폐암 환자에 대한 부가적인 침치료 효과 - 단일군, 전후, 예비임상 연구 - (Effectiveness of Additional Acupuncture Therapy for Lung Cancer Patients - A single group before-and-after, preliminary study -)

  • This study is a single group before-and-after study aimed to evaluate the effectiveness of additional acupuncture therapy for lung cancer patients being treated with chemotherapy. Acupuncture therapy was conducted twice a day at least for a week. The effectiveness outcome measures were the Korean Functional Assessment of Cancer Therapy (FACT-L) and the Korean Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F) at the baseline, discharge and four weeks after discharge, and visual analogue scale(VAS) checked daily from the baseline to discharge. Ten patients were enrolled, one patient had excluded due to acute exacerbation of lung cancer. The data of nine patients showed some improvement in all of FACT-L, FACIT-F and VAS, however, there were no statistical significance. Additional acupuncture therapy for lung cancer patients treated with chemotherapy might be beneficial to improve the quality of life(QoL) and to improve fatigue. Prospective trials with complex traditional Korean medicine interventions are needed to further investigation.

An Increased Proportion of Apoptosis in CD4+ T Lymphocytes Isolated from the Peripheral Blood in Patients with Stable Chronic Obstructive Pulmonary Disease

  • Background: The pathophysiology of chronic obstructive pulmonary disease (COPD) includes inflammation, oxidative stress, an imbalance of proteases and antiproteases and apoptosis which has been focused on lately. Abnormal apoptotic events have been demonstrated in both epithelial and endothelial cells, as well as in inflammatory cells including neutrophils and lymphocytes in the lungs of COPD patients. An increased propensity of activated T lymphocytes to undergo apoptosis has been observed in the peripheral blood of COPD patients. Therefore, the apoptosis of T lymphocytes without activating them was investigated in this study. Methods: Twelve control subjects, 21 stable COPD patients and 15 exacerbated COPD patients were recruited in the study. The T lymphocytes were isolated from the peripheral blood using magnetically activated cell sorting. Apoptosis of the T lymphocytes was assessed with flow cytometry using Annexin V and 7-aminoactinomycin D. Apoptosis of T lymphocytes at 24 hours after the cell culture was measured so that the T lymphocyte apoptosis among the control and the COPD patients could be compared. Results: Stable COPD patients had increased rates of $CD4^+$ T lymphocyte apoptosis at 24 hours after the cell culture, more than the $CD4^+$ T lymphocyte apoptosis which appeared in the control group, while the COPD patients with acute exacerbation had an amplified response of $CD4^+$ T lymphocyte apoptosis as well as of $CD8^+$ T lymphocyte apoptosis at 24 hours after the cell culture. Conclusion: Stable COPD patients have more apoptosis of $CD4^+$ T lymphocytes, which can be associated with the pathophysiology of COPD in stable conditions.

젊은 성숙 알라스칸 말라뮤트에서 특발성 다발성근염 증례 (Idiopathic Polymyositis in a Young Mature Alaskan malamute)

  • 20 개월령의 알라스칸 말라뮤트 견에서 특발성 다발성근염의 임상증상과 병리학적 소견을 서술하였다. 임상 증상은 급성 허약을 동반한 진행성 운동불내성, 근 위축, 후지의 동시적 걸음걸이, 계란 위를 발끝으로 걷듯 짧고 경직된 걸음 등을 보였다. 신체검사와 임상검사에서는 신경계나 골격계 그리고 다른 질병과 관련된 이차적인 근 질환의 증거가 없었다. 그래서 가장 근 위축이 심한 부위에서 병리조직 검사를 위한 근 생검을 실시하였다. 골격근의 병리검사 결과 근 섬유의 괴사와 함께 단핵세포의 침윤이 관찰되어, 특발성 다발성근염으로 진단하였다. 초기치료는 통증경감과 보조치료를 시작하여 프레드니손 2 mg/kg를 경구로 매일 투여하는 면역억제 요법을 시행하였다. 3주 후 환자는 혈액, 혈청학적 검사에서 정상으로 회복될 뿐만 아니라 걸음걸이, 식욕, 운동의 향상을 나타냈다.

