"위해성" 개념을 이용한 오염지역 정화 및 관리의 과학적 타당성 (Scientific Feasibility on the Risk-Based Clean-up and Management of Contaminated Sites)

  • 지난 수십년 동안 토양내 수착된 오염물질이 접촉시간(contact time)이 경과함에 따라 생물학적 또는 화학적 이용성의 감소는 일반적으로 인식되어 오고 있다. 이와 같은 aging 또는 격리(sequestration)라 불리는 현상이 위해성에 직접적인 영향을 미친다는 것이 알려져 있다. 이러한 현상은 소수성 오염물질을 중심으로 보고되어 왔으나, 최근 연구결과에 의하면 중금속의 경우에도 이와 같은 현상이 발견되고 있으며, 이는 오염물질의 흡-탈착 이력(hysteresis) 현상, 비가역 흡착, 탈착저항성, 비평형 흡착 등과 직접적으로 연관된 것으로 알려지고 있다. Aging 또는 sequestration에 의한 오염물질의 생이용성(bioavailability)의 감소는 인체에 대한 실질적인 위해성의 감소를 의미한다. 최근 들어 이와 같은 과학적인 증거를 토대로 단순한 오염물질의 농도 측정이 아니라 위해성에 근거한 오염 복원(또는 관리)의 개념이 도입되어 환경친화적인 복원수준의 선정에 적용하고 있다. 이는 토양내 오염물질의 총농도가 아니라 오염물질의 생이용성 또는 독성을 기준으로 하여 정화수준을 결정함으로써 오염부지의 처리비용과 노력의 절감효과를 동시에 기대할 수 있기 때문이다. 토양내 잔류오염물질의 생이용성 또는 독성은 오염물질의 이동 또는 거동 특성에 의해 영향을 받으며 결국 생물체로의 노출 경로와 생체축적에 직접적인 영향을 미친다. 본 논문에서는 토양내 오염물질의 위해성을 평가하여 토양오염복원에 적용시 과학적인 타당성에 대해 살펴보고자 하였다.

반도체 PR 공정의 인화성 물질 누출 빈도분석을 통한 위험성 평가 (Risk Assessment of Semiconductor PR Process based on Frequency Analysis of Flammable Material Leakage)

  • 반도체 Photo Resist (PR) 자동화 장비는 여러 인화성 물질을 혼합하여 사용하며, 인화성 물질이 공정 중 누출되는 경우 다양한 사고로 이어질 수 있어 위험성 평가가 필요하다. 본 연구는 PR 자동화 장비에서 사용되는 Acetone, PGMEA의 누출 빈도와 이러한 누출이 화재 사고로 이어질 수 있는 빈도를 빈도분석 방법을 통해 분석하였으며, 현 설비의 추가적인 위험성 경감 조치의 필요성을 평가하였다. IOGP의 공정 누출 데이터와 점화 확률 데이터를 기반으로 누출 빈도 및 점화 확률을 도출하였으며, 이를 조합하여 실제 화재 사고의 빈도를 분석하였다. 반도체 PR 공정 중에 발생할 수 있는 물질 누출에 대한 빈도는 7.30E-03/year이며, 화재 사고는 물질이 누출되었을 때 인화점 이상의 상태로 존재하는 Acetone에 의해 발생할 수 있으며, 빈도는 1.24E-05/year의 수준으로 계산되었다. UK HSE에서 제시하는 자료에 따르면, 1.24E-05/year의 빈도로 발생하는 주요 사고에 대해서는 7명 이내의 사망자를 발생시킬 때 위험성 경감을 위한 추가 조치가 필요 없는 수준 "Broadly Acceptable" 이라고 정의하고 있어, 2인 1조로 운영되는 공정의 특성상 별도의 위험성 경감 조치가 요구되지 않는다.

