Treatment of Latent Tuberculosis Infection and Its Clinical Efficacy

  • The role of the treatment for latent tuberculosis infection (LTBI) has been underscored in the intermediate tuberculosis (TB) burden countries like South Korea. LTBI treatment is recommended only for patients at risk for progression to active TB-those with frequent exposure to active TB cases, and those with clinical risk factors (e.g., immunocompromised patients). Recently revised National Institute for Health and Care Excellence (NICE) guideline recommended that close contacts of individuals with active pulmonary or laryngeal TB, aged between 18 and 65 years, should undergo LTBI treatment. Various regimens for LTBI treatment were recommended in NICE, World Health Organization (WHO), and Centers for Disease Control and Prevention guidelines, and superiority of one recommended regimen over another was not yet established. Traditional 6 to 9 months of isoniazid (6H or 9H) regimen has an advantage of the most abundant evidence for clinical efficacy-60%-90% of estimated protective effect. However, 6H or 9H regimen is related with hepatotoxicity and low compliance. Four months of rifampin regimen is characterized by less hepatotoxicity and better compliance than 9H, but has few evidence of clinical efficacy. Three months of isoniazid plus rifampin was proved equivalence with 6H or 9H regimen in terms of efficacy and safety, which was recommended in NICE and WHO guidelines. The clinical efficacy of isoniazid plus rifapentine once-weekly regimen for 3 months was demonstrated recently, which is not yet introduced into South Korea.

자문의뢰된 입원환자의 특성과 정신과 치료 순응도에 대한 연구 (A Studyof Psychiatric Treatment Compliance in Referred Patients at a General Hospital)

  • 연구목적: 종합병원에서 정신과에 자문 의뢰된 입원 환자 및 그 자문의뢰의 특성을 살펴보고 이를 통해 정신과 치료의 순응도에 관련된 요인을 찾아보고자 하였다. 방 법: 2009년 9월 1일부터 2010년 8월 31일까지 고려대학교 안산병원에서 정신과에 자문 의뢰된 입원환자 333명을 대상으로 자문기록지와 의무기록을 참고하여 연구를 진행하였다. 기본적인 인구 역학적 정보, 자문을 의뢰한 임상 각과, 의뢰 요청자, 의뢰된 사유, 정신과적 진단과 과거력, 입원 중 정신과 치료의 시행 여부 및 추후 통원 치료의 여부 등을 조사하였다. 이들 중 정신과 치료의 시행 여부 및 통원 치료의 여부를 치료순응도로 정하였다. 순응도와 기타 변수간의 이분형로지스틱 회귀분석을 시행하여 순응도에 영향을 미치는 요인을 확인하였다. 결 과: 입원 중 정신과 치료를 권유 받은 310명의 환자들 중, 치료에 순응한 환자는 82.9%이었으며, 외래 통원치료를 권유 받은 111명의 환자들 중에는 통원치료에 순응한 환자가 55.8%이었다. 64세 이하 연령층보다 65세 이상의 노인에서 입원 중 치료에 대한 순응도가 높았다(OR=4.838, p=0.004). 외래 통원치료를 권유받은 환자들 중 내과적 질환으로 인해 이차적인 정신과적 증상이 나타난 경우가 외래 순응도가 더 높았다(OR=8.520, p=0.008). 결 론: 노인 환자는 입원중 정신과 치료에 대해서는 순응도가 높았으나 신체 질환의경과에 영향을 미치는 섬망 및 기분장애가 흔하므로 정신과적 증상에 대한 평가와 치료가 보다 적극적으로 이루어져야 할 것이다. 또한, 내외과적 질환과 관련이 없는 정신과 문제로 자문 의뢰된 환자들은 외래 통원치료 순응도가 낮으므로 치료순응도를 높이기 위해 다방면의 치료적 접근 방법을 모색하여야 할 것으로 판단된다.

