A Study of Usefulness for Megavoltage Computed Tomography on the Radiation Treatment Planning (메가볼트 에너지 전산화 단층 촬영을 이용한 치료계획의 유용성 연구)

  • The purpose of this study was to investigate image differences between KVCT vs MVCT depending on a high densities metal included in the phantom and to analyze the r values for the purpose of the dose differences between each methods. We verified the possibilities for clinical indications that using MVCT is available for the radiation therapy treatment planning. Cheese phantom was used to get a density table for each CT and CT sinogram data was transferred to radiation planning computer through DICOM_RT. Using this data, the treatment dose plan has been calculated in RTP system. We compared the differences of r values between calculated and measured values, and then applied this data to the real patient's treatment planning. The contrast of MVCT image was superior to KVCT. In KVCT, each pixel which has more than 3.0 of density was difficult to be differentiated, but in MVCT, more than 5.0 density of pixels were distinguished clearly. With the normal phantom, the percentage of the case which has less than 1($r\leq1$, acceptable criteria) of gamma value, was 94.92% for KVCT and 93.87% for MVCT. But with the cheese phantom, which has high density plug, the percentage was 88.25% for KVCT and 93.77% for MVCT respectively. MVCT has many advantages than KVCT. Especially, when the patient has high density metal, such as total hip arthroplasty, MVCT is more efficient to define the anatomical structure around the high density implants without any artifacts. MVCT helps to calculate the treatment dose more accurately.

Rapid onset of efficacy predicts response to therapy with certolizumab plus methotrexate in patients with active rheumatoid arthritis

  • Background/Aims: The objective of this study was to determine the efficacy and safety of add-on therapy with certolizumab pegol (CZP) in active rheumatoid arthritis (RA) patients of a single ethnicity. Methods: In this 24-week, phase 3, randomized, double-blind, placebo-controlled trial, eligible patients (n = 127) were randomized 2:1 to subcutaneous CZP + methotrexate (MTX; 400 mg at week 0, 2, and 4 followed by 200 mg every 2 weeks) or placebo + MTX. Results: At week 24, the American College of Rheumatology criteria for 20% (ACR20) response rate was significantly greater with CZP + MTX than with placebo (66.7% vs. 27.5%, p < 0.001). Differences in ACR20 response rates for CZP vs. placebo were significant from week 1 (p < 0.05) and remained significant through week 24. The CZP group reported significant improvement in physical function and disability compared to the placebo group (p < 0.001) at week 24, as assessed by Korean Health Assessment Questionnaire-Disability Index (KHAQ-DI). Post hoc analysis indicated that the proportion of patients who had ACR70 responses, Disease Activity Score 28 (DAS28) low disease activity, and DAS28 remission at week 24 was greater in CZP + MTX-treated patients who achieved a decrease in DAS28 ${\geq}1.2$ (43.8%) at week 4 than in nonresponders. Among 18 (22.2%) and 14 patients (35.0%) in CZP and placebo groups who had latent tuberculosis (TB), none developed active TB. Most adverse events were mild or moderate. Conclusions: CZP treatment combined with MTX in active RA patients with moderate to severe disease activity and an inadequate response to MTX resulted in rapid onset of efficacy, which is associated with better clinical outcome at week 24 and has an acceptable safety profile, especially in an intermediate TB-burden population.

Development of Measuring Tool for Health Promotion Behavior of Nurses (간호사의 건강증진행위 측정도구 개발)

