A Study of Methodology to Examine Organizational Root Causes through the Retrospect Error Analysis of Railroad Accident Cases

  • Objective: This study proposes a systematic process to present the analysis methods and solutions of organizational root causes to human errors on the railroad. Background: In fact, organizational root cause such as organizational culture is an important factor in the safety concerns on human errors in the nuclear power plant, railroad and aircraft. Method: The proposed process is as follows: 1) define analysis boundary 2) select human error taxonomy 3) perform accident analysis 4) draw root causes with FGI 5) review root causes analysis with survey 6) chart analysis of root causes, and 7) propose alternatives and solutions. Results: As a result, root causes of the organizations like railroad and nuclear power plant came from the educational problems, violations, payoff system, safety culture and so forth. Conclusion: The proposed process does predict potential railroad accident through retrospect error analysis by building new human error taxonomies and problem solution. Application: This study would contribute to examination of the relationship between human error-based accidents and organizational root causes.

The Gender Difference in the Occupational Hazards and Injuries of Cleaning Workers and Janitors

  • Objective: The purpose of this study is to analyze the accident characteristics according to the gender of the injured workers in building cleaning and to reflect them in the Industrial Accident Prevention Policy. Background: An analysis of industrial accidents is an essential process for establishing systematic industrial accident prevention measures. In order to establish industrial accident prevention measures for workers effectively, it is necessary to analyze accident characteristics by job type for workers who do the same work. Method: In this study, we analyzed the accident characteristics of 1,645 janitors who were approved of work-related injuries in 2015. We also analyzed the characteristics according to gender by dividing them into worker-related factors and accident-related factors. Results: The accidents caused to the janitors showed different characteristics according to gender, age, work experience, agency of accident, and distribution of original cause materials. In other words, 70.2% occurred to workers over 60 years old and 56.2% occurred to unskilled workers with less than a year of work experience. In the case of accident pattern, 79.1% occurred in tripping (slip) hazards, and 68.2% of accidents occurred on the floor (including the ground) and the stairs, indicating that the accident occurred most frequently during cleaning work on the floor or stairs. Conclusion and Application: The results of the study on the accident characteristics of the janitors can be used as basic data for systematic preventive measures against accidents occurring to the elderly female workers in the service industry.

차량크레인 전도 사고의 구조 안정성 평가에 관한 법공학적 연구 (Forensic Engineering Study on the Evaluation of the Structural Stability of the Mobile Crane Accident)

  • Forensic Engineering is the area covering the investigation of products, structures that fail to perform or do not function as intended, causing personal injury or damage to property. To investigate the mobile crane's overturn accident in terms of the forensic engineering, in this study, we identified the accident mobile crane's position and posture before accident by the analysis of the trace resulted by the contact between the outrigger and the ground, and the accident remodeling has been performed using CATIA modeling program in the basis of the accident mobile crane's position and posture information. The accident analysis has been performed by comparing this accident remodeling and the crane's specification, the table of the allowance load about the boom's length and the working radius. Through these studies, the safety accident that may occur in mobile crane can be minimized by performing specialized and systematic investigation of the accident cause in terms of the forensic engineering.

  • Industrial disaster caused the deaths of 2,114, construction workers among them was the highest of 621 deaths. In the construction industry, has established a number of safety alternatives to prevent accidents. But until now, the cause of the accident has stopped being superficial analysis, awareness on the root cause of the acciden did not reflect. In this study, we analyze the characteristics and causes in G contractors' safety accidents. And innovation strategy, organization-wide safety management system and detailed tasks to derive essentially was to prevent the occurrence of large construction companies. A lot of business for accident prevention effect was transient and formal, to reflect a management style and organizational culture, and try to prevent construction accidents. we will strive to prevent the disaster from the construction site through the improvement of these.

산재원인을 최소화하기 위한 예방인자 및 전략의 우선순위 평가에 관한 연구 (A Study on the Priority Evaluation of Prevention Factors and Strategies to Minimize Industrial Accident Cause)

  • It is very important to evaluate the priority of prevention factors and strategies in order to minimize industrial accident. It provides decisive information for accident prevention and safety management. Therefore, this paper proposes evaluation method of the priority through statistic testing with prevention factors and strategies by the cause analysis of cause and effect models. Especially, this paper uses priority matrix criterion to justify application of rank and objectivity. This paper is based on the results of a questionnaire of workers and managers who are engaged by real manufacturing and construction industries with less than 300 workers in the central region of Korea, where most of the fatal accidents have happened. Finally, the result provides one way to implement safety management for industrial accident prevention.

정전기 화재·폭발 위험성평가 및 사고조사에 필요한 발생원인 변수 도출 (Derivation of Cause Variables necessary for Electrostatic Fire/Explosion Risk Assessment and Accident Investigation)

  • Static-electricity-induced fires and explosions persistently occur every year, averaging approximately 80 and 20 cases annually according to fire statistics provided by the National Fire Agency and industrial accident statistics provided by the Ministry of Employment and Labor, respectively. Despite the relatively low probabilities of these accidents, their potential risks are high. Consequently, effective risk assessment methodologies and accident investigation strategies are essential for efficiently managing static-electricity hazards in fire- and explosion-prone areas. Accordingly, this study aimed to identify the causal variables essential for accident investigations, thereby facilitating risk assessments and the implementation of effective recurrence prevention measures to mitigate static-electricity hazards in fire-and explosion-prone regions. To this end, industrial accident statistics recorded over the past decade (2012 to 2021) by the Ministry of Employment and Labor were analyzed to identify major fire and explosion incidents and related industrial accidents wherein static electricity was identified as a potential ignition source. Subsequently, relevant investigation reports (63 cases) were thoroughly analyzed. Based on the results of this analysis, existing electrostatic fire and explosion risk assessment techniques were refined and augmented. Moreover, factors essential for investigating electrostatic fire and explosion disasters were delineated, and the primary causal variables necessary for effective risk assessments and scientific investigations were derived.