전략물자 수출통제 자율준수제도(CP)에 관한 한.일 비교 연구 (A Comparative Study on the Compliance Program(CP) of Strategic Export Control System between Korea and Japan)

  • 수출자에게 이중용도 품목의 최종용도와 수요자 확인의무를 부여하는 상황허가(Catch-all) 제도가 본격 시행되면서 대량살상무기 (WMD) 확산 방지 등을 위한 기업의 전략물자 수출통제 자율준수제도(Compliance, Program: CP)에 대한 관심이 높아지고 있다. 일본은 아시아 최고의 전략물자 수출통제 국가로서 현재 1,600개 이상의 기업이 자율준수무역거래자로 지정되어 있다. 반면 한국은 2005년 자율준수무역거래자 제도가 도입되었음에도 불구하고 2011년 10월말 현재 148개 기업만이 자율준수무역거래자로 지정되어 있다. 본 논문은 한국과 일본의 전략물자 수출통제 자율준수제도의 발전과정, 주요 내용 및 현황에 대한 비교 분석을 통해 한국의 자율준수제도 개선방안을 다음과 같이 제시한다. 첫째, 전략물자 수출기업의 자율준수제도에 대한 긍정적인 인식 전환 노력이 시급하다. 둘째, 자율준수제도에 대한 신속한 정보 제공과 함께 홍보 및 교육이 강화되어 야 한다. 셋째, 자율준수제도 관련 정부 기업 간 커뮤니케이션이 활성화되어야 한다. 넷째, 자율준수제도의 발전 및 정착을 위한 한 일간 다각적인 협력방안의 모색이 필요하다.

넙다리뒤근 단축 증후군이 있는 젊은 여성에서 오금각과 넙다리뒤근 순응성에 대한 신경가동화기법과 정적신장운동의 즉각적인 효과 비교 (Comparison of the Immediate Effects of the Neural Mobilization Technique and Static Stretching Exercise on Popliteal Angle and Hamstring Compliance in Young Women With Short Hamstring Syndrome)

  • Background: Limitation of hamstring extensibility is often associated with various musculoskeletal problems such as alterations in posture and walking patterns. Thus, certain appropriate strategies need to be established for its management. Objects: The aim of this study was to compare the effects of the neural mobilization technique and static stretching exercises on popliteal angle and hamstring compliance in young women with short hamstring syndrome (SHS). Methods: Thirty-three women with SHS were randomly assigned to either group-1 ($n_1=17$) that underwent the neural mobilization technique or group-2 ($n_2=16$) that underwent the static stretching exercises. Outcome measures included the active popliteal angle (APA) and a hamstring's electromyographic (EMG) activity at a maximum popliteal angle of the baseline. Intervention for each group was performed for a total time of 3-min (6 sets of a 30-sec application). Results: There were significant interactions between time and group in the APA [group-1 (pre-test to post-test): $69.70{\pm}8.14^{\circ}$ to $74.14{\pm}8.07^{\circ}$ and group-2: $68.66{\pm}7.42^{\circ}$ to $70.52{\pm}7.92^{\circ}$] (F1,31=6.678, p=.015) and the EMG activity of the hamstring (group-1: $1.12{\pm}.30{\mu}N$ to $.69{\pm}.31{\mu}V$ and group-2: $1.19{\pm}.49{\mu}V$ to $1.13{\pm}.47{\mu}V$)(F1,31=6.678, p=.015). Between-group comparison revealed that the EMG activity of the hamstring was significantly different at post-test between the groups (p<.05). Furthermore, in within-group comparison, group-1 appeared to be significantly different for both variables between pre- and post-test (p<.05); however, group-2 showed significant difference in only the APA between pre- and post-test (p<.05). Conclusion: These findings suggest that the neural mobilization technique and static stretching exercises may be advantageous to improve hamstring compliance in young women with SHS, resulting in a more favorable outcome in the neural mobilization technique.

  • In this paper, a direct drive scalar robot manipulator is constructed and its mechanical machanism for operation is explained. Also, a motion controller board for the direct drive robot manipulator was developed where the IBM 486 computer is the main controller. For the developed direct drive robot, a force/motion control algorithm based on an active compliance scheme is developed. A preliminary experiment using the developed direct drive for a peg-in-hole job was done by implementing the control algorithm.

  • In this paper, we presented an active RFID system in 2.45GHz range including LNA, Mixer and gain block. And in this work, a link budget model for RFID applications are proposed. We describe the detailed design and implementation of our system. Our components in RFID system has features such as low Noise Figure, reliable energy budget, and standard compliance with ISO 18000-4. Our receiver is effective for development and evaluation of prototype applications because of the flexibility of the design hardware. So, our platform will be suitable for versatile item management applications.

