EFFECTS OF WAVE-PARTICLE INTERACTIONS ON DIFFUSIVE SHOCK ACCELERATION AT SUPERNOVA REMNANTS

  • Nonthermal radiation from supernova remnants (SNRs) provides observational evidence and constraints on the diffusive shock acceleration (DSA) hypothesis for the origins of Galactic cosmic rays (CRs). Recently it has been recognized that a variety of plasma wave-particle interactions operate at astrophysical shocks and the detailed outcomes of DSA are governed by their complex and nonlinear interrelationships. Here we calculate the energy spectra of CR protons and electrons accelerated at Type Ia SNRs, using time-dependent, DSA simulations with phenomenological models for magnetic field amplification due to CR streaming instabilities, Alf$\acute{e}$enic drift, and free escape boundary. We show that, if scattering centers drift with the Alf$\acute{e}$en speed in the amplified magnetic fields, the CR energy spectrum is steepened and the acceleration efficiency is significantly reduced at strong CR modified SNR shocks. Even with fast Afv$\acute{e}$nic drift, DSA can still be efficient enough to develop a substantial shock precursor due to CR pressure feedback and convert about 20-30% of the SN explosion energy into CRs. Since the high energy end of the CR proton spectrum is composed of the particles that are injected in the early stages, in order to predict nonthermal emissions, especially in X-ray and ${\gamma}-ray$ bands, it is important to follow the time dependent evolution of the shock dynamics, CR injection process, magnetic field amplification, and particle escape. Thus it is crucial to understand the details of these plasma interactions associated with collisionless shocks in successful modeling of nonlinear DSA.

Fyn Kinase: A Potential Therapeutic Target in Acute Kidney Injury

  • Acute kidney injury (AKI) is a common disease with a complex pathophysiology which significantly contributes to the development of chronic kidney disease and end stage kidney failure. Preventing AKI can consequently reduce mortality, morbidity, and healthcare burden. However, there are no effective drugs in use for either prevention or treatment of AKI. Developing therapeutic agents with pleiotropic effects covering multiple pathophysiological pathways are likely to be more effective in attenuating AKI. Fyn, a non-receptor tyrosine kinase, has been acknowledged to integrate multiple injurious stimuli in the kidney. Limited studies have shown increased Fyn transcription level and activation under experimental AKI. Activated Fyn kinase propagates various downstream signaling pathways associated to the progression of AKI, such as oxidative stress, inflammation, endoplasmic reticulum stress, as well as autophagy dysfunction. The versatility of Fyn kinase in mediating various pathophysiological pathways suggests that its inhibition can be a potential strategy in attenuating AKI.

  • This study was performed to examine the clinical discussion records where Lee Gong-yoon, participated in Royal treatment at Joseon Dynasty, was involved. Lee Gong-yoon who scouted from the outside had participated in the medical center of the Royal Family for several years as a royal doctor. But he preferred aggressive prescriptions and behaved indiscreet. Eventually, he was exiled when Kyung-Jong who was the 20th king of the Joseon Dynasty died. He was harshly criticized until the year crown prince ascended the throne following Kyung-Jong. But some officials started to defend him since March of the next year. They recognized Lee Gong-yoon's aggressive prescriptions and indiscreet behavior. But mentioned that some very aggressive prescriptions didn't come from his own idea. After they proved their insistence true, his place of exile was changed from island to outland. Different from other doctors who focused on patients' 元气 ($yu{\acute{a}}nq{\grave{i}}$), Lee Gong-yoon focused on removing patients' symptoms when he decided to cure a disease. And also he had considerable insight of "醫學入門 ($y{\bar{i}}xu{\acute{e}}r{\grave{u}}m{\acute{e}}n$)". Despite of his high degree of medical knowledge, unconventional character and behavior made the end of his life unfortunate.

Effects of Low intensity Cardiac Rehabilitation Exercise on Weight and Histological Changes of Rat Models with Acute Myocardial Infarction

  • The purpose of the study was to investigate effects of Low intensity cardiac rehabilitation, using a treadmill, on the myocardial structure. We identified the effects by analyzing changes in the rats' weights and the results of biopsies. Twenty Sprague-Dawley male rats, 50 weeks old, were randomly divided into the exercise group and the control group. myocardial infarction(MI) was induced by ligaturing their left anterior descending artery. After the acute MI induction, two rats of each group began to fall dead, therefore, eight of each group completed at the end of the experiment. We used treadmills for animals for the exercise group. This exercise group performed 30 minutes of exercise five times per week for six weeks, while the control group did not perform any exercise. No statistically significant differences in weight were found in within group comparison and between group comparison. Furthermore, we observed histological changes in the myocardium using Hematoxylin & Eosin and Masson's trichrome staining in both groups. Low-intensity exercise inhibited myocardial fibrosis, may serve as a reference in the cardiopulmonary field, which plays a role in rehabilitating patients with cardiac disorders, including acute MI.