산업현장에서 쉽게 적용할 수 있는 근본원인 사고조사기법 개발에 관한 연구 (Development of RCA Incident Investigation Method as Easily Adopted Industry Field)

  • Incident investigation is one of the most important processes among various other safety management methods to prevent industrial accidents. Finding the root causes of accidents, eliminating hazards, and improving safety are the most important purposes of investigating accidents. During the investigation process, root cause analysis (RCA) techniques are used to effectively identify RCA. Over the past few decades, over 30 RCA methods have been developed. These techniques are being widely used in some industries, such as the nuclear and aircraft industries; however, most of the RCA techniques require professional knowledge and special training, making it difficult for safety managers in their respective fields to understand and apply them. Therefore, managers of general industrial sites are rarely present at the scene of actual accident investigations, and they cannot contribute much to the purpose and effectiveness of these investigations. In this study, to address these issues, we developed an RCA technique to facilitate root cause investigation of accidents in real-world industrial sites. To develop new techniques, Systematic Cause Analysis Technique (SCAT), one of the RCA techniques, was used to investigate incidents in the enterprise over three years. We also utilized feature analysis and other papers from existing RCA techniques. To verify its effectiveness, the technique proposed was also applied to the accident case. The technique developed can easily identify and analyze the root cause of an accident and help industrial managers. It can also identify the root cause category where accidents are concentrated and use this data to establish guidelines for preventing future accidents and, thus, focus on prioritizing improvement initiatives.

해빙기 가스사고분석 및 사고예방 대책 제시 (Gas Accident Analysis and Suggestion of Countermeasure at Thawing Season)

  • 연료가스의 사용이 늘어남에 따라 이로 인한 가스사고를 적절하게 관리할 필요성이 증대되고 있다. 최근 4년 동안 발생했던 사고를 체계적으로 분석하여 사고 발생에 영향을 미치는 주된 요인을 찾아내고 이를 사고감소 대책을 제시하는데 활용한다. 해빙기는 연평균 치보다 사고발생비율이 높아 상대적으로 취약한 기간으로 분석되었다. 동 기간동안 LPG(Liquefied petroleum gas) 사고는 연 평균치와 비슷하나 전체적으로 차지하는 비중이 높으며 원인별로는 사용자취급부주의에 의한 사고와 제품불량 혹은 시설미비에 의한 사고가 많아 이에 대한 대책이 시급한 것으로 나타났다. 동 기간동안 도시가스사고는 연 평균치보다 높아 전반적으로 주의 깊게 관리할 필요가 있으며 특히 원인별로 시설미비로 인한 사고 및 타공사로 인한 사고가 비중이 높았다. 비중이 높은 사고나 연 평균치보다 사고비율이 높은 사고에 대해 이를 저감할 수 있는 대책을 제안하였다.

  • 건설산업에서의 안전 수준 향상 노력에도 불구하고, 건설산업은 타 산업에 비해 높은 사망자수를 보이고 있다. 이러한 높은 사고율은 건설산업의 안전관리 측면에서 효과적인 대책이 필요함을 명확히 의미한다. 기존의 연구에서 주장하듯, 사고의 발생 가능성이 높은 공종이나 기인물을 파악하고 이에 적합한 안전대책을 세우는 것이 중요하다. 다수의 추락관련 건설재해가 작업발판 및 안전통로에서 발생한다는 점을 고려해보면, 안전관리 측면에서 가시설물을 중점적으로 다루어야 할 것이다. 이를 위해 본 연구에서는, 작업발판 및 안전통로와 관련된 6개년 1663개의 사망사고를 공사규모, 공정률, 공종, 직종에 의해 분류하고, 각 사망재해가 발생한 사례들을 분석함으로써 보다 중점적으로 관리되어야할 요소를 추출하였다.

국내 민간 무인항공기 사고조사 HFACS 적용중점 (Applicable Focal Points of HFACS to Investigate Domestic Civil Unmanned Aerial Vehicle Accidents)

  • 국내외 연구들은 무인항공기 사고의 주원인으로 인적요인을 지목하고 있고, 이러한 인적요인을 효과적으로 분석하는 기법으로 HFACS를 소개하고 있다. 현재까지는 HFACS를 이용해 무인항공기 사고의 인적요인을 분석했던 국내외 사례는 주로 군용 무인항공기가 대상이었는데, 항공사고를 유발하는 인적요인 정보를 분석하여 객관적 원인 규명과 유사 사고 예방 도구로 사용할 수 있는 HFACS가 국내 민간 무인항공기 분야에서도 활용이 필요한 시점이다. 특히, 국내 민간 무인항공기의 성능과 운용 여건을 고려한 HFACS 적용중점을 식별한다면 사고 발생 시 원인 규명과 재발 방지에 큰 도움이 될 것으로 예상된다. 본 연구는 HFACS version 7.0을 근간으로 우리나라 항공철도사고조사위원회가 수행했던 사고조사 결과보고서 자료를 분석하여 국내 민간 무인항공기 사고조사에 활용할 수 있는 HFACS 적용중점을 식별하였다.