A Single-Center Experience of Robotic-Assisted Spine Surgery in Korea : Analysis of Screw Accuracy, Potential Risk Factor of Screw Malposition and Learning Curve

  • Objective : Recently, robotic-assisted spine surgery (RASS) has been considered a minimally invasive and relatively accurate method. In total, 495 robotic-assisted pedicle screw fixation (RAPSF) procedures were attempted on 100 patients during a 14-month period. The current study aimed to analyze the accuracy, potential risk factors, and learning curve of RAPSF. Methods : This retrospective study evaluated the position of RAPSF using the Gertzbein and Robbins scale (GRS). The accuracy was analyzed using the ratio of the clinically acceptable group (GRS grades A and B), the dissatisfying group (GRS grades C, D, and E), and the Surgical Evaluation Assistant program. The RAPSF was divided into the no-breached group (GRS grade A) and breached group (GRS grades B, C, D, and E), and the potential risk factors of RAPSF were evaluated. The learning curve was analyzed by changes in robot-used time per screw and the occurrence tendency of breached and failed screws according to case accumulation. Results : The clinically acceptable group in RAPSF was 98.12%. In the analysis using the Surgical Evaluation Assistant program, the tip offset was 2.37±1.89 mm, the tail offset was 3.09±1.90 mm, and the angular offset was 3.72°±2.72°. In the analysis of potential risk factors, the difference in screw fixation level (p=0.009) and segmental distance between the tracker and the instrumented level (p=0.001) between the no-breached and breached group were statistically significant, but not for the other factors. The mean difference between the no-breach and breach groups was statistically significant in terms of pedicle width (p<0.001) and tail offset (p=0.042). In the learning curve analysis, the occurrence of breached and failed screws and the robot-used time per screw screws showed a significant decreasing trend. Conclusion : In the current study, RAPSF was highly accurate and the specific potential risk factors were not identified. However, pedicle width was presumed to be related to breached screw. Meanwhile, the robot-used time per screw and the incidence of breached and failed screws decreased with the learning curve.

앉은 자세에서 힘 수준에 따른 상지관절 동작별 최대 수용 반복 빈도수 분석 (Analysis of Maximum Acceptable Frequencies for Upper Extremity Motions with Forces in a Seated Position)

  • Evaluation of repetitiveness for upper extremity intensive tasks is essential to determine the level of risk for upper extremity musculoskeletal disorders at the workplace. However, experimental data available to establish the acceptable levels of repetitiveness for various postures and forces is lacking. The present study examined the maximum acceptable frequencies(MAFs; motions/min.) of shoulder, elbow, wrist, and index finger motions at different forces(1kgf and 4kgf for shoulder, elbow, and wrist; 0.25kgf and 1 kgf for index finger) in sitting. Seventeen right-handed males in 20s without having any history musculoskeletal disorders participated in the MAF experiment. The participants determined their MAFs for the upper extremity motions by using the self-adjustment method and their work pulse(increase in heart rate; beats/min.) and rating of perceived exertion(RPE) were measured when working at MAF. The MAFs of elbow, wrist, and index finger motions for each force level were about 2, 3, and 6 times the corresponding MAF(9 at the high force and 24 at the low force) of shoulder motion and the MAFs at the low force increased about 2 times those at the high force. The work pulses of elbow, wrist, and index finger motions for each force level were 70%, 50%, and 30% of the corresponding work pulse(17 at the high force and 12 at the low force) of shoulder motion and the work pulses at the low force were about 70 % of those at the high force. Lastly, the RPEs of the upper extremity regions were about level 3(moderate) or below.

Learning Curve of C-Arm Cone-beam Computed Tomography Virtual Navigation-Guided Percutaneous Transthoracic Needle Biopsy

