Risk Factors for Hypertension of Middle Aged Male Workers using Data from Health Check-ups (중년 남성 근로자의 고혈압 발생에 관련된 요인의 검토)

  • The present study was conducted to identify the factors related to the hypertension, and to offer the evidence of 1st prevention of hypertension. 2,230 male workers aged 30-59 years were observed the relationships between hypertension and age, BMI, glucose intolerance, smoking, alcohol drinking, regular exercise, eating habits using data from health check-ups in 2011. As a results, the incidence rate of hypertension was 18.8% of 30-39 year old group, 23.8% of 40-49 year old group, 33.0% of 40-49 year old group. The incidence rate of hypertension was significantly higher as age and BMI ascend, and it was higher in the group of abnormal glucose intolerance, regular alcohol intake, no regular exercise than their respective counterparts. The multivariable-adjusted odds ratio of hypertension increased significantly as there is an increase in age, and the group of abnormal glucose intolerance, smoking, regular alcohol intake, no regular exercise. In conclusion, obesity, glucose intolerance, alcohol intake, and physical inactivity are risk factors for hypertension, therefore we need the control of these factors for 1st prevention of hypertension.

Dumping Syndrome in a Child with Gastrojejunal Tube Feeding (위공장 경관식이(Gastrojejunal Tube Feeding) 환아에서 동반된 덤핑 증후군(Dumping Syndrome) 1례)

  • Dumping syndrome is a known complication of gastric surgery in adults, but a very rare disease in the pediatric population. We report on a case of dumping syndrome in a 19-month-old child, who underwent gastrojejunal feeding tube insertion for the treatment and prevention of gastroesophageal reflux and frequent aspiration pneumonia. At 17 months of age, 2 months after the beginning of gastrojejunal tube feeding, postprandial diaphoresis, palpitation, lethargy, bloating, and diarrhea occurred, and a single episode of convulsion with hypoglycemia were noted. Early and late dumping syndrome was confirmed by an abnormal oral glucose tolerance test with early onset hyperglycemia followed by delayed onset hypoglycemia. Diet therapy including uncooked corn starch then improved the postprandial diaphoresis, abnormal glucose levels, and her nutritional status. We conclude that dumping syndrome may be considered as a complication of gastrojejunal tube feeding in a child.

Correlation Between Unidentified Bright Objects on Brain Magnetic Resonance Imaging (MRI) and Cerebral Glucose Metabolism in Patients with Neurofibromatosis Type 1

  • Purpose: Neurofibromatosis type 1 (NF1), which is caused by mutations of the NF1 gene, is the most frequent single gene disorder to affect the nervous system. Unidentified bright objects (UBOs) are commonly observed on brain magnetic resonance imaging (MRI) in patients with NF1. However, their clinical and pathologic significance is not well understood. The purpose of this study was to investigate the correlation between UBOs and cerebral glucose metabolism measured by $^{18}F$-2-Fluoro-2-deoxy-D-glucose ($^{18}F$-FDG) positron emission tomography (PET) in Korean patients with NF1. Materials and Methods: Medical records of 75 patients (34 males and 41 females) with NF1 who underwent brain MRI and PET between 2005 and 2011 were evaluated retrospectively. Clinical data including demographics, neurological symptoms, and brain MRI and PET findings, were reviewed. Results: UBOs were detected in the brain MRI scans of 31 patients (41%). The region most frequently affected by UBOs was the basal ganglia. The most frequent brain PET finding was thalamic glucose hypometabolism (45/75, 60%). Of the 31 patients with UBOs, 26 had thalamic glucose hypometabolism on brain PET, but the other 5 had normal brain PET findings. Conversely, of the 45 patients with thalamic glucose hypometabolism on brain PET, 26 showed UBOs on their brain MRI scans, but 19 had normal findings on brain MRI scans. Conclusion: UBOs on brain MRI scans and thalamic glucose hypometabolism on PET appear to be 2 distinctive features of NF1 rather than correlated symptoms. Because the clinical significance of these abnormal imaging findings remains unclear, a longitudinal follow-up study of changes in clinical manifestations and imaging findings is necessary.

Changes in Blood Pressures, Blood Profiles and Physical Conditions among Adults in the Daejeon Area (대전지역 일부 직장인의 혈압, 혈액성분 및 체위의 변화)

