노인 중풍 발생에 미치는 요인 분석 (Analisis of chief influencing factor on aged man's cerebrovascular accidents - case control study -)

  • This study was carried out during 3 years from March 1992 to March 1995, 140 healthy people and 140 C.V.A patients were investigated, at an oriental clinic in Taejon. The data collected was analysed using a SAS. The results are as follows. 1. The Relationships between patients who have undergone one or more abortion procedures and C.V.A occurrence. A statistical significant difference was recorded between the control group and the outpatients group of P<0.05 2. Number of years after initial C.V.A diagnosis before C.V.A symptom onset. within 1 years 52% (2-3 years, 3-4 years, 4-5 years, 5 years and over) 3. 소아에서 골절된 하악과두의 골개조 양상에 관한 임상 및 방사선학적 연구 (A CLINICO-RADIOLOGIC STUDY OF BONY REMODELING OF THE FRACTURED CONDYLES IN CHILDREN)

  • Bony remodeling pattern of condyle fractures in children are different from in adult for growing of condyle, also might affect treatment and prognosis of the condyle fracture. Subjects of this clinical and radiologic study were 26 temporomandibular joints diagnosed as condyle fracture in 23 patients under 15 years old age. They were treated with conservative method at Dental Hospital of Yonsei University from Jan., 1986 to Oct., 1994. Bony remodeling related with fracture pattern was evaluated. The results obtained are as follows: 1. The ratio of male to female in patients with condyle fracture was 1 : 0.9 and the difference of sex ratio was not noted. Comparing with preschool-age group and school-age group, age frequency was higher in preschool-age group(83%). 2. Fallen down(54%) was the most frequent cause of condyle fractures. Traffic accident and slip down were followed. 3. The most common clinical sign of condyle fractures was tenderness to paipation09 cases). Mouth opening limitation07 cases), swelling(7 cases), malocclusion(3 cases) were next in order. 4. According to sites of condyle fractures, unilateral fractures were in 20 patients and bilateral fractures in 3 patients, therefore total 23 patients-26 cases of condyle fracture were observed. According to fracture distribution, condyle fractures were in 10 patients(44%). Condyle fractures with symphysis fracture(9 patients, 39%), condyle fractures with ascending ramus fracture(2 patients, 9%), condyle fracture with mandibular body fracture(1 patient, 4%), and condyle fractures with mandibular angle fracture(1 patient, 4%) were followed. 5. In displacement pattern of fractured fragment of mandibular condyle, dispiacement(17 cases, 66%) was most common. Dislocation(5 cases, 19%) and deviation (4 cases, 15%) were next in order. 6. During the observation period of fractured condyles, remodeling patterns of fracture sites related with articular fossa were observed with usual condylar shape in 23 cases and with prominently different shape in 3 cases.

수리모형을 이용한 선박 항해기기 패널의 인간공학적 배치 (The Ergonomic Layout of Ship's Bridge Panels using the Mathematical Programming)

  • When designing a ship, ergonomic considerations are crucial when minimizing a navigator's fatigue due to the burden of work, and to appropriately operate the navigational equipment for each given situation by helping the operator to understand the surroundings as well as the physical functions of the ship. However, insufficient consideration of ergonomic elements in the actual design of ship Bridges is lowering the performance of safe navigation and allows for the possibility of operation or readout errors. Consequently, these errors lead to an increase in maritime accidents. Therefore, this study conducted a usability evaluation on the importance of and the usage frequency of navigational equipment, their influence on actual navigation, and the possibility of error upon operation or readout between training ship officers, to derive an optimized layout that includes the consideration of ergonomic factors for on-Bridge navigational equipment, which are currently arranged differently according to their type or size. The optimized layout of on-Bridge navigational equipment was carried out based on the evaluation results, using the Lingo program. Through the process of optimization, revised layouts of on-Bridge navigational equipments(control and display device) were suggested, considering emergency situations(ship collision, stranding, fire and explosion, sinking, etc.) during navigation.

