The capability of lactic acid bacteria (LABs) to adhere to intestinal epithelial cells and vaginal epithelial cells is an important factor in the formation of a barrier to prevent the colonization of pathogenic bacteria. In addition, the ability to coaggregate with pathogens and production of antimicrobial agents also allow LABs to fight against pathogens. In this work, Weissella confusa PL9001 was tested for its ability to inhibit the growth and adherence of genitourinary pathogens, including Candida albicans, Escherichia coli, Staphylococcus aureus, and vancomycin-resistant Enterococcus faecium (VRE), isolated from the urine of hospitalized female patients. W. confusa PL9001 was found to coaggregate with the four pathogens, as observed with a light microscope and scanning electron microscope. In competition, exclusion, and displacement tests, the adherence of the pathogens to T24 bladder epithelial cells was also inhibited by W. confusa PL9001. Accordingly, these results suggest that W. confusa PL9001 is potentially useful for both preventive and therapeutic treatment of genitourinary infections.
Purpose: This study tested the effectiveness of brochure- and video-based education on managing surgical site infections by operating room health personnel. Methods: From April 20 to May 4, 2021, 34 operating room health personnel were subjected to training on surgical site infection management using brochures and educational videos. A survey was then conducted on knowledge, perception, and adherence regarding surgical site infection management. Results: After receiving training on surgical site infection management, the knowledge score increased significantly (15.15±2.09 vs.19.70±1.96, p<.001). However, the perception and adherence scores were already near perfect before the intervention and did not further increase after the intervention. Conclusion: It is necessary to develop and utilize continuous and substantive educational programs to improve perception and adherence of surgical site infection management.
Objective: Using the Korean Health Panel data (KHP) of 2008 and 2009, this paper analyzed the effect of antihypertension medication adherence on the changes in medical service utilization and medical expenditure in the next year. Method: Through a face-to-face interview survey, KHP has provided an extensive data on demographic characteristics, medical utilizations behavior, medical expenditure and health behaviors of the targeted households and their members since 2008. KHP is recognized as highly accurate regarding medical expenditure in that it makes the target households to record 'Health diary' whenever they use healthcare service, which could minimize their cognitive bias or memory distortion. The 2010 KHP data was based on the interview from 6,432 households and 19,697 household members. Two part model was used to explore the effects of medication adherence on medical use (logistic regression analysis) and medical expenditures (OLS). Result: Our study result demonstrated that the 74.7% of the patients who strictly adhere to their medication in both years, in 2008 and in 2009, were likely to use inpatient service in 2010. This shows the noncompliant patients were in fact use emergency service less than the compliant patients. Conclusion: Based on our analysis, this paper concludes that the high medication adherence of hypertensive patients could contribute to reduce the emergency service use. Therefore it is highly recommendable for the Korean government and the insurer, NHIS, to actively invest in developing education and promotion program to improve medication adherence among hypertensive patients.

Purpose: The purposes of this study were to identify the level of health literacy and to determine its impact on disease-related knowledge and adherence to self-care in adults with hypertension. Methods: This was a cross-sectional, descriptive study in which patients with hypertension were recruited from outpatient hypertension clinics in Seoul, Korea from December, 2009 to February, 2010. A total 186 patients completed series of questionnaires to assess health literacy, disease-related knowledge, and adherence to self-care. Hierarchical linear regression was used to determine whether health literacy was associated with disease-related knowledge and adherence to self-care. Results: Seventy-one (38.2%) and seventy-two (38.7%) patients had inadequate and marginal health literacy, respectively. In hierarchical linear regression, health literacy independently predicted disease-related knowledge (${\beta}$= .43, p < .001) and adherence to self-care (${\beta}$= .37, p < .001) after controlling for age, education level, having a job or not, and body mass index. Conclusion: These findings show that health care providers need to pay attention to patients with hypertension who have inadequate health literacy. Further, it is recommended to develop and implement new strategies for assessing health literacy in clinical practices. Interventions to improve health literacy could promote disease-related knowledge and adherence to self-care in patients with hypertension.
Purpose: The purpose of this study was to examine the mediating effect of regimen distress in the relationship between medication adherence and glycemic control in men with type 2 diabetes. Methods: A total of 116 patients with type 2 diabetes was recruited for the cross-sectional survey design. Data were analyzed using descriptive statistics, independent t-test, ANOVA, Pearson's correlation coefficient, and multiple regression using Baron and Kenny steps for mediation. Results: The mean score for medication adherence was 6.32, and the mean glycemic control level (HbA1c) was 7.47%. The mean score was 2.37 for regimen distress. There were significant correlations among medication adherence, regimen distress, and HbA1c. Regimen distress had a partial mediating effect (${\beta}$=.22, p=.005) in the relationship between medication adherence and HbA1c (Sobel test: Z=2.47, p=.013). Conclusion: Regimen distress was found to be associated with glycemic control in men with type 2 diabetes. Based on the findings of this study, nursing intervention programs focusing on decreasing regimen distress are highly recommended to improve level of glycemic control in patients with type 2 diabetes.