The Long-term Efficacy of Domiciliary Noninvasive Positive-Pressure Ventilation in Chronic Obstructive Pulmonary Disease: A Meta-Analysis of Randomized Controlled Trials

  • Background: We evaluated the long-term effects of domiciliary noninvasive positive-pressure ventilation (NIPPV) used to treat patients with chronic obstructive pulmonary disease (COPD). Methods: Databases were searched to identify randomized controlled trials of COPD with NIPPV for longer than 1 year. Mortality rates were the primary outcome in this meta-analysis. The eight trials included in this study comprised data from 913 patients. Results: The mortality rates for the NIPPV and control groups were 29% (118/414) and 36% (151/419), suggesting a statistically significant difference (risk ratio [RR], 0.79; 95% confidence interval [CI], 0.65-0.95). Mortality rates were reduced with NIPPV in four trials that included stable COPD patients. There was no difference in admission, acute exacerbation and quality of life between the NIPPV and control groups. There was no significant difference in withdrawal rates between the two groups (RR, 0.99; 95% CI, 0.72-1.36; p=0.94). Conclusion: Maintaining long-term nocturnal NIPPV for more than 1 year, especially in patients with stable COPD, decreased the mortality rate, without increasing the withdrawal rate compared with long-term oxygen treatment.

펜타닐 첩포 남용으로 인한 지연성 저산소성 백색질뇌증: 증례 보고 (Delayed Post-Hypoxic Leukoencephalopathy Induced by an Overdose with Fentanyl Patches: A Case Report)

  • 펜타닐 첩포가 의학적으로 이용됨에 따라 간간이 펜타닐 중독 증례가 보고되고 있다. 지연성저산소성 백색질뇌증(delayed post-hypoxic leukoencephalopathy)은 저산소증에 의한 합병증의 하나이다. 이로 인해 급성 중독으로 인한 혼수상태에서 완전히 회복하더라도, 뒤늦게 신경병적 증상이 발생하고, 이후 백색질뇌증으로 진행하기도 한다. 여기 10개의 펜타닐 첩포를 한 번에 사용하고 13시간 동안 깨어나지 못하여 본원으로 내원한 65세 환자의 증례를 소개하였다. 첫 내원 당시 촬영한 CT 소견은 정상이었으나, 20일 뒤 지연성 신경병적 증상이 발생하여 촬영한 MRI 상 양측 대뇌 백색질에 대칭적인 불균일하고 융합하는 고신호 강도 병변이 T2와 FLAIR 영상에서 관찰되었으며 이 병변은 확산 제한을 보였다. 이후 환자의 신경병적 증상은 빠르게 악화되었고 1년 후 심한 전반적 뇌 위축으로 진행하였다.

Obesity Exacerbates Coxsackievirus Infection via Lipid-Induced Mitochondrial Reactive Oxygen Species Generation

  • Coxsackievirus B3 (CVB3) infection causes acute pancreatitis and myocarditis. However, its pathophysiological mechanism is unclear. Here, we investigated how lipid metabolism is associated with exacerbation of CVB3 pathology using high-fat diet (HFD)-induced obese mice. Mice were intraperitoneally inoculated with 1×106 pfu/mouse of CVB3 after being fed a control or HFD to induce obesity. Mice were treated with mitoquinone (MitoQ) to reduce the level of mitochondrial ROS (mtROS). In obese mice, lipotoxicity of white adipose tissue-induced inflammation caused increased replication of CVB3 and mortality. The coxsackievirus adenovirus receptor increased under obese conditions, facilitating CVB3 replication in vitro. However, lipid-treated cells with receptor-specific inhibitors did not reduce CVB3 replication. In addition, lipid treatment increased mitochondria-derived vesicle formation and the number of multivesicular bodies. Alternatively, we found that inhibition of lipid-induced mtROS decreased viral replication. Notably, HFD-fed mice were more susceptible to CVB3-induced mortality in association with increased levels of CVB3 replication in adipose tissue, which was ameliorated by administration of the mtROS inhibitor, MitoQ. These results suggest that mtROS inhibitors can be used as potential treatments for CVB3 infection.