가정용 인공호흡기 관련 안전사고 특성 및 손상 영향 요인 분석: 상급종합병원 일반병동 환자 중심으로 (Factor Associated with Injury Related to Home Mechanical Ventilation in General Ward Patients: A Retrospective Study)

  • Purpose: This study aims to describe the characteristics of safety incidents and factors associated with injury for patients with Home Mechanical Ventilation (HMV) at the hospital. Methods: This is a retrospective study. Data were collected from the work log of respiratory home care nurses and the patients' electronic medical records were investigated. In order to compare group differences, independent t-test and χ2 test were used. Associated factors with injury development were identified by generalized mixed modeling analysis controlling for age and gender. Results: A total of 304 patients on HMV were included in this study, among which 129 (42.4%) experienced 352 HMV-related incidents. Mean frequency of incidents for each patient was 5.11±3.98, ranged from 1 to 15 times. In 19.0% of the incidents, injury was developed. Types of incident and persons involved in the incidents were significantly associated with the patient's injury. In the case of the safety incidents, patient's injury was significantly higher in accidents caused by respiratory circuit problems compared to those caused by problems with the ventilator operation by the medical staff (coefficient=1.25, p=.020). In addition, in the case of those involved in the safety incidents, patient's injury was significantly higher in the accident caused by the patient family members or caregivers than that caused by the medical personnel (coefficient=1.25, p=.019). Conclusion: In order to minimize injury caused by incidents in patients with HMV, hospitals need to provide systemic education to their medical staff and caregivers to enhance awareness of the importance of reporting and safety management.

Effectiveness of Direct Safety Regulations on Manufacturers and Users of Industrial Machines: Its Implications on Industrial Safety Policies in Republic of Korea

  • Background: Despite considerable efforts made in recent years, the industrial accident rate and the fatality rate in the Republic of Korea are much higher than those in most developed countries in Europe and North America. Industrial safety policies and safety regulations are also known to be ineffective and inefficient in some cases. Methods: This study focuses on the quantitative evaluation of the effectiveness of direct safety regulations such as safety certification, self-declaration of conformity, and safety inspection of industrial machines in the Republic of Korea. Implications on safety policies to restructure the industrial safety system associated with industrial machines are also explored. Results: Analysis of causes in industrial accidents associated with industrial machines confirms that technical causes need to be resolved to reduce both the frequency and the severity of such industrial accidents. Statistical analysis also confirms that the indirect effects of safety device regulation on users are limited for a variety of reasons. Safety device regulation needs to be shifted to complement safety certification and self-declaration of conformity for more balanced direct regulations on manufacturers and users. An example of cost-benefit analysis on conveyor justifies such a transition. Conclusion: Industrial safety policies and regulations associated with industrial machines must be directed towards eliminating the sources of danger at the stage of danger creation, thereby securing the safe industrial machines. Safety inspection further secures the safety of workers at the stage of danger use. The overall balance between such safety regulations is achieved by proper distribution of industrial machines subject to such regulations and the intensity of each regulation. Rearrangement of industrial machines subject to safety certification and self-declaration of conformity to include more movable industrial machines and other industrial machines with a high level of danger is also suggested.

최근 5년간 국내 연근해에서 발생한 해양사고에 대한 주성분분석 (Principal Component Analysis on Marine Casualties Occurred at Korean Littoral Sea in Recent 5 Years)

  • 본 연구에서는 2010년부터 2014년까지 최근 5년간 우리 나라 주변해역에서 발생하여 중앙해양안전심판원의 재결을 마친 1417건의 해양사고에 대해 이를 25개 요인별로 분류하고, SPSS 통계 프로그램에 의한 주성분분석(Principal Component Analysis; PCA)을 행하여 이들 각 요인들의 상관성 및 주요 해양원인을 분석 고찰하였다. 얻어진 주요한 결과들을 요약하면 다음과 같다. 1. 해양사고의 주된 원인은 기관설비취급불량, 화기취급불량, 항행법규소홀, 침로선정유지불량, 경계소홀 등 기관실 및 조타실 관련 인적요인에 의해 발생한다. 2. 조타실 관련 인적요인에 의해 발생하는 사고는 충돌과 좌초 등이 큰 비중을 차지하며, 기관실 관련 인적요인에 의해 발생하는 사고유형은 주로 기관손상이나 화재폭발 등이다. 3. 주성분분석의 결과 제1주성분은 해양사고의 출현율을, 제2주성분은 해양사고의 원인을, 제3주 성분은 해양사고의 유형을 나타낸다.