  • The purpose of this study was to develop a measuring tool for the health promotion behavior of Korean nurses. This would address the lack of a proven tool that reflects the nature of the nurses' nursing environment. This study was conducted on 530 nurses from January to December 2019. A literature review and focus group interview were conducted, data analysis was carried out to measure validity and reliability, and the conceptual framework was constructed by applying the IMB model. Five factors namely self-concept (2 questions), hospital life management (4 questions), knowledge and information regarding health (5 questions), physical and mental stress management (3 questions), and work adaptation (2 questions) were framed into 16 questions. The model fit was 346.23 (��<.001), Parsimonious Normed Fit Index (PNFI) was 0.60, and Parsimonious Comparative Fit Index (PCFI) was 0.63, which met the acceptance criteria, and the Root Mean Square Error of Approximation (RMSEA) was 0.10. Goodness of Fit Index (GFI) was 0.88, Comparative Fit Index (CFI) was 0.85, and Incremental Fit Index (IFI) was 0.85 which were found to be acceptable as per the applicable standards. All items had a Cronbach's �� score of .85, which ensured stable reliability. The nurse's health promotion behavior measurement tool developed in this study will be used to measure the nurse's health promotion behavior in terms of nursing practice which will help in understanding the broad contours of this behavior.

A simple method to determine lycopene in solid supplementary food preparations using saponification and liquid chromatography (비누화 및 액체크로마토그래프를 활용한 고상 건강기능식품 중 라이코펜 분석법 개발)

  • Lycopene, a carotenoid hydrocarbon is known to have effects on reducing cardiovascular risk factors, blood lipids, and blood pressure. Thus, a lot of supplementary foods with lycopene in several dosage forms like soft capsule filled with liquid and hard capsule filled with powder are available in a market. Recently, however, our research group found that the lycopene assay in Supplementary Food Code of South Korea is only valid for oily lycopene preparation. Thus, here, we developed a simple method to determine lycopene in solid preparations for Supplementary Food Code of South Korea using saponification and liquid chromatography with an absorbance detector. The method was validated following Ministry of Food and Drug Safety guidelines. All validation parameters observed in this study were within acceptable criteria of the guidelines (selectivity, linearity of r2 ≥ 0.991, lower limit of quantification of 0.0149 mg/mL, accuracy as recovery (R) between 92.70 and 97.18 %, repeatability as relative standard deviation (RSD) values of R between 0.85 and 1.59 %, and reproducibility as the RSD value of interlaboratory R of 3.70 %). Additionally, the practical sample applicability of the validated method was confirmed by accuracy between 98.81 and 101.59 % observed from its lycopene certified reference material (CRM) analyses. Therefore, the present method could contribute to fortify the supplementary food safety management system in South Korea.

A Groundwater Potential Map for the Nakdonggang River Basin (낙동강권역의 지하수 산출 유망도 평가)

  • A groundwater potential map (GPM) was built for the Nakdonggang River Basin based on ten variables, including hydrogeologic unit, fault-line density, depth to groundwater, distance to surface water, lineament density, slope, stream drainage density, soil drainage, land cover, and annual rainfall. To integrate the thematic layers for GPM, the criteria were first weighted using the Analytic Hierarchical Process (AHP) and then overlaid using the Technique for Ordering Preferences by Similarity to Ideal Solution (TOPSIS) model. Finally, the groundwater potential was categorized into five classes (very high (VH), high (H), moderate (M), low (L), very low (VL)) and verified by examining the specific capacity of individual wells on each class. The wells in the area categorized as VH showed the highest median specific capacity (5.2 m3/day/m), while the wells with specific capacity < 1.39 m3/day/m were distributed in the areas categorized as L or VL. The accuracy of GPM generated in the work looked acceptable, although the specific capacity data were not enough to verify GPM in the studied large watershed. To create GPMs for the determination of high-yield well locations, the resolution and reliability of thematic maps should be improved. Criterion values for groundwater potential should be established when machine learning or statistical models are used in the GPM evaluation process.