REGULATORY EFFECT OF DEXAMETHASONE ON TRACHEAL CALCIUM PROCESSING PROTEINS AND MUCOSAL SECRETION

  • Dexamethasone inhibits mucin secretion considering the primary option for treating acute asthma exacerbation. However, the mechanism underlying dexamethasone-induced decreased in mucosecretion is unclear. Recent studies have reported that dexamethasone exerts an inhibitory effect on mucosecretion in the lung by modulating the expression of calcium processing genes. However, the expression of the calcium processing genes in the trachea is not examined yet. Thus, the present study is the first to report the localization of calcium processing proteins such as transient receptor potential vanilloid-4 (Trpv4), transient receptor potential vanilloid-6 (Trpv6), calbindin-D9k (CaBP-9k) and plasma membrane Ca2+-ATPase 1 (Pmca1) in the mouse trachea and their glucocorticoid-induced response. In this study, mice were subcutaneously injected with dexamethasone for 5 days, and their tracheal samples were collected by dividing the trachea into the cervical, and thoracic sections based on its anatomical structure. The localization of TRPV4, TRPV6, CaBP-9k, and PMCA1 proteins was detected in the tracheal epithelium, submucosal glands, cartilages and muscles. Dexamethasone treatment downregulated the mRNA expression of the four calcium processing genes and mucin producing genes. The dexamethasone-induced decrease in the secretion of mucosubstances in the trachea was determined by performing Alcian blue-periodic acid-Schiff staining. Thus, the findings of the present study suggest that glucocorticoids simultaneously can regulate the expression of calcium processing genes and tracheal mucosecretion.

만성 폐쇄성 폐질환 급성 악화 시 C-반응단백과 폐동맥 고혈압의 관계 (Elevated C-reactive Protein Levels are a Sign of Pulmonary Arterial Hypertension in AECOPD)

  • 연구배경: COPD 환자에서 혈청 C-반응단백은 증가하는 것으로 알려져 있으며 이러한 변화는 급성 악화 시 보다 두드러진다. 폐동맥 고혈압은 COPD의 흔한 합병증 중 하나이며, C-반응단백은 전신적 심혈관계 질환 발생 위험과 밀접한 관련이 있다고 알려져 왔다. 하지만, COPD에서 이차적으로 발생하는 폐동맥 고혈압에 대한 C-반응단백의 영향에 대해서는 연구가 미비한 상태이다. 방법: 본 연구는 AECOPD에 대해 입원 치료를 시작한 72명의 환자를 대상으로 전향적으로 연구하였다. 환자들은 AECOPD에 대한 즉각적인 치료를 받았고 입원 2일 또는 3일째 실내 환기 하에서 혈청 C-반응단백, 동맥혈 산소 분압, 폐동맥 고혈압에 대한 이환 여부 등에 대한 검사를 시행 받았다. 결과: 폐동맥 고혈압에 이환된 환자는 47명으로 전체 환자 중 65.3%에 달하였다. COPD의 중증도가 심할수록 폐동맥 고혈압의 이환율과 C-반응단백 평균치가 증가하였고, C-반응단백 평균치가 증가할수록 평균 우심실 수축압 역시 증가하는 것을 관찰할 수 있었다. 폐동맥 고혈압 환자군과 비환자군에서 C-반응단백은 각각 $37.6{\pm}7.4mg/L$$19.9{\pm}6.6mg/L$ 통계적으로 의미 있게 폐동맥고혈압 환자군에서 높았지만, 동맥혈 산소분압은 양 군간 의미 있는 차이를 보이지 않았다($77.8{\pm}3.6mmHg$ vs. $87.2{\pm}6.0mmHg$). 결론: 본 연구는 COPD의 급성 악화 시 증가된 C-반응 단백은 폐동맥 고혈압의 이환 여부와 밀접한 관련이 있는 것을 보여 주고 있으며, 이는 COPD의 예후에 심혈관계 질환의 이환 여부가 중요하다는 점을 감안할 때 C-반응단백의 COPD에 대한 독립적 예후인자로서의 가능성을 시사해 준다.

