room revealed staphylococci (66.7%), Gram negative bacilli (33.6%), and revealed staphylococci (75%), Gram positive diplococci (8.3%), and fungi (8.3%), in delivery room. 5. Most of strains isolated in the ward of O.B. & G.Y. revealed staphylococci (100.0%), Gram positive diplococci (8.3%), and Gram negative bacilli (6.7%). 6. The strain isolated in the surgical ward revealed staphylococci (91.7%), fungi (33.3%), Gram positive diplococci (25%), Gram negative bacilli (25%) and Bacillus subtilis (8.3%). 7. The strain isolated in the pediatric ward revealed staphylococci (75%), fungi (25%), Gram positive diplococci (8.3%), Bacillus subtilis (8.3%), and Gram negative bacilli (8.3%). 8. The strain isolated in the internal ward revealed staphylococci (91.7%), fungi (33.3%), Gram positive diplococci (25%), and negative bacilli (16.7%). The strains isolated from the nasal cavity of those doctors and nurses who and enaged in the ward of O.B. & G.Y. revealed staphylococci (80%), Bacillus subtilis (10%), and Gram negative bacilli (10%), from doctors and Gram positive diplococci (10%), instead of Gram negative bacilli (10%), from nurses. 10. The strain isolated from nasal cavity of parturient women on admission revealed staphylococci (90%), and Gram negative bacilli (10%), but after admission revealed staphylococci (70%), Gram positive diplococci (10%), and Gram negative bacilli (10%). 11. Of the total 91 staphylococci isolated from the air of the ward, the Coagulase pastive was 36 (39.6%), and the negative 55 (60.4%), As a result of the coagulase experiment of the staphylococci isolated from the nasal cavity of those who are engaged in the ward of O.B. & G.Y. all were revealed as negative that belonged to non-pathogenic. 12. Consequence of the biochemic examination of the gram negative bacilli isolated from the air of the ward the aerobacter aerogens revealed was (16.7%) E-coli 5% in the nasal cavity of those came and went to the of O.B. & G.Y. and Aerobacter aerogens 7.5%.

  • Purpose: In extensive deep burn of the lower limb, due to less amount of soft tissue, bone is easily exposed. When it happens, natural healing or reconstruction with skin graft only is not easy. Local flap is difficult to success, because adjacent skins are burnt or skin grafted tissues. Muscle flap or free flap are also limited and has high failure rate due to deep tissue damage. The authors acquired good outcome by performing one - stage operation on bone exposed soft tissue defect with AlloDerm$^{(R)}$(LifeCell, USA), an acellular dermal matrix producted from cadaveric skin. Methods: We studied 14 bone exposed soft tissue defect patients from March 2002 to March 2009. Average age, sex, cause of burn, location of wound, duration of admission period, and postoperative complications were studied. We removed bony cortex with burring, until conforming pinpoint bone bleeding. Then rehydrated AlloDerm$^{(R)}$(25 / 1000 inches, meshed type) was applicated on wound, and thin split thickness(6 ~ 8 / 1000 inches) skin graft was done at the immediately same operative time. Results: Average age of patients was 53.6 years(25 years ~ 80 years, SD = 16.8), and 13 patients were male(male : female = 13 : 1). Flame burn was the largest number. (Flame burn 6, electric burn 3, contact burn 4, and scalding burn 1). Tibia(8) was the most affected site. (tibia 8, toe 4, malleolus 1, and metatarsal bone 1). Thin STSC with AlloDerm$^{(R)}$ took without additional surgery in 12 of 14 patients. Partial graft loss was shown on four cases. Two cases were small in size under $1{\times}1cm$, easily healed with simple dressing, and other two cases needed additional surgery. But in case of additional surgery, granulation tissue has easily formed, and simple patch graft on AlloDerm$^{(R)}$ was enough. Average duration of admission period of patients without additional surgery was 15 days(13 ~ 19 days). Conclusion: AlloDerm$^{(R)}$ and thin split thickness skin graft give us an advantage in short surgery time and less limitations in donor site than flap surgery. Postoperative scar is less than in conventional skin graft because of more firm restoration of dermal structure with AlloDerm$^{(R)}$. We propose that AlloDerm$^{(R)}$ and thin split thickness skin graft could be a solution to bone exposured soft tissue defects in extensive deep burned patients on lower extremities, especially when adjacent tissue cannot be used for flap due to extensive burn.