A Review of the Systemic Analysis Method on Dental Sedation for Children (소아 치과환자에 대한 진정법의 체계적 분석 방법 고찰)

  • The first priority of sedation for incorporative children in pediatric dentistry is a safety. Therefore, evidence-based practices in health care are needed for preventing medical accidents. In accordance with the rise of the evidence based medicine, the interest in Evidence-Based Dentistry is increasing in the field of dentistry. However, systematic research about Evidence-Based sedation in Korea has rarely been done. As such, the purpose of this systematic review is to critically analyze the available scientific literature regarding dental sedation and to seek the next developmental strategies about evidence based pediatric dental sedation. A broad search of the 5 databases of the systematic reviews manual of the National Evidence-based Healthcare Collaborating Agency in Korea were referenced: 1) Core search database- KMbase, KISS; 2) Academic information and portal; 3) the National Assembly Library; 4) DBpia, and 5) RISS. Of a total 470 themes limited to the search term of "dental sedation", in accordance with the PRISMA statement for reporting systematic reviews of health sciences interventions, a literature selection process, which includes the removal of overlapping down the flow chart, was performed. Of the remaining 31 articles, two authors read through articles independently and added or removed articles using the exclusion criteria. Finally, twenty published papers of acceptable quality were identified and reviewed. This systemic review of Korean pediatric dental sedation practices for the last twenty-five years was based on the objective criteria defined in the GRADE process and identified consistent evidence. The results were evidence of moderate quality. Therefore, more systemically well-designed clinical studies are needed about the safe use of a sedative medicines (drugs).

  • Purpose: Bacille Calmette-$Gu{\acute{e}}rin$ (BCG) lymphadenitis is a relatively frequent local adverse reactions after BCG vaccination. Its incidence rate is usually <1%. However, this rate may be different according to BCG strain, vaccination method or skill, etc. In the Republic of Korea, two BCG strains are used: intradermal Danish-1331 or percutaneous Tokyo-172. We surveyed the incidence rates of BCG lymphadenitis. Methods: This survey was performed in total 25 centers (5 general hospitals, 20 private pediatric clinics). Immunized type of BCG strain in study subjects was verified by directly observing the scar. The occurrence of BCG lymphadenitis was asked to their parent. In cases of BCG lymphadenitis, location, diameter size, progression of suppuration, and treatment method were investigated, as well. Results: The total number of study subjects was 3,342. Among these, the subjects suitable for enrollment criteria (total 3,222; Tokyo strain 2,501, Danish strain 721) were analyzed. BCG lymphadenitis regardless of its size developed in each five of subjects per strains, therefore, its incidence rate was 0.20% in Tokyo and 0.69% in Danish strain, respectively (P=0.086). However, when applying the WHO criteria - the development of lymph node swelling with diameter 1.5 cm or more, the incidence rate of BCG lymphadenitis was 0.16% (4 cases) in Tokyo and 0.42% (3 cases) in Danish strain, respectively. Conclusions: The incidence rate of lymphadenitis in two BCG types, percutaneous Tokyo and intradermal Danish strain BCG, is 0.20% and 0.69%, respectively. Both rates are acceptable.

  • As various state restrictions on individual freedom were imposed during the COVID-19 pandemic, concerns have been raised that excessive infringements on fundamental rights were indiscriminately permitted based on the public interest of preventing infectious diseases. Therefore, the question of how to set acceptable limits of liberty restrictions on individuals has emerged. However, since the phenomenon of infections spreading to the population is only predicted statistically, how to deal with the risk of the infected individual as a subject of legal analysis has become a problem. In the absence of a theoretical framework of legal analysis of risk, the risk of infected individuals during the pandemic was not analyzed strictly, and proportionality review of infection prevention measures was often only an abstract comparison of the importance of public interest and individual rights. Therefore, this research aims to conduct a theoretical review on how risk can be conceptualized legally in a public health crisis, and to develop a theoretical framework for proportionality review of the risk of liberty-limiting measures during a pandemic. Chapter 2 analyzes the legal philosophical concepts of risk, which are the basis for liberty restrictions during a public health crisis, and applies and extends them to the pandemic. Chapter 3 reviews previous studies related to liberty restriction measures in the context of the COVID-19 pandemic, and points out they have a limitation that specific criteria for the proportionality review of public health measures in the pandemic have not been presented. Accordingly, Chapter 3 specifies the methodological framework for proportionality review, referring to the theoretical discussion on risks in Chapter 2. Chapter 4 reviews the legitimacy of gathering restriction orders, applying the theoretical discussion in Chapter 2 and the criteria for proportionality review established in Chapter 3. In particular, Section 4 examines logic of proportionality review in judicial precedents over the ban on gathering restrictions implemented in the COVID-19 pandemic. In analyzing the precedents, the logic of proportionality review in each case is critically reviewed and reconstructed based on the theoretical framework presented in this research.

