Acute kidney injury in pediatric patients with rhabdomyolysis

  • Purpose: This study aimed to evaluate the clinical findings in pediatric rhabdomyolysis and the predictive factors for acute kidney injury (AKI) in Korean children. Methods: Medical records of 39 Korean children, who were newly diagnosed with rhabdomyolysis from January 2008 to December 2015, were retrospectively analyzed. The diagnosis was made from the medical history, elevated serum creatinine kinase level >1,000 IU/L, and plasma myoglobin level >150 ng/mL. Patients with muscular dystrophy and myocardial infarction were excluded. Results: The median patient age at diagnosis was 14.0 years (range, 3-18 years), and the male to female ratio was 2.5. The most common presenting symptom was myalgia (n=25, 64.1%), and 14 patients (35.9%) had rhabdomyolysis-induced AKI. Eighteen patients (46.2%) had underlying diseases, such as epilepsy and psychotic disorders. Ten of these patients showed rhabdomyolysis-induced AKI. The common causes of rhabdomyolysis were infection (n=12, 30.7%), exercise (n=9, 23.1%), and trauma (n=8, 20.5%). There was no difference in the distribution of etiology between AKI and non-AKI groups. Five patients in the AKI group showed complete recovery of renal function after stopping renal replacement therapy. The median length of hospitalization was 7.0 days, and no mortality was reported. Compared with the non-AKI group, the AKI group showed higher levels of peak creatinine kinase and myoglobin, without statistical significance. Conclusion: The clinical characteristics of pediatric rhabdomyolysis differ from those observed in adult patients. Children with underlying diseases are more vulnerable to rhabdomyolysis-induced AKI. AKI more likely develops in the presence of a high degree of albuminuria.

Changes in 40m sprint times after acute squat exercise (일회성 스쿼트 운동 후 40m 달리기 기록의 변화)

  • In most sports, a key evaluator of athletic performance is sprinting speed. There are two trainable factors associated with speed including mechanical efficiency and force production. The squat exercise is one of the beneficial exercises implemented to enhance lower extremity power for sprinters. However, there has been little research on the optimal exercise intensity for sprinting performance. This study was to investigate whether performing different squat exercises prior to a 40m sprint would improve running speed. Ten male high school sprinters performed six 40m sprints following the different squat intensities (30%, 50%, 70%, and 90% of 1RM). A one-way ANOVA with repeated measures revealed that 40m sprint times had significantly improved after 50% ($5.27{\pm}0.13$, p<0.0001) and 70% ($5.26{\pm}0.15$, p<0.0001) of 1RM squat exercises compared to sprint times not following a squat % ($5.33{\pm}0.16$). Performing squat exercises with intensities of 50% and 70% of 1RM may improve 40m sprint times. Therefore, it is suggested that coaches and sprinters could include a medium intensity squat exercise (70%) in the warm-up procedure in order to improve sprint performance.

Effects of Formula (JR-22) Maybe Containing Traditional Herbs on Maximal Exercise Performance and Antioxidant Meterials in Murine Model (생약조성물 투여가 지구력 향상과 항산화 물질에 미치는 영향)

  • The effects of dietary supplementation of JR-22, formula containing distilled extracts of traditional herbs on maximal exercise performance and endurance were evaluated in mouse and rat model. In acute forced swimming test with 4%∼8% of the body weight attached to the tail, it was shown that dietary JR-22 supplementation increased endurance in exercise performance. There was no change of blood lactic acid, ammonia, inorganic phosphorous ion and creatine kinase activity, however ATP concentration in muscle was increased by JR-22 supplementation. Also, insulin-like growth factor-l (IGF-1) concentration in blood was significantly increased by JR-22 supplementation. In addition, the oxidative damage induced by exercise was reduced by JR-22 supplementation. In these results, we suggested that JR-22 supplementation enhanced maximal endurance exercise performance by the mechanism of increasing ATP and IGF-1 concentration and reducing oxidative damage.

Characteristic of Cross-sectional Area of Lumbar Paraspinal Muscle in Patients of Acute and Chronic LBP (20대와 40대의 급성 및 만성요통환자의 척추주위 근육에 관한 횡단면의 비교)

  • The purpose of this study was to compare acute and chronic LBP patient in twenties and forties, respectively by size measure paraspinal muscle (cross-sectional area; CSA). CSA of paraspinal muscle (psoas, multifidus, erector muscle) size was measured by free-handling technique of the picture archiving and communication system(PACS) using MRI at the level(lower end-plate of L4) in twenties(9 males, 10 females) and forties(9 males, 8 females) in acute and chronic LBP patient. The results of this study showed no significantly difference between acute and chronic LBP (p>0.05) in twenties patients. However, there was significant difference between acute and chronic LBP (p<0.05) in forties patients. Also, there was significant difference in paraspinal muscle CSA between chronic LBP patients in twenties and chronic LBP patients in forties (p<0.05). This study showed that paraspinal muscle atrophy was observed in forties with various cause, but Not chronic LBP patients in twenties. Accordingly it is required for chronic LBP patients in forties to minimize trunk muscle atrophy through immediate back muscle dynamic exercise and early functional activity.