  • The body mass index, blood pressures and blood profiles of adults, who worked at three colleges in the Daejeon area, were evaluated for use in developing well-balanced menus at their workplace cafeterias and/or in creating nutrition education programs tailored to their health statuses. The data analyzed were from physical examinations performed in 2002 and in 2004 by the National Health Insurance Corporation. Overall, levels of systolic and diastolic blood pressure and serum cholesterol were lower in the 2004 samples than in those of 2002, but most samples in 2004 showed higher levels of SGOT. In 2002, statistical differences occured in the levels of blood pressure and serum cholesterol among age groups, but not in 2004. And in 2004, the older the age group, the higher the BMI. Among subjects in their twenties, three variables (systolic and diastolic blood pressure, and serum cholesterol) showed statistical differences; this increased to four variables (systolic and diastolic blood pressure, blood glucose and serum cholesterol) among subjects in their thirties and to six variables (Hb, SGOT, systolic and diastolic blood pressure, blood glucose and serum cholesterol) among those in their forties, followed by a decline to four variables (systolic and diastolic blood pressure, Hb and serum cholesterol) among subjects in their fifties. In general, the results of physical examinations showed improvements in blood pressure and blood profiles in 2004 in comparison with results in 2002. The level of SGOT was the exception. Despite these improvements, of those employees who had physical examinations in 2004, $5.2\%$ had high blood glucose and $18.1\%$ hypertension, $27.9\%$ hypercholesterolemia, $8.3\%$ registered abnormal liver functions, and $31.3\%$ were classified as overweight and obese. Moreover, because some vulnerable blood profiles were found in the older age groups, various programs, including nutrition education, and/or development of healthful diets as well as excercise activities, may be needed within their workplaces.

Relationship among impaired fasting glucose and diabetes and periodontal disease (공복혈당장애 및 당뇨병과 치주질환의 관련성)

  • This study examined the relationships among impaired fasting glucose (IFG), diabetes and periodontal disease. The data from 10,856 adults (aged over 19 years) was derived from the Fifth Korean National Health and Nutrition Examination Survey, which was conducted in 2010 and 2012. Adjusting the related confounders, multiple logistic regression analysis showed that periodontitis were related to gender, age, education level, smoking and diabetic status (p<0.001). These findings suggest that abnormal fasting glucose, which is a predisposing factor for diabetes mellitus, does not appear to be a risk indicator for periodontal disease. On the other hand, if patients do not take steps to prevent or delay diabetes, prediabetes is likely to develop into type 2 diabetes within 10 years. Therefore, patients with prediabetes need to undergo continuous examinations and management of periodontal disease.

Effects of Legume Supplementation on the Glucose and Lipid Metabolism and Lipid Peroxidation in Streptozotocin-Induced Diabetic Rats (두류의 첨가가 당뇨병 쥐의 내당능과 지질대사 및 지질과산화에 미치는 영향)

  • The present study was conducted to evaluate the usefulness of common Korean legumes as a high-fiber supplement in therapeutic diets for diabetic patients. Streptozotocin-induced diabetic rats were used as animal models and four kinds of legumes, black soybean (BS), yellow soybean (YS), green pea (GP) and soybean curd residue (SCR) were tested as high-fiber supplements. Seven groups of normal and streptozotocin-induced diabetic rats were fed isocaloric experimental diets containing 8% dietary fiber from one of four legumes or purified cellulose and pectin for 6 weeks. The effects of the legumes on the glucose and lipid metabolism of diabetic rats was examined and compared with the effects of cellulose and pectin. The legume supplementation did not show any beneficial effect on glucose tolerance, however, it exhibited a plasma cholesterol-lowering effect in diabetic rats. The cholesterol-lowering action was especially strong in BS and the degree of the effect was comparable to that of pectin. The levels of total lipids, cholesterol, and triglyceride in the hepatic tissues of rats fed legume diets were similar to those of the pectin group. All legume supplements induced an increase in fecal steroid excretion. The fecal cholesterol contents were significantly high following the supplementations of YS and SCR (p < 0.05). The excretion of fecal bile acids in the BS and YS groups was significantly higher than it was in the pectin group (p < 0.05). Concentration of lipid peroxidation products in the blood and urine of diabetic animals was lower in the legume groups than in the cellulose group. The levels of hepatic lipid peroxidation products were significantly lower in the BS and YS groups than in the pectin group (p < 0.05). From the results of this study, the plasma cholesterol-lowering effect of BS is possibly due to the significant (p < 0.05) in-crease in fecal steroid excretion, which suggests that BS could be beneficial in improving abnormal lipid metabolism in diabetic rats. Anti-Diabetic Studies of Mass Cultured Mycelia from Ganoderma applanatum in db/db Mice and Human (잔나비걸상버섯(Ganoderma applanatum) 균사체의 db/db Mice 및 인체에서의 항당뇨 효능연구)

  • Anti-diabetic activities of cultured mycelia from Ganoderma applanatum are being evaluated in this study. The OGTT and 4-weeks of repeated oral efficacy tests are conducted in mice at the doses of 0 (vehicle treatment), 25, 75 and 225 mg/kg/day, respectively. In human study, the test article was administered orally every day for 8-week at a dose of 1,500 mg/kg, tid and placebo group. The blood glucose levels (BGL) at 0.5 hour after treatment are significant decreased in all treatment groups of OGTT test. In the 4-week test, BGL of 75 and 225 mg/kg/day group is continuously decreased during all treatment periods and the BGL of 25 mg/kg/day group show decreasing trends at the final week, the pancreas weight of all treatment groups are being increased, and the Langerhans-islet numbers were increased at all treatment groups with a dose-response manner. There are no test article-related abnormal signs and the fasted blood glucose (FBG), postprandial blood glucose (PPG) and HbA1c are decreased significantly after 8-week treatments. These results that the cultured mycelia from Ganoderma applanatum could decrease BGL by protecting the degeneration of Langerhans islets.