  • Objective: To evaluate the learning curve for C-arm cone-beam computed tomography (CBCT) virtual navigation-guided percutaneous transthoracic needle biopsy (PTNB) and to determine the amount of experience needed to develop appropriate skills for this procedure using cumulative summation (CUSUM). Materials and Methods: We retrospectively reviewed 2042 CBCT virtual navigation-guided PTNBs performed by 7 novice operators between March 2011 and December 2014. Learning curves for CBCT virtual navigation-guided PTNB with respect to its diagnostic performance and the occurrence of biopsy-related pneumothorax were analyzed using standard and risk-adjusted CUSUM (RA-CUSUM). Acceptable failure rates were determined as 0.06 for diagnostic failure and 0.25 for PTNB-related pneumothorax. Results: Standard CUSUM indicated that 6 of the 7 operators achieved an acceptable diagnostic failure rate after a median of 105 PTNB procedures (95% confidence interval [CI], 14-240), and 6 of the operators achieved acceptable pneumothorax occurrence rate after a median of 79 PTNB procedures (95% CI, 27-155). RA-CUSUM showed that 93 (95% CI, 39-142) and 80 (95% CI, 38-127) PTNB procedures were required to achieve acceptable diagnostic performance and pneumothorax occurrence, respectively. Conclusion: The novice operators' skills in performing CBCT virtual navigation-guided PTNBs improved with increasing experience over a wide range of learning periods.

퍼지이론을 사용한 다기준의사결정기법에 의한 질산의 위해성 관리 (Nitrate Risk Management by Multiobjective Decision-making Technique Using Fuzzy Sets)

  • 지하수를 음용수로 공급시 질산오염문제는 한국을 포함한 세계의 여러나라에서 보고되어 왔다. 질산($NO_3$)은 청색증 유발물짙로서 뿐만 아니라 발암물질일 가능성도 동물실험을 통해 제기되고 있는 실정이다. 질산에 오염된 지하수로 인한 인체의 위해성을 감소시키기 위하여 여러 가지의 질산 위해성 관리 대책들은 개발될 수 있다. 이러한 여러가지의 대책들을 비교 평가할 때 기준이 되는 항목으로는 (1) 허용할 수 있는 인체의 위해도(Risk Level), (2) 비용, 그리고 (3) 적용하는 방법의 기술적 타당성을 들 수 있다. 그러나 불충분한 자료와 인간지식의 한계 때문에 각 기준항목을 대표하는 값의 대부분은 불확실성을 내포하게 된다. 본 논문에서는 의사결정권자들이 불확실성을 고려하면서 여러가지 대책들 중에서 최적의 대책을 선정하는데 이용하기 위한 다기준의사결정기법은 보여진다. 그리고 각 기준값의 불확실성을 표현하고 다기준의사결정기법과 불확실성을 결합시키기 위하여 퍼지집합이론(Fuzzy Set Theory)은 응용되어 진다.

강원도 횡성지역의 지하수 수질 모니터링 및 인체 위해성 평가 (Water Quality Monitoring and Risk Assessment for Groundwater at Hoengseong, Gangwon-do Province)

  • Background: Concerns have been raised regarding the criteria of groundwater, in particular in Gwangwon-do Province where many residents drink groundwater due to the poor supply of tap water and a high nonconformity rate with water quality criteria nationwide. Objectives: Water quality monitoring and risk assessment were conducted for groundwater in Hoengseong, Gangwon-do Province. Methods: A total of 46 items required for meeting drinking water criteria were analyzed from 258 samples collected from March 2017 through August 2018 (152 sites in 2017 and 106 sites in 2018). Risk assessment was conducted for two non-carcinogens (F- and NO3-N), and one carcinogen (i.e., arsenic) based on their high nonconformity to water quality criteria. Results: Water quality analysis revealed that the total proportion of nonconformities was determined to be 27.9%. The nonconformity rate for each content item is as follows: total colony counts (1.6%), total coliform (6.2%), Escherichia coli (1.2%), F- (8.1%), arsenic (4.7%), NO3-N (8.1%), pH (1.2%), manganese (0.4%), and turbidity (5.8%). Risk assessment indicated that fluoride induced a hazard quotient greater than 1 with the 95% UCL (Upper Confidence Limit) concentration of the total 258 sites and average, median, and 95% UCL concentrations of nonconformity sites. For NO3-N, there was no human health risk. For arsenic, the excess cancer risk exceeded the acceptable cancer risk of 1×10-6 with the average and 95% UCL concentrations of total 258 sites and average, median, and 95% UCL concentrations of nonconformity sites. Conclusions: This study suggests that it is necessary to expand water quality monitoring of groundwater and conduct a more detailed risk assessment in order to establish a health care plan for the residents of Hoengseong, Gangwon-do Province.