Effects of Occupational Therapeutic Craft Activity on improvement cognitive function and hand function in acute patients with stroke (수공예 작업치료 프로그램 활동이 급성기 뇌졸중 환자의 손 기능에 미치는 효과)

  • This study investigatedwhether occupational therapeutic craft activity improves cognitive function and hand function in patients with acute stroke, with the hope of providing clinically useful results for further studies. From March 2016 to November 2016, thirty acute patients with stroke at A general hospital and B rehabilitation hospital in Seoul and Gyeonggi-do, Korea were enrolled and randomly divided into two groups. One group was treated with occupational therapeutic craft activity,and the other general exercise. The subjects performed each activity for 30 minutestwice weekly, for four weeks. To test the effect of intervention, hand function was tested with the Box & Block Test and the Jebsen-Taylor Hand Function Test.Cognitive function measures were the NCSE and Trail-Making Test. Over the four weeks of treatment activities, in the occupational therapeutic craft activity group, hand function measures(p<.05) increased significantly,as did cognitive function measures(p<.05). This study shows that occupational therapeutic craft activity improves hand function and cognitive function effectively in patients with acute stroke. Occupational therapeutic craft activity programscan be based on this study to prepare intervention programs for further studies with acute stroke patients.

  • Background: Chronic obstructive pulmonary disease (COPD) is now regarded as a heterogenous disease, with variable phenotypes. Acute exacerbation of COPD is a major event that alters the natural course of disease. The frequency of COPD exacerbation is variable among patients. We analyzed clinical features, according to the frequency of acute exacerbation in COPD. Methods: Sixty patients, who visited Gyeongsang National University Hospital from March 2010 to October 2010, were enrolled. Patients were divided into two groups, according to their frequency of acute exacerbation. Frequent exacerbator is defined as the patient who has two or more exacerbation per one year. We reviewed patients' medical records and investigated modified Medical Research Council (MMRC) dyspnea scale, smoking history and frequency of acute exacerbation. We also conducted pulmonary function test and 6-minute walking test, calculated body mass index, degree of airway obstruction and dyspnea and exercise capacity (BODE) index and measured CD146 cells in the peripheral blood. Results: The number of frequent exacerbators and infrequent exacerbators was 20 and 40, respectively. The frequent exacerbator group had more severe airway obstruction (forced expiratory volume in one second [$FEV_1$], 45% vs. 65.3%, p=0.001; $FEV_1$/forced vital capacity, 44.3% vs. 50.5%, p=0.046). MMRC dyspnea scale and BODE index were significantly higher in the frequent exacerbator group (1.8 vs. 1.1, p=0.016; 3.9 vs. 2.1, p=0.014, respectively). The fraction of CD146 cells significantly increased in the frequent exacerbator group (2.0 vs. 1.0, p<0.001). Conclusion: Frequent exacerbator had more severe airway obstruction and higher symptom score and BODE index. However, circulating endothelial cells measured by CD146 needed to be confirmed in the future.

Acute Achilles Tendon Rupture : - Isokinetic plantarflexion torque evaluation after Krackow suture technique - (급성 아킬레스건 파열 - Krackow 봉합술 후 등속성 족저 굴곡력의 분석 -)

  • Purpose: We try to evaluate the functional outcomes of the ankle with isokinetic plantaflexion torque for acute achilles tendon rupture cases those treated by primary repair with the Krackow suture technique and early rehabilization. Materials and Method: The authors studied retrospectively, 15 patients of acute achilles tendon rupture treated and followed over six months, from July 1997 to May 2001. There were 12 men and 3 women, and mean age was 39.6year. The repair method of ruptured tendon was single or double Krackow suture technique. One week(5days-2weeks) after operation, early ROM with ankle-foot orthosis was started. We used Arner-Lindholm Scale for the clinical evaluation and analyzed patient's satisfaction and subjective strength deficit. We analyzed the dorsiflexion peak torque and plantarflexion peak torque of the ankle statistically with strength test with Cybex dynamometer. Result: In clinical results, we had 11 excellent cases(73%) and 3 good cases(20%). In patients satisfaction degree, 11 excellent cases(66.6%), 3 good cases(20%) and 2 fair cases(13.4%). And in strength deficit, 3 none deficit (20%), 9 minimal deficit(60%). We evaluated the isokinetic plantar flexion torque in 3months or 6months after operation. After 3 months, isokinetic test showed the mean functional deficits, 32% and 25% at $30^{\circ}$ and $120^{\circ}/sec$, in 7 cases(46.6%) of 15 cases respectively. After 6 months, the mean deficits were in 21%, 24% at 30. and $120^{\circ}/sec$, respectively. At 3 and 6 months' follow up, absolute value of isokinetic test showed increase of 25. 31bs to 421bs and 19.61bs to 271bs at $30^{\circ}$ and $120^{\circ}/sec$, respectively. Conclusion: We had good result for acute achilles tendon rupture treated by Krakow suture technique and early range of motion exercise of the ankle. After 6 months, strength deficit was 21% in all of cases but were able to return pre-injured state. This study shows Krakow suture technique was recommended method for primary repair and early rehabilization of achilles tendon ruptue.