  • Objectives : The purpose of this study is to evaluate the affected part acupuncture moving treatment for acute radial nerve palsy patient who has Cervical Herniation. Methods : We examined 1 patient with Radial nerve palsy who admitted Jaseng Hospital of Oriental Medicine. The patient was treated by the affected part acupuncture moving treatment including herbal mixture. Results : In this study, in the end of affected part acupuncture moving treatment, patient's condition has improved. Muscle strength was also increased. Conclusions : The affected part acupuncture moving treatment for acute radial nerve palsy was effective in restore of functional activities, but this treatment needs more study.

  • Periodic lateralized epileptiform discharges (PLEDs) had been debated whether it is ictal or non-ictal phenomenon. As most of PLEDs occur in patients with acute structural lesions, some epileptologists prefer PLEDS as a non-ictal phenomenon, rather an obscure epiphenomenon of etiological diseases. But, almost half of the patients with PLEDs do not have acute structural lesions in the brain and metabolic disorders or old CNS lesions may cause PLEDs and even more, no brain lesion was identified in some patients. There are many data supporting PLEDs as ictal phenomena. Occurrence of PLEDs usually accompanied by decreased mentality and is improved as PLEDs disappeared. Current SPECT study showed marked hyperperfusion in the lesion side of PLEDs, that is striking evidence of PLEDs as ictal phenomena. Also careful review of EEG with PLEDs revealed it is a dynamic process rather than a static state. Despite of these evidences, as PLEDs are an end-stage of animal status epilepticus models, it may be a transition of ictal to interictal state.

  • We present a case of a colonic involvement associated with necrotizing pancreatitis, with a review of the literature. A 10 year old boy had an appendectomy at the local clinic ten days ago. On admission, he complained nausea, vomiting and severe constipation. His abdomen was distended and he had tenderness on the left abdomen. Laboratory and radiologic studies revealed findings consistent with acute pancreatitis with colonic complication. He was treated conservatively for 30 days but did not improve. On hospital 30th day, abdominal pain developed and his vital sign changed. Abdominal CT suggested ischemic change of the transverse colon. At laparotomy, the left colon showed stenosis. The greatly distended transverse colon was resected and a transverse end colostomy was done. He was discharged at postoperative 45th day with improvement and colostomy closure was performed 8 months later.

  • Hypertensive encephalopathy is an acute neurologic syndrome that occurs in association with abrupt and marked elevation of blood pressure and is characterized by headache, vomiting, seizure, visual disturbances and altered mental status. Hypertensive encephalopathy is most commonly associated with renal disease in children, including acute glomerulonephritis, reno-vascular hypertension, and end-stage renal disease. Hypertensive encephalopathy associated with nephrolithiasis has not been reported. We have experienced a 10-year-old boy with hypertensive encephalopathy associated with ureteral stone.

  • The protective effect of human urinary trypsin inhibitor (UTI) on acute pancreatitis was studied in beagle model. After pancreatitis was experimentally induced in beagle dogs by infusing trypsin and sodium taurocholate into the accessory pancreatic ducts under laprotomy, effects of UTI were compared to those of aprotinin. Routine physical examination, hematology and blood chemistry values were used as parameters and checked on a periodical basis during the experiment. At the end of experiment, surviving animals were sacrificed and liver, kidney, and pancreas were histologically examined. The results of this study warrant the development of UTI as new therapeutics for acute pancreatitis.

  • In order to investigate the clinical efficiency based on Oriental Medical References of Hyang-Sa-Yuk-Gun-Ja-Tang and Oh-Pe-San, histological studies have been done by using the experimental animal. The following results were obtained. 1. The Hyang-Sa-Yuk-Gun-Ja-Tang showed a significant promoting effect of repair on the epithelial layer from the basement membrane of duodenal ulcer in rats. 2. The Oh-Pe-San showed efficiencies about repair of the necrosis in epithelial layer on the acute gastritis in rats. According to above results, it is concluded that Oh-Pe-San and Hyang-Sa-Yuk-Gun-Ja-Tang were recognized to be effective on the acute gastritis and the duodenal ulcer in rats, and this study may suggest a clinical guidance to the prescriptions for the treatment of gastritis end gastric ulcer.