해양플랜트 폭발사고 위험도 평가/관리를 위한 실증시험기법에 관한 연구 (A Research on the Verification Test Procedure for Quantitative Explosion Risk Assessment and Management of Offshore Installations)

  • The structural design of offshore installations against explosions has been required to protect vital areas (e.g. control room, worker's area etc.) and minimize the damage from explosion accidents. Because the explosion accident will not only result in significant casualties and economic losses, but also cause serious pollution and damage to surrounding environment and coastal marine ecosystems. Over the past two decades, an incredible efforts was made to develop reliable methods to reduce and manage the explosion risk. Among the methods Quantitative Risk Assessment and Management (QRA&M) is the one of cutting-edge technologies. The explosion risk can be quantitatively assessed by the product of explosion frequency based on probability calculation and consequence analyzed using computer simulations, namely Computational Fluid Dynamics (CFD) and Finite Element Analysis (FEA). However to obtain reliable consequence analysis results by CFD and FEA, uncertainties associate with modeling and simulation are needed to be identified and validated by comparison with experimental data. Therefore, large-scaled explosion test procedure is developed in this study. And developed test procedure can be helpful to obtain precious test data for the validation of consequence analysis using computer simulations, and subsequently allow better assessment and management of explosion risks.

체질량지수와 출혈성 뇌졸중 발생간의 관련성에 대한 환자-대조군연구 (Body Mass Index and Risk of Hemorrhagic Stroke in Korean Adults: Case-control Study)

  • Objectives : To evaluate the association between body mass index (BMI) and hemorrhagic stroke. Methods : A case-control study was conducted on 2,712 persons (904 cases, 904 hospital controls, and 904 community controls) participating in an Acute Brain Bleeding Analysis study from October 2002 to March 2004. Two controls for each case were matched according to age and gender. The information was obtained by trained interviewers using standardized questionnaire. A conditional logistic regression model was used to estimate the association between BMI and the frequency of having a hemorrhagic stroke. Results : Obese men (25.0 $\leq$ BMI < $30.0\;kg/m^2$) had an odds ratios (OR) of 1.39 (95% CI 1.03 to 1.87) a hemorrhagic stroke, compared to men with a normal BMI (18.5 to $24.9\;kg/m^2$). Conversely, women with lower BMI had a higher risk of having hemorrhagic stroke. With respect to subtypes of hemorrahagic stroke, we observed about a three-fold increase in the risk of intracerebral hemorrhage (ICH) in the highly obese group. However, these trends were not significant in patients with subarachnoid hemorrhages. Conclusions : Obesity was identified as one of the risk factors in hemorrhagic stroke, in particular ICH. Conversely, in women, a lean body weight increases the risk of hemorrhagic stroke. Consequently, managing one's weight is essential to reduce the risks of hemorrhagic stroke.

Safety analysis to derive safety requirement in the railway system

  • The safety of the railway system is important because the system is a mass transit system and the results of the accident are inconceivable. The railway system is operated by train operation system such as train control system. So the train control system requires safety critical characteristics. In the European railway, ETCS (European Train Control System) project has been finished to accomplish the interoperability of each national railway signaling system. According to the interoperability degree, ETCS levels are suggested. As the highest level, ETCS level 3 suggests a radio communication. Also recently urban railway system is operated by driverless and automatic train control system. In this circumstance, more safety is required than before in the railway system. In order to accomplish the safety of a system, the requirements considering safety have to be suggested. The requirement is a set of several functions such as general function, environment, safety etc. For the safety critical system, safety function is more important than any other functions. The safety functions are deduced by safety analysis. In order to perform the safety analysis, the system hazards have to be identified and then risk analysis for each hazard should be performed. The risk is related to the frequency and the severity of each hazard. And then countermeasures for each risk have to be prepared. The summary of the countermeasures is about a kind of safety functions in a system. In this paper, the safety functions for a train control system are presented according to the above procedure.

LTE 기반 초고속 해상무선통신(LTE-M) 라우터 시스템 (LTE based High-speed Maritime Wireless Communication(LTE-M) Router System)

  • 해양안전에 정보통신기술을 적용하려는 세계 각국의 많은 노력을 바탕으로 국제해사기구(IMO)에서는 최신 정보통신기술을 해양 서비스에 적용하기 위해 e-Navigation 기준을 제정하였다. 해양수산부에서는 국내 환경을 고려한 한국형 e-Navigation을 추진하고 있으며, 해상에서의 초고속 무선통신 방식으로 LTE 통신 방식을 적용하려 하고 있다. 본 논문에서는 초고속 해상무선통신 서비스를 위해 할당된 700Mhz 대역의 주파수를 지원하는 LTE 기반 선박용 라우터 시스템의 개발에 대해서 기술한다. LTE 기반 선박용 라우터를 이용하여 기지국에서 100km 거리 통신이 가능함을 실험으로 보여준다. LTE 기반 초고속 해상무선통신 라우터 시스템은 해상에서 발생할 수 있는 다양한 사고예방 기술 개발에 필요한 통신 기반을 제공할 수 있을 것으로 사료된다.