Effects of Exhaustive Exercise and Aged Garlic Extract Supplementation on Weight, Adipose Tissue Mass, Lipid Profiles and Oxidative Stress in High Fat Diet Induced Obese Rats (탈진적 운동과 마늘진액 섭취가 고지방식이로 비만이 유도된 흰쥐에 체중, 지방량, 혈중지질 및 산화적 스트레스에 미치는 영향)

  • The purpose of this present study was to investigate the effects of long-term treadmill running and aged garlic extract supplementation on weight, adipose tissue mass, lipid profiles and oxidative stress after exhaustion in high fat diet-induced obese rats. Thirty male Sprague-Dawley rats (3 weeks old) were used as subjects. High fat diets were fed to all subjects for 6 weeks in order to induce obesity. The subjects were divided into five groups - N (normal diet group), HD (high fat diet group), E (exercise group), EA (exercise with AGE diet group) and A (AGE diet group). Aerobic exercise was performed through treadmill running and AGE at a dosage of 2.86 kg/g was administered to rats 30 min before every exercise for 4 weeks. After 4 weeks, all groups completed acute treadmill running (speed increasing gradually to 25 m/min, 15% uphill grade) until exhaustion. Immediately after exhaustive treadmill exercise, the weight, adipose tissue mass, lipid profiles and oxidative stress of the rats were assayed. At the end of 6 weeks of high fat diets, body weight and body weight gain were significantly higher in the high fat diet groups than in the normal diet group (p<0.001). At the end of treadmill exercise with AGE intake for 4 weeks, body weight gain, visceral and epididymal fat of the E and EA groups were significantly decreased compared to other groups (p<0.05). There were no significant differences in gastrocnemius and soleus. T-C, HDL-C, TG and LDL-C were not significant in any of the groups. TBARS was significantly lower in the A group than in the E group (p<0.05). These results indicated that body weight gain, visceral and epididymal fat decreased in the E and EA groups, and TBARS levels were lower in the A group than the E group. Regular aerobic exercise intervention with AGE supplementation may also modify the adipose weight and improve the oxidant stress in obese rats.

  • Background : Although the long term adverse effects of cigarette smoking on health are well known, the acute possible detrimental effects of smoking on pulmonary or cardiovascular function, especially when these systems are stressed by the metabolic demands of exercise, have not been well studied The purpose of this study is to determine the acute action of cigarette smoking on cardiopulmonary function under stress. Method : Twenty -one healthy smoking subjects were studied. Before exrecise testing, history taking, physical examination and baseline studies, including CBC, chest PA, PFT and EKG, were done. The subjects performed an incremental bicycle exercise test to exhaustion on two occasions, one without smoking and the other after smoking 5 cigarettes/h for 2 hours. All indices of P.F.T. and bicycle ergometry were compared between before and after smoking. Results : 1. $VO_2$max and $O_2$ pulse showed significant decrease in smoking day. 2. Although there were no significant differences, anaerobic threshold showed a tendency of decrease and HRmax showed that of increase in smoking day. 3. P.F.T. and respiratory indices showed no significant change io smoking day. Conclusion : Cigarette smoking has immediate adverse effect, especially on the cardiovascular system rather than the respiratory system. These results would be due to the effect of elevated HbCO and/or impaired blood flow in response to the exercise stimulus.

Water Workout Recovery Program on the Patient with Guillain-Barre Syndrome (부상회복 프로그램에 따라 실시한 Guillain-Barre 증후군 환자의 수중운동)

  • Guillain-Barre syndrome(GBS) is one of the common motor unit diseases and defined as acute postinfectious polyneuropathy, It is not known most effective medical intervention for GBS, but generally benefits from an intensive physical therapy program. In this report, hydrotherapy was applied for a patient with GBS to improve muscle power and functional abilities. Two weeks later, the patient's functional abilities, muscle power, and physical endurance were improved without complications. Since the hydrotherapy does not load maximally to the patient with GBS, the patient can exercise himself actively for the maximal tolerance. It is the reason of fast recovery. In summary, the hydrotherapy is important to the patient with GBS and it must apply to these patients more frequently.