Effectiveness of Clinical Examination for Detection of Respiratory Tuberculosis (호흡기 결핵 환자에서 임상 검사의 유용성)

  • To find out the effectiveness of clinical examination for the diagnosis of respiratory tuberculosis, a 78 respiratory tuberculosis patient,s group was matched by sex and age with 78 control healthy subjects. In the result of blood chemistry, mean values of $123.5{\pm}62.04mg/dL$ in glucose, $429.01{\pm}150.77IU/L$ in LDH, and $44.51{\pm}43.76IU/L$ in ${\gamma}$-GTP, were higher than that of the controls (healthy subjects), and $3.51{\pm}0.68mg/dL$ in albumin was lower than that of the controls. In the result of the haematology examination, mean values of $12.52{\pm}3.27g/dL$ in hemoglobin, $36.72{\pm}7.28%$ in hematocrit, and $24.61{\pm}12.36%$ in lymphocyte, were lower than that of the controls, $9.23{\pm}5.25%$ in monocyte $78.30{\pm}37.35mm/hr$ in ESR, and $48.45{\pm}35.15U/L$ in ADA were higher than that of the controls. For the comparisons of the tuberculosis patients values from normal reference values, 22.2% in glucose, 22.4% in LDH, 25.0% in ${\gamma}$-GTP, 35.4% in albumin, 88% in ESR, and 88.6% in ADA, showed abnormal values. We concluded that the values of glucose, ${\gamma}$-GTP, albumin, WBC, RBC, hemoglobin, hematocrit, lymphocyte, monocyte, ADA, and the ESR were useful in tuberculosis diagnosis.

Decreased Expression of TRPV4 Channels in HEI-OC1 Cells Induced by High Glucose Is Associated with Hearing Impairment

  • Purpose: Previous reports have shown that hyperglycemia-induced inhibition of transient receptor potential vanilloid sub type 4 (TRPV4), a transient receptor potential ion channel, affects the severity of hearing impairment (HI). In this study, we explored the role of TRPV4 in HI using HEI-OC1 cells exposed to high glucose (HG). Materials and Methods: HEI-OC1 cells were cultured in a HG environment (25 mM D-glucose) for 48 hours, and qRT-PCR and Western blotting were used to analyze the expression of TRPV4 at the mRNA and protein level. TRPV4 agonist (GSK1016790A) or antagonist (HC-067047) in cultured HEI-OC1 cells was used to obtain abnormal TRPV4 expression. Functional TRPV4 activity was assessed in cultured HEI-OC1 cells using the MTT assay and a cell death detection ELISA. Results: TRPV4 agonists exerted protective effects against HG-induced HI, as evidenced by increased MTT levels and inhibition of apoptosis in HEI-OC1 cells. TRPV4 overexpression significantly increased protein levels of phosphorylated p38 mitogen-activated protein kinase (p-p38 MAPK), while TRPV4 antagonists had the opposite effect. Our results indicated that TRPV4 is a hyperglycemia-related factor that can inhibit cell proliferation and promote cell apoptosis by activating the MAPK signaling pathway in HEI-OC1 cells. Conclusion: Our results show that the overexpression of TRPV4 can attenuate cell death in HEI-OC1 cells exposed to HG.

Acute Toxicity Study on Coptidis Rhizoma in Mice (황련의 급성독성에 관한 연구)

  • In order to evaluate acute toxicity of Coptidis rhizoma, 6 week- and 13 week-old male ICR mice received Coptidis rhizoma extract (600~4,800 mg/kg body weight) orally, and toxicological responses were observed for consecutive 7 days. In the mice received relatively high concentration of Coptidis rhizoma($\geq$1,200mg/kg), death occurred within 3 hrs after oral administration, and its ratio in 13 week-old mice was conspicuously higher than that in 6 week-old mice. $LD_{50}$ of Coptidis rhizoma were estimated to bi 2,575 mg/kg and 1,490 mg/kg body weight in 6 week and 13 week-old mice, respectively. Coptidis rhizoma-treated animals manifested a variety of abnormal clinical findings such as ptosis, crouching, lethargy, convulsion, bizarre behavior and truning sideway. These abnormalities also ranked highly in the 13 week-old mice compared to those in the 6 week-old mice. In addition to abnormal behaviors, Coptidis rhizoma($\geq$1,200 mg/Kg) significantly elevated the urinary contents of bilirubin, urobilirubin, protein and glucose, and values in 13 week-old mice was higher than those in 6 week-old animals. No toxicological response was observed at concentration less than 600 mg/kg. Our results clearly demonstrate that susceptibility of mice to Coptidis rhizoma may be related with age, indicating that younger age mice is more resistant to the Coptidis rhizoma than the older, and toxicological mechanism of Coptidis rhizoma may be closely associated with its pharmacological mechanism.