Topology optimization of variable thickness Reissner-Mindlin plate using multiple in-plane bi-directional functionally graded materials

  • This paper introduces a novel approach to multi-material topology optimization (MTO) targeting in-plane bi-directional functionally graded (IBFG) non-uniform thickness Reissner-Mindlin plates, employing an alternative active phase approach. The mathematical formulation integrates a first shear deformation theory (FSDT) to address compliance minimization as the objective function. Through an alternating active-phase algorithm in conjunction with the block Gauss-Seidel method, the study transforms a multi-phase topology optimization challenge with multi-volume fraction constraints into multiple binary phase sub-problems, each with a single volume fraction constraint. The investigation focuses on IBFG materials that incorporate adequate local bulk and shear moduli to enhance the precision of material interactions. Furthermore, the well-established mixed interpolation of tensorial components 4-node elements (MITC4) is harnessed to tackle shear-locking issues inherent in thin plate models. The study meticulously presents detailed mathematical formulations for IBFG plates in the MTO framework, underscored by numerous numerical examples demonstrating the method's efficiency and reliability.

국내 웹 개발자의 웹표준 준수 및 전문성 수준에 대한 연구 (Research on Web standards compliance and professional level of domestic Web Developer)

  • 최근 인터넷환경에서 Active-X 등 비표준 기술은 인터넷 비즈니스의 장애 요인이 되고 있다. 따라서 인터넷 이용환경 개선이 정책적으로 요구된다. HTML5 기반 융합 기술은 특정 플랫폼이나 디바이스에 종속되지 않고 사용자, 디바이스, 네트워크 및 응용환경을 지원하기 위한 웹 표준으로 다양한 인증기술, 보안 및 웹 앱 게임 등 인터넷과 기존 산업이 융합된 형태의 비즈니스 서비스 모델과 시장을 창출할 수 있는 웹 플랫폼으로 주목받고 있다. 본 논문에서는 국내 웹 개발자들의 웹표준 준수 및 웹개발 전문성 수준에 대한 설문조사를 수행하여 그 결과를 분석하였다. 분석된 결과를 기반으로 정부의 소프트웨어 정책방향 및 소프트웨어 인력 자원 양성 정책 수립의 기본 방향을 제시한다. 이는 웹 산업 경쟁력 강화, 디지털 융복합 산업의 지원 정책 수립, 글로벌 웹 표준(HTML5) 확산을 위한 기술 지원 강화 및 전문 인력 양성 프로그램 구축에 기여할 수 있을 것이다.

이족 보행 로봇을 위한 빠르고 안전한 접촉 생성 전략 (Fast and Safe Contact Establishment Strategy for Biped Walking Robot)

  • One of the most challenging issues when robots interact with the environment is to establish contact quickly and avoid high impact force at the same time. The proposed method implements the passive suspension system using the redundancy of the torque-controlled robot. Instead of utilizing the actual mechanical compliance, the distal joints near the end-effector are controlled to act as a virtual spring-damper system with low feedback gains. The proximal joints are precisely controlled to push the mid-link, which is defined as the boundary link between the proximal and distal joints, towards the environment with high feedback gains. Compared to the active compliance methods, the contact force measurements or estimates are not required for contact establishment and the control time delay problems do not occur correspondingly. The proposed method was applied to the landing foot control of the 12-DoF biped robot DYROS-RED in the simulations. In the results, the impact force during landing was significantly reduced at the same collision speed.

Leading for Safety: A Question of Leadership Focus

  • Background: There is considerable evidence that leadership influences workplace safety, but less is known about the relative importance of different leadership styles for safety. In addition, a leadership style characterized by an emphasis and a focus on promoting safety has rarely been investigated alongside other more general leadership styles. Methods: Data were collected through a survey to which 269 employees in a paper mill company responded. A regression analysis was conducted to examine the relative roles of transformational, transactional (management-by-exception active; MBEA), and safety-specific leadership for different safety behavioral outcomes (compliance behavior and safety initiative behaviors) and for minor and major injuries. Results: A safety-specific leadership contributed the most to the enhanced safety of the three different kinds of leadership. Transformational leadership did not contribute to any safety outcome over and above that of a safety-specific leadership, whereas a transactional leadership (MBEA) was associated with negative safety outcomes (fewer safety initiatives and increased minor injuries). Conclusion: The most important thing for leaders aiming at improving workplace safety is to continuously emphasize safety, both in their communication and by acting as role models. This highlights the importance for leadership training programs aiming to improve safety to actually focus on safety-promoting communication and behaviors rather than general leadership. Furthermore, an overly monitoring and controlling leadership style can be detrimental to attempts at achieving improved workplace safety.