  • Key concepts in South Korean nuclear safety regulation are safety and risk. Nuclear regulation in South Korea has required reactor designs and safeguards that reduce the risk of a major accident to less than one in a million reactor-years-a risk supposedly low enough to be acceptable. To date, in South Korean nuclear safety regulation has involved the establishment of many technical standards to enable administration enforcement. In scientific lawsuits in which the legal issue is the validity of specialized technical standards that are used for judge whether a particular nuclear power plant is to be licensed, the concept of uncertainty law is often raised with regard to what extent the examination and judgment by the judicial power affects a discretion made by the administrative office. In other words, the safety standards for nuclear power plants has been adapted as a form of the scientific technical standards widely under the idea of uncertainty law. Thus, the improvement of nuclear safety regulation in South Korea seems to depend on the rational lawmaking and a reasonable, judicial examination of the scientific standards on nuclear safety.

  • The importance of the risk analysis tool has been recognized and its use also has been emphasized by a number of researchers recently The methodology were examined but neither algorithms nor practical applications have been implemented or practiced in Korea. In this paper, the architecture of the Buddy System, one of the automated risk assessment tools. is analyzed in depth to provide the algorithmic understanding and to promote the development of the risk analysis methodology. The Buddy System mainly uses three main factors of vulnerability, threat and countermeasures as a nucleus of the qualatative analysis with the modified loss expectancy value. These factors are identified and assessed by the separation of duties between the end user and security analyst. The Buddy System uses five axioms as its bases of assessment algorithm and the assessed vulnerability level is strictly within these axioms. Since the In-place countermeasures reduce the vulnerability level up to a certain level. the security analyst may use "what if " model to examine the impact of additional countermeasures by proposing each to reduce the vulnerability level further to within the acceptable range. The emphasis on the qualitative approach on vulnerability leveling is very well balanced with the quantitative analysis that the system performance is prominent.prominent.

단축형 노인 낙상위험 사정도구의 타당도 (Validation of the Short Form Bobath Memorial Hospital Fall Risk Assessment Scale at a Specialized Geriatric Hospital in Korea)

  • Purpose: This study was conducted in order to evaluate the reliability, validity, sensitivity, and specificity of the Short Form of Bobath Memorial Hospital Fall Risk Assessment Scale (BMFRAS-SF). Methods: A validation study was conducted on 207 elderly patients aged over 65 who were admitted to Bobath Memorial Hospital. Fall risk scores of BMFRAS, composed of eight subscales (age, fall history, physical activity, consciousness level, communication, fall risk factors, underlying disease, and medications) were assessed from the electronic medical record. BMFRAS-SF was derived from eight subscales of the BMFRAS representing the significance between fallers and non-fallers (fall history, physical activity, fall risk factors, underlying disease, and medications). Internal consistency reliability and interrater reliability were assessed by Cronbach's alpha and kappa coefficient. Validity was assessed by Spearman correlation analysis, factor analysis. Sensitivity, specificity, positive predictive and negative predictive values, and a receiver-operating characteristic curve (ROC) were generated. Results: Fallers had significantly higher risk scores than non-fallers in fall history, physical activity, fall risk factors, underlying disease, and medication scales. The BMFRAS-SF demonstrated acceptable Cronbach's alpha (.706) and kappa coefficients of .95. The BMFRAS-SF subscales showed good convergent validity and construct validity. The BMFRAS-SF presented good sensitivity(86.7%), specificity(67.9%), positive predictive value(42.9%) and good negative predictive value(94.8%) at a cut-off score of 5. Areas under the ROC curves were .860 for the BMFRAS and .861 for the BMFRAS-SF. Conclusion: The BMFRAS-SF was proved to be reliable and valid. It could be used for time-saving assessment and evaluation of the high risks for falls in clinical practice settings.
