Effects of Exposure Dose Reduction by Optimization of Automatic Exposure Control Factors in Digital Radiographic Examination of Paranasal Sinus (부비동 디지털 엑스선검사에서 자동노출제어 조절인자의 최적화를 통한 조사선량 감소 효과)

  • The purpose of this study was to reduce dose while maintaining image quality during digital radiographic examination of paranasal sinus by using the automatic exposure control (AEC) system. The tube voltage was set as six stages that increased by about 10 kVp to 70 kVp, 81 kVp, 90 kVp, 102 kVp, 109 kVp and 117 kVp. And then the AEC system conditions were consisted of 9 setting environments, that change mode of the sensitivity (S200, S400, S800) and the density (+2.5, 0, -2.5). We measured automatically exposed tube current (mAs) under 54 conditions with combined these, and assessed SNR and PSNR through the acquired images. In addition, four radiologists performed a qualitative assessment of the acquired images for each combination on a five-point scale of the Likert. As a result, the lowest dose and the highest values of SNR and PSNR in images with a qualitative assessment more than 4 point were the AEC control factors of 90 kVp, S800, D2.5. We applied this condition to the clinical trial, it showed an effect of 83.1% reduction in exposure radiation dose (mR). Therefore, AEC system could be used as dose reduction technology if it understood and used related regulatory factors and physical characteristics.

  • The doctor blade is equipped in a toner cartridge and is a device to maintain the uniform thickness of a toner by controlling the pressure on the developing roller. The contact pressure between the developing roller and the doctor blade is one of the significant factors for image quality and durability of toner cartridge. The purpose of this study is to develop approximation model in order to minimize the time and cost which are needed much required in making optimal design of the doctor blade. Central composite design was used for the design of experiment and response surface design was used for approximation. The data for contact pressure were acquired through finite element analysis and data of image density and toner weight were acquired through experiment. The approximation model developed in this study has presented very high fitness.

  • Human immunodeficiency virus is a retrovirus that causes acquired immunodeficiency syndrome. Acquired immunodeficiency syndrome is defined in terms of "either the occurrence of specific diseases in association with a HIV infection or a CD4 cell count below 200cells/ul" by centers for disease control and prevention(CDC). When performing the surgery of human immunodeficiency virus infected patients, several factors should be considered. First, standard precautions should be performed to prevent infection. It is safe to treat human immunodeficiency virus infected patients if we follow the standard precautions. Second, when making a surgical plan, surgeons have to take account of delayed bone healing and postsurgical infection. This case report presents a case of orthognathic surgery of human immunodeficiency virus infected patient.

  • The calyceal diverticulum is a cystic cavity lined by a transitional epithelium, is encased within the renal substance, and is situated peripheral to a minor calyx, to which it is connected by a narrow channel. Both congenital and acquired factors have been suggested to explain the formation of a calyceal diverticulum. We experienced a case of a huge calyceal diverticulum that was newly developed after a renal injury.

  • Pneumonia is the cause of significant morbidity and mortality, despite advances in diagnosis and antibacterial treatment. Pneumonia is often misdiagnosed and mistreated up until recently. Recent classification of pneumonia consists of community-acquired pneumonia, health care-associated pneumonia, hospital-acquired pneumonia, and ventilator-associated pneumonia. The etiology, risk factors, and treatment are different among them. This article briefly introduces new concepts and ideas in biomarkers, diagnosis, treatment, prognosis, and prevention of pneumonia during the past 2 years. One of the most frequent subjects of recent papers was those about pandemic H1N1 in 2009.

  • This paper is on the luster property of fertile fabric materials which is one of the important factors related to the development of clothing materials leading to the value-added products. Using several image analysis techniques, the optical properties, especially luster related ones, of the commercially available fabrics were measured and analyzed. The fabric samples included 6 kinds of preliminary experimental fabrics along with 22 kinds of fabric which were commercially available. The images acquired at various observing angles are taken by 44 Points and these Pictures are analyzed into luster through the image analysis program. The preliminary experiment demonstrated the effects of weave types and the number of yarn twist on the luster of the fabrics through the use of the image analysis program.

Current Treatment and Clinical Outcomes of Community Acquired Pneumonia According to Pneumonia Severity Index (Pneumonia Severity Index에 따른 원외획득폐렴 환자의 치료 현황 및 성과)