소아에서 13종 호흡기 바이러스에 의한 급성 하기도 감염의 임상 양상 (Clinical characteristics of acute lower respiratory tract infections due to 13 respiratory viruses detected by multiplex PCR in children)

  • 목 적 : 급성 하기도 감염으로 입원하는 소아에서 최근 알려진 hRV C 및 hBoV를 포함하여 13종 호흡기 바이러스의 임상 양상 및 역학을 알아보고자 하였다. 방 법 : 2008년 5월부터 2009년 4월까지 충북대학교병원 소아과에 급성 하기도 감염으로 입원한 소아 중 325명을 대상으로 비인두 흡인물에서 multiplex RT-PCR법을 이용하여 총 13종의 바이러스를 검출하였으며, 대상 소아의 의무기록을 검토하였다. 결 과 : 대상 소아 중 270례(83.1%)에서 호흡기 바이러스가 검출되었으며, 혼합 감염은 71례(26.3%)에서 관찰되었다. 바이러스 검출 빈도는 RSV 108례(33.2%), hRV 62례(19.1%), Flu A 55례(16.9%), hMPV 50례(15.4%), PIV 27례(8.3%), hBoV 26례(8.0%), ADV 19례(5.8%), hCoV 7례(2.2%)였다. 임상진단은 세기관지염 37.5%, 폐렴 34.5%, 급성 천식 악화 20.9%, 크룹 7.1%이었으며, 세기관지염과 폐렴으로 진단된 소아에서 가장 높은 빈도로 검출된 호흡기 바이러스는 RSV, hRV, hMPV, Flu A였다. Flu A와 hRV는 3세 이상의 천식 악화로 진단된 소아에서 가장 높은 빈도로 검출되었다. hRV A와 hRV C는 각각 48명(14.8%)와 14명(4.3%)에서 검출되었으며, hRV C가 검출된 소아의 평균 연령은 $4.1{\pm}3.5$세로 hRV A가 검출된 소아에서의 $1.7{\pm}2.3$세에 비해 유의하게 높았다(P =0.009). hBoV는 세기관지염 또는 폐렴으로 진단된 소아에서 주로 검출되었고, 이들의 평균 연령은 $2.3{\pm}3.4$세였다. 결 론 : 본 연구에서는 한국 소아에서 13종의 바이러스에 의한 하기도 감염의 양상을 관찰하였다. 소아 하기도 감염에서 새로 알려진 바이러스들의 역할을 명확히 알기 위해서는 향후 지속적인 연구가 필요하다.

A Multicenter Study to Identify the Respiratory Pathogens Associated with Exacerbation of Chronic Obstructive Pulmonary Disease in Korea

  • Background: Although respiratory tract infection is one of the most important factors triggering acute exacerbation of chronic obstructive pulmonary disease (AE-COPD), limited data are available to suggest an epidemiologic pattern of microbiology in South Korea. Methods: A multicenter observational study was conducted between January 2015 and December 2018 across 28 hospitals in South Korea. Adult patients with moderate-to-severe acute exacerbations of COPD were eligible to participate in the present study. The participants underwent all conventional tests to identify etiology of microbial pathogenesis. The primary outcome was the percentage of different microbiological pathogens causing AE-COPD. A comparative microbiological analysis of the patients with overlapping asthma-COPD (ACO) and pure COPD was performed. Results: We included 1,186 patients with AE-COPD. Patients with pure COPD constituted 87.9% and those with ACO accounted for 12.1%. Nearly half of the patients used an inhaled corticosteroid-containing regimen and one-fifth used systemic corticosteroids. Respiratory pathogens were found in 55.3% of all such patients. Bacteria and viruses were detected in 33% and 33.2%, respectively. Bacterial and viral coinfections were found in 10.9%. The most frequently detected bacteria were Pseudomonas aeruginosa (9.8%), and the most frequently detected virus was influenza A (10.4%). Multiple bacterial infections were more likely to appear in ACO than in pure COPD (8.3% vs. 3.6%, p=0.016). Conclusion: Distinct microbiological patterns were identified in patients with moderate-to-severe AE-COPD in South Korea. These findings may improve evidence-based management of patients with AE-COPD and represent the basis for further studies investigating infectious pathogens in patients with COPD.