Biological Toxicity Assessment of Sediment at an Ocean Dumping Site in Korea (폐기물 배출해역 퇴적물의 생물학적 독성평가 연구)

  • The effect of sediments in a waste dumping area on marine organisms was evaluated using sediment toxicity tests with a benthic amphipod (Monocorophium acherusicum) and bioluminescent bacterium (Vibrio fischeri) in accordance with the Korean Standard Method for Marine Wastes (KSMMW). Nine sites in the East Sea-Byeong, East Sea-Jeong, and Yellow Sea-Byeong areas were sampled from 2016 to 2019. The test results showed that the relative average survival rate (benthic amphipods) and relative luminescence inhibition rate (luminescent bacteria) were below 30%, which were judged to be "non-toxic." However, in the t-test, a total of 12 benthic amphipod samples (6, 1, 1, and 4 in 2016, 2017, 2018, and 2019, respectively) were significantly different (p<0.05) from the control samples. To identify the source of toxicity on benthic amphipods, a simple linear regression analysis was performed between the levels of eight heavy metals (Cr, As, Ni, Cd, Cu, Pb, Zn, and Hg) in sediments and the relative average survival rate. The results indicated that Cr had the highest contribution to the toxicity of benthic amphipods (p = 0.000, R2 = 0.355). In addition, Cr was detected at the highest concentration at the DB-85 station and exceeded the Marine Environment Standards every year. Although the sediments were determined as "not toxic" according to the ecotoxicity criteria of the KSMMW, the results of the statistical significance tests and toxicity identification evaluation indicated that the toxic effect was not acceptable. Therefore, revising the criteria for determining the toxic effect by deriving a reference value through quantitative risk assessment using species sensitivity distribution curves is necessary in the future.

A Study for the development of the Korean orthodontic bracket (한국형 교정치료용 Bracket의 개발에 관한 연구)

  • The aim of this study was development of the Straight-Wire Appliance(SWA) suitable lot the treatment or Korean. To accomplish the object of this study, Korean adult with normal occlusion were selected with following criteria : 1) no functional abnormality in the craniofacial area, 2) good dental arch form and posterior occlusal relationship, 3) Angle Class I occlusal relationship, 4) no experience of orthodontic, nor prosthodontic treatment, especially, no dental treatment on labial and buccal surfaces of teeth, 5) good racial profile. Impression were taken for upper and lower dental arches or the selected normal occlusion samples and the orthodontic dental stone models were fabricated. 5 well-trained orthodontists had examined the acquired dental stone models to select study samples which satisfy the Six keys to optimal occlusion of Andrews. 155 pairs of dental stone models (92 pairs of Male, 63 of Female) were finally selected. 3 dimensional digitization were performed with the Coordinate Measuring Machine(CMM, MPC802, WEGU-Messtechnik, Germany) and measuring of Angulation, Inclination, In-and-Out, Molar offset angle and Arch form were accomplished with a measuring software to achieve data for the development of SWA. Before the measurement, error study was performed on the 3 dimensional digitization with CMM, and the analysis of reliability of computerized measuring method adapted in this study and conventional manual method Presented by Andrews was performed. Results of this study were as to)lows : 1. Equi-distance digitization with mesh size 0.25 mm, 0.5 mm and 1.0 mm were acceptable in 3 dimensional digitization of dental stone model with the CMM, and the digitization with 1.0 mm mesh size was recommendable in terms of efficiency. 2. Computerized measuring method with 3 dimensional digitization was more reliable than manual measuring method of Andrews. 3. Data were collected for the development of SWA suitable for the morphological characteristics of Korean with the computerized measuring method with 3 dimensional digitization.