  • Purpose: There is considerable variability in rates of hospitalization for patients with community-acquired pneumonia (CAP) in part because of physicians' uncertainty in assessing the severity of illness at presentation. The purpose of the study was to examine the current treatment patterns and factors influencing the Pneumonia Severity Index (PSI) and clinical outcomes in the patient with CAP. Method: The retrospective data collection of the patients with CAP was conducted and the data were reviewed. The collected data included demographic, clinical, laboratory and microbiological medical information. All patients were stratified into three risk groups according to PSI: low risk (PSI score I-II), moderate risk (III) and high risk (IV-V) groups. The examined treatment patterns were the appropriateness of admissions, category of antibiotics used. The prognostic factors associated with PSI and clinical outcomes were examined. Results: One hundred and six patients' medical data were reviewed. The overall appropriateness of admissions was low presenting many of patients were admitted or intensely treated in the hospital despite of lower risk of prognosis and treated with intravenous antibiotics instead of oral fluoroquinolones. Primary pneumonia pathogens were Klebsiella pneumoniae (27%) and Streptococcus pneumoniae (21.6%). Mean LOS was 8.5 days and was significantly longer (10.0days) (p<0.001) in high risk group. The patients with age >65 (p<0.001), diabetes mellitus (p<0.001), mental alteration (p<0.001), and/or $PaO_2$ <60 mmHg (p<0.001) had a tendency to have higher PSI. The prognostic factors associated with longer LOS were age >65 years (p=0.008), mental status alteration (p<0.001), dyspnea (p=0.002) and PSI score (p=0.001). The prognostic factors associated with mortality were congestive heart failure (p=0.038), systolic blood pressure <90 mmHg (p=0.002) and arterial pH <7.35 (p=0.013). Conclusion: Most of patients were found to over-utilize medical service according to appropriateness of admissions. The elderly, mentally altered patients with low $PaO_2$ had higher PSI score with increased risk of LOS. The mortality could be increased in the patient with disease state of congestive heart failure, high blood pressure, and/or acidosis.

  • The purpose of study is to find out common factors and variables that affect common factors in level of right knowledge. attitude behavior with respect to AIDS. During the period form Apr.1. 1995 to Jun. 30. guestionnaires, written both in Korean and English. were distributed to Korean Malaysion employees woring at a certain Korean Video companies. and results of replies. given by 80 workers by country. were used as the research matrials. The gathered marials were analyzed through the SPSS package t-test. ANOVA. factor analysis. and the following results were obtained. The 4 extracted common factors could be named 'general publicity', 'transmissive knowledge', 'diagnosis. prevention'. and 'limitted publicity'. The means of trasimssive knowledge. diagnosos. prevention. limtted publicity factors showed higher means in groups who had beyond university level of education than in showed higher means in groups who beyond university level of education than in groups who had below high school level of education. There was a significant relationship between general publicity factor and econmic state. and between limitted publicity factor and preventive education

Study on a Difference of Health Behavior and Health Promotion between Korean American and Korean (한국인과 재미동포간의 건강증진행태의 차이에 관한 연구)

  • Objectives: The purpose of this study is to analyze how the different lifestyles between Koreans and Korean Americans have significant effects on their health. Despite being the same race, Koreans and Korean Americans have different health conditions and health attitudes due to the acquired environmental factors such as social-economic factors, lifestyle risk factors, healthcare systems, and medical utilization. It is crucial to examine how the different lifestyle habits between Koreans and Korean Americans lead to various health conditions for establishing an effective health promotion policy. Methods: In this study, a comparative analysis was conducted using the National Health and Nutrition Survey of Korea and CHIS data of the United States in 2005 and 2015 to provide valuable insights when establishing such a policy. Results: The specific research purpose is as follows: First, socioeconomic factors, such as (1)living habits risk factors, (2)health satisfaction levels, (3)disease outbreaks, and (4)medical uses, are analyzed to find the distinct characteristics among Koreans, Korean Americans, and Americans. Second, the three groups --Koreans, Korean Americans, and Americans-- were compared based on their exposure to disease-related lifestyle risk factors related to their body mass index and their general health condition. The research results are as follows: First, all three groups improved health conditions in 2015 better than in 2005. Koreans maintained relatively higher general health conditions compared to other groups: their prevalence rate of chronic diseases such as diabetes, high blood pressure, heart disease, and asthma was lower than that of U.S. residents. Second, in regards to health behavior factors, the lifetime smoking experience for Koreans and Americans both decreased in 2015 compared to 2005, while the lifetime smoking experience for Korean Americans increased slightly. The number of smokers for Koreans has greatly decreased over a decade while that of Americans has moderately increased. Third, according to the results of the multiple regression, the general health conditions, which is a dependent variable, suggests that the number of men who answered they are healthy is greater than that of women in Korea, compared to the United States. Conclusions: In conclusion, the acquired environmental factors had more significant impacts on health than the racial factors did. Compared to 2005, the health behaviors and health levels of Korean Americans in 2015 gradually became more similar to those of Americans.

Community-Acquired Necrotizing Pneumonia Caused by ST72-SCCmec Type IV-Methicillin-Resistant Staphylococcus aureus in Korea

  • Methcillin-resistant Staphylococcus aureus (MRSA) has emerged as an important cause of community-acquired infections, which has been recently designated as community-associated (CA) MRSA. Panton-Valentine leukocidin (PVL)-negative multilocus sequence type 72 (ST72)-staphylococcal cassette chromosome mec (SCCmec) type IV has been reported as the predominat CA-MRSA strain in Korea and is commonly associated with skin and soft tissue infections in addition to healthcare-associated pneumonia. However, community-acquired pneumonia (CAP) for this strain has not yet been reported. We hereby report two cases of CAP caused by PVL-negative ST72-SCCmec type IV strain in patients who had no risk factors for MRSA acquisition. While CA-MRSA infections are not yet prevalent in Korea, our cases suggest that CA-MRSA should be considered in cases of severe CAP, especially for cases associated with necrotizing pneumonia.