Purpose: In this study an examination was done of the effect of self-efficacy promoting vestibular rehabilitation (S-VR) on dizziness, exercise self-efficacy, adherence to vestibular rehabilitation (VR), subjective and objective vestibular function, vestibular compensation and the recurrence of dizziness in patients with vestibular hypofunction. Methods: This was a randomized controlled study. Data were collected 3 times at baseline, 4 and 8 weeks after beginning the intervention. Outcome measures were level of dizziness, exercise self-efficacy, and level of adherence to VR. Subjective and objective vestibular function, vestibular compensation and the recurrence of dizziness were also obtained. Data were analyzed using Windows SPSS 21.0 program. Results: After 4 weeks of S-VR, there was no difference between the groups for dizziness, subjective and objective vestibular functions. However, exercise self-efficacy and adherence to VR were higher in the experimental group than in the control group. After 8 weeks of S-VR, dizziness (p =.018) exercise self-efficacy (p <.001), adherence to VR (p <.001), total-dizziness handicap inventory (DHI) (p =.012), vision analysis ratio (p =.046) in the experimental group differ significantly from that of the control group. The number of patients with recurring dizziness were higher in the control group than in the experimental group (p <.001). Conclusion: The results indicate that continuous 8 weeks of S-VR is effective in reducing dizziness, and improving exercise self-efficacy, subjective vestibular function and adherence to VR. Objective vestibular function and vestibular compensation were also improved in the experimental group at the end of 8 weeks of S-VR.
Purpose: The aim of this study was to evaluate the level of self-care adherence in the elderly with diabetes mellitus (DM) who have lived alone, and to investigate the association between health literacy, diabetic knowledge and self-care adherence. Methods: Descriptive research using the cross-sectional approach was conducted. Data was collected by using a convenience sampling of 201 participants who aged sixty-five years old or more and have lived alone. Statistical analysis was conducted by using an independent t-test, one-way ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficient and stepwise multiple regression analysis. Results: Mean scores of health literacy, diabetic knowledge and adherence of self-care were $4.41{\pm}3.54$ points out of a possible $12, 6.22{\pm}2.55$ points out of a possible $15, 58.56{\pm}17.28$ points out of a possible 112, respectively. Adherence of self-care showed a statistical association with health literacy (r=.26, p<.001) and diabetic knowledge (r=.30, p<.001). Statistically significant factors in the regression model were monthly income, diabetic knowledge and exercise. Conclusion: The level of self-care adherence in the elderly with diabetes living alone was relatively low. Diabetic knowledge was an important factor to improve the level of self-care adherence for the elderly with diabetes living alone.
Objective: This study was conducted to evaluate payer-driven medication adherence intervention program from the patient's and counselor's perspectives. Methods: Target patients for intervention were selected by retrospective adherence measures based on national health insurance claims data for hypertension, diabetes and hyperlipidemia. As a serial intervention for higher risk groups of medication non-adherence, initial direct mailing, the first direct telephone call and the second direct call or a home visit were followed. Interview approach to qualitative inquiry was used to evaluate intervention results. Results: Participants including 4 patients received telephone calls, and 4 National Health Insurance Service staff and 4 pharmacists participated as counselors were interviewed regarding their impression of the intervention program. Three major themes arose: overall perception; necessities; and suggestions for success, of the intervention. Despite short period of intervention, educational intervention by telephone counseling involving pharmacists shows potential to improve self-management of chronic disease, and pharmacist-involvement. But more sophisticated selection of target patients requiring the intervention and complementation of electronic database system would be necessary. In addition, personal disposition of counselor was revealed to be an important factor for achieving successful outcome of intervention. Conclusion: The findings suggest that the individualized counseling intervention would be an efficient option for improved medication adherence. Further researches should include longer periods of interventions, a quantitative analysis using adherence measures based on claims data and consideration of clinical benefits associated with the intervention.
The purpose of the study was to analyze the relationship between motivation, psychological euphoria and adherence intention in elders. To achieve this purpose, subjects of this study were sampled 249 dancesports participant from J and K city in Jeonbuk. Data were collected through the self-administrated questionnaires which were used by preceding study. The factor analysis method was used correlation analysis, multiple regression analysis and three-step mediated regression analysis. The results of the analysis are as follows: First, motivation(health, hobby) had positive effects on psychological euphoria(accomplishment, satisfaction). Second, motivation(health, hobby) had positive effects on adherence intention. Third, psychological euphoria(accomplishment, satisfaction) had positive effects on adherence intention. Forth, psychological euphoria mediated between motivation and adherence intention.
This study is to analyze the relationship of speciality of personal trainer, PT satisfaction and exercise adherence. To accomplish this goal, this study sampled 240 members of sports facilities of Seoul utilizing purposive sampling method in 2013. But this study finally used 219 samples in data analysis. Validity and reliability of instruments tested by expert meetings and reliability analysis. Chronbach's ${\alpha}$ is over .697. Data analysis is SEM using AMOS 18.0. Conclusions are the followings. Firstly, speciality of personal trainer influence on PT satisfaction of members. Secondly, speciality of personal trainer influence on exercise adherence. Thirdly, PT satisfaction influence on exercise adherence. Fourthly, PT satisfaction have not a mediating role between speciality and exercise adherence.
